Its gaining efficiency. How close of contact do a 15, 20 and 25 year old have. This is not like a mother caring for a baby. Some of the other victims have not neccesarily had close contacts either. IE., the reporter and keep thinking of the little girl that died a while back. Usual explanation..she lived near where chickens died...they were 50 feet from her house. She did not have direct contact with them. AIRBORNE.

The man, 25, died on Sunday with bird flu symptoms after being treated at a Christian hospital in Bandung, the capital of West Java province, said Hadi Yusuf, head of the bird flu ward at the city's Hasan Sadikin hospital.

The dead man's brother, 20, and sister, 15, were being treated in Hasan Sadikin hospital and tests carried out by a government laboratory showed the surviving brother had bird flu, Yusuf said

These folks sometimes sleep in crowded quarters - may to a room, so contact between them is not out of the question.

If the little girl did not have contact with chickens, we cannot assume it is airborne. The feces can travel on shoes and people sometimes sleep on floors.

If you reread some posts regarding actual sequences, especially those about the chickens and humans have different strains of H5N1 (elusive unknown vectors) we can see that the chickens are not always the source.

The patient Confirm Bandung Bird Flu was Still being Critical
          The writer: Sugeng

Bandung -- MIOL: the Condition for Zakaria Divine Guidance, 20, the patient confirm bird flu that was treated in RS Handsome Sadikin, Bandung, till Wednesday still was critical. His pulse was still being not uniform, the temperature of the high body and the crowded breath.
"He must be still making use of respiratory aids."

His condition did not yet improve, so as we continued monitored in a tight manner, explained the Chairman Tim the Handling of RSHS Hadi Jusuf Bird Flu, on Wednesday.

Now his brother, the precious stone Diamond, 15, that began to be treated on Tuesday night (26/9), his condition has begun to be normal.

Although results of the laboratory inspection were not yet available, but the temperature of his body has descended and breathless has been far decreased.

Apparantly he's decided to rule out a human source because the younger sister(brother?) has seasonal flu, not H5N1, and the older brother died without having his blood tested.

Well, it's not like that's particularly good news (except, perhaps, for the girl). H5N1 and the regular flu in one household? Perfect opportunity for recombination, reassortment (take your pick).... Yikes!

Indonesia confirms bird flu in possible cluster

                Agence France-Presse
Last updated 01:18pm (Mla time) 09/27/2006

JAKARTA -- A 20-year-old Indonesian man has contracted bird flu, the country's health minister said Wednesday, after the death of his brother in a case that has raised fears of possible human transmission.

"On the matter of bird flu, another case is positive," Health Minister Siti Fadillah Supari said, referring to Taufik Zakaria, a patient being treated in hospital in the West Java city of Bandung.

Zakaria's 25-year-old brother died Sunday after showing bird flu symptoms, while the pair's 15-year-old sister has also been hospitalized complaining of similar symptoms, Fatimah Resmiati from the West Java health office said.

The 25-year-old was buried without samples being taken from his body.

Resmiati said Zakaria remained in a "critical condition" on Wednesday.

The official said on Tuesday the trio were being investigated as a possible cluster case but cautioned that further tests needed to be carried out.

Nadirin, a doctor from the national bird flu information centre, said that the family kept ducks and fighting cocks which had shown immunity to the H5N1 virus, meaning they had been infected at one point.

This, along with the fact the family fed dead chickens to their dog, raises the possibility they caught the virus directly from the poultry, rather than each other.

So-called cluster cases, where the H5N1 virus is spread from human to human rather than from poultry, increase the chance of the virus mutating slightly to become easily transmissible between people.

Such a development, scientists fear, could lead to a global pandemic with a potential death toll of millions.

Indonesia is grappling with ongoing outbreaks of the H5N1 virus in its poultry and has reported 51 human fatalities from avian influenza, the highest number anywhere in the world.

The World Health Organization estimates that there have been about half a dozen instances of human-to-human transmission, mainly involving relatives or close carriers who were infected by someone who caught the virus from poultry.

Re: INDONESIA - Bandung Cluster, West Java

Obviously there is some controversy around this subject but suggesting it is airborne is based on my own belief personal research and experience. The poultry transmit the h5n1 via eye mouth and nasal secretions and the virus particles are airborne. This is why not even the n95 mask offers complete protection from the disease.

Am aware of the research regarding close contact tranmission and there are other studies suggesting it is airnborbe. There is not enough scientific information in this area to conclude one way or the other. Again looking at the cases that defy explanation the airborne theory is completely plausible.

The brothers often bought dead chickens to feed their dog, but the girl had no known history of contact with chickens, the doctor said.

Ok hopefully they are going to include the DOGS in testing also.

Avian influenza ? situation in Indonesia ? update 33

                            27 September 2006

The Ministry of Health in Indonesia has confirmed the country?s 68th case of human infection with the H5N1 avian influenza virus. The patient is a 20-year-old male from Bandung, West Java. He developed symptoms of fever and cough on 17 September and was hospitalized on 24 September. He remains hospitalized.

The man?s 23-year-old brother developed symptoms on 16 September. He died of respiratory disease on 24 September, two hours after admission to hospital. Infection with the H5N1 virus is suspected for the 23-year-old male, but cannot be confirmed as no samples were collected for testing. Both men had direct contact with dead chickens when feeding carcasses to their dogs. Local agricultural authorities also found evidence of H5 infection in household birds.

A third sibling, a 15-year-old female, was hospitalized on 25 September after developing symptoms of fever and cough. Initial test results received on 27 September were negative for the H5 virus subtype and positive for the H1 subtype, indicating an infection with normal seasonal influenza. She remains hospitalized in a stable condition.

Of the 68 cases confirmed to date in Indonesia, 51 have been fatal.
