Genetically, there is also prior evidence for exchanges between Vietnam and South Korea / Japan. The 2003 / 2004 isolates were precursors to clade 2.2 (Qinghai strain), which has become dominant in migratory birds that summer in Siberia and Mongolia. All H5N1 west of China has been clade 2.2 which was also true for South Korea and Japan in the 2006/2007 season.
However, the 2003/2004 isolates form South Korea / Japan had a novel cleavage site RERR_KKR (delected an R), which was subsequently found in northern Vietnam in early 2005. This acquisition was appended onto a clade 1 genetic background, indicating it was acquired by recombination. The change was associated with larger clusters and milder H5N1 cases.
Media reports in South Korea also suggest that the US CDC has confirmed H5N1 in patient(s) in South Korea, as had been previously seen in the soldier that was H5 positive. Korea claimed the soldier was H5N1 “negative” because they couldn’t confirm the N1. However, they did not deny confirmation of H5, which meets the WHO definition of a confirmed case. It seems that the confirmation by the CDC is being withheld, and a Friday meeting is planed to discuss the development.
These developments may be related to recent WHO comments on sharing of research results. The withholding of this information remains hazardous to the world’s health.

The report of clade 2.3.2 in whooper swans in Japan increases the likelihood that these type of exchanges will increase, and clade 2.2 migrating into Europe, the Middle East, and Africa will have additional clade 2.3.2 polymorphisms. Moreover, the report of 2.3.2 in South Korea and northern Japan increases the likelihood that clade 2.3.2 will migrate into areas where clade 2.2 was reported previously, as well as Alaska, which lies within the East Asia flyway that is delivering clade 2.3.2 to Korea and Japan.
The finding of clade 2.3.2 in Korea and northern Japan significantly increases pandemic concerns and the likelihood that H5N1 wsill once again significantly expand its global reach and genetic diversity.

The current H5N1 strain of bird flu sweeping the nation is a different variation of the virus from Korea¡¯s two previous outbreaks, a government body announced yesterday. But National Veterinary Research and Quarantine Service officials said they need more time to confirm what exactly the difference is.
¡°We have found that this is a different H5N1 virus than that of 2003 and 2006,¡± said Kim Ki-seuk, head of the team researching the virus. ¡°We don¡¯t yet know if this season¡¯s virus is a mutation. Korea has clade 2.3.2 (a fujian subclade). Prior to the soldier, all human fujian cases were 2.3.4.

- Australian travellers, long-term residents and businesses overseas should inform themselves about the risks of avian influenza, be prepared to take personal responsibility for their own safety and put appropriate contingency plans in place.
- Australians who live in an avian influenza affected area for an extended period should consider, as a precautionary measure, having access to influenza antiviral medicine.
- Australians intending to travel to affected countries for shorter periods are at much lower risk of infection, but should discuss the risk of avian influenza with their doctor as part of their routine pre-travel health checks.
- If you are visiting or living in a country affected by avian influenza, you should follow sensible precautions to reduce infection risk.
- If the threat of sustained human-to-human transmission appears serious, we will advise Australians in affected countries to consider leaving. If they don't leave when first advised to do so, they may be prevented from leaving later.
- Australians should ensure that their travel documents are up-to-date in case they need to depart an affected country at short notice.
- If a widespread outbreak occurs, the delivery of consular assistance to Australians could be severely constrained. Australian missions and offices overseas will not be in a position to provide influenza antiviral medicines to Australians in affected areas. Australian travellers, long-term residents and businesses are responsible for securing their own supply of influenza antiviral medicine, if required.
