Re: Indonesia: Human Cases 8 Oct 2008 +

I received a note from Ida at Bird Flu Information Corner

that clears up the genders, and relationships, of the patients (and fatalities) in Bogor Indonesia.

I would like to confirm that Ruslani (30) previously reported as 'sister' is actually brother of Nurul, the first victim who died. In the other hand, Nurul is a female. Nurul and Ruslani had died due to infection, meanwhile now their brother Ridwan (35) is developing same symptoms as his siblings. I can't confirm anything about this, but just say they are strongly suspected to contract bird flu virus until the next update.

So Ruslani is a male. Nurul is a female. And Ridwan is a male. Two brothers and a sister.

The two fatalities in mid-January were therefore, brother and sister.

A big hat tip to Ida, and the Bird Flu Information Corner.

Re: Indonesia: Human Cases 8 Oct 2008 +

In mid January we have an index case for a potential cluster with the infection of Nurul Hikmah (27F) about Jan 14. She dies, as does her brother, Ruskani (30M), about Jan 16. A third sibling, Ridwan (35M), begins exhibiting symptoms on February 7. I believe all three of these individuals are from Cilendek Barat, an outlier of Bogor City.

Meanwhile, on February 3, Sumarni, a young child from Kedung Waringin in the Subdistrict Tanah Sareal, Bogor City, develops fever and possible bird flu symptoms. The map belows shows that Cilendek Barat and Kedungwaringin are only a couple of miles apart. The specific location of the RT/RW groups noted by Commonground above are uncertain, but are mostly likely located in this same general area.

This small area could potentially have 12-15 human H5N1 infections since mid January.

Treated as an outpatient, he is at home, not in hospital.

                        Bali resident bird flu suspect

VIVAnews - Did not yet end the handling of the case of rabies in Bali, bird flu currently threatens the Island of the Gods. One of the residents, Putu Prana Yoga (20) that beralamat in the Sibang Series, Desa Jagapati, Abiansemal, Badung was expected by bird flu.

The assumption headed to bird flu because Yoga had the story of direct contact with the poultry that died as a result of bird flu. According to the official surveillance from the Community Health Centre of Abiansemal Ii, Dewi Tresnaningsih Yoga has been handled.

Has been given by us tamiflu twice a day with the dose 75 milligram but also other medicines, said Dwi when being met in the Yoga house, on Tuesday February 10 2009.

Giving tamiflu, said Dwi, was given while seven days were endless. Each five hours continued to be carried out by observation, added Dwi. Until this the Yoga status was still being expected by bird flu.

According to Dwi, the sample of Yoga blood has been sent to the Penelitian Dan Pengembangan Kesehatan Body of the Department of the Health. Results just went out two weeks more, said Dwi. The temperature of the Yoga body through to 38 levels celcius was accompanied breathlessly.

However after being carried out by the checking, evidently his respiration was normal that is 16 times per the minute. Nevertheless, Yoga still in the supervision from the Community Health Centre.

Separately, the Secretary of the Handling of Rumah Sakit Sanglah Bird Flu, Dr Ken Wirasandi again warned in the service rank for his vigilance if finding the patient that experienced hot high and breathless to immediately asked whether having the story of poultry contact or not.

Hopefully after 1.5 expired years did not have again the case of bird flu in Bali, hoped he. The Bird Flu command post recorded, up until January 16 2009, from 12 provinces that were infected by bird flu to humankind, 6 provinces were not found again by the case of bird flu.

The province that has been free to be the Lampung Province (the infection on September 22 2005), the South Sulawesi Province (the infection on June 18 2006), the South Sumatra Province (the infection on March 10 2007), the East Java Province (the infection on March 19), the North Sumatran Province (the infection on May 3 2007), and the Balinese Province (the infection on August 14 2007).

Up to the middle of January 2009, according to the Department's data of the Health, the spread of bird flu was still coming from the poultry to humankind. Not from humankind to humankind. The proof is, the specimen inspection of all case contact showed the negative.

Chicken Cull in Bali After Avian Influenza Outbreak

On the heels of a rabies outbreak in Bali, officials on the tourist island are now wrestling with the shadow of avian influenza.

Made Badra, the head of Badung district?s animal husbandry, marine and fisheries agency, said on Tuesday that one person was being treated for suspected bird flu infection, while 133 chickens from the Jagapati Abiansemal village had been culled after one out of five of the sick birds, selected randomly from the group, tested positive for the H5N1 virus last week.

Indonesia remains the world?s most-affected country from the virus, with 113 human deaths from 139 cases as of Jan. 19, according to the World Health Organization. Indonesia?s failure to reign in the disease ? which is endemic among domestic birds in the vast majority of Indonesia?s 33 provinces ? has prompted critics to warn a global pandemic could originate from Indonesia.

?We conducted the culling [last] Thursday, Friday and [Tuesday] morning and eliminated 133 chickens,? he said, adding that local residents had voluntarily given up their birds to be destroyed.

Badra also said that a local resident, Putu Pramayoga, was experiencing bird-flu like symptoms after coming in contact with sick poultry. He was given antibiotics and antiviral drug Tamiflu and his condition was said to be improving.

?This morning I have checked. The fever and the cough have gone,? he said, adding that Putu?s tissue samples had been sent to Sanglah hospital in Denpasar to find out whether or not he was infected with bird flu.

?There is no rapid test for humans. Laboratory tests take time, so we cannot confirm whether or not Putu was infected by the H5N1 virus.?

Badra said that in an effort to curb the spread of the virus, the agency would conduct a drive to disinfect chicken farms and slaughter houses.

?We will also reactivate surveillance in nearby villages to monitor the transportation of chickens,? he said.

Badra said awareness of bird flu in Badung had improved after a small outbreak of the virus struck the district in 2007, killing two people.

The state-run Antara news agency reported on Tuesday that 11.5 percent of Lampung Province?s 2,330 villages, a total of 268 villages, had tested positive for bird flu among birds in January.

Sunaryo Kasman, section head of animal and veterinary health for Lampung?s husbandry and animal health agency was quoted as saying that the survey conducted by the agency in January 2009 indicated that the virus had spread to eleven subdistricts.

Sunaryo also said that in 2004, the H5N1 virus killed more than 1.8 million birds in Lampung, the highest rate in the province, while in 2008 the number of dead birds fell to 25,282.

The decreasing figure was the result of increasing awareness in the community and faster response times when bird flu cases were detected, he said.

Avian flu detected in cockfighting village

Andra Wisnu , The Jakarta Post , Denpasar | Wed, 02/11/2009 12:23 PM | The Archipelago

A local man suspected of having bird flu rested in his home in Jagapati village, Badung, a place well-known for cockfighting, while officials continued to cull fowls in the area.

Putu Pranayoga, 20, was resting in his bedroom when The Jakarta Post visited. Dwi Tresna Ningsih, a field doctor who has been treating and observing Putu, said he began showing symptoms of influenza Monday, just a few days after the report of a possible bird flu outbreak in the area.

"We've given him Tamiflu *the medicine required to treat bird flu suspects* and we'll continue to observe him every five hours," Tresna said Tuesday.

She said Putu seemed to be getting better but declined to rule out the possibility of bird flu, saying that she was still waiting for results of Putu's blood test, which was sent to Jakarta on Monday.

"We can't know for sure until we've received the results of his blood tests, which we will get in two weeks," she said.

It is the latest resurgence of the much-dreaded avian flu in Bali since the death of a 29-year-old woman, who allegedly died after being infected by the H5N1 virus on August 2007.

The virus was first detected on Wednesday last week, when a villager reported the sudden and nearly simultaneous death of 15 fowls in the area to the Badung Animal Husbandry and Fisheries Agency.

The dead fowl tested positive of bird flu.

The agency has since culled as many as 133 fowls in the district, with further culling to continue in the days to come until all the villages' estimated 180-fowl population has been eliminated.

"We have culled 133 fowls so far, including the 40 we aim to finish off today. We'll continue the elimination for as long as it takes," said I Made Badra, head of the Badung Animal Husbandry and Fisheries Agency, in Jagapati on Tuesday.

According to Badra, the resurgence of the virus was related to the wet season, which may have weakened a certain fowl's immune system enough to catch bird flu, quickly spreading to others.

Another possibility, he said, was the transportation of an infected chicken from Java to Jagapati to use for cockfighting.

The Bali government has banned unlicensed live animal transportation into Bali and cockfighting, but the latter remains a staple in the island due to the religious and traditional nature of the so-called sport.

On the other hand, the ban on animal transportation seems to have been fully implemented since the detection of rabies virus late last year. However, Badra said there was a good chance that infected fowls escaped detection because they might have been transported before the rabies scare.

"For now we'll continue to urge the public to not transport live animals into Bali," he said.

Authorities in Indonesia awaited the results of blood tests to determine if a resident of a village known for cockfighting has contracted avian flu.
The illness of Putu Pranayoga, 20, may signal the first human case of avian flu, commonly called bird flu, in Bali since a 29-year-old woman died of the disease in August of 2007, The Jakarta Post reported Wednesday.

The virus was first detected a week ago when a villager reported the nearly simultaneous death of 15 fowls that all ended up testing positive for avian flu.
We have culled 133 fowls so far, including the 40 we aim to finish off today,</q> said I Made Badra, head of the Badung Animal Husbandry and Fisheries Agency in Jagapati.
The Indonesian government has banned unlicensed live-animal transportation into Bali as well as cockfighting, but the sport remains a staple on the island.
