This is from Sep. 24 It is stating that the Mexican Health Authorities were developing a plan to prevent/control Dengue in Sinaloa State (Pacific Coast).

Mexican Sinaloa State Fights Dengue

Mexico, Sep 24 (Prensa Latina) Mexican health authorities are developing an emergency operation to prevent and control dengue in Sinaloa state, on the Pacific coast, which was affected by Lane hurricane.

Cleaning of houses, fumigation of over 120 communities, entomologic measurement to learn about the behavior of the population of Aedes Aegipty, the transmitting mosquito of the disease, are some of the actions currently carried out to avoid an epidemic.

Oscar Velazquez Monroy, director of the National Center for Epidemiology and Disease Control (CENAVECE) reported that the district has enough larvicides and trained personnel.

He insisted that fumigation will only work as long as mosquito breeding sources are eradicated inside and outside the houses, and exhorted all settlers to keep taking effective and permanent action to this end.

Health Secretary of the state Hector Ponce asserted they reinforced the health infrastructure and increased the resources in Sinaloa, in order to avoid outbreaks of the disease.

He reiterated that the most important task must be carried out by the population in their houses, eliminating any gap for the mosquito to reproduce, avoiding open garbage bins and water containers.

Jorge Mendez, director of the Program to Prevent Diseases Transmitted by vectors, warned that after the heavy rains the meteor left, the presence of Aedes Aegipty grubs in house patios is between 30 and 60 percent.

Those larvae will soon become mosquitoes; hence the urgent need to pay attention to the risk of having mosquito deposits, the expert said.

Mexico Speculation

Due to the proximity of Mexico, I wanted to start this thread off with some background information.

CDC: Dengue Fever at the U.S.-Mexico Border, 1995-1996

Dengue is a mosquito-transmitted acute disease caused by any of four virus serotypes (DEN-1, DEN-2, DEN-3, and DEN-4) and is characterized by acute manifestations that can include fever, headache, myalgia, arthralgia, rash, nausea, and vomiting (1). On August 25, 1995, public health authorities in Mexico notified the Texas Department of Health (TDH) of an ongoing outbreak of dengue fever in the state of Tamaulipas, which borders south Texas (Figure_1). Because of the year-round presence of the Aedes aegypti mosquito (a major vector for dengue) in southernmost Texas and the frequent movement of persons across the U.S.-Mexico border, the outbreak in adjacent Tamaulipas suggested an increased potential for imported and autochthonous cases in Texas, as had occurred during 1980 and 1986 (2). In response to the notification from Mexico, TDH intensified surveillance efforts for dengue, resulting in identification of 29 laboratory-diagnosed cases in Texas residents, including seven persons with no history of travel outside the state. This report summarizes results of dengue surveillance in the U.S.-Mexico border area during 1995-1996. Mexico

During July-December 1995, health authorities in Tamaulipas (1994 population: 2,459,087) reported 4758 suspected cases of dengue to health authorities in Mexico. Dengue hemorrhagic fever (DHF) * was reported in 37 (1%) of these cases. The largest numbers of cases were reported from the cities of Reynosa (2706 cases), adjacent to McAllen (Hidalgo County), Texas; Tampico (1404 cases), approximately 250 miles south of the border; and Matamoros (408 cases), adjacent to Brownsville (Cameron County), Texas. Dengue infection was laboratory-diagnosed by positive immunoglobulin M (IgM)-capture enzyme-linked immunosorbent assay (ELISA) (578 cases) and viral isolation (64 cases). Viral isolates included DEN-1 from southern Tamaulipas, DEN-3 and DEN-4 from northern Tamaulipas, and DEN-2 from both areas.

In Reynosa, epidemic activity began in July and peaked in October. DEN-2, DEN-3, and DEN-4 viruses were isolated from cases in Reynosa. Although cases occurred in all age groups, most (70%) were in persons aged 15-44 years; 56% of cases occurred in females. Hemorrhagic manifestations were noted in 218 patients (8%), but only 28 (1%) patients developed DHF. Mosquitoes were collected in Reynosa from mid-October through mid-November and included 847 Ae. aegypti, 1033 Ae. albopictus, and 420 Aedes spp. DEN-2 was recovered from two pools of Ae. aegypti mosquitoes.

After recognition of the outbreak, health authorities initiated community education and mosquito-control activities in Reynosa. High school students distributed educational pamphlets, and vector-control personnel conducted clean-up campaigns and treatment of larval habitats with Abate(Registered) ** (temephos) in all sections of the city. Ultra-low volume (ULV) applications of malathion were conducted in 179 neighborhoods.

In Tamaulipas, transmission of dengue ended by late December 1995. However, during the first week of July 1996, cases of dengue-like illness were reported in Tampico. Of 28 acute- or convalescent-phase serum samples obtained during August and sent to CDC for testing, 17 were positive for IgM dengue antibodies, and cultures of five samples yielded DEN-1. Texas

Because of the reported outbreak in Tamaulipas, on August 25, 1995, TDH issued a dengue alert memorandum by facsimile to all local health departments, infection-control practitioners, and infectious disease physicians in south Texas. Packets of information about dengue prevention were mailed to 13,000 primary-care and emergency department physicians throughout the state. Community education efforts included a press release advising the public about the threat of dengue and recommendations for preventing mosquito exposure, the distribution of informational material on dengue prevention and the mosquito life cycle (6000 posters and 200,000 pamphlets in English and Spanish). TDH conducted active surveillance for dengue-like illness through telephone calls and personal visits to area hospitals and clinical and reference laboratories. Studies of vector densities in selected habitats confirmed that both Ae. albopictus and Ae. aegypti were abundant in this area (4).

Specimens from 273 Texas residents with suspected cases were tested at CDC; of these, 23 had virologic or serologic evidence of dengue infection. TDH received reports from commercial laboratories of seven additional patients with positive serologic tests. Of the 29 patients with laboratory-diagnosed dengue, eight reported recent travel to areas with endemic dengue outside Mexico. Of the remaining 21 persons, 14 reported recent travel to Mexico, and seven reported no travel outside Texas. The seven persons with domestically acquired dengue were women aged 20-90 years (median: 40 years). Dates of onset of illness among these seven cases were from mid-September through mid-November 1995. Four of these patients resided in Hidalgo County and three in Cameron County. DEN-2 was isolated from one of the four Hidalgo County residents, and DEN-4 was isolated from one of the three Cameron County residents. TDH and CDC are investigating an additional case of suspected dengue in a person with no travel history.

Dengue Fever Outbreaks in Mexican Coastal Areas

November 6, 2006

Francisco Cardoza, Secretary of Health of Baja California Sur, reported that the increase in dengue fever and dengue hemorrhagic fever cases in Los Cabos, BCS, Mexico, that total 63, requires strengthened epidemiological measures. The state agency has sent 70 additional workers to support the 40 who are working to lower larvae indices of the mosquito that transmits dengue fever and to control the outbreak.

In Acapulco, Guerrero, authorities report a rise of 166 dengue cases in just six days, in both of its manifestations, which brings the total to 1,156 cases in this tourist destination. The Secretariat of Health has invested [US$1.4 million] in the fight against the transmitting mosquito.

The incidence of dengue, that in its hemorrhagic fever form is life threatening, remains at high levels in the states of Veracruz, Guerrero and Quintana Roo, where 10,757 cases and five deaths have been recorded this year due to the resistance of the people to collaborate in official prevention programs, and in programs to fight the mosquito that transmits the disease.

Mexico Dengue Goes Deadly

Mexico, Mar 22 (Prensa Latina) One out of every four dengue cases in Mexico is hemorrhagic, the most dangerous and deadly form, Mexican Health Secretary Jose Angel Cordoba asserted.

The fight against dengue is one of the main health programs today, said Cordoba, who called for all social sectors to participate actively in its elimination.

He recalled that there was one case of hemorrhagic dengue in Mexico per every 50 cases of classical dengue seven years ago, but there is one per every four cases now, hence the urgent need to adopt the necessary measures, he explained.

According to an epidemiological general view by the Mexican Health Secretary s Office, there were 4,477 hemorrhagic-dengue cases, which caused 20 deaths.

The official said the most effective way to control the epidemic is to constantly destroy deposits of the virus transmitting agent, the Aedes Aegypti mosquito.

Cordoba visited Quintana Roo state, some 746 miles west of this capital, and called to intensify preventive actions and continue the joint programs among the municipal, state, and federal government levels.

Surge in dengue fever forecast

Associated Press

Deadly form of dengue fever increasing in Mexico

MEXICO CITY ? The deadly hemorrhagic form of dengue fever is increasing dramatically in Mexico, and experts predict a surge throughout Latin America fueled by climate change, migration and faltering mosquito eradication efforts.

Overall dengue cases have increased by more than 600 percent in Mexico since 2001, and worried officials are sending special teams to tourist resorts to spray pesticides and remove garbage and standing water where mosquitoes breed ahead of the peak Easter Week vacation season.

Even classic dengue ? known as "bonebreak fever" ? can cause severe flu-like symptoms, excruciating joint pain, high fever, nausea and rashes.

More alarming is that a deadly hemorrhagic form of the disease, which adds internal and external bleeding to the symptoms is becoming more common.

It accounts for one in four cases in Mexico, compared with one in 50 seven years ago, according to Mexico's Public Health Department.

While hemorrhagic dengue is increasing around the developing world, the problem is most dramatic in the Americas, according to the Centers for Disease Control and Prevention.

Dengue is driven by longer rainy seasons some blame on climate change, as well as disposable plastic packaging and other trash that collects water.

Migrants and tourists ? including the many thousands of Americans expected for spring break this year ? carry new strains of the virus across national borders, where mosquitoes can spread the disease.

The CDC says there's no drug to treat hemorrhagic dengue, but proper treatment, including rest, fluids and pain relief, can reduce death rates to about 1 percent.

Dengue is increasing dramatically in Mexico, and specialists predict
a big increase in dengue incidence in all of Latin America.
This
epidemic is being driven by a combination of factors, such as climate
change, migration, urbanization and deficiencies in mosquito control
campaigns.

Dengue incidence has increased 600 percent since 2001, and the authorities
have sent special [mosquito] eradication teams to the principal tourism
centers in anticipation of the Easter Week vacation period.

The epidemic also reflects a longer rainy season as well as the
abundance of disposable plastic containers, which retain water and
serve as breeding places for mosquitoes. Immigrant and tourist
travelers transport new strains [dengue serotypes - Mod.TY] of the
virus. The disease is becoming more common and more deadly.

DHF, which can be fatal, is becoming more frequent, with one [DHF
case] in 4 dengue cases, compared with one in 50 six years ago,
according to the Mexican Secretary of Health. [This is an
exceptionally high ratio, probably biased by lack of reporting of
mild cases and a liberal case definition of DHF. Every case with
petechiae or a low platelet count does not automatically qualify as
DHF. Information on the DHF case definition being used would be very
helpful. - Mod.JW]

In past weeks, the Mexican [Health] authorities sent hundreds of
their employees to eradicate mosquitoes in Puerto Vallarta, Cancun
and Acapulco -- the 3 principal tourist destinations -- with the
objective of stopping possible outbreaks during Easter Week, when the
beaches are jammed
. "We, the federal and state governments, are
working intensively in these 3 places, which we want to maintain
under control so that there will not be a risk [of dengue infection]
for the tourists," said the Mexican Chief of Epidemiology, Dr. Pablo
Kuri. The Canadian Embassy has issued an alert, after 5 of its
citizens contracted the disease in Puerto Vallarta at the beginning
of the year [2007].

However, the problem is more serious in the poor suburbs that circle
the tropical cities, where trash collection and sanitation are
inferior to those of the tourist areas. The Mexican Secretary of
Health says that dengue is one of the primary public health problems
of the country.

In January and February [2007], 1589 dengue cases were registered, an
increase of 389 percent compared to the same period of last year
[2006], said Kuri. In 2001, there were just 1781 confirmed [dengue]
cases in the entire country. In 2006, there were more than 27 000
cases, with a score of deaths.

The Intergovernmental Commission on Climate Change, which represents
the principal world specialists in the climate change area, predicted
in its March 2007 draft report that global warming and climate change
might cause a sudden increase in dengue incidence. This is what has
happened in Mexico with the prolonged rainy season.

ProMED-mail
<promed@promedmail.org>

[This is another illustration of the potential economic impact that
dengue could have, if tourists stay away from popular vacation
destinations. Tourism is a substantial generator of economic activity
in Mexico. As the International Commission points out, significant
changes in temperature and precipitation can be expected to alter
both distribution of dengue and the duration and intensity of outbreaks.

The depenedencia informed that the suffering presented/displayed by several people, diagnosed like a strange disease, is dengue classic and that there is confirmed 31 cases in the city of Leon.

Leon, Gto. The suffering that several people presented/displayed this week in the city, diagnosed in principle like a strange disease, is dengue classic, and it is not controlled, reason why it could propagate, indicated the Secretariat of state Health.

In press conference, the holder of the dependency, Jorge Armando Aguirre Torres, said that until the moment they have themselves confirmed 31 cases of ill people with dengue, in the colonies Duraznal and Obreg?n, who comprise of the District of Above, and in the Woods of Ibarrilla, to the north of the city.

?We knew that it was a virus, we had the name and we required of the last names, today already we have the last names and we want to inform to them, he is dengue and it is in the municipality of Leon?, limited.

It showed that at the moment the virus is not controlled, and the eradication of the mosquito that propagates it will depend on the work that is made between the society and the sector health.

For that reason, it said, ?we are working in a sanitary wall, nebulizando, to be able to finish with mosco and abatizando to eliminate the larvae?.

It clarified that, at a first moment, the presence of dengue refused because the laboratory tests had not confirmed it because the period of incubation of the virus takes between seven and 14 days.

The diagnosis was confirmed by the National Institute of Diagnosis and Reference Epidemiologist, who emitted the results of the samples that took with the 64 detected cases, of which, 31 infected people were only confirmed.

Warming Ignites Dengue in Mexico

Mexico, Dec 14 (Prensa Latina) Forty thousand dengue cases were reported in Mexico this year while Veracruz registered twelve thousand, informed Health secretary, Jose Angel Cordoba, who blamed this epidemic on the worldwide climate changes.

The majority of the cases, infected by the Aedes aegypti mosquito, were the classic strain while six persons died of the hemorrhagic variety.

Cordova insisted on the need to maintain fumigation campaigns throughout the country, mostly in the state of Tabasco under emergency conditions during floods last month and to prevent proliferation in that part of the country.

"Attention of dengue cases is a priority of the federal government as it is not exclusive of Mexico but of all Latin America as a result of migration and climate change," he insisted.

He pointed out that 26 million dollars were set aside in 2007 to confront the illness, a figure that could triple in 2008.

Six new cases of dengue in the state were confirmed by the secretary of Health, Raymundo Lopez Vucovich, which already added 23 so far this year.

"As I had anticipated, we have a dengue outbreak in Navojoa, and we are adding six more to the 17 already reported making a total of 23 cases confirmed to date in this disease," he said.

The official said that in the week were added to the account three of Navojoa, two in Benito Ju?rez and one more in Cajeme.

In the process of analysis are 49 samples, so that next week could increase the number of dengue cases in Sonora.

He also stressed that all cases of classic dengue have been 23 people affected and are in good health or restoring the condition.

Latest update from ProMED mail on Dengue Fever in Mexico:

Mexico (Jalisco)

Date: Tue 9 Sep 2008

Source: Radiorama [in Spanish, trans. & summ. Mod.TY, edited]

For the past 2 years, dengue outbreak foci have been in the coastal
municipalities of Jalisco. Today, there is a [dengue] alert in
Metropolitan Guadalajara, where there are the greatest concentration
of cases.
This year [2008], Zapopan is the municipality [in
Metropolitan Guadalajara] in Jalisco with the greatest concentration
of reported cases, with 47 of 86 reported cases.

Norma Jacobo Hernandez, Chief of the Department of Vectors and
Zoonoses of the state Secretariat of Health (SSJ) recognized that, in
concentrating their efforts in coastal communities, preventive
actions in Metropolitan Guadalajara have been relaxed.

"In coastal areas, in contrast to Zapopan, there have been frequent
and continuous efforts to eliminate breeding sites (of the mosquito
vector of the diseases [virus]), within houses, and we have not
carried out this activity with such frequency in Metropolitan
Guadalajara," said Jacobo Hernandez.

This official indicated that the factor which most favors the
presence of the mosquito vector of dengue disease [virus] is the
presence in homes of containers where clean water is stored, an ideal
site for mosquito breeding.

Up until now, the outbreak has occurred in poorer neighborhoods of
Zapopan. However, Jacobo Hernandez stated that this disease is no
respecter of social status and can occur in the most exclusive parts
of the city because it is not related to poverty or marginalization.

Up until now, the actions and operations have not been sufficient to
eliminate all the [mosquito] breeding sites. For this reason,
community participation is necessary so that every person does their
part in the elimination of breeding sites in their homes.

"The idea that simply applying insecticides can control [dengue]
outbreaks is false, but it is simply not that way, it requires the
participation of the community, because I could apply insecticides
that would give me results for a week, but if people don't eliminate
breeding sites, we will have problems with dengue again a week
later," she emphasized. The mosquitoes have great reproductive capacity.

The facts that vector mosquitoes are found in Zapopan increases the
possibility that the outbreak might extend to other municipalities in
the Metropolitan area. The mosquito can fly up to 200 meters, but the
possibility exists that they can travel greater distances, for
example when people transport them in their cars to other places by accident.

[There is perhaps a greater possibility that there are significant
_Aedes aegypti_ populations in other municipalities, and an infected,
viremic person could introduce dengue virus into these areas and
extend the outbreak. The collective municipalities within
Metropolitan Guadalajara essentially present one large populated
area, and there would be no barriers to the spread of vectors or
virus within it, with the exception of those buildings that are air
conditioned, where dengue virus-infected mosquitoes would have
difficulty entering. - Mod.TY]

Hermosillo, Sonora .- At the risk of living a dengue epidemic is located south of Sonora where he was an outbreak of 52 cases of dengue fever as a result of flooding that left the tropical storm "Lowell", reported today the Secretary for Health, Raymundo Lopez Vucovich.

At a press conference, Lopez Vucovich said that the most serious case is in Navojoa which recorded 45 of the 52 cases detected in the south, and where "Lowell" left 19 colonies flooded and at least 10 thousand homeless.

Two other cases were recorded in Benito Juárez, two more in Cajeme, two in Huatabampo, and one in Santa Ana, this town located 200 kilometers north of Hermosillo.

These 52 cases are in addition to the 23 who had confirmed until Sept. 19, total of 75 cases so far this year.

The Secretary of Health stressed that the south of Sonora is in epidemiological alert and made a strong call for the Sonoran families to participate in the campaign "Everyone Against Dengue" and the strategy "Patio Clean," to prevent more cases.

Lopez Vucovich noted that there has been little involvement of families in the municipalities of Navojoa, Etchojoa Huatabampo and the actions of "Patio Clean" of the campaign "Everyone Against Dengue."

He warned that continuing weak society cooperation in this region, it runs the risk of living another epidemic similar to the one that occurred in 2003, when 500 thousand were registered laboratory-confirmed cases in the state.

He also assured that the patients were located in a timely manner through the work of intensive house-to-house search conducted by the brigades of the Ministry of Public Health (SSP) in the areas affected by tropical storm where people had not gone to consultation medical, presenting a picture of the infection no longer than 72 hours.

The areas considered at greatest risk of transmission are in the town of Navojoa, with 24 cases in the Colonia Francisco Villa; at the Jacaranda, seven; in Sports, three; in Sonora, five; in Tierra y Libertad, five more; and Rosario Ruelas in a case.
