      Guillain-Barre is a diagnosis of exclusion; there is no test that "confirms" Guillain-Barre, the case is declared Guillain-Barre when all other possible diagnoses have been excluded.

      I would like to see poliovirus (both wild and vaccine-derived), other enterovirus, several other pathogens (including H1N1), and several toxins conclusively excluded by the CDC from the samples they have been sent in the next few days before I conclude this is a cluster of Guillain-Barre cases. And then, at that point, the hard work would begin to try to explain why. Keep in mind we still can't agree on whether and why the 1976 swine flu vaccine may have caused GBS.

              Unfortunately, the more likely cause for AFP with that prodome is an infection, not an autoimmune response. Enteroviruses are highly suspect, and polio needs to be excluded. There are also some toxins that could produce such a syndrome, but that seems less likely due to the wider geographical spread. Campylobacter infection should also be considered (see below).

              Apparently, this has been going on since early September.

                [ProMED has reported a few clusters of Guillain Barre Syndome (GBS)
                before (see below). GBS is thought to be an aberrant immune reaction
                elicited by an infection or, in rare cases, immunizations. Previous
                studies have linked GBS to infections with campylobacter, but a
                cluster of 30 cases closely related in time and geographical area
                would mean that many cases should be diagnosed with the infection, but
                without GBS, and only upper respiratory tract symptoms are reported
                here.

                The risk of GBS after influenza vaccination is estimated to be 0.8 per
                million doses administered after the pandemic H1N1 vaccine (see below)
                and cannot explain this cluster of cases. ProMED will be happy to post

                  The title says Nayarit in Spanish and English, but perhaps an error by the author? They may mean Naranjal? As most of the cases accept 1 in Nogales appear to be in Veracruz? On the opposite side of Mexico...
