      In dengue shock syndrome, an acute increase in capillary permeability results in leakage of plasma into the interstitial space. Pleural effusion is commonly seen in dengue shock syndrome. We report three cases of dengue-associated adult respiratory distress syndrome (ARDS) in children, in all of whom dengue haemorrhagic fever, presenting with grade 3 or grade 4 dengue shock syndrome with disseminated intravascular coagulopathy, was confirmed. The criteria for the diagnosis of ARDS were based on the expanded definition of ARDS by Murray et al. Treatment consisted of fluid resuscitation, correction of coagulopathy and mechanical ventilation. All three children had multi-organ impairment, but it was more severe in the two who died. The one survivor was well at discharge.

        Thirteen people in Indonesia suspected of having bird flu have tested
        negative for the feared disease, the country's health ministry said
        Saturday [9 Aug 2008]. Experts from the World Health Organisation
        (WHO) arrived Friday in the affected village in North Sumatra to help
        investigate a possible outbreak after 3 people died, and the 13 were
        admitted to hospital. "All specimens collected from suspect cases
        have given negative results. They are all recovered," I Nyoman
        Kandun, director general of the ministry's communicable diseases
        department said on a text message.

        Officials and residents in Asahan district in North Sumatra province
        said villagers began showing symptoms of avian flu after a large
        number of chickens died suddenly last week in Air Batu village. The
        local husbandry office took preventive action this week by
        slaughtering and burning some 400 chickens and ducks. The ministry,
        which has stopped giving regular bird flu updates, announced earlier
        this week that the human toll from avian influenza in Indonesia had
        risen to 112 following the recent death of a 19-year-old man.

        The deadly H5N1 strain of bird flu has killed more than 240 people
        worldwide since late 2003. The virus typically spreads from bird to
        human through direct contact, but experts fear it could mutate into a
        form easily transmissible between humans, with the potential to kill
        millions in a pandemic.

        [Contrary to a statement by local health officials on Wed 6 Aug 2008
        (see ProMED-mail reference below), the suspected human cases of avian
        H5N1 influenza in North Sumatra have not been confirmed by the
        Indonesian Ministry of Health. The World Health Organisation
        cumulative table of human cases still lists the number of confirmed
        avian influenza fatalities in Indonesia as 110, rather than the 112
        fatalities recorded by the Indonesian Ministry of Health.

        Until further notice, ProMED-mail will not report suspected human
        cases of avian influenza in Indonesia until they have been confirmed
        by the Indonesian Ministry of Health.

                              SINGAPORE (Kyodo) -- Japan reaffirmed with the 10-member ASEAN Wednesday their aim for a comprehensive free trade agreement to take effect by the end of this year, and assured that preparations are underway to appoint an ambassador to ASEAN once the group's landmark charter is ratified.
                              At ministerial talks in Singapore, Japanese Foreign Minister Masahiko Komura also told his counterparts that Japan will provide an additional stockpile of antiviral drug Tamiflu that could treat 500,000 people to be distributed to members of the Association of Southeast Asian Nations, a Japanese Foreign Ministry official said.
                              This will double Japan's total contribution of Tamiflu to ASEAN to help prevent a pandemic in the region that is vulnerable to deadly strains of bird flu. The earlier stockpile of the drug is stored in a warehouse in Singapore.
                              "Japan would like to work with ASEAN for the FTA to take effect as early as possible by the end of this year," Komura was quoted as saying at the meeting, which was part of a series of annual talks hosted by ASEAN with its key dialogue partners this week.
                              The ASEAN-Japan Comprehensive Economic Partnership Agreement, Japan's first FTA with a regional bloc, was endorsed by the Japanese parliament in June. It will come into force on the first day of the second month after Japan and at least one of the 10 ASEAN members complete domestic procedures to ratify it.
                              Foreign ministers of Cambodia, Indonesia, Laos, Malaysia and the Philippines took part in the meeting with Komura, while ministers of the other five ASEAN countries attended similar talks with Australia held concurrently on Wednesday morning.
                              Japan has been eager to catch up with China and South Korea amid an intensifying race for regional FTAs. The accord with ASEAN is not only an FTA for goods but also covers services and investment.
                              It will enable Japanese companies that are expanding their markets in Southeast Asia to reduce trade costs. For example, tariffs on Japanese exports from one ASEAN country to another will be removed.
                              In particular, electronics and automobile manufacturers will also benefit as parts from various countries in the region can be assembled without tariffs being levied.
                              Japan will repeal tariffs on 93 percent of imports from ASEAN by value within 10 years of the deal taking effect. Meanwhile, six major ASEAN members -- Brunei, Indonesia, Malaysia, the Philippines, Singapore and Thailand -- will eliminate tariffs on 90 percent of imports from Japan such as automobiles within 10 years.
                              A more gradual tariff elimination table has been set for the remaining four ASEAN members with smaller economies -- Cambodia, Myanmar, Laos and Vietnam.
