Re: Indonesia: Health Ministry Summary of Bird Flu Cases June 26 2007

From Dep of Health Indonesia

With the article comes a table and a diagram, may be some one who is smarter with IT than I am, can copy them..

Two Bird Flu years in Indonesia, 101 Victims

On July 05 2007

Already 2 virus years H5N1 had infected humankind in Indonesia since the first case was found in Tangerang, in June, afterwards died in July 2005.

But, the virus that since 2003 infected the poultry in Indonesia this, still became the source of the discussions material that was interesting in the international community with various debates concerning the method of controlling him.

Unfortunately, from all these discussions, much less the method of controlling him, his virus procedure personally still was difficult to be depicted by the international experts.

The government has tried to drop the number of casualties off, but help of the community that important to wage war on Bird Flu.

From four semesters of the bird flu outbreak, the number of cases of the year to two, clear descended the amount compared to the first year.

If in two first semesters (July 2005-June 2006) the number of residents who was infected by bird flu 59 people,

the number in two semesters to two (July 2006-June 2007) descended as far as 29%, to 42 people.

The table 1 along with showed the number of cases of Bird Flu to humankind had a basis the semester.

Hoped with the control that continued to be carried out, especially with help and the community's co-operation, the number of residents who was infected by Bird Flu could continue to be pressed.

The department of the Health tried as well as possible to reduce the number of residents that tertular the Bird Flu virus, that was spread from the poultry to humankind.

With help of the Regional Government, casualties Bird Flu descended the amount.

However, in fact the key to the prevention of the spreading of the virus H5N1 from the animal to focusing humankind in independent efforts the community to maintain the cleanliness himself and the environment, as well as separated the poultry from the settlement of humankind, especially for them who had or raised poultries.

So that the community was avoided from Bird Flu, only awareness of each one of the community's members personally that could ascertain the resident to carry out efforts that were suggested by the government.

Independent efforts the community must be also supported by the regional government, because of the local government that was given by the authority and responsibility to respond to the report on the community.

The follow-up pelaporan also must be supported by the regional government, especially in the level of the government most close to the community like the village head, the district and the subdistrict.

Moreover, the regional government that have authority and was responsible towards the handling of the poultry in their respective territory.

At this time, already 11 provinces that his resident was affected by Bird Flu, like appearance to the table 2 along with.

The last province, Riau, just got infected last May 2007.
Not only the province that already infected that must try hard to prevent the further spreading.

The not infected provinces also must make an effort prevented so that his territory is free from the possibility of the spread of Bird Flu from the poultry to humankind.

Prevention efforts became very important in the control because Bird Flu to humankind was the very deadly illness.

Observed Case Fatality Rate or the percentage of the sufferer Bird Flu that died compared to the number of the total case (79%) like appearance to the table 1, or, from every time 5 sufferers, 4 people died and only 1 surviving person.

Moreover, some sufferers were late carried to health means of receiving first help.

One of them was Tamiflu giving that only was effective if being given in time 48 hours or oldest 2 days from when the sign began to be shown by the sufferer.

Tamiflu personally, not killed the virus but hindered the development of the virus.

So, the community must be on the alert and preceptive towards signs cough-pilek, the fever, hot high through to more than 38?C, was sick the throat, and especially if deliberately or not deliberate, bersinggungan or was affected by the waste/the liquid from the poultry that was sick or the poultry carcass.

The diagram 1 pointed out CFR that increased from the semester to the semester.

The virus H5N1 indeed very deadly, this was important to be known by the community.

For reasons of this, the government, especially the Department of Health tried hard informed about the preceptive need towards the spreading of Bird Flu.

Moreover, Bird Flu was very potential caused the pandemic, that is the death of many short time insiders to the wide area.

Precisely because Indonesia did not yet enter the pandemic stage, this during that was exact to prevent the death of millions of people who definitely caused the big loss for our nation.

Re: Indonesia: Health Ministry Summary of Bird Flu Cases June 26 2007

Indonesia’s 103th Bird Flu Case Confirmed

Jakarta, August 13, 2007 - Ministry of Health

A female bead-embroidery worker, “NLP” (F, 29 years old) from Tukad Aya Hamlet, Negara sub district, Jembrana District, Bali Province was confirmed as Indonesia’s most recent human H5N1 case.

The tests conducted by the Research and Development Center of the Ministry of Health and the Eijkman Insitute in Jakarta on August 12, 2007 and August 13, 2007, respectively, confirmed that “NLP” is infected by the H5N1 virus. “NLP” is the 82th fatalities in the country and is the first human bird flu case confirmed in Bali.

There are 12 provinces that have recorded human bird flu cases with the total number of 103 confirmed human H5N1 and 82 fatalities. With this case, the human H5N1 case fatality rates now stands at 79.6%, which means from 5 people infected by bird flu, only 1 person survives.

Public Communication Center receives this information from the Bird flu post of Director General of Disease Control & Environmental Health, Ministry of Health on August 13, 2007. As for today, Bali Provincial Health Office together with the surveillance team from Ministry of Health are investigating the case in the field.

NLP experienced onset of illness on August 3 2007 with symptoms of fever, coughing with phlegm, and shivering. She was examined at a private nurse practice on August 6, 2007.

On August 7, 2007 she was examined at a private doctor practice. Victim was examined at the Negara sub-district hospital on August 8-10, 2007 and diagnosed with serious pneumonia. On August 10, 2007, NLP was taken to Sanglah Hospital with fever up to 40 degree Celcius, coughing, and breathing difficulties.

NLP died August 12, 2007. Earlier, D (F, 5 year-old), NLP’s daughter died on August 3, 2007 from pneumonia. From the investigation, on early-mid July 2007, chickens raised by NLP and her neighbor died suddenly. D played with the chickens before she died.

From the Livestock office laboratory, it is confirmed that the dead chickens were H5N1 positive.

On July 17, 2007, the Livestock Office did poultry culling at Jembrana District because the area is bird flu endemic to poultry and an outbreak in poultry occured.

At the moment, NLP neighbour, PN (F, 2 years 9 months) is being treated as a bird flu suspect at Sanglah Hospital. PN experienced onset of illness on August 10, 2007 with symptoms of fever and cold.

PN is treated at Sanglah Hospital since August 12, 2007 and was given Oseltamivir
.

Until now, sick birds remain the the primary source of human infection with Bird Flu. Because the risk of contracting bird flu from poultry to human, it is advised for people to keep their poultry away from home.

Be aware of sick poultry or suddenly dead poultry around you. People are urged to learn more about of bird flu and to reduce their risk of contracting the virus by adopting clean and healthy behaviors:

Don’t touch sick or dead chickens. If you have had contact with sick or dead chickens, promptly wash your hands with soap and report the incident to your village head. Wash your hands and cooking appliances with soap before cooking. Cook chicken and eggs well. Separate poultry from humans. And also separate new poultry from existing poultry flocks for 2 weeks.

Go to a Public Health Center or Hospital immediately if you have flu and fever symptoms after having contact with poultry/chicken.

Re: Indonesia: Health Ministry Summary of Bird Flu Cases August 16 2007

Official Statement from Indonesian Department of Health on the 103rd case from Bali

NLP, 103rd AI Sufferer
14 Aug 2007
According to observation at laboratory of National Institute of Health Research and Development (Balitbangkes) MOHRIon Sunday, August 12, 2007, NLP (f, 29 years) who lived in Dangin Tukad Aya Sub village, Negara Sub district, Jembrana District, Bali Province was positive of H5N1 virus. NLP was a bead-embroider. In addition, on Monday, August 13, 2007, Eijkman Laboratory also stated she was positive of Avian Influenza (AI). Therefore, she is the 82 nd person died by H5N1 virus infection and the first AI case in Bali . Until August 13, 2007, there are 12 provinces reporting AI case among human with 103 total cases. The CFR (Case Fatality Rate) is 79.6 %. It means from 5 cases, only 1 is alive.

This information is received by Center for Public Communication from AI Outbreak Post of DG of Disease Control & Environmental Health MOHRI until August 13, 2007. Today, Bali Provincial Health Office and MOHRI surveillance team go to location for an investigation.

NLP started her onset on August 3, with symptoms of fever, cough with sputum, and shiver. On August 6, she went to private practice of nurse and on August 7 to private doctor practice. On August 810, she was hospitalized in Negara District Hospital and on August 10, referred to Sanglah Hospital with diagnose of severe pneumonia with symptoms of high temperature (up to 40oC), cough with sputum and difficulty in breathing. She died on August 12, 2007. Previously, her child, D (5 years old), died on August 3 by pneumonia. According to observation, on July there were hers and her neighbor's chicken suddenly died, and previously D played with those chicken. Laboratory of Farming Office found that dead chicken surrounding her house were positive of AI.

On July 17, 2007, farming office conducted avian cull at Jembrana District for AI endemic and outbreak among avian.

Now, her neighbor, PN (f, 2 years 9 months) is hospitalized in Sanglah Hospital as AI suspect. She began her onset on August 10, with symptoms of high temperature and influenza. Since August 12, 2007, she was hospitalized in Sanglah Hospital and provided with oseltamivir.

---------------------------

Transmission source of AI still comes from avian. For the big risk factor of AI transmission among human, people are suggested to separate their avian from settlement. They should be careful and aware of sudden dead and sick chicken. Preventions are important to avoid AI infection.
The preventions include:
1. never touching sick or dead chicken. if not, cleaning body with soap and reporting it to RT/RW (lowest administrative unit) or head of village.
2. using water and soap to wash hand and kitchen utensil, and always cook avian meat and eggs well.
3. separating avian from human as well as new avian from old ones for at least 3 weeks.
4. going to health center or hospital (especially referral hospital for AI) when finding symptoms of influenza and fever, especially after having contact with avian.

16 Aug 2007

There in another Avian Influenza (AI) sufferer. L (f, 17 years), who lived at Tangerang District, Banten, is the 104th AI positive case and the 83rd dead victim, based on observation in Laboratory of National Institute of Health Research and Development, while result from Eijkman Laboratory is still awaited.

Therefore, in Banten there are 14 AI positive cases, with 12 of them die. AI case in Indonesia cumulatively has reached 104 cases, with 83 people die. The Case Fatality Rate (CFR) is relatively high (79.81%).

This information is received by center for Public Communication from AI Outbreak Post, DG of Disease Control & Environmental Health MOHRI at August 16, 2007.

There has not been effective drug for AI. Recent drug, oseltamivir, is only to obstruct virus growth. The only effective way is by avoiding AI infection.

The transmission still comes from avian. For the high risk factor of AI transmission among human, people are suggested to separate avian from settlement.

They should be aware of sudden dead and sick avian. When they find it, they should report it to RT/RW, head of village, health center or local health/farming office.

According to data from Sari Asih Hospital , L, who lived at Jl. Danau Laut Tawar No. 1 Tangerang District, had her onset on August 8, 2007 with symptoms of fever and difficulty in breathing.

She was hospitalized in Sari Asih Hospital, Karawaci, Tangerang on August 13, 2007 and died on August 14, 2007.

Her x-ray reveals covering on right lung. Now, surveillance team from Tangerang Dsitrict Health Office goes to location for anamneses to her family, while risk factor remains unknown.
