A 9-year-old Indonesian boy who had tested positive for bird flu died on Monday, the health ministry said in a statement, taking the country's death toll from the deadly disease to 99.

The boy from the outskirts of Jakarta died at the Sulianto Saroso hospital on Sunday after being treated in different hospitals for two weeks, said Joko Suyono, an official at the ministry's bird flu information centre.

A 31-year-old woman and 32-year-old man hospitalised at Persahabatan hospital for fever and respiratory problems also tested positive for the deadly H5N1 virus on Monday, the ministry said.

According to the statement, the woman lived in East Jakarta near a poultry slaughterhouse that kept many fowl believed to be the source of her H5N1 infection.

The man from Tangerang, west of Jakarta, is believed to have contracted H5N1 from his neighbour's pet doves, the ministry said.

Authorities are also expecting genetic test results to come back for a fourth case, a 23-year-old housewife from East Jakarta, who died on Sunday from bird flu symptoms.

The above comments described four confirmed or suspect H5N1 patients in the Jakarta area. Two of the four have died within 24 hours of each other.

The clustering of cases in the Jakarta area has been seen previouslyludin the first confirmed cases in Indonesia, but this is among the highest number of cases announced at the same time.

Since the cases are at four distinct locations, and do not appear to be linked, the clustering reflects H5N1 in the Jakarta area that is more efficiently transmitted to humans.

Release of sequences from these patients would be useful.

A 23-year-old Indonesian woman from East Jakarta has died from bird flu, taking the country's death toll to 100, according to a report from Indonesia's bird flu information centre on Monday.

The above comments indicate the suspect H5N1 described earlier, was confirmed. Thus, four confirmed cases in the Jakarta area have been announced today. Two of the four have died.

The most recent confirmed case, like the 97th fatal case, lived near a slaughter house. The high cases fatality rate in Indonesia raises concerns that the number of infections is markedly higher than the number of confirmed cases.

Since there have now been two confirmed fatal cases who lived near slaughter houses in the Jakarta area, screening of workers for H5N1 antibodies may be a useful start for a more accurate determination of the incidence of H5N1 in people in the Jakarta area.

Testing of H5N1 in Indonesia has been suspect. Many patients are not tested until they are transferred to an infectious disease hospital, allowing many mild cases to be missed. In addition, application of a Tamiflu blanket may keep H5N1 levels below detection in mild cases.

More testing of suspect H5N1 in Indonesia would be useful.

Indonesia Reports 100 Bird Flu Fatalities

Jakarta, January 29, 2008 - Ministry of Health - Indonesia has recorded four new confirmed cases of human avian.Nas from Tangerang District, MIY from Depok Municipality, Res and Vir from East Jakarta are confirmed to be infected by H5N1 virus based on the test results from the Health Research and Development Center of the Ministry of Health and Eijkman Institute dated January 27 2008.

Since it was found in mid july 2005, the total number of confirmed bird flu cases in human up to 28 January 2008 has reached 124 cases with 100 fatalities. The Case Fatality Rate (CFR) stands at 80.6 %.

Nas is a 32 year old chicken rice seller from Tangerang District, Banten Province. He experienced onset of illness on 17 January 2008 with symptoms of headache, stomach pain, breathing difficulties and fever. He was the admitted to Bakti Asih Hospital Tangerang on 24 January 2008 when two days later, 26 January 2008, he was then reffered to Persahabatan Hospital in Jakarta. Nas is still being treated in the ICU at Persahabatan Hospital in Jakarta. Nas neighbour who lives 500 meter from his house has 12 pigeons.


MIY, a 9.5 year old boy from Depok Municipality, West Java Province experienced onset of illness on 16 January 2008 with symptoms of bad headache, bad breathing difficulties, high fever up to 37.8 Celcius. He was examined at Sentra Medika Hospital in Cimanggis on 23 January 2008, where then he was reffered to Sulianti Saroso Infectious Disease Hospital (RSPI-SS) in Jakarta on 27 January 2008. MIY died on January 27 2008 at 13.55 at Sulianti Saroso Infectious Disease Hospital in Jakarta. The risk factors still being investigated.

Res, a 31 year old female from East Jakarta experienced onset of illness on 18 January 2008. She was then admitted to Harum Hospital on 22 January 2008 with the complain of coughing. Four days later, 26 January 2008, she was then reffered to Persahabatan Hospital in Jakarta with symptoms of fever, coughing, and headache. RF is still under treatment with a ventilator at Persahabatan Hospital, Jakarta. Based on the investigation conducted, the risk factors are: 100 meter from Res house there is a backyard chicken farm, where 500 meter from the house there is a chicken slaughter house. Beside that, Res visited a poultry market on 15 January 2008.

Vir, a 23 year old female from East Jakarta experienced onset of illness on 19 January 2008 with symptoms of fever and breathing difficulties. She was admitted to Ananda Hospital in Bekasi on 24 January 2008, where two days later, 26 January 2008, she was reffered to Sulianti Saroso Infectious Desease Hospital (RSPI-SS) in Jakarta. Vir died on 27 January 2008 at 05.15 in RSPI-SS Jakarta. The risk factors are still being investigated.

Indonesia's 100th bird flu death shows disease out of control: experts

JAKARTA - CONFIRMATION of the 100th human bird flu death in Indonesia shows the virus is out of control in the country most ravaged by outbreaks of the disease, experts said on Tuesday.

Indonesia's health ministry on Monday confirmed the death of Virda Sari, a 23-year-old woman from eastern Jakarta who died in hospital early Sunday morning.

Her death brought Indonesia's death toll to the symbolic milestone, highlighting the deadly nature of a virus that has infected a confirmed 124 people in the country, according to the ministry.

More than half of all bird flu deaths worldwide since 2005 have occurred in Indonesia, World Health Organisation figures show.

More than twice as many Indonesians have died of the disease than in Vietnam, which with 48 deaths is the second most affected country.

'The virus is uncontrollable in Indonesia ... it means that viral contamination of the environment is quite high,' said Ngurah Mahardika, a virologist at Udayana University on the resort island of Bali, which recorded its first two human deaths from the disease last year.

'The reason is that the virus is not under control in animals right now,' he said.

The H5N1 virus is mostly spread to humans through contact with infected poultry, but scientists fear it could mutate into a form easily transmittable between humans.

The resulting pandemic could kill millions.

Avian influenza has been particularly prevalent in areas surrounding Jakarta, Mahardika said, with the satellite city of Tangerang in neighbouring Banten province of particular concern with its large population living close to poultry.

Tangerang has seen eight confirmed human infections since October last year, all of them fatal.

A quarter of Indonesia's bird flu deaths have occurred in Jakarta, with another 45 percent striking in Banten and the nearby province of West Java, according to health ministry figures.

Both provinces abut the massive capital and receive the overflow of its urban sprawl.

A lack of coordination between government agencies means authorities are not sharing disease samples, raising the possibility of missing detecting strains of the virus that have adapted to humans, Mr Mahardika said.

'The people working in this field never come to the table to talk about it and analyse it together,' he said.

Devolution of power over the last 10 years since the fall of authoritarian president Suharto has also hindered coordination efforts, Australian National University Indonesia expert James Fox said.

'You have an area that will try to contain the bird flu epidemic, but there's no guarantee the neighbouring area will do the same. So (bird flu) will always just come back,' Mr Fox said.


Vaccinate or exterminate

Unlike other Asian nations which have managed to contain the bird flu virus, Indonesia's gross oversight was to not begin exterminating poultry three or four years ago when the disease first began spreading, he said.

'Most of the other nations decided they would not vaccinate, they would exterminate. The problem is on a veterinary level, (Indonesian authorities) were in denial,' Mr Fox said.

'(Exterminating poultry) is not possible anymore, you can only do that when there's a small outbreak, when it first begins. It is now endemic,' Mr Fox said.

Muchtar Ihsan, the doctor at the head of the avian influenza team at Jakarta's Persahabatan hospital - one of two bird flu referral hospitals in the capital - said Indonesia's long practice of families living close to their poultry has proved hard to break.

The Ministry of Health in Indonesia has confirmed an additional four cases of human infection with the H5N1 avian influenza virus. Two of these cases were fatal. There is no evidence of an epidemiological link between the cases.

The first case, a 31-year-old female from East Jakarta, Jakarta Province, developed symptoms on 18 January, was hospitalized on 22 January and is currently in hospital. The investigation indicated that she visited a wet market where live poultry are sold three days prior to symptom onset.

The second case, a 9-year-old male from Depok Municipality, West Java, developed symptoms on 16 January, was hospitalized on 23 January and died on 27 January. Investigations into the source of his infection indicate that the case lived next door to a wet market where live poultry are sold.

The third case, a 32-year-old male from Tangerang Municipality, Banten Province, developed symptoms on 17 January, was hospitalized on 24 January and is currently in hospital. Investigations into the source of his infection are ongoing.

The fourth case, a 23-year-old female from East Jakarta, Jakarta Province, developed symptoms on 19 January, was hospitalized on 24 January and died on 27 January. Investigations into the source of her infection are ongoing.

Of the 124 cases confirmed to date in Indonesia, 100 have been fatal.

JAKARTA : A 32-year-old Indonesian man has died of bird flu, the health ministry said Wednesday, bringing the death toll to 101 in the nation worst hit by the deadly virus.

The man, who died Tuesday, was from the Jakarta satellite district of Tangerang, the ministry's bird flu information centre said in a statement.

He was the seventh confirmed death from bird flu this year.

The victim was said to have first shown symptoms similar to bird flu on January 17, but he was only taken to a village clinic on January 21 before being referred to a hospital in the district town of Tangerang on January 24.

The man was then moved to a bird flu referral hospital in Jakarta on January 26, suffering from fever, breathing difficulties, low blood cell count and pneumonia, the health ministry's bird flu centre has said.

The centre said it was not known whether the victim had contact with infected birds but that several of his neighbours were keeping pigeons.

Humans are typically infected with bird flu by coming into direct contact with infected poultry, but experts fear the H5N1 virus may mutate into a form easily transmissible between humans, sparking a deadly global pandemic.

The concern stems from past influenza pandemics. A pandemic in 1918, just after the end of World War I, killed 20 million people worldwide.

Indonesian woman tested positive with bird flu

JAKARTA, Jan. 30 (Xinhua) -- A 32-year-old woman has been tested positive of having bird flu and is now in critical condition at a Jakarta hospital, local press said Wednesday.

The woman was admitted to the Persahabatan Hospital on Jan. 26 and laboratory tests confirmed later she had the H5N1 virus, the Metro TV reported.

"She is very critical now with pneumonia and respiratory problems," Dr. Mukhtar Ikhsan with the hospital told the television.

The patient is put on ventilator to assist her breathing. She was the country's 125th confirmed bird flu case, which already killed 101 people since the disease began ravaging in 2003.

Experts have blamed widespread bird flu attacks across the huge archipelago on million of backyard chicken commonly found in rural families.
