It is unlikely that these birds had symptoms, and a survey of birds near Beijing failed to identify dead or dying poultry. However, a health care worker in Beijing did develop symptoms, raising concerns of increased efficiency of transmission to humans.
The Beijing case(s) were followed by the fatality in Shandong (27F), who developed symptoms on January 5. The possible exposure has not been disclosed.
These two cases were followed by the case (2F) in Hunan, who is in critical condition in Shanxi. Surveys of markets in Hunan and Shanxi failed to identify dead or dying poultry.
Thus, there has been no linkage of any of the three confirmed H5N1 to dead or dying poultry. However, vaccination of poultry with a poorly matched vaccine would yield asymptomatic birds shedding H5N1 virus, which would facilitate spread in immunized birds that could lead to fatal human infections.
More information on the H5N1 isolated from the three cases this month would be useful.
Clade 7 is readily distinguished from clade 1 or clade 2.1, 2.2, or 2.3, and the distinction can be seen in the HA cleavage site (clade 7 has a G at the third position instead of an R or S).

Avian influenza situation in China -- WHO Update 3

The Ministry of Health in China has announced 3 new confirmed human
cases of H5N1 infection. The 1st, a 31-year-old female from Urumqi,
Xinjiang Autonomous Region, had onset of symptoms on 10 Jan 2009. She
received treatment in hospital but died on 23 Jan 2009.
Investigations into the possible source of her infection indicate
recent visits to a live poultry market. The local authorities are
currently conducting epidemiological investigations and close
contacts are being monitored. To date, no clinical symptoms have been
reported among the contacts.
The 2nd case is a 29-year-old male from Guiyang city, Guizhou. He had
onset of symptoms on 15 Jan 2009 and remains in a critical condition.
Investigations into the source of his infection indicate possible
exposure at poultry market.
The 3rd case is an 18-year-old male from Beiliu City, Guangxi
Province. He had onset of symptoms on 19 Jan 2009 and died on 26 Jan
2009. Investigations into the source of his infection indicate a
recent history of exposure to sick and dead poultry. Close contacts
of the case are being monitored and to date all remain well.
Of the 37 cases confirmed to date in China, 25 have been fatal.

of infection. In virtually all recent human cases of avian A/H5N1
influenza virus infection in China there has been some form of
contact with poultry (although the disease status of the poultry is
rarely known. The pattern of disease continues to be that of sporadic
infection of a few (possibly genetically) susceptible individuals
with no onward transmission of virus to their contacts.

So....a lot has happened since the 22nd. Maybe they need to update their post?
But the main date on the article is the 27th.....yet the Shandong comment is from the 21st.....right?

of infection. In virtually all recent human cases of avian A/H5N1
influenza virus infection in China there has been some form of
contact with poultry (although the disease status of the poultry is
rarely known. The pattern of disease continues to be that of sporadic
infection of a few (possibly genetically) susceptible individuals
with no onward transmission of virus to their contacts.

The comments are made when the report was posted yesterday. The above is the most recent from ProMED and the CP comments have been similar since 2004. He doesn't think H5N1 is much of a problem for the general population. He maintains that only a small number of genetically susceptable individuals get infected by H5N1 positive poultry.
Last season the H2H case in Jiangsu (that was written up in NEJM), was said to be due to poultry consumed at a restaurant, and the son who died ate dark meat, while his father had white meat, which accounted for the gap in disease onset dates! (and the son also had a diminished immune response becase he was receiving rabies shots for a dog bite). I really am not making this up. CP also invented a sibling in a cluster in central Vietnam in 2005 to prove that the H2H cluster wasn't H2H because the imaginary sibling wasn't infected.
Although entertaining, the comments are not reality based, regardless of posting date.
