    Summary of UK surveillance of influenza and other seasonal respiratory illnesses
    3 January 2013 – Week 1 report (up to week 52 data)

    Increases continue to be seen for several indicators of influenza activity. A letter has been issued to the NHS that GPs may now prescribe antiviral medicines for the prophylaxis and treatment of influenza in accordance with NICE guidance.
    Due to bank holidays in week 52 (ending 30 December 2012), GP surgeries were only open for three days which will have impacted on GP consultation rates – data should be interpreted with caution.

     Overall weekly influenza GP consultation rates across the UK
    o In week 52 (ending 30 December 2012), the weekly primary care ILI consultation rate increased in England (32.7 per 100,000) Scotland (37.1 per 100,000) and Northern Ireland (43.7 per 100,000) and decreased in Wales (3.4 per 100,000).
    o In week 52 2012, the weekly proportion of NHS Direct calls for cold/flu increased and the proportion for fever (5-14 yrs) decreased slightly but both remained above their respective influenza thresholds of 1.6% and 11.7%.
    o 12 acute respiratory disease outbreaks (seven in care homes, three in schools and two in hospitals) have been reported in the past seven days. Out of the three with a virological result available, one was confirmed as influenza B, one was influenza A(unsubtyped) and one was RSV.

     Virology
    o 138 (16.6%) of the 684 respiratory specimens reported to DataMart (England) tested positive for influenza in week 52 (110 B, 14 A subtype not known, 8 A(H1N1)pdm09 and 6 A(H3)).
    o The proportion of samples positive increased slightly for RSV and hMPV, decreased for rhinovirus and remained stable for adenovirus and parainfluenza.
    o Five influenza B positive detections were recorded through the two English GP-based sentinel schemes in week 52.

     Disease severity and mortality
    o 40 new admissions to ICU/HDU with confirmed influenza (21 B, 11 A(subtype not known), seven A(H3N2), and one A(H1N1)pdm09) were reported through the USISS mandatory ICU surveillance scheme across the UK (137 Trusts in England) in week 52. 63 new hospitalised confirmed influenza cases have been reported through the USISS sentinel hospital network across England (24 Trusts).
    o No excess all-cause mortality was seen by age group or region in week 50 across the UK as calculated with the EuroMOMO algorithm.

     Influenza vaccination
    o Up to week 50, 71.7% of people in England aged 65 years and over had received the 2012/13 influenza vaccine (provisional data). For those in a clinical risk group aged under 65 years, provisional uptake was 48.3%.
    o Amongst pregnant women, the provisional uptake was 39.6%.
    o Provisional data from the second monthly collection of influenza vaccine uptake by frontline healthcare workers show 40.8% were vaccinated by 30 November 2012.

     International situation
    o During week 51 2012, despite surveillance limitations over the holiday period, there are clear indications that influenza transmission continues to rise in Europe, although the impact and characteristics of the epidemics are yet to be determined.
    o Many countries of the northern hemisphere temperate region are now reporting elevated detections of influenza, particularly in North America.

     Novel coronavirus 2012
    o The total number of global laboratory confirmed human cases of novel coronavirus 2012 reported to WHO currently stands at nine. In the UK there has been one confirmed imported case to date and the public health response around this case has been published here. Information on management and guidance of possible cases is available on the HPA website.

    In week 52, 40 new admissions to ICU/HDU with confirmed influenza infection (21 B, 11 A unknown
    subtype, seven A(H3N2) and one A(H1N1)pdm09) were reported across the UK (137/163 Trusts in England)
    through the USISS mandatory ICU scheme (Figures 12 and 13). Five new confirmed influenza deaths were
    reported in week 52 2012. A total of 115 admissions (46 B, 35 A unknown subtype, 19 A(H3N2) and 15
    A(H1N1)pdm09) have been reported since week 40 2012. Nine confirmed influenza deaths have been
    reported across the UK in ICU/HDU since week 40 (Figure 12).

       Overall weekly influenza GP consultation rates across the UK o In week 1 (ending 6 January 2013), the weekly primary care ILI consultation rate decreased slightly in England (29.2 per 100,000) and increased in Wales (33.0 per 100,000), Scotland (46.3 per 100,000) and Northern Ireland (87.0 per 100,000).
      o In week 1 2013, the weekly proportion of NHS Direct calls for cold/flu decreased slightly but remains above the influenza threshold of 1.6%. The weekly proportion of calls for fever (5-14 yrs) decreased below the influenza threshold of 11.7%.
      o 14 acute respiratory disease outbreaks (12 in care homes, and two in schools) have been reported in the past seven days. Out of the three with a virological result available, two were confirmed as influenza B and one was confirmed as influenza A(H3).

       Virology
      o 242 (23.2%) of the 684 respiratory specimens reported to DataMart (England) tested positive for influenza in week 1 (175 B, 38 A(H3), 24 A subtype not known and 5 A(H1N1)pdm09).
      o The proportion of samples positive in DataMart (England) decreased slightly for RSV and remained stable for hMPV, rhinovirus, adenovirus and parainfluenza.
      o 32 influenza positive detections were recorded through the two English GP-based sentinel schemes in week 1 (26 B, 4 A(H3) and 2 A(H1N1)pdm09).

       Disease severity and mortality
      o 51 new admissions to ICU/HDU with confirmed influenza (23 B, 16 A(subtype not known), nine A(H1N1)pdm09 and four A(H3N2) were reported through the USISS mandatory ICU surveillance scheme across the UK (142 Trusts in England) in week 1. 65 new hospitalised confirmed influenza cases have been reported through the USISS sentinel hospital network across England (28 Trusts).
      o In week 1 2013, no excess all-cause mortality was seen by age group or region in England, an excess was seen in Scotland and no excess was seen in Wales and Northern Ireland.

      o Up to week 1, 72.5% of people in England aged 65 years and over had received the 2012/13 influenza vaccine (provisional data). For those in a clinical risk group aged under 65 years, provisional uptake was 49.7%.
      o Amongst pregnant women, the provisional uptake was 39.4%.
      o Provisional data from the second monthly collection of influenza vaccine uptake by frontline healthcare workers show 40.8% were vaccinated by 30 November 2012.

       International situation

      o Influenza activity appears to have continued to rise in most countries of the northern hemisphere temperate region.

        Indicators of influenza activity across England have decreased compared to the previous week. A letter has been issued to the NHS that GPs may now prescribe antiviral medicines for the prophylaxis and treatment of influenza in accordance with NICE guidance.

         Overall weekly influenza GP consultation rates across the UK o In week 2 (ending 13 January 2013), the weekly primary care ILI consultation rate decreased slightly in England (24.8 per 100,000) and Northern Ireland (72.4 per 100,000), increased in Scotland (52.22 per 100,000) and remained stable in Wales (26.1 per 100,000).
        o In week 2 2013, the weekly proportion of NHS Direct calls for cold/flu decreased below the influenza threshold of 1.6%. The weekly proportion of calls for fever (5-14 yrs) decreased and remained below the influenza threshold of 11.7%.
        o Three acute respiratory disease outbreaks in care homes have been reported in the past seven days. Out of the two with a virological result available, two were confirmed as influenza A unsubtyped.

         Virology
        o 144 (17.1%) of the 843 respiratory specimens reported to DataMart (England) tested positive for influenza in week 2 (91 B, 22 A(H3), 16 A subtype not known and 15 A(H1N1)pdm09).
        o The proportion of samples positive in DataMart (England) increased for rhinovirus, decreased for RSV and hMPV and remained stable for adenovirus and parainfluenza.
        o 25 influenza positive detections were recorded through the two English GP-based sentinel schemes in week 2 (18 B and 7 A(H3)).

         Disease severity and mortality
        o 44 new admissions to ICU/HDU with confirmed influenza (24 B, 9 A(subtype not known), seven A(H1N1)pdm09 and four A(H3N2) were reported through the USISS mandatory ICU surveillance scheme across the UK (134 Trusts in England) in week 2. 61 new hospitalised confirmed influenza cases have been reported through the USISS sentinel hospital network across England (26 Trusts).
        o In week 2 2013, no excess all-cause mortality was seen by age group or region in England, an excess was seen in Scotland and no excess was seen in Wales and Northern Ireland.

        subtype, seven A(H1N1)pdm09, and four A(H3N2) were reported across the UK (134/163 Trusts in England)
        through the USISS mandatory ICU scheme (Figures 12 and 13). Four new confirmed influenza deaths were
        reported in week 2 2013. A total of 254 admissions (109 B, 61 A unknown subtype, 50 A(H3N2) and 34
        A(H1N1)pdm09) have been reported since week 40 2012. Twenty-eight confirmed influenza deaths (nine B,
        nine A(H3N2), five A(H1N1)pdm09 and five A unknown subtype) have been reported across the UK in
        ICU/HDU since week 40 (Figure 12).

             Overall weekly influenza GP consultation rates across the UK o In week 4 (ending 27 January 2013), the weekly primary care ILI consultation rate decreased in England (11.1 per 100,000) and Northern Ireland (47.9 per 100,000), increased slightly in Scotland (40.5 per 100,000) and remained stable in Wales (10.5 per 100,000).
            o In week 4 2013, the weekly proportion of NHS Direct calls for cold/flu and fever (5-14yrs) remained below their respective influenza thresholds of 1.6% and 11.7%.

              While some indicators of influenza activity across England have remained stable compared to the previous week, some indicators have slightly increased and outbreaks and hospitalisations continue to be reported.
              A letter has been issued to the NHS that GPs may now prescribe antiviral medicines for the prophylaxis and treatment of influenza in accordance with NICE guidance.
               Overall weekly influenza GP consultation rates across the UK o In week 5 (ending 3 February 2013), the weekly primary care ILI consultation rate remained stable in England (12.7 per 100,000) and Northern Ireland (48.4 per 100,000) and increased in Wales (19.9 per 100,000) and Scotland (44.2 per 100,000).
              o In week 5 2013, the weekly proportion of NHS Direct calls for cold/flu and fever (5-14yrs) remained below their respective influenza thresholds of 1.6% and 11.7%.
              o 28 acute respiratory disease outbreaks have been reported in the past seven days compared to 18 the previous week. Eleven had a virological result available (four influenza A(unsubtyped), four B, two A(H3) and one A(H1N1)pdm09).

               Virology
              o 218 influenza positive detections (97 B, 66 A subtype not known, 38 A(H3) and 17 A(H1N1)pdm09) were recorded through the DataMart scheme (overall positivity of 23.2% compared to 19.2% the previous week).
              o The proportion of samples positive in DataMart (England) increased for rhinovirus and remained stable for RSV, hMPV, adenovirus and parainfluenza.
              o 42 influenza positive detections were recorded through the two English GP-based sentinel schemes in week 5 (18 B, 17 A(H3) and 7 A(H1N1)pdm09), giving a positivity of 42% compared to 44% in week 4.

               Disease severity and mortality
              o 36 new admissions to ICU/HDU with confirmed influenza (14 A(subtype not known), 12 B, five A(H1N1)pdm09 and five A(H3N2) were reported through the USISS mandatory ICU surveillance scheme across the UK (137 Trusts in England) in week 5. 78 new hospitalised confirmed influenza cases have been reported through the USISS sentinel hospital network across England (26 Trusts).
              o In week 5 2013, overall excess all-cause mortality was seen in Northern Ireland and none was seen in England, Scotland and Wales. This data is provisional due to the time delay in death registration.

               Influenza vaccination
              o Up to week 4, 73.2% of people in England aged 65 years and over had received the 2012/13 influenza vaccine (provisional data). For those in a clinical risk group aged under 65 years, provisional uptake was 50.8%. Amongst pregnant women, the provisional uptake was 39.7%.
              o A mid-season analysis assessing the effectiveness of the trivalent influenza vaccine in the UK in this season (2012/13) has been published in Eurosurveillance. An overall adjusted vaccine effectiveness of 51% (95% confidence interval 27% - 68%) was reached against all laboratory-confirmed influenza in primary care, which is within the expected range of effectiveness of a trivalent influenza vaccine.

               International situation
              o During week 4 2013, influenza activity continued to rise across Europe, although the epidemic may have passed its peak in some north-western countries.
              o Influenza activity in North America remains high regionally, though nationally most indicators of transmission have begun to decrease.
              o The patterns of influenza virus co-circulation being identified in Europe are different from that being reported by North America.

                  Indicators of influenza activity across England have either remained stable or decreased compared to the previous week. Outbreaks and hospitalisations continue to be reported. A letter has been issued to the NHS that GPs may now prescribe antiviral medicines for the prophylaxis and treatment of influenza in accordance with NICE guidance.

                   Overall weekly influenza GP consultation rates across the UK o In week 6 (ending 10 February 2013), the weekly primary care ILI consultation rate remained stable in England (14.5 per 100,000), increased in Northern Ireland (61.4 per 100,000) and decreased slightly in Wales (16.1 per 100,000) and Scotland (41.6 per 100,000).
                  o In week 6 2013, the weekly proportion of NHS Direct calls for cold/flu and fever (5-14yrs) remained below their respective influenza thresholds of 1.6% and 11.7%.

                  o In the past seven days, two cases of novel coronavirus have been diagnosed in UK residents in the UK. The first case had recently travelled to the Middle East and Pakistan. The second case is a family member and was in close personal contact with the first case, with no recent travel history. While it is understood the second case had an existing medical condition that may make them more susceptible to respiratory infections, this suggests person-to-person transmission has occurred and that it occurred in the UK. Close contacts are being identified and followed up.
                  o These latest two cases bring the total number of confirmed cases globally to 11.

                   International situation
                  o During week 5 2013, influenza activity continued to rise across Europe. In a few countries the epidemics seemed to have passed their peaks, although some countries experienced a resurgence of ILI rates.
                  o Influenza activity in North America remains high, although it has decreased in most areas in the United States.
                  o The patterns of influenza virus co-circulation being identified in Europe are different from that being reported by North America.

                             In February 2013, three cases of novel coronavirus were diagnosed in UK residents in the UK. The first case had recently travelled to the Middle East and Pakistan.                               28 March 2013 ? Week 13 report (up to week 12 data)

                              Indicators of influenza activity across England are decreasing but continue to show influenza is circulating. A letter has been issued to the NHS that GPs may prescribe antiviral medicines for the prophylaxis and treatment of influenza in accordance with NICE guidance.
                              Overall weekly influenza GP consultation rates across the UK o In week 12 (ending 24 March 2013), overall weekly influenza GP consultations remained stable in England (11.6 per 100,000) and Scotland (18.1 per 100,000) and decreased in Wales (14.2 per 100,000) and Northern Ireland (32.1 per 100,000).
                              o In week 12 2013, the weekly proportion of NHS Direct calls for cold/flu and fever (5-14yrs) remained below their respective influenza thresholds of 1.6% and 11.7%.
                              o 16 new acute respiratory disease outbreaks have been reported in the past seven days. 11 had a virological result available (seven influenza A (unsubtyped), two A(H3), one B/A(unsubtyped) and one B).

                               Virology
                              o 188 influenza positive detections (71 A subtype not known, 69 A(H3), 29 B and 19 A(H1N1)pdm09) were recorded through the DataMart scheme (overall positivity of 17.2% compared to 20.3% the previous week).
                              o The proportion of samples positive in DataMart (England) remained stable for adenovirus, parainfluenza, RSV, hMPV and rhinovirus.
                              o 9 influenza positive detections were recorded through the two English GP-based sentinel schemes in week 12 (5 A(H3), 2 B and 2 A(H1N1)pdm09), giving a positivity of 34.6% compared to 36.6% in week 11.

                               Disease severity and mortality
                              o 23 new admissions to ICU/HDU with confirmed influenza (nine A(subtype not known), seven A(H3N2), six B and one A(H1N1)pdm09 and were reported through the USISS mandatory ICU surveillance scheme across the UK (130 Trusts in England) in week 12. 54 new hospitalised confirmed influenza cases have been reported through the USISS sentinel hospital network across England (22 Trusts).
                              o In week 12 2013, excess all-cause mortality was seen in England and Northern Ireland through the EuroMOMO algorithm. This data is provisional due to the time delay in death registration.

                               International situation
                              o During week 11 2013, influenza activity continued to decline across Europe; after more than three months of active transmission, the 2012/13 influenza season is on the wane.
                              o Influenza activity in North America remains elevated but is continuing to decrease in most areas.
                              o Low levels of influenza activity were reported across the tropical regions of the world and activity in countries of the southern hemisphere remains at inter-seasonal levels.

                               Novel coronavirus
