Fièvre jaune au Sénégal

Le 20 septembre 2010, le Ministère de la Santé du
Sénégal a signalé un cas présumé de fièvre jaune
dans le district sanitaire de Mbour (Région de
Thiès), à environ 50 kilomètres de Dakar, où le
cas en question a été hospitalisé.

Ce cas concerne un pêcheur âgé de 27 ans qui
travaille en Gambie (localité de Tandji) et a
manifesté les symptômes cliniques de fièvre et
d'ictère. Les échantillons sanguins du patient
ont été analysés par l'Institut Pasteur de Dakar
et ont révélé une positivité des IgM par ELISA,
confirmée par PNRT (test de neutralisation par
réduction de plages), un test plus spécifique. Ce
patient n'avait pas d'antécédents de vaccination antiamarile.

Un second cas présumé a été signalé dans la
région de Thiès (Sénégal), lui aussi un pêcheur
de la localité de Tandji en Gambie, présentant
des symptômes neurologiques, notamment une
altération de l'état mental. Ce cas particulier
semble ne pas avoir été confirmé par l'Institut Pasteur de Dakar.

La Gambie a été informée de la situation par
l'intermédiaire de l'OMS (Organisation mondiale
de la santé), afin d'évaluer la situation
épidémiologique dans la localité de Tandji.

Le Sénégal a organisé une campagne préventive de
vaccination antiamarile de masse en 2007, ciblant
plus de 3,1 millions de personnes dans l'ensemble
des 18 districts qui n'avaient pas été protégés
jusque-là par des ripostes à des flambées. Dans
la région sanitaire de Thiès, 314 713 personnes
étaient ciblées et l'on a obtenu une couverture
vaccinale de 91,8 pour cent (88,3 - 95,3 pour
cent). Cette campagne préventive s'est inscrite
dans le cadre de l'initiative mondiale de lutte
contre la fièvre jaune qui vise à éviter les
épidémies et à garantir un approvisionnement
suffisant en vaccins antiamarils en Afrique. La
couverture de la vaccination systématique des
nourrissons contre la fièvre jaune était au Sénégal de 79 pour cent en 2009.

Après la poussée épidémique de fièvre jaune en
Gambie en 1978-1979, une enquête sérologique
rétrospective dans les villages a permis
d'estimer que 8000 cas et 1700 décès s'étaient
produits. Selon certains rapports, la couverture
nationale de la vaccination était estimée à 95
pour cent en janvier 1979. Pour 2009, on a
signalé une couverture nationale de la
vaccination systématique des nourrissons de 99 pour cent.

Au vu de la couverture élevée de la vaccination
systématique et des campagnes récentes de
vaccination préventive au Sénégal comme en
Gambie, on ne prévoit pas d'épidémie et la
vaccination d'urgence ne s'impose pas pour le
moment. L'enquête autour des cas sporadiques
pourrait révéler des groupes d'individus non
vaccinés et orienter les stratégies de
vaccination et de lutte antivectorielle au niveau
local pour ceux qui sont exposés au risque.

[Malgré cette bonne apparente couverture
vaccinale, il y a nécessité de faire une
investigation profonde dans les localités
concernées, et les populations à risques. Souvent
derrière ces bonnes couvertures vaccinales il y a
des poches de populations non vaccinées qui
peuvent toujours être source d'une importante épidémie.
