Zhao Qingchao, an official with Langfang municipal government, said Huang Yanqing apparently was the only one of 13 people who ate the duck to become fatally ill, the state-run news agency Xinhua reported Tuesday.
Huang bought nine ducks at the Yanjiaoqingong Market in Langfang City of Beijing's neighboring Hebei Province Dec. 19, then cleaned the ducks' internal organs before giving three ducks to other people, the Beijing Municipal Health Bureau said.
"According to the initial investigation, a total of 13 people have eaten the duck and Huang is the only one who fell ill," Zhao said. "We had thoroughly combed all stands in the market."
He said the ducks' origin was traced to Jixian County in Tianjin Municipality.
No bird flu case had been found in Langfang City so far, a spokesperson with China's Ministry of Agriculture told Xinhua.
The Beijing Municipal Health Bureau said 116 people were in close contact with the victim. One healthcare professional in contact with Huang contracted bird flu, but recovered, the bureau said.
Beijing officials reported the incident to the World Health Organization, as well as health authorities in Hong Kong and Macao. Official also have begun bird flu prevention and control measures.
Gene sequencing by the National Avian Influenza Reference Laboratory indicated that this virus is very similar to the avian influenza virus variant isolated in Shanxi in 2006.
A 19-YEAR-OLD Beijing woman who suffered the city's first human case of bird flu since 2003 has died, municipal health authorities said yesterday.
The sequencing comments above are from the OIE report on the H5N1 from the Jiangsu outbreaks last month, followed by the most recent confirmed human case in China (see updated map), who died yesterday. The “virus variant” is clade 7, as seen in phylogenetic analysis of A/chicken Shangxi/02/2006 in the latest WHO on vaccine targets. On the same branch is A/Beijing/01/2003, which was isolated from the first H5N1 confirmed case in mainland China.
It is likely that the latest case was also infected by clade 7 H5N1, which is vaccine resistant because most vaccines target clade 1 or 2, which are significantly different than clade 7.
The details of the Jiangsu outbreaks are unclear. Some local media reports suggested the outbreak was large and involved dead poultry that was shipped to market. Other reports acknowledged the H5N1 infection, but noted that the infected birds were asymptomatic and identified through routine screening.
However, if the latest case was fatally infected by clade 7 from an asymptomatic duck, then the potential for additional cases is high because the virus can travel and dispress undetected. Moreover, since clade 7 is resistant to the most common vaccines, it could be widespread.
Clade 7 was also reported in Vietnam in August, and a clade 7 target was recently identified by the WHO. There currently is no clade 7 pandemic vaccine, and the selected target is only proposed as a target, as indicated in the most recent WHO vaccine update. The earlier presence of clade 7 in Vietnam may be related to the recent confirmed human case in northern Vietnam, which was announced today.
Thus, more detail about the H5N1 in Beijing and Vietnam, including sequence data from these isolates, as well as the WHO vaccine target, A/chicken/Vietnam/NCVD-016/2008, would be useful.
The presence of vaccine resistant H5N1 in asymptomatic ducks in Beijing would create significant pandemic concerns.

SHANGHAI -- A 19-year-old Beijing woman has died of bird flu, the first human case of the virus in China since February last year, the government said Tuesday, putting public-health officials on higher alert for a possible resurgence of the disease this winter.
The woman, who lived on the outskirts of China's capital, succumbed Monday morning to the H5N1 strain of avian influenza, according to a statement by the Beijing health bureau. It didn't say how she became infected.
Mao Qun'an, the spokesman for the Ministry of Health, said public-health officials have forecast a likely increase in avian influenza among birds this year, raising the possibility of more human infections.
"We have been paying great attention" to the possibility of human illnesses, Mr. Mao said. But, he added, because bird flu isn't readily transmissible to people, there is "no need to get nervous."
Bird flu remains primarily a threat to poultry, not humans, but the World Health Organization and national public-health authorities remain on guard because the virus has the potential to mutate into a more contagious form and spark a pandemic.
Also on Tuesday, health officials in Vietnam said an 8-year-old girl had tested positive for the disease, the Associated Press reported. It was the first human case reported in that country in nearly a year.
Human infections with H5N1 peaked in 2006, when 115 cases -- 79 of them fatal -- were reported to the WHO. In 2008, there were 40 cases of human infection with H5N1, leading to 30 deaths -- a tiny number compared with the death toll from ordinary flu.
Since fall, outbreaks of avian influenza in birds have been reported in countries including Vietnam, Thailand, India, Togo and Germany. One person died of bird flu in Indonesia in November, and a teenager in Egypt died of the disease in December.

Chinese health officials said they have stepped up monitoring and infection-control measures since the Beijing woman's diagnosis. People who had close contact with the woman have been placed under medical observation, but none have shown any sign of infection, officials said. An official in the village where the woman lived said disinfection teams had visited the area Monday.
The Ministry of Agriculture and local agriculture officials said they have received no reports of avian-flu outbreaks in poultry in or around Beijing. Last month, the authorities ordered the slaughter of more than 350,000 chickens after routine testing of eggs turned up signs of bird flu in an east-coast province, more than 1,000 kilometers from Beijing.
Nyka Alexander, a spokeswoman for the WHO in Beijing, said the Chinese government had informed the organization of Monday's death.
Authorities in Hong Kong -- where tens of thousands of chickens were culled in December after dead birds tested positive for H5N1 -- as well as in Macau and Taiwan also were alerted, China said.

One report said the victim cleaned all 9 birds.
H5N1 in a HCW is a major red flag. UPI said the HCW had bird flu. Several reports said the nurse had a fever. No report said she had a fever due to something other than bird flu.
Clade 7 has a LOT of RBD changes and some are in patients from Indonesia.
We do not know with certainty that this is a Clade 7.
I don't think anyone knows with certainty for the latest human case, but the Jiangsu outbreak is clear, while Shangxi in 2006 and Beijing in 2003 are public.
For the latest case, the timing and location strongly point toward clade 7. I would be VERY surpised if this isn't a clade 7 case.
