Doctor claims he was asked to lie about dengue deaths

Mumbai: Last week, a report in MiD DAY spoke of the ongoing dissension between the municipal corporation and some NGOs over the number of reported dengue cases in the city, with the latter pointing out that figures released by the BMC fall far short of accurate ('Is dengue really on the decline in Mumbai?' December 7).
Turns out the NGOs aren't the only ones who feel that the civic body is trying to manipulate figures to mislead the masses.
A doctor who is a member of the Maharashtra Malarial Death Review Committee has added credence to their claims, admitting that he and his colleagues have been facing constant pressure from officials of BMC and the State Health Department to avoid mentioning dengue as the primary cause of death while they review cases.
The Malarial Death Review Committee consists of several doctors and experts appointed by the state health department and the BMC. Its primary responsibility is to review the cause of death of a malaria or dengue patient and then declare the primary cause of death.
The doctor who requested anonymity, said, "We at the committee are asked to review the actual cause of death of malaria and dengue patients. Though the death certificate issued to a victim may already mention the cause of his death - both primary, such as dengue, and morbid, such as the organ failure stemming from dengue - a case can come to our committee if the BMC or the state health department feels that there is need for reviewing."
Asked if BMC's statistics regarding dengue deaths were accurate, he said emphatically, "One word answer to this question is 'NO'. The actual numbers are much higher than the declared ones. Being in the committee, I know very well how many people die due to dengue."

Secondary infections make Dengue deadlier

KOLKATA: With dengue fear still looming large over the city and its suburbs, virologists have found out the three viruses that helped the disease take epidemic proportions in the state.
According to them, t hree serotypes of the virus - DEN1, DEN2 and DEN3 - were in circulation during the dengue outbreak a few months back.
While primary infection by any of these strains causes the self-limiting disease, doctors, entomologists and virologists suspect that a secondary or dual infection had added to the severity of the infection this time.
Though the official number of dengue deaths was recorded at 11, more than 30 patients who were detected with dengue died during the outbreak.
The health department, however, reportedly refused to register the deaths of patients in private hospitals as triggered by the infection.
"The mortality rate due to dengue infection is just about 0.25%. When it is combined with a secondary viral infection or dual infection by more than one strain of the dengue virus, the severity could be alarming. Those below the age of 15 are most vulnerable and are likely to get dengue haemorrhagic fever or dengue shock syndrome. Now, if a person with other underlying health complications dies after he is infected by dengue, it is not fair to blame dengue as the cause," said Dr Shyamalendu Chatterjee, senior virologist with Indian Council of Medical Research's (ICMR) viral unit.

Kolkata records high cases of Dengue in 2012 than other cities in India

New Delhi: The mosquito-borne dengue fever is mostly affected all metro cities of India but Kolkata leads among the metros with the highest number of dengue cases (3321). Chennai with 988 cases and Mumbai with 907 cases in 2012.
Dengue spread increased by 283% in Mumbai, by 100% in Chennai in 2012 when compared with 2011. Where as Delhi dengue numbers increased rose 199 in 2011 to as high as 3,321 in a year.
Delhi, has shown the highest circulation of the virus serotype 2 which causes hemorrhagic fever and hence records a high mortality rate.
Dr AC Dhariwal, chief of India’s National Vector Borne Disease Control Programme, told TOI that the genetic analysis of the virus strain conducted by the National Centre for Disease Control (NCDC) has found that it is of genotype 4 that didn’t exist earlier.
Dr Dhariwal said, “Type 2 strain of dengue is known to result with severe manifestations of the disease. On the other hand, we have seen a completely new genotype this season.”
Dr Shobha Broor, head of microbiology at the All India Institute of Medical Sciences (AIIMS), added, “Delhi is seeing an outbreak of type 2 dengue virus which is the most dangerous type. Chandigarh too is facing a similar type 2 outbreak. Genotype tells us how the virus is changing. Since genotype 4 hasn’t occurred earlier, the population in Delhi is more susceptible to getting infected and they aren’t naturally protected against this particular type,” Dr Broor said.

Neem-papaya juice passes dengue test

CHENNAI: After more than 37,000 dengue cases, including 227 which resulted in death, across India, a grandma's antidote for the virus seems to be getting scientific endorsement. A herbal concoction — juice of papaya leaf, common neem and hill neem — being given to dengue patients in government hospitals in Tamil Nadu has been found to have anti-viral properties. Tests conducted at the King Institute of Preventive Medicine found that the Siddha preparation brought down symptoms and speeded up the recovery of patients.
Scientists dropped the four strains of the dengue virus on the cell lines (drawn from the gut cells of dengue causing albopictus mosquito) and added the herbal preparation. Normally, the virus would have entered the cells. "After two days, we found the cell-lines were unaffected. The virus was neutralized by the herbal formulation. Further tests showed it blocked the virus from entering the cell. In cells where the virus had already entered, it prevented multiplication," said King Institute director Dr P Gunasekaran.

Outbreak of Dengue Across Major Metro Cities
An increase in the number of dengue cases during current year was reported from major metropolitan cities of the country.
Dengue Cases reported from four metropolitan in the current year upto 11th December, 2012 and in the corresponding period in 2011 are given below:

The Government of India advises and helps the States/UT Governments in tackling vector borne diseases, including dengue, through the National Vector Borne Diseases Control Programme. The Government of India has taken the following steps for prevention and control of Dengue in the country:
1. Technical Assistance:
a) A Long Term Action Plan was formulated and circulated to the State/UTs for implementation in January, 2007.
b) A Mid Term Plan was developed for prevention and control of Dengue and approved by the Committee of Secretaries on 26-05-11. This plan was also circulated to the State/UTs for implementation.
c) National guidelines for clinical management of cases have been sent to the States for circulation and use in all hospitals.
2. The dengue situation in the country is closely monitored and reviewed with the States/UT Government.
a) Union Health & FW Secretary reviewed the situation in the States with the State Health Secretaries on 12th September, 2012.
b) Committees of Secretaries (CoS) reviewed the situation through video conferencing in Karnataka, Kerala, Maharashtra, Pudducherry, Tamil Nadu and West Bengal on 05th October, 2012.
c) The Union Minister for Health &FW reviewed the situation with the State Health Ministers of Delhi and Haryana on 08th October, 2012.
d) The situation in Andhra Pradesh, Karnataka, Kerala, Tamil Nadu and Puducherry was reviewed by the Union Minister for Health & FW on 12th October, 2012. at Chennai.
e) A review with Mission Directors, NRHM was undertaken on 31st October, 2012.
3. In view of the importance of the issue, the Union Health & FW Minister had written on 31st May, 2012 to all CMs of States & UTs to tackle the Dengue problem. An Advisory was earlier issued by the Union Health & FW Secretary on 26th March, 2012 to all Chief Secretaries and by Director NVBDCP to all Director, Health Services on 28th February, 2012.
4. Field visits were carried out to assess the preparedness and to provide technical guidance to the States.
5. Training is imparted to clinicians on case management as per GOI guidelines and to other health care functionaries on programme activities.
6. Diagnostic Facilities Provided:
(a) For augmenting diagnostic facilities, the numbers of Sentinel Surveillance Hospitals (SSHs) with laboratory support has been increased to 347 across the country from 110 and linked with 14 Apex Referral laboratories with advanced diagnostic facilities for back up support for Dengue across the country.
(b) ELISA based IgM test kits were provided to these institutes through National Institute of Virology, Pune free of cost. During current year (till 16.11.12), a total of 2312 dengue diagnostic kits (1 Kit= 96 tests) have been provided by Centre to SSHs.
(c) ELISA based NS1 test kits for early detection of cases from 1st day of disease can be procured by the States/UT Governments locally using NRHM/NVBDCP funds.
(d) to(f) Controlling dengue is an ongoing activity under the National Vector Borne Disease Control Programme (NVBDCP).
The situation is closely monitored for the prevention and control.
The GOI has taken steps as mentioned in the answer to the para (c) of the question.
This information was given by Minister for Health & Family Welfare Shri Ghulam Nabi Azad in written reply to a question in the Rajya Sabha today.

New Delhi - When the monsoon season ends in India, locals breathe a sigh of relief as the pools of standing water dry up, and the mosquitoes lose their breeding grounds.
This season's official tally of people who fell ill with dengue fever, transmitted by the Aedes aegypti mosquito, was the highest for years at more than 37,000, with 227 recorded fatalities.
But experts say the figures could belie a much higher count, as many cases go undetected or unrecorded.
US-based dengue specialist Scott Halstead says dengue probably strikes at least 1 million Indians per year, extrapolating from data in Thailand and Vietnam, where 0.1 per cent of the population is estimated to catch the disease in any given year.
Even assuming a similar rate to nearby Sri Lanka or Indonesia, both reporting cases in 0.05 percent of the population each year, would put India's real rate above half a million cases annually.
“The government is not making any serious efforts to calculate the true number of people infected,” says Halstead, an adjunct professor in the Department of Preventive Medicine and Biometrics at the McMaster University in Rockville, Maryland.

Girl succumbs to dengue in Madurai

MADURAI: A 13-year-old girl, who had been suffering from dengue, succumbed at the Government Rajaji Hospital (GRH) here on Monday. Swasthiga, 13, daughter of Natarajan of Anna Street in Palanganatham, tested positive for dengue and succumbed in the morning. Sources from the hospital said she had been suffering from fever for several days. She was treated in a private hospital in Madurai before being admitted in GRH on Saturday afternoon. Since she was found with symptoms of dengue, her blood sample was collected and sent to a virology lab in the Department of Microbiology in Madurai Medical College.
The results obtained on the next day confirmed dengue. The girl was isolated and the required treatment was provided to her. Her condition deteriorated on Sunday night following which intensive treatment was provided. However, the girl succumbed to dengue with shock syndrome at 8.45 am, not responding to the treatment, said doctors from the hospital.
Meanwhile, Manimegalai, 13, another girl succumbed to suspected dengue in the hospital. Manimegalai, daughter of Pandiaraj of Sandhaiyur from Peraiyur block in the district, had been suffering from fever. She was taken to a hospital in Srivilliputhur for treatment, where her health condition did not improve. She was brought to GRH on Sunday night. However, she died on Monday morning not responding to the treatment.

GURGAON: Dengue cases trebled in 2012 as compared to the previous year. If private hospitals' reports are to be taken into consideration, the figure for dengue fever broke all previous records. Besides that there was no slowdown in malaria cases in 2012 drawing attention to poor sanitation conditions of the city.
Health department officials said that 469 cases of dengue and one death were reported in 2012, while 599 malaria cases and six swine flu cases were reported this year. However, according to private hospitals, around 2,000 dengue cases had been reported, besides seven deaths. Last year, 156 dengue cases were reported.
The huge gap in figures the government and private hospitals reflected defensive attitude of the district health department rather than focusing on preventive measures.
