A woman has died and 10 more in hospital after a flu-like outbreak on board a Via Rail train. The train, travelling from Vancouver to Toronto, is now under quarantine near Timmins.
Sgt Laura Nichols OPP said that she was called by CN at 8:35am Friday. "There was one person who had vital signs absent and five other people that were sick with flu-like symptoms," she reported.
That number quickly increased to ten, and all were taken to Timmins and District Hospital.
"The Timmins District Hospital is a regional hospital, and I'm confident they can handle it," Nichols allowed.
The train is stopped in Foleyet.
"The whole place is being overrun with ambulances and police cars, and we've got helicopters," said Deborah DesRochers, chairwoman of the town. "They've got the train quarantined. They're trying to isolate what it is."
There were 260 passengers and 30 crewmembers on board the train. Emergency crews in full protective gear searched the affected cars. It's believed, but not certain, that the outbreak is confined to only two cars. No one except emergency personnel is allowed on or off the trains.

Since no details about the ten people with flu-linke illness were released, one cannot definitely draw conclusion until new info releasing. If people were from a single group or family one may think about an infectious agent. Otherwise, a toxic agent - carbon monoxide (CO) from heating system - may also explain the symptoms. Other contaminants in the train enviroment may be responsible as a rapid onset of symptoms probably exclude an infectious pathogen. However, no travel hystory was dispatched with above newswires. (CO) acute intoxication symptoms are often mistakenly diagnosed as ILI syndromes, with severe consequence for patients, who need immediate treatment in hyperbaric facilities.

It is important to see patients' blood level of carboxyemoglobin to exclude(CO) intoxication from heating system and immediate life-saving proper treatment.

It is advisable that passenger will be keep in ventilated area to avoid further contamination from either toxin, (CO) or infectious agents (at this point less likely than toxic agents).

This directly point to a intoxication by cooling or heating system. A respiratory disease with two o three days of incubation are also possible, but environmental contaminant may be the culprit. Chemical agents in use for cleaning of toilet, for example (aerosols).

This, I believe, opens the symptoms to include: nausea, sweating, diarrhea, fever, and respiratory systems, among others.
And high fever, cough, sense of breathless, cardiac and pulmonary failure in the more severe cases. CNS signs, seizures. Rash. Chemical agents into disposable sleeping car equipment, powder for cleaning, cooling system (CO). It is happened in Italy, where a derelict rail system operates and north-south (more than 2.000 Km long) trips are common.

Without a preliminary blood inspection of the cases? This is pure madness. Even a toxin was inhaled, or dispersed though the body of passengers, trasported out of the scene may represent a risk. Ricin scare in Las Vegas is the most recent case.
