    Status Report of Avian Influenza in Assam, India

    The Department of Animal Husbandry, Dairying and Fisheries in the Agriculture Ministry has confirmed further outbreaks of avian influenza in Block Sidli Chirang (District Chirang) and Bezera, Guwahati Municipality (District Kamrup) and Dibrugarh Municipality (District Dibrugarh).

    This is in addition to earlier outbreaks in Block Gobardhana, Barpeta District and Block West Nalbari, Nalbari District (on 4.12.08) in Rani and Rampur block of Kamrup district (01.12.2008) and Hajo block of Kamrup district (27.11.08).

    The Ministry of Health and Family Welfare, India, has strengthened isolation facilities to admit and treat suspect cases.

    15,000 capsules of Tamiflu, 9,000 surgical masks, 900 personal protective equipments, 300 N-95 masks and five ventilators have been supplied by MoHFW.

    Another 5,000 capsules of Tamiflu has reached the affected area on 14.12.2008.

    There is no suspect case of human avian influenza.

      Malda, Dec. 16: District officials in Malda are worried about a family of six and two couples suffering from fever for the past week at Satgharia village.

      The family owned 12 chickens. All died days before bird flu was confirmed in this pocket of the Narhatta gram panchayat area.

      Labourer Kurban Ali, his wife, mother and three children have been taking a quack?s medicines.

      Ali said the high temperature they have been running in the evenings has made them weak and they have no appetite. About the birds, he said: ?When they died, we threw them into the river.?

      Four others ? Ilam Mistry and Nazrul Islam, and their wives ? are also running a high temperature.

      The couples, too, are being treated by quacks.

      A team of doctors visited the Satgharia and Budhia villages today and collected blood samples of those suffering from fever.

        Malda, Dec. 16: The district administration is worried about a family of six suffering from fever for the past one week in Satghoria, 12km from here. The family owned 12 chickens, which died just before bird flu was detected in the village located in Narhatta gram panchayat

        Four others in the same panchayat area are also suffering from high temperature.

        A day labourer, Kurban Ali, and his wife, mother and three children are depending on medicines prescribed by a quack. According to him, after dusk they start shivering with fever. The disease has made them weak and they have no appetite for food.

        About the birds, Kurban said: ?When they died we threw them into the river. It was at that time when the Prime Minister had come to Malda.? Manmohan Singh was supposed to visit the district on December 7 but Kurban did not know that the trip was cancelled at the last moment.

        Iliam Mistry and Nazrul Islam, along with their wives, from Budhia village are also visiting quacks.

        ?We cannot afford to bear the expenses to visit the district hospital for medical treatment. We are solely dependent on the village quack. But his medicines are not working,? Mistry said.

        A team of doctors visited the Satghoria and Budia villages today and collected the blood samples of those suffering from the unknown fever.

        ?The disease will be diagnosed only after we receive the blood report,? chief medical officer of health of the district Srikanta Roy said. He said his department would arrange for the treatment of the patients suffering from the unknown disease.

          ? Brief of containment operations

          - Culling activities has been completed in blocks of Hajo and Rani/ Azara in Kamrup district.
          - Culling activities are on in other epicenters. A total of 18,864 birds were culled [cumulative 3,56,446].
          - A total of 546 Animal Health workers were deployed for the culling activity. They are on Chemoprophylaxis.


          ? Surveillance

          - Central government has deployed RRTs in all epicenters. They are working with the state RRT teams.
          - Active house to house surveillance is being followed in 0-3 km zone in all epicenters and in 3-10 km in phases. A population of 1,77,538 was covered in 0-3 Km area and 1,83,270 in 3-10 km area.
          - 144 cases of Upper Respiratory Infection [URI] with fever were detected [85 in 0-3 Km and 59 in 3-10 Km]. None of them had history of handling dead or sick birds.
          - 126 cases of URI with fever in the outpatient in the identified health facilities.
          - 1147 health workers were deployed for surveillance.
          Available details of epicenter-wise surveillance is at annexure-I.


          ? Logistics

          - 30,000 capsules of Tamiflu, 9,000 surgical masks, 900 personal protective equipments, 300 N-95 masks and 8 ventilators have been supplied by MoHFW. Sufficient anti-viral drugs and PPE are in stock with the State Government.


          ? Communication

          - Communication materials in Assamese (both print and media) has been provided to the state Government through the UNICEF.

            CULLING BIRDS in districts of Assam and Meghalaya is continuing.

            Health authorities in Assam and Meghalaya have sounded an alert as about 150 people are reported to be suffering from fever and upper respiratory infections, fuelling fears that the poisonous virus has started affecting humans too.
            Segregation norms have been strengthened though no cases of influenza resembling sickness through contact with affected rooster has been detected so far, Parthajyoti Gogoi regional director (North-East) of the Central Health and Family Welfare department, said.

            About 150 people were treated for fever and upper respiratory infections in bird flu areas. We have put the patients in isolation wards, even if the symptoms do not point to influenza, he added.
            The bird flu virus has spread across six Assam districts with authorities claiming it has unspecified pandemic percentage. More than 2,50,000 birds were culled in the past two weeks in Assam and estimated 1,50,000 more have been ordered to be killed. The districts hit by bird flu are Kamrup (metro), Dibrugarh, Nalbari, Barpeta and Chirang.

            ?We are concerned over the bird flu virus thinning out to humans as the strains travel on the air speedily,? said Assam health minister Himanta Biswa Sarma. ?We want the people of Assam to lend a hand with the veterinary department so that the culling operations are carried out appropriately. If not we might countenance a catastrophe,? he added.

            To put off multiplying of the H5NI virus from bird flu struck Assam, the state veterinary department started culling operations late evening on December 14, 2008 at Pilangkata, Maikailu, Upper Kholi and Patarkuchi falling within the three-kilometre radius of Khanapara farm in Assam where cases of bird flu have been detected.

            State animal husbandry and veterinary department (AH&VD) director Dr D Khonglah, who is managing the crisis said about 3,000 household chickens would be culled in about four villages in Meghalaya?s portion of Khanapara. Over 70 medical and veterinary staff is serving in culling operations. Section 144 of the CrPC has been imposed in the entire Ri Bhoi district to prevent any outburst in the state.

            The veterinary department has collected blood samples from birds and poultry from Assam-Meghalaya border areas and has sent them for test to Bhopal.

            Besides Assam and Meghalaya, bird flu has also returned to West Bengal after an outbreak in 14 districts of the states in January this year.

            Authorities in West Bengal say they have identified the deadly H5N1 strain of bird flu in thousands of dead chickens. Tests on poultry from two villages around English Bazaar town in the Maldah district have returned positive results, a health official said.

            Laboratory tests have confirmed that the birds in West Bengal died of the deadly infection, secretary of animal resources development department, Dilip Chakraborty said in Kolkata.

            Culling would begin on Wednesday, the government said. The virus spread to 14 of the 19 districts in West Bengal, which has a population of more than 80 million. However, no human case was reported.

            There are many types of bird or avian flu. The most contagious strains, which are usually fatal in birds, are H5 and H7.

            There are nine types of H5. They take different forms ? some are highly pathogenic, while some are pretty harmless. The type currently causing concern is the highly pathogenic Asian strain of the H5N1 virus. The H5N1 strain of bird flu is regarded as highly pathogenic and can also cause disease and death in humans. Scientists have discovered four different sub types of H5N1, and there could well be more. However, all are deadly for birds and can cause disease and death in humans too.

            Bird flu was thought only to infect birds until the first human cases were seen in Hong Kong in 1997. Humans catch the disease through close contact with live infected birds. Birds excrete the virus in their faeces, which dry and become pulverised and are then inhaled.

            Symptoms are similar to other types of flu-fever, sore throats and coughs. People can also develop conjunctivitis.

            There may have been examples of human to human transmission, but so far not in the form which could fuel a pandemic.

            A case in Thailand indicated the probable transmission of the virus from girl who had the disease, to her mother, who also died. The girl?s aunt, who was also infected, survived the virus.

            Though there is no fail safe way of preventing its spread, yet we can minimise its danger up to minimum level. Experts say that proper poultry controls ? such as preventing wild birds getting into poultry houses are vital.
            In addition, they say monitoring of the migratory patterns of wild birds should provide early alerts of the arrivals of infected flocks -- meaning they could be targeted on arrival.

            There is not yet a definitive vaccine, but prototype, which offers protection against the H5N1 strain are being produced. But antiviral drugs as Tamiflu which are already available and being stockpiled by countries such as UK, may help limit symptoms and curbing the disease from spreading.

            However, the sudden death of thousands of chickens over the course of a few days has been cause for panic in Haryana?s Bhiwani district, as poultry farmers fear that the highly-pathogenic H5N1 virus was to blame for the death.

            Moreover, there is also possibility of spread of this virus all over India. Well, it is high time for all concerned authorities to coordinate among themselves and try to check this epidemic as early as possible.

                    /B]Malda (WB): Fresh reports of death of poultry and crows were reported today from three areas of Malda district, where the N5N1 disease has returned after 11 months, as 22 culling teams targetted to kill 16,000 birds in three days.

                    The fresh bird deaths were reported from Baharlal in Ratua Block 1, Chandmoni 1 and 2 blocks, where blood samples were collected by Animal Resources Development personnel for for tests, District Magistrate Sreedhar Ghosh said.[/B]

                    The number of culling teams has been raised to 22 from 18 and 6000 of 16,000 birds were eliminated by evening, in two gram panchayat areas of English Bazar block, he said.

                    The teams would continue to work at night, he said.

                    The chief medical officer has been asked to inquire, about a family of six and two couples suffering from fever for the past week in Narhatta gram panchayat area after death of chickens in their households, he said.

                    Compensation for culling poultry has been raised following resistance from villagers, Animal Resource Development minister Anisur Rahaman said in Baharampore in Murshidabad district.

                    The minister said compensation for a full-grown chicken has been raised to Rs 50 from Rs 35, Rs 75 for ducks from Rs 40 and for ducklings Rs 25 from Rs 10.

                    Poultry owners would also receive compensation for eggs and chicks destroyed, the DM said.

                    He said Rs 40 lakh has been sanctioned as compensation for poultry farmers.

                    The borders of the district with Bangladesh and Assam, have been sealed to stop entry of poultry and poultry products.

                    A state-wide alert had been sounded yesterday.

                            NEW DELHI: With the deadly bird flu hitting urban areas of the country, for the first time a visibly worried Union health ministry has stepped up
                            surveillance on human beings in the affected districts.

                            Officials told TOI that about 17 lakh people in Assam alone have already been checked for symptoms of Avian Influenza
                            . The ministry plans to check 95% of the population living within a 0-3 km radius of the outbreak. At least 1,149 health workers were deployed for the surveillance.

                            Around 30% of the population in the 3-10 km radius is also being checked for symptoms like cough and cold, history of poultry handling and lower respiratory infection. At least 143 cases of Upper Respiratory Infection (URI) with fever have already been detected (78 in 0-3 km radius and 65 in 3-10 km radius). None of them, however, had any history of handling dead or sick birds.

                            Seven districts in Assam -- Kamrup Rural and Metro, Nalbari, Barpeta, Darrang, Dibrugarh and Bongaigaon -- have been affected by the virus.

                            On Thursday, blood samples of crows found dead in the Chatribari area in Guwahati were confirmed as H5N1 positive by laboratories in Bhopal and Pune.


                            Over four lakh birds have been culled and Rs 1.11 crore paid as compensation to the affected poultry farmers. The Union health ministry has already rushed 30,000 tablets of Tamiflu to Assam.

                            Meanwhile, animal husbandry department notified a bird flu
                            outbreak in Englishbazar block of Malda in West Bengal on December 15. Active surveillance in a 0-3 km has started and the state government has been provided with ventilators, personal protective equipments and Tamilfu. An additional 5,000 capsules of Tamiflu have been kept on hold for West Bengal and will be sent soon.

                            Sources said 86 animal health workers who are under Tamilfu are conducting culling activities.

                            A health ministry official said: "We have never had a re-infection in India. The return of the virus in Malda has raised serious concern that the virus may have got embedded in the state. Scientists are trying to find whether West Bengal has become a reservoir or not."

                            The official added: "What's worse is that the outbreaks this time have occurred in urban settings which are also more risky, considering the population."


                            In Assam, health minister Himanta Biswa Sarma had earlier expressed his serious fears about strains of the deadly bird flu virus spreading to humans. "We are really worried about the bird flu virus spreading to humans as the strains transmit rapidly," he said.

                              - The active surveillance in 0-3 Km has started to cover a total population of 33,065 in 0-3 Km area.
                              - 94 health workers have been deployed for active surveillance.
                              - Isolation ward in Malda district hospital has been identified.

                              - The State Govt has been provided with ventilators, PPE and Oseltamivir during the last outbreak the stock of which is available with them.
                              - Additional 5000 capsules of Tamiflu has been kept at Regional Director office of MOHFW, Kolkata. Culling activity has started.
                              - 127 animal health workers are under chemoprophylaxis.


                              ? Assam

                              - Department of Animal Husbandry had notified avian influenza in Block Gobardhana, Barpeta District and Block West Nalbari, Nalbari District (on 4.12.08) in Rani and Rampur block of Kamrup district (01.12.2008) and Hajo block of Kamrup district (27.11.08).
                              - Outbreaks have also been notified on 10.12.2008 in Block Sidli Chirang [District Chirang] and Bezera, Guwahati Municipality [District Kamrup] and Dibrugarh Municipality [District Dibrugarh].

                              - Further outbreaks have been notified on 16.12.2008 in Bojali block [District Barpeta], Boitamari block [District Bongaigaon] and Guwahati Block/Municipality [District Kamrup (Metro)].
                              - Central RRTs already stationed in these districts would cover the containment operations.


                              ? Brief of containment operations

                              - Culling activities has been completed in blocks of Hajo and Rani/ Azara in Kamrup district.
                              - Culling activities are on in other epicenters.
                              - A total of 11,234 birds were culled [cumulative 3,80,169].
                              - A total of 564 Animal Health workers were deployed for the culling activity. They are on Chemoprophylaxis.


                              ? Surveillance

                              - Central government has deployed RRTs in all epicenters. They are working with the state RRT teams.
                              - Active house to house surveillance is being followed in 0-3 km zone in all epicenters and in 3-10 km in phases.
                              - A population of 1,94,876 was covered in 0-3 Km area and 2,26,975 in 3-10 km area.
                              - 130 cases of Upper Respiratory Infection [URI] with fever were detected [78 in 0-3 Km and 52 in 3-10 Km]. None of them had history of handling dead or sick birds.
                              - 104 cases of URI with fever in the outpatient in the identified health facilities.
                              - 1357 health workers were deployed for surveillance.
                              - Available details of epicenter-wise surveillance is at annexure-I.


                              ? Logistics

                              - 30,000 capsules of Tamiflu, 9,000 surgical masks, 900 personal protective equipments, 300 N-95 masks and 8 ventilators have been supplied by MoHFW.
                              - Sufficient anti-viral drugs and PPE are in stock with the State Government.


                              ? Communication

                              - Communication materials in Assamese (both print and media) has been provided to the state Government through the UNICEF.


                              ? Command and Control

                              - Nodal Officers have been identified for the epicenters and control room established at the State / District/ block level for both animal and human health sector.
                              - The situation is being monitored on a daily basis.
