• The Indiana State Department of Health Laboratory identified a suspect swine-origin influenza A virus on October 28, 2011. The specimen was forwarded to the CDC where genome sequencing confirmed the specimen to be positive for the presence of a triple reassortant H3N2 (SOtrH3N2) influenza virus. The case has been investigated by ISDH Surveillance and Investigations Division, the Indiana State Department of Health Laboratory, and the CDC. The affected Local Health Department and BOAH have been notified and have been cooperative in the investigation. The case patient had very close contact with pigs in the week prior to illness onset. No illness has been reported in the case’s household or close contacts. This case is the 7th recognized case of human infection with S-OtrH3N2 with the M segment gene from the pH1N1virus (previously identified in Maine [2], Indiana [1], and Pennsylvania [3]) in 2011. This case from Indiana marks the second case in the past 3 months and the 15th human case of S-OtrH3N2 reported to CDC since 2005. No community transmission has been documented in this case. General information about swine influenza is available at http://www.cdc.gov/flu/swineflu/.

Sentinel surveillance system ILI: 1.28%; at baseline.
No specimens tested positive for influenza. Two were positive for Rhinovirus, and one was positive for Parainfluenza 1.

Week 45:
ILI: 0.89%
ED ILI: 1.2%
No positives for influenza detected
UPDATE: Two human infections with novel influenza A viruses were detected in individuals from Indiana. Both patients were infected with swine-origin influenza A (H3N2) viruses. Both were hospitalized but have been discharged and both have recovered from their illnesses. One case was hospitalized in July, 2011 and one in October, 2011. There has been no evidence of an epidemiological link between these two patients and no sustained human-to-human transmission of these viruses has been found.

Discussions were held among U.S. and international organizations to determine what to call a non-human influenza virus that is detected in humans. Following discussions with the World Health Organization (WHO), the World Organization for Animal Health (OIE), the Food and Agriculture Organization (FAO) and other U.S. Federal agencies, influenza viruses that originate in swine but that are identified in humans will now be referred to as ?variant? viruses and denoted with a ?v.? Since August 2011, 12 cases of human infection with an influenza A (H3N2) virus with avian, swine and human genes that has been found in U.S. swine have been detected. In the past, these viruses have been called ?triple reassortant swine origin viruses?. Following the new naming convention, the H3N2 virus detected in 12 people in the U.S. will now be called an ?influenza A (H3N2) variant virus with genes from avian, swine and human viruses,? and will be abbreviated ?A (H3N2)v? for scientific purposes and ?H3N2v? for general public use.

The Indiana State Department of Health (ISDH), in collaboration with the National Centers for Disease Control and Prevention (CDC) is requesting certain medical professionals to participate in a special enhanced influenza surveillance program. The enhanced influenza surveillance program involves a limited number of selected sites to submit at least 4 nasopharyngeal specimens for RT PCR testing of patients with influenza like illness (ILI) to the ISDH Laboratory every week until further notice. The ISDH will providethe shipping (by using a provided pre-paid air bill) and the testing free of charge. Our enhanced system would like to rely heavily on specimens from children. In order to further identify and follow influenza strains and to track any changes in the virus, we need extra specimens to test. The submitter will receive lab results for all positive specimens, all testing supplies and shipping for the specimens submitted. If you are interested in joining our enhanced influenza surveillance program, please e-mail Shawn Richards at srichard@isdh.in.gov to request an enrollment form or discuss the opportunity further. Unauthorized influenza specimens submitted to the ISDH Laboratory will not be tested.
