OK... so contained to 1 family in one house.
symptoms apeared May 5... means the caught it around May 1-3. They could have been spreading it from May 2 - May 8 when they went to the hospital.
IF this is THE pandemic strain emerging, Then we should start to see others they interacted with getting sick may 6 or 7 and getting bad enough by May 10 - 11 to get medical treatment.
HCW should start showing symptoms May 11-13.
This is as current as we've gotten with onset dates. It would be interesting to see the length of time between the family members or did they all get sick at the same time?
If it was the same time, it was probably the consumed chicken.
It still bears intenses watching... cause Flu mutates at an astonishing rate. And there is mention of pigs.

I've commented on this elsewhere. This is a cluster. What's missing from this report is that the MOTHER died previously, and of course, there's no samples taken and no info about her, and she's of course, per standard WHO action, not reported as "the" index case.
I don't know if these are all from one mother. The ages here are 29 to 8.
That is unusual...a 21 year fertility period. I know they're out there, but that's rare. I saw the place and didn't see that. I saw people have many children but not such a large age range gap (with exceptions among the more well to do who could afford new wives and babies).
As to when we would see more signs of infection, my answer is not "LATER". We here have played that game. It's a losing proposition.
My answer is "NOW". If this were easily transmissable, the chicken farmer, the market butcher, the neighbor kids, and their friends (as they are very socially gregarious and they have friends wandering in and out of their homes, even in slum areas) would NOW or by tomorrow (I'll give you one day and that's it, one day only) would be reporting fever and diarrhea (sp) and headaches.
We hear no such news. This is another "H2H" cluster. It's obvious because of the fact that the mom died first and isn't being reported here.
The WHO system is broken. We suffer from ignorance on this topic. BlueJay at CE is covering this story as he covers all Indonesian stories.

Familial clusters of H5N1 in Indonesia have been noted previously, but this is the largest suspected cluster reported to date.

Watch the next few days to week for an increasing number of victims. Since we learned the virus can live on exposed surfaces for as long as 6 days, we have to take into consideration that the cases may be infected from the index case at longer intervals then previously suspected. As for the long time span between kids, well accidents do happen. Just because a woman may have a twenty year time period between kids hardly makes it odd. A woman who may have married at 18 would still be ovulating at 38, trust me on this one. LOL

Also take into account the stigma that BF imposes on people and the consquences.
All of their poultry will be destroyed. People will shun them. I suspect even if this is an H2H strain, or in future clusters, many won't report and just die quietly at home. The backyard burial?
Lets keep an eye out over the next week or so for increase in local hosptials

The above detail on the suspected H5N1 bird flu fatalities raises additional pandmeic concerns. The mother died on May 4 followed the death of her son on May 9 and another family member on May 10. The delay of 5-10 days between the index case and family members is the hallmark of person-to-person transmission of H5N1. In Indonesia, as many as 2/3 of cases are linked to family clusters and the vast majority of the clusters have the 5-10 day gap.
However, in the past familial clusters were smaller, generally invoving 2 or 3 people. This cluster has 8 family members who have died or are hospitalized with bird flu symptoms, signaling a more efficient transmission of H5N1.
H5N1 has been present in Indonesia since 2003, but the first reported human cases were reported last year. If the current cluster is positive for H5N1, they will be the largest cluster in Indonesia and the largest confirmed cluster if all test positive.
In any event, the large number of family members, ranging in age from 40 to 1.5 and the differences in onset dates suggests this cluster is cause for concern.

It's not the number of dead from this cluster. We know that without treatment the disease is fatal 90++% - 100% of the time. It kills.
We are looking for the casual contacts to get sick, for the father to get sick, for the neighbor kids who play with them to get sick, for the chicken vendor to get sick.
This is the "sign" we look for.l
The most bothersome part of news article above is that the newspapers insist on acting the fools. The only possible way to get this in their feeble minds is via a bird. Therefore, if the family bought a chicken or a "piece" of chicken, each and every cluster instance must include a Jonestown-esque scene in which everyone sits around the communal pot and eats the chicken. While it's possible, they love chicken satay (possibly way too undercooked...very possible), and yet again, everyone would have had to take their communion around the food...BUT NOT THE FATHER AND NOT THE GRANDPARENTS WHO ARE PROBABLY AROUND.
I am willing to eat so much horseradish and then I'm not going to do it anymore. The Indonesians continue to be inept, incapable of seeing fact and reporting disinterestedly.

Anyone recall that?

Goju, it isn't in any of the Indonesian/English papers. I will check now to see if anything has popped up n the last day.
I can find nothing that is written in English at any of my Indonesian links. I really wish we had an Indonesian translator. There are stories out there but I cannot translate them.

He said the assistance was badly needed for the implementation of the national strategic plan for the handling of bird flu infection in Indonesia.
"We have strong commitment to fighting this disease and have drawn up a national strategic plan but we still had no enough capacity, funds and knowledge. Without regional and international support, we would face problem in carrying out the plan," the minister said.
He said that based on the Indonesian government`s calculation, Indonesia needed some US$900 million to implement the strategic plan for a period of three years.
The government has in the state budget set aside some US$50 million per annum for bird flu fight funds while the private sector was expected to provide some US$20 million per annum, he said.
According to the minister, the total funds the government could collect for fighting the influenza virus infection in the coming three years was only about US$200 million, far below the needed US$900 million.
He said that donor countries attending bird flu conference in Beijing last January pledged a total fund of US$50 million to help fight bird flu infection in Indonesia.
The minister hoped that the donor countries would realize their commitment soon to help the Indonesian government carry out its bird flu elimination program.
"It must be carried out soon because the number of infection cases tended to increase, though there is no indication as to the outbreak of bird flu pandemic disease," he added.
He said 401 suspects had been examined. Of the number, 330 were declared negative with the bird flu virus, 128 were still under observations and 128 had been confirmed positive with the virus.

Three suspected bird flu patients die in Medan Apriadi Gunawan, The Jakarta Post, Medan
Three suspected bird flu patients have died in North Sumatra during the past week, with the latest victim passing away in a Medan hospital Wednesday morning.
According to the North Sumatra Bird Flu Prevention Team, the three victims, all from Kubu Simbelang village in Karo regency, died several days after being admitted to hospitals in Medan.
The team said three other suspected bird flu patients are currently being treated at Adam Malik Hospital in Medan.
Nur Rasyid Lubis, the head of North Sumatra's Suspected Bird Flu Patient Management team, said Wednesday the three dead victims had eaten chicken together.
Several days after eating the chicken, "the first suspected victim, Fuji Ginting, experienced breathing problems and a high fever. The same symptoms were also suffered by the two other victims, Anta Ginting and Roy Karo-Karo", Nur said.
Anta, 29, died at Adam Malik Hospital on Wednesday morning, while Roy, 18, died at the same hospital Tuesday. Roy's father, Fuji, 40, passed away May 4 at Elisabeth Hospital.
Nur, who is also deputy director of medical services at Adam Malik, said the hospital did everything in its power to treat the patients, but was unable to save them.
North Sumatra Health Office chief Fatni Sulani said the office was investigating the cases and had sent blood samples from the three to Jakarta for testing.
"We expect the results of the blood tests to be completed in the next two weeks. For the meantime, the three victims are still suspected, not positive, bird flu patients," Fatni said.
She said that North Sumatra has not had a confirmed case of bird flu in humans.
"So far, the bird flu virus in North Sumatra has only affected fowl. Thirteen regencies/cities have been put on bird flu alert after the virus was found in local birds," Fatni said.
She said the cities on bird flu alert included Binjai, Deli Serdang, Langkat and Medan.
North Sumatra Husbandry Office head Rahim Siregar said Wednesday his office had added Karo regency to the list of bird flu prone areas in the province.
"Fowl in North Sumatra have been affected by bird flu since July 2005, but there have been no confirmed cases in humans," Rahim said.
