5000 doctors and paramedics to get vaccinated
Sumitra Deb Roy, TNN, 1 February 2010, 12:23am IST

MUMBAI: The death of an 8-year-old girl in Pune on Friday rekindled fears in the minds of many doctors and paramedics who had earlier refused to get vaccinated thinking that the swine flu virus had lost its potency. Now, 5,000 doctors have given their consent for vaccination as against 100 doctors who have said no to it.

The health department of the state has already drawn up a list of 5,000 doctors and paramedics, who will get vaccinated, when the first lot arrives next week. Additional chief secretary (health) Sharvaree Gokhale confirmed that more doctors have come forward asking for the vaccine following the death in Pune.

These doctors are paramedics who man the 109 isolation centres for influenza H1N1 in Maharashtra. With 313 deaths, the state has topped the count of fatalities due to swine flu in the country.

A senior state government officer, on condition of anonymity, said that all medical staffers were given forms wherein they were asked if they want to take the vaccine. Following the recent death in Pune, many doctors wanted to rework their forms, the officer said. Till the final count, only 100 doctors had replied saying they did not want the vaccine, he added.

Dean of Pune Sassoon General Hospital, Dr Arun Jamkar, said that though there were apprehensions about the adverse effects of the vaccine, doctors were right in taking it. According to him, there are concerns that as the virus go on changing their anti-genecity, efficacy of the vaccine will be variable. "The trend suggests there will be one or two swine flu deaths every month. It may be a wise decision to take the vaccine,'' he said. About 90% of the doctors at Sassoon Hospital, that has treated the largest number of swine flu cases, have chosen to take the vaccine.

The state is all set to start the vaccination process and also study side-effects of the vaccine, if any. Members of the Adverse Events Following Immunisation (AEFI) committee of the state will be closely watching all kinds of small or major reactions. Nodal officers, from the district-level upwards, have been alerted to record and report any untoward health incident following vaccination, said the state government official.

The Centre, on its part, has maintained that by and large doctors from all states have faith in the vaccines that the government is procuring. Director general, Indian Council of Medical Research and secretary, Department of Health Research Dr V M Katoch agreed that it was a crucial stage. "Vaccination results will be crucial but we are hopeful that it will protect our doctors,'' he said. He, however, refused to comment on the amount that the Indian government had shelled out to procure vaccines for 15 lakh health care workers.

Thursday, February 18, 2010 AT 11:42 AM (IST)

PUNE: H1N1 flu claimed one more on Wednesday morning, State health officials reported on Thursday pushing the death toll in Pune district to 162. The virus so far has claimed 335 lives in the State.

Twenty-seven years old Aditya N Chite from B T Kawade Road in Ghorpuri died at Inlaks and Budhrani Hospital around 10.15 am, a Government communiqu? said. The city has witnessed 16 H1N1 deaths since February 1, this year.

Meanwhile, 20 patients are admitted to intensive care units of various hospitals and four of them are on ventilators, an official press release said.

Ro's Note: Numberswise, this report is odd. Currently, 180+ fatalities have occurred in Pune District, and 345 in Maharashtra. Death #162 occurred between Jan 8 and Jan 15. I'm not 100% sure at this point whether this is a late report of that death, or a new fatality with poor reporting.

            H1N1 strikes Pune again!
By: Alifiya Khan Date: 2010-02-19 Place: Pune

Doctors in city say rising number of swine flu deaths should serve as wake-up call for careless citizens. Alifiya Khan reports

On an average, one person has succumbed to the deadly swine flu virus each day in the past 20 days. This is a statistic that should not be ignored warn city doctors.

"In three days, there have been three deaths this week; last week there were eight deaths," said Dr Kalyani Deshpande, a doctor in the swine flu monitoring cell.

"It is worrying; too much complacency has set in among people," added Dr Ashok Kante, in-charge of the swine flu control room.

City doctors agree with Kante's theory that people, including doctors have started taking the swine flu virus too lightly, which could be dangerous in this weather and lead to rising deaths.

"People have stopped taking safety precautions. Doctors have also stopped referring cases of high fever, cough and cold to swine flu centers. Patients are being referred as a last resort, but sometimes it is too late," said Avinash Bhondwe, senior member of Indian Medical Association, Pune.

            Blame the weather
Dr Sharad Agarkhedkar, head of pediatric department at D Y Patil Medical College, Pune, said that the cold wave that has swept the city has led to the increase in number of cases and severity.

"It is a known fact that this virus thrives in cold conditions, and in fact the World Health Organisation had warned against this phenomenon. Patients are not getting treated on time," said Agarkhedkar.

Two patients who died recently were victims of late referrals by doctors. 53-year-old Gani Mohammed Shaikh, the 189th victim of virus who died yesterday fell ill on Janaury 29 but wasn't admitted until February 13.

Even in case of Aditya Chitte (27) who died on February 17, he was admitted to hospital on February 13 while the illness had started on February 3.

A Third Of City?s Population Immune To Virus, Says NIV Director
The initial panic and fear may have subsided but the H1N1 virus continues to thrive in the city. By Saturday, the number of deaths had risen to 201 with the deaths of Raghunath Shivram Pawar (61) a resident of Ghorpadi Peth and Dhiraj Chordia (26) of Belgaum.

According to Uddhav Gavande, deputy director of the state health services, Pawar died at Sassoon hospital at 8.30 pm on Friday night. Chordia died at 7.20 am on Saturday at the Deenanath Mangeshkar Hospital.

A C Mishra, director of the National Institute of Virology (NIV), however, insists that it is not all bad news. Around a third of the city?s population is immune to the virus, he says.

?The continuous exposure of the city?s population to the Swine Flu virus has led to the development of anti?bodies among 30 to 35 per cent of the city?s population. These are the findings of our study which is yet to be released. The study is still going on,? Mishra told TOI on Saturday.

Asked whether the virus had become endemic, Mishra said, ?The virus continues to be in the pandemic phase. We cannot say that it has become endemic. If the virus continues its activity even after summer, then we can come to the conclusion that it has become endemic.?

Microbiologist Siddharth Dalvi, however, is of a different opinion. ?Unlike other strains of influenza viruses, the current novel H1N1 strain has spread more rapidly from person to person. A large number of people were infected in a short span of time. This led to a relatively rapid build?up of herd immunity towards this novel strain and as a result, the strain has already become endemic,? he said.

According to the state health services, the first H1N1 death was reported in the city on August 3, 2009, while the 100th death was reported on November 5. The first 100 deaths were recorded in almost four months while the next 100 deaths came in five months.

The first death had triggered intense panic in the city and thousands of people had thronged the PMC?run Naidu Hospital to be tested. Men, women and children with the slightest hint of a cough and cold, would gather at the hospital and the doctors and hospital staff soon found themselves swamped by the jostling crowds. During that time, general practitioners were reluctant to check patients suffering from a cold and fever as they feared action would be taken against them if a patient later tested positive for H1N1.

On August 5, the Maharashtra government invoked the Epidemic Diseases Act in two districts, Pune and Satara, to prevent spread of the virus. On August 10, it was decided to shut down all schools, colleges and malls for a week in the wake of the rising death toll.

Thousands of patients were examined at the 15 screening centres started by the Pune Municipal Corporation for the initial screening of patients with H1N1 flu?like symptoms. Given the burden on state?run testing centres, the state government on August 7 decided to rope in private hospitals to tide over the rush of probable patients and the district administration arranged for four private hospitals to treat Swine Flu cases in Pune.

Around this time, the state government decided to administer Tamiflu even to suspected H1N1 patients and the NIV decided to only test severely ill patients for the flu.

In view of the heavy burden on government?run testing laboratories, the Union government, on August 28, gave the go?ahead to private labs to conduct tests for H1N1 influenza. City?based Golwilkar Metropolis Health Services Pvt Ltd In Pune started offering testing facilities for H1N1 from August 30.

Gita's PrinciplesOpportunityDon?t Glorify NaxalismRole-Model MPs, MLAs! Even as state health officials will distribute imported swine flu vaccine for doctors and paramedics on Saturday, the Serum Institute of India Limited has now embarked upon Phase II and III of the indigenous injectable vaccine against the virus.

Dr Prasad Kulkarni, director of the Clinical Trials, said the Drug Controller General of India had given the approval for the clinical trial for an injectable vaccine. The trial for the vaccine began on Thursday. Like the ?nasal spray vaccine? trial, here too 330 people will be involved at medical colleges and hospitals at Pune, Ahmedabad, Indore and Bangalore. ?We need to wait for 21 days after the nasal spray has been administered to each person in the clinical trial.

The results will be available in the third week of April,? Kulkarni said.

The state health officials will hold a meeting on Saturday to discuss the demand for the dose from each district. This vaccine is meant for doctors and paramedical workers who have been treating swine flu patients.

While the vaccine will not be administered immediately, vials will be distributed to all 33 districts. Dr V M Kulkarni, in charge of the vaccination programme, said assistant medical officers of district rural hospitals will place their demand at the meeting following which the vials will be given.

Pune has registered a demand for 3,893 doses to be administered to the medical and para medical staff at the isolation wards of some 20-odd hospitals in the city.

So far there is no demand from Mumbai and Gondia, whereas areas from Sindhudurg have had a higher demand, said Kulkarni. ?It is not compulsory but a precautionary measure that has been advised by the government for the medical community,? said Kulkarni. Swine flu has claimed 208 lives and the progressive number of people getting infected with the virus in Pune is 3,195 people.
