      I would only highlight this point: what is the likelihood that a traveler from the US - currently experiencing a severe winter seasonal A/H3N2 sustained influenza epidemic, this virus accounting for 90%+ of type A strain isolations - to remain infected with an H1N1pdm09 and then causing a out-of-season nosocomial outbreak in Argentina?

      Is it possible that another pathogen could be involved in an hyperacute outbreak among health care providers?

        Another question (if this is just H1N1) is why would pH1N1, after a very large circulation over the last four years and inevitable multiple exposures for healthcare workers (who very likely have had vaccines in the past) suddenly cause such a high level of severe disease and/or virulence?

          Be careful, because the public authorities tend to minimize the seriousness of this type of event and to allocate to external causes rather than point on internal weaknesses.

                  Gov't denies Swine Flu outbreak, suggests flu shot before visiting US
                  After four people were diagnosed with Swine Flu in a Carmen de Areco hospital, the head of the BA province Health Ministry?s Primary Care office, Luis Crovetto, suggested that people planning on visiting the US should get a flu shot before traveling.

                  He also rejected rumours saying there was a Swine Flu outbreak in the country.

                  Regarding the patients currently being treated, he stated that ?one of them recently visited the US, and the Malbr?n Institute is treating him to determine which virus strain they are dealing with.?

                  The Government official clarified that ?neither the country nor the province are dealing with a Swine Flu outbreak? and said the population should not be worried.

                  He concluded by assuring that ?every infected patient was treated quickly? and that no other cases were registered.

                    This is apparently a quote from an official source. If the traveler is a nurse, it seems unlikely to be the index case. Might there be some co-infection going on that worsened the illness of the two nurses?

                          Which flu A strain do these patients have? Yes, there is flu in Florida. This is not new. It is flu season here. Here is the Florida forum on FluTrackers.

                          From the latest state report - week 4:

                          In Florida, around two thirds of the specimens that have been submitted for influenza testing at BPHL in recent weeks are testing positive for influenza.

                          Most of these are influenza A H3, but influenza B is also circulating, and small numbers of 2009 influenza A H1N1 specimens have also been reported.

                          All of these are seasonal strains of influenza.

                          Influenza virus is circulating in all regions of the state.

                            I've seen enough that I have removed the mention of H1N1 from the thread title. I suspect the subtype has yet to be determined, and some of the reporters are using "H1N1" or "swine flu" incorrectly as a synonym for "Influenza A" , especially in light of the doctor's comments about H3N2 above.

                              This event is interesting for sure.

                              Perhaps we are missing the fact that the proposed "index case from Florida" had to travel by air. By doing so, she was in contact with people from all over the world - from India? Yemen? Iraq? Israel? Hong Kong? All of which are locations reporting H1N1 cases - whether en route to or from Orlando (which happens to be a world vacationing location, especially this time of year.)

                              Additionally, I agree with the most recent Dr. discussion regarding the influenza A in US. It is too vague to determine the cases in Argentina are or are not H1N1.

                              The demographics of the disease in Argentina suggest a virus more sinister to the middle and younger age groups - where the current H3N2 situation in the US - although creating disease in all populations - is very obviously posing a significant danger to those persons 65 and older.

                              This event is worthy of all of your ongoing efforts in epidemiology and surveillance.

                                I find this statement not credible:

                                Flu is flu. It is not containable. It circulates all over the world in a continuous cycle.
