Acute case search done to find, new cases. Information sent for stool test. Health education given regarding hygiene and sanitation. Stool sent for Test-01, Result Awaited RRT Visited the affected area. House to house survey done Daily surveillance done. Investigation Closed No

      Passage of watery Stool along with vomiting and nausea. Burtoll T.E. under SC of Burtoll SC of Harinagar BPHC. 228 cases 1 fatality 10/05/2012 2012-05-12 23:44:07 7000 Many patient of different age groups mainly young adult presented with symptoms of passage of watery stool with vomiting and nausea. Few presented with severe dehydration and some mild dehydration. First case reported on 28-4-2012 who attended a marriage party subsequently there is gradual rise of cases and sudden increase of no. of cases of diarrheal disease on 10-5-2012. Most of the people consumed drinking water from unprotected well. Seven Nos of the stool sample collected for Bacterial examination.Two nos. of the water sample collected for MPN test. All sample sent to SMCH, Microbiology Deptt. for examination. Result yet to received. Medical team from SMCH visited spot, render necessary medical treatment. Another Medical team constituted by SDM&HO,IC Harinagar BPHC also delaminated the cases and render medical treatment. Dr. Atanu Chakraborty,Asstt. Prof. Department of Microbiology,SMCH and DSO accompanied with Birendra Roy,Lab.Tech and DM visited the area. Stool sample were collected in Carry Blair Media under supervision of Dr. Chakraborty . PHE Deptt. supplying save drinking water to the affected area . All wells were disinfects by bleeching Power by the Garden authority. Occurrence of Cases are gradually decreasing. IEC activities initiated by the Public Health Staffs. RRT team started started outbreak investigation since 11-5-2012. Investigation Open Joint Director of Health Services Visited the area and observed the situation and assured all possible medical help from her side. She stress upon on supply of pure drinking water and proper hygienic care.

        Cholera Village- Noolpuzha PHC : Noolpuzha Block: Bathery District-Wayanad 13 0 08/05/2012 2012-05-14 13:28:36 114 All cases belong to the adjacent houses to the index case. The index cases were reported on 05.05.2012, who had watery diarrhoea. She is a MGNREGA worker who said to have consumed water from the stream nearby at her workplace. The stream passes near by the colonies where open defecation is persistent. This stream is suspected to be the source of index case. Water from the stream has also been collected for examination. There are immigrants from the colonies who had gone like Coorge in Karnantaka where cholera is reported. The colony has only one well, and people mostly prefer open defecation. One sample tested positive for Vibrio cholera (O1 ?Ogawa) result of water samples awaited * Informed SSU and District Collector * Convened a meeting of the staffs of the PHC and discussed the situation, also provided information on control measures. * Medical Camp organised * RRT Visited the area * Health Education Provided * Water Source Chlorinated * ORS packets Distributed * Day to day Surveillance in the affected colony * Active case search Done. * Special Meeting of Intersectoral committee including the Local Self government Elected representatives convened and discussed the Situation and control measures. * Colony is under day to day surveillance Investigation Open

Result is awaited. All the field staff of the PHC Kulumani have been involved in the ADD surveillance and treated the ADD cases in the field.The pipe line breakage is rectified and chlorination has been strengthened in this village and ensure the residual chlorine 2ppm. And instructed the tank operator to give protected( chlorinated) water supply to this village daily.Instructed the field staffs to disinfect affected streets and houses with bleaching powder with lime and Phenyl. Investigation Open Intrcted to strengthend Chlorination in all OHTs in the drinking water supply.

            Diarrhoea Thottiam,Kattuputhur,Chinnapalipalayam & Chinnapalipalayam 80 0 15/05/2012 2012-05-16 11:04:47 1424 The piping Station to the OHT is near by the Kavery River both water supply well and OHT has not been chloriated regularly. This may be leeds to the outbreak. 2 Water Samples and 4 Stool Samples were collected and sent for Bacteriological analysis to KAPV Medical College, Trichy. Result is awaited. All the field staff of the PHC Kattuputhur have been involved in the ADD surveillance and treated the ADD cases in the field.The OHTs are chlorinated and give the safe water. Instructed the field staffs to disinfect affected streets, open air defecated area and houses with bleaching powder with lime and Phenyl. Health education was given to the public to drink Hot water and safe food.Further, I submit to state that the village kept under vigil by the KattuputhurPHC Rapid Response Team. Palorbond T.E,Palorbond SC under Harinagar BPHC 2 cases 2 fatalities 13/05/2012 2012-05-16 18:28:56 7000 Kanti Bagti along with her wife and son went outside where they took food and came with symptoms of passage of watery stool and vomiting. No case of similar symptoms found in the area except 2 nos. of diarrhea cases who are doing well after receiving ORS and antidiarrhoeal drugs. The death is due to suspected food poisoning. Not done. House to House visit done by ANM and ASHA . Medical team from Harinagar BPHC headed by Dr. B.P.Nath,SDM&HO IC Harinagar BPHC and Dr.S.Muriya of Joypur SHC visited the Place and render medical aid.DSO also visited the area .No such cases found in the area. Investigation Open nil

                  Acute Diarrheal Disease CHNC Mydukur, PHC Mydukur, Sub-Centre Annaluru, Village Bheemalingaiahpalli 29 0 16/05/2012 2012-05-18 15:54:15 376 Water contaminated due to the leakage of drinking water pipe in the village, as the drinking water pipe is running beneath the drinage canal. Immediately identified the source of contamination and rectified the problem with the help of RWS and Revenue department. Two number of water samples collected in the village,and sent to the DPHL, Kadapa for water analysis. Results awaited. Stopped the usage of contaminated water. Provided safe drinking water. Pot chlorination adopted. ORS packets distribution and health education given to the people. Investigation Open Continuous monitoring is going on. Health camp is arranged in the village.

                  Acute Bloody Dysentery with convulsion BPHC : Sissiborgaon BPHC PHC : Silapathar MPHC Sub-centre : Kulajan Sc Affected Village: Patiri 2 cases 2 fatalities 12/05/2012 2012-05-18 16:05:34 1661 1.It is reported from Civil Hospital Dhemaji that the first death occurred due to convulsion following Acute Bloody Dysentery. 2. Another death who did not reach Civil Hospital,Dhemaji after referral occurred at home. Age of the two deaths cases are of 6 years(M) and 3? years(M) respectively No sample could be collected. BPHC ic Sissiborgaon has been contacted and advised to do house to house survey in the village for detection of any other lying case and to take immediate measures for control. Situation being investigated by concerned BPHC. No new case has been reported after 16.05.12 Investigation Open Though the deaths at Civil Hospital was labeled as AES, the death might be due to Shigella dysentery and yet to be investigated and confirmed after reports of Survey.

Victim person was visited his father in law?s house in Morigaon district on 130512, where he had taken some traditional food for Bihu occasion. On 14512 he became very sick with loose Motion (7 times). But patient was treated in his home by quacks. Patient was admitted on 15512 at 12:30 AM at Khetri CHC with very shock & dehydration and expired at 2:00AM after two hours. No new cases admitted or reported from the affected area. Stool Sample : Not Available, Water Sample : 2 H2S Strip Collected and 2 sample send to GMC for MPN test. Result awaited. 1) Local PHC has advised to keep monitor the affected area. 2) DSO, DM from IDSP visited affected area and Khetri CHC for investigation the case. 3) HE has advised to give IEC and health education to the people from affected area. 4) ASHA and ANM are involved.                   Pain Abdoman, Vomiting and Loose Motion (Food Poisoning) Chariapuria Village, Block: Kamarbandha Ali BPHC, PHC: Baruagaon, SC: Balijan 4 0 09/05/2012 2012-05-18 10:55:49 450 On 9th May 2012 morning one of the family in Chariapuria Village having their breakfast with rice, gourd and potato. After two hours all the 4 family members having severe pain abdoman with vomiting and loose motion. They immediately admitted in KK Civil Hospital, Golaghat by 108 service. Age group are: 0-5 yrs 1, 6-15 yrs 1, 16-30 yrs 1, 31-50 yrs 1 1. Two stool sample collected and sent to District Priority Laboratory, Golaghat for CS test Result: Shigella. 2. Drinking water collected from two sources. One from tubewel and another from pond for H2S test on 09052012 Result: Not contaminated 1. A medical team from Baruagaon PHC visited the area and searched for any lying cases 2. ASHA and ASHA facilitator involved for house to house survey for any diarrhoeal cases 3. IEC given specially for safe drinking water and hand wash 4. DSO and Epidemiologist visited to KK Civil Hospital and meet the patients. Investigation Closed Nil

Hospital under supervision DHO,Dr. K.B.Shah,and PHC MO, 35 patients were admited Noor- E- Mohammed Hospital under supervison BHO KALOL and others PHC MO,12 patient admmited were dr.Viral Hospital.All the Patients treated accourdingly discharged Mid night.
