      Relatives say a 38-year-old man has died of bird flu, raising the unofficial toll in the world's hardest hit nation to 111.

      Abdul Kadir said Thursday his brother-in-law, Asnawi Sandri, died on July 10. He had symptoms of the disease, including high fever and coughing, when he was hospitalized.

      Kadir says doctors told the family the father of two died of bird flu.

      The above comments from an AP wire report confirm local media reports on a lab confirmed H5N1 case in Tangerang. The lab confirmation was announced on July 12 and carried in local media. Thus, the confirmation has been acknowledged in local media and one wire service, but has not yet been acknowledged in the WHO update.

      Since this lab confirmed case has not been acknowledged as an official case, it is unclear if contacts were tested.

      The last WHO update is on the death of the last official case, who died in May, but was not reported until June, in violation of International Health Regulations on H5N1 in humans, which is a reportable disease. The WHO update in June failed to acknowledge the death of the brother of the confirmed case, who was diagnosed with typhus, or the hospitalization of a second brother. Both brothers had bird flu symptoms, but the index case was diagnosed as typhus and the second brother tested negative. However, the two fatalities had the same cyanosis of the extremities, leaving little doubt that both were H5N1 infected.

      The report of the one case was delayed for a month and associated with an announcement by Indonesia that they would no longer announcement lab confirmed cases at the time of confirmation. The above cluster was preceded by two additional clusters which were also reported as individual clusters, with index cases dying from lung inflamation or dengue fever.

      The failure of Indonesia or WHO to acknowledge the clusters, which almost certainly involved human to human transmission, remains a cause for concern.

        Dozens of Chickens in Temanggung Affected with Bird Flu


        Liputan6.com, Temanggung; Dozens Of chickens in the Gemawang Village, Temanggung, Central Java, just recently, was found died suddenly. The resident was increasingly restless after two local village children suddenly the high fever.

        The resident suspected, the two children were affected by bird flu. The Dinas official of Temanggung Livestock Breeding that got the report segara checked to the location.

        From results of the fast test was known, chickens that died positive was infected by the bird flu virus. To interrupt the virus link, the official destroyed hundreds of the resident's poultries.

          LANGSA- Yusniar (50) the resident of the Village Antara Desa Seuneubok Teungoh, Kecamatan Peureulak Timur, that was known as the patient suspect bird flu had finally been reconciled to the Hospital of Adam Malik Medan, on Saturday (26/7) after could be treated in RSU Langsa, on Friday (25/7) in the III class JPS space.

          Yusniar was evacuated by the Peureulak Timur Community Health Centre side with the patient's family. However, because minin him means and the infrastructure in RSU Langsa, finally the patient was severed to be reconciled to Medan. Beforehand, Yusniar experienced the deviation to his face that became red and his body mengigil.

                    The Public Health Ministry said yesterday [30 Jul 2008], 4 people in
                    Phichit province who were suspected of having contracted the bird flu
                    virus have tested negative for the disease.

                    Deputy permanent secretary Dr Paijit Warachit said he ordered the
                    mobile rapid disease investigation unit to collect blood samples from
                    the 4 for testing against the H5N1 strain of the virus at Nakhon
                    Sawan's provincial Science Centre. The laboratory results found that
                    none of the 4 had the H5N1 virus, which can be lethal to humans.
                    Three of them are suffering from seasonal influenza, while the 4th
                    has no flu whatsoever.

                    The 4 people tested [included a 6-year-old child, a 10-year-old
                    child, and 2 women aged 62 and 70. They all remain under observation
                    in Phichit's Sam Ngam Hospital. The 6-year-old does not have the flu.
                    The 4 reported they had come into contact with dead chickens in their
                    village before falling ill. Three days later, they had a high fever,
                    and it was feared they had been infected with the H5N1 strain of the
                    bird flu virus.

                    Paijit has asked healthcare volunteers in Sam Ngam district to
                    monitor the symptoms of the relatives of the 3 patients with seasonal
                    flu for the next 12 days. If any suspicious symptoms are found, they
                    will also be sent to hospital immediately. He also urged villagers to
                    avoid cooking food using chickens or ducks that have died
                    suspiciously. Villagers should incinerate any such poultry in order
                    to minimise the risk of the H5N1 virus spreading to other areas, he
                    said.

                    [The 4 suspected human cases of avian influenza reported in "Avian

                    not been confirmed.

                    It remains true, therefore, that there have been no confirmed human
                    cases of avian influenza in Thailand since 27 Sep 2006 (and none
                    anywhere for several months
                    ). There is also no confirmation that
                    avian influenza was responsible for the deaths of local poultry.
                    Thailand's last outbreak in poultry was reported to the OIE (World
                    Organisation for Animal Health) in January 2008. In March, a "final"
                    report was submitted, stating that the outbreak was resolved.

                            The 4 suspected human cases of avian influenza reported in "Avian influenza, human (57): Thailand (Phichit), susp 20080730.2336" have not been confirmed.

                            It remains true, therefore, that there have been no confirmed human cases of avian influenza in Thailand since 27 Sep 2006 (and none anywhere for several months).

                            The above comments are from today's ProMED commentary on suspect cases in Thailand, as well as a July 19 commentary on media reports on an H5N1 confirmed case in Indonesia.

                            Recent reports that three of the five suspect cases in Thailand had tested positive for seasonal flu decreased the likelihood that the patients were H5N1 positive.

                            However, the comment that there had been no confirmed H5N1 cases anywhere for several months is curious. The situation in Indonesia is far from clear, and the lack of transparency has been a cause for concern for some time.

                            Indonesia declared H5N1 endemic in their last OIE report in the fall of 2006. Although the endemic declaration lowered required reporting frequencies of H5N1 in poultry to once every six months, Indonesia has not filed a report since 2006.

                            In early 2007 they halted the shipment of clinical samples from suspect H5N1 patients and the number of samples sent for confirmation since early 2007 has been limited to a few patients. Moreover, there have been no public human H5N1 sequences from Indonesia
                            since early 2007.

                            This year there were a number of obvious clusters which were reported as individual sporadic cases. Index cases were said to have died from lung inflammation, typhus, or dengue fever. Media questions on these index cases have led to cluster denials and a new policy of delayed reporting.

                            However, IHR regulations require reporting of human H5N1 cases to be made within 24-48 of confirmation, which was noted when Indonesia announced its new policy of delayed reporting. These concerns led to announcements that reporting would be done on a monthly basis.

                            However, the last government report was made in mid June on two patients. One patient, who was part of a cluster, died in mid-May, while a second case died in early June. Thus, the reporting for both cases violated IHR regulations.

                            WHO had indicated that they had a good relationship with Indonesia, even though sequences and samples were withheld. WHO maintained that it had no control over Indonesian notifications to its citizens, but WHO had not received any indication that Indonesia had changed policy on notifications to WHO.

                            Thus, it remains unclear as to when Indonesia is notifying WHO. For other countries, WHO publishes a situation update shortly after they receive notice of a confirmed case. The long delays following lab confirmation of the past two cases raises concerns that either Indonesia is not notifying WHO in a timely manner, or WHO is delaying situation updates for Indonesia, or both.

                            On July 10, another patient in Indonesia died, She was lab confirmed on July 12. However, Indonesia refused to deny or confirm the confirmation two weeks ago.

                            It has now been three weeks since the patient died and six weeks since Indonesia issued its last update.

                            The delays in reporting or acknowledging these lab confirmed cases by Indonesia, WHO, and ProMED remain causes for concern.

                              Suspect Flu Burung di Tangerang Meninggal

                              Tangerang: After could be treated two days in RSUD Tangerang, Joko (19), the resident of the Village of Jambu Tegal, RT 06/02, the Village Jambu Great, the Rejeg Subdistrict, the Tangerang Regency that was stated suspect bird flu, on Thursday night (31/7) died.
                              According to Anang, 55, casualties's father, the Joko indication was affected by bird flu it was said the doctor in RSUD Tangerang. The "big possibility" of "the bird flu virus came from the chicken that died suddenly," he said during the Joko burial, on Friday (1/8).
                              Four days before died Joko suffered flu and was run off with to the clinic of Citra Raya, Kecamatan Cikupa, Kabupaten Tangerang. Because never recovered, on Tuesday night (29/7) casualties were run off with to RSUD Tangerang to be treated. Because his condition has been serious, Joko was placed in isolation space.
                              Section Head Kesehatan of the Tangerang Regency, Hany Heryanto admitted to getting the report. However Hany still could not confirm whether casualties died resulting from bird flu or not. (SM/OL-06)

                                Can't find Rejeg Subdistrict on a Tangerang Regency map. Googling it results in a map of Rejeg identified in a neighboring Regency (Labak), in the 'sticks', a tiny village at the edge of a rice paddy.
