    Measles is back
    Pierre Pelchat
    The Sun

(Quebec) region of Quebec has just experienced its first cases of measles since 1996. Notifiable disease, measles was confirmed in 19 people in the region in recent weeks. For Quebec, there are 26 people infected with measles virus.

"They are mostly infants and young people in their twenties and thirties whose vaccination had not been completed. There have been hospitalizations for tracking children and to avoid cases of dehydration, "said Tuesday at Sun, Dr. Jasmin Villeneuve, the Regional Directorate of Public Health.

Although it refuses to speak of an epidemic, the situation is sufficiently worrisome to bring public health precautions. Thus, doctors' offices and emergency hospitals were notified of the presence of measles cases in the region.

"People who have fever and redness are invited to provide this information upon arrival at the clinic or emergency room. They will be given a mask or they will be segregated. Measles is a highly contagious disease. If a child is sick, we try not to expose other children at daycare, "said the doctor.

It is estimated that the virus can stay alive in the air in a room two hours after the passage of an infected person.

The disease can seriously affect people whose immune systems already weakened by other health problems, even if they were vaccinated. For the same reason, pregnant women are also at higher risk of developing the disease.

"Once a case is confirmed, we investigated to find out who the patient was in contact and inform those whose immune system is weakened a possible onset of disease and complications that may arise" said Dr. Villeneuve.

"For children who were not vaccinated, they can make antibodies to fight disease," he said. The vaccine is administered to children from the age of 12 months. Earlier, this protection is not effective.

    French origin

Everything indicates that the outbreak of measles in Quebec origin of Europe, especially France. "After investigation, we were able to trace cases from France," said the director of public health protection for Quebec, Dr. Horacio Arruda.

For three years and particularly since early 2011, the number of measles cases in France has increased considerably. More than 14,500 cases were reported since 2008 and over 3,000 in March alone. The epidemic affects mainly the south-east.

In France, only 60% of residents were vaccinated, while in Quebec, the inoculation rate is around 95%. "This shows that vaccination is the best tool possible to prevent an epidemic and the importance of vaccination in our day," argued Dr. Arruda.

In Quebec, we do not expect that the number of measles cases increased dramatically, due to the high rate of vaccination. "It should disappear by itself," argued Dr. Villeneuve.

Even if they have not been vaccinated, it is considered that people aged 50 and older are protected because they have already been in contact with the disease before the vaccination programs are introduced in the early 70 .

"Measles is like a cold. There is a fever, runny nose and conjunctivitis [eye irritation]. After three or four days, the redness will appear on the body. It lasts four to seven days. There is no treatment. That part alone. In young children, make sure they are well hydrated, "said Dr. Villeneuve.

The disease can cause major complications, including encephalitis. We are talking about one death per 1000 cases of measles. In the past, Quebec has experienced years of 10 000 to 12 000 cases of this disease before the introduction of vaccination programs.

    By the numbers

    >> 95% of Quebecers are vaccinated against measles

    >> 60% of French people are vaccinated against measles

    >> 3000 cases reported in France in March

Public health officials in the Quebec City region are dealing with a minor outbreak of the measles.

Of the 36 cases reporting in the province, 19 are in the capital region, and officials are now warning people who have not been vaccinated for measles to make an appointment.

The vaccination rate in Quebec is 95 per cent, according to Dr. Horatio Arruda, the head of prevention for Quebec's public health department.

Arruda said he didn't expect the current outbreak to spread, as it did in the 1989 measles epidemic when 10,000 Quebecers became sick and several died...

(Sherbrooke) After touching the Drummondville, measles has now reached the Eastern Townships. The Directorate of Public Health (DSP) Estrie confirmed the presence of cases in the Estrie region, on Tuesday, including in Sherbrooke.

On the territory of the CSSS Drummond, 196 cases have been confirmed so far, including 16 in the territory of the CSSS Arthabaska-and-l'?rable. It is therefore not surprising that the Eastern Townships have contracted the disease, according to Dr. Suzanne Menard, medical adviser to the DSP of the Eastern Townships.

"In Quebec, the measles virus is circulating. There are periodic reports that are made. We have not finished talking about, "says Dr. M?nard, who would not reveal the exact number of cases here yet.

Notifiable Disease (NND), measles is characterized by high fever, runny nose and cough, as well as redness. The most frequent complications associated with it are the ear and lung infections.

However, it may cause more serious complications, including pregnant women, people with compromised immune systems (such as people with HIV or patients receiving chemotherapy) and infants under one year.

        Measles cases rise
Posted by Krystel Dub? CKOI 106.9 Mauricie Sunday, June 5, 2011 at 7:38.

Quebec is grappling with a rise in measles cases. Since the beginning of the year, 250 cases were recorded in the province. It is in the Mauricie and Centre-du-Qu?bec that the outbreak is greatest.

64% of sufferers are young people aged between 10 and 19. The vaccine is the only way to protect against measles.

Moreover, the Department of Health and Social Services will review the situation on Monday.

The last major outbreak of measles in Quebec dates back to 2007 when seven regions had been affected.

Quebec public health officials are dealing with a growing measles outbreak in the province, with 254 reported cases since the beginning of 2011, according to the latest data.

Most of the measles patients are in the Mauricie and Central Quebec, but 5 cases have been reported in Montreal and 26 in Quebec City.

Health authorities are expected to provide a full update on the situation Monday afternoon, and they are already advising the population that the best tactic against measles is vaccination...

          Measles epidemic spreads in Quebec
254 cases; disease leapfrogged from Europe
By Charlie Fidelman, Gazette Health Reporter June 6, 2011 1:49 PM

MONTREAL - An epidemic of measles, a highly contagious disease that's leapfrogged here from Europe, has already affected 254 people in Quebec, and officials expect more infections as the virus spreads among people who have not been vaccinated.

Outbreaks have been reported in England, Wales and France. The outbreaks are often cited as a legacy of a now discredited study by a United Kingdom doctor who scared parents away from immunization.

Quebec's last measles outbreak, in 2007, sickened 94 people. The 254 cases cited Monday are for the last six months...

Quebec is in the midst of a resurgence of measles with 700 cases so far this year.

Public health officials are urging parents to check their children's vaccination records.

So far this year there have been just 11 cases of measles on the Island of Montreal but it's a different story to the South and West of the Island.

There, 140 cases are reported and letters are being sent home through the schools asking parents to check vaccination records...

Measles cases among vaccinated Que. kids raises questions about vaccine schedule

TORONTO - Measles cases have surged in parts of Canada and the United States this year, with cases among unvaccinated children and teens driving the high numbers, public health officials from both countries will tell a major infectious diseases conference this weekend.

But an unusual observation from a large outbreak in Quebec may raise some alarm among those who attend the conference, the annual meeting of the Infectious Diseases Society of America.

An investigation into an outbreak in a high school in a town that was heavily hit by the virus found that about half of the cases were in teens who had received the recommended two doses of vaccine in childhood ? in other words, teens whom authorities would have expected to have been protected from the measles virus.

It's generally assumed that the measles vaccine, when given in a two-dose schedule in early childhood, should protect against measles infection about 99 per cent of the time. So the discovery that 52 of the 98 teens who caught measles were fully vaccinated came as a shock to the researchers who conducted the investigation.

"That's the real question. How could that have happened?" said Dr. Gaston De Serres,...

                Portrait epidemiological outbreak of measles

                Epidemic curve

As of October 12, 2011, 12 h, 747 cases of measles were reported to the Office to Monitor and watch (BSV) MSSS since 3 April 2011, at the moment there is a sustained local transmission. Before this date, 12 cases reported between January 1 and April 2, 2011 are sporadic cases many of which were exposed to the disease during a stay abroad and they will no longer be considered in the epidemiological picture.

Since the last update October 5, 2011, two new cases were reported. There are:

                * 674 cases confirm?s1
                * 73 cases cliniques2

The cases occurred during the period, from April 4 to October 8, 2011. Over the past three weeks, from September 18, 2011, 10 cases have been reported and, in the Mont?r?gie region of Montreal, the Laurentians and the Eastern Townships. It is possible that other cases are reported later in view of the school year and the incubation period of the disease is 7 to 14 days ...

Transmission of the disease and affected areas

The first reported cases of travelers who had contracted the disease during a stay in Europe. Local transmission began in April 2011 and from May, sustained transmission has been installed in schools and continued until late June. The current outbreak is mainly driven by people who contracted the disease in Quebec.

To date, 10 regions reported at least one case, and the region of the Mauricie and Centre-du-Qu?bec reported alone, 70% of cases, 525 of 747 cases. The Mont?r?gie region is second with 146 cases or 19.5% of cases ...

Cumulation, it is estimated that nearly 42% of cases are associated with transmission in schools and 33% in the community compared to only 2% that would have contracted during a trip. 21% of cases, the source could be identified and documented.

                Age and Sex

The majority of cases (56%) were male. The age of those affected vary from 3 months to 54 years, the average age being 15 years. Found a high proportion (63%) of young people aged from 10 to 19 years ...

                Morbidity cases

One in nine (11%) required hospitalization. This proportion is higher among young children aged 12 to 17 months (25%) and young adults 30 years or more (24%).

Complications have been reported for 64 cases (9%), most likely including 27 cases of respiratory pneumonia.

                Vaccination status of cases

It is estimated that 81% (604) patients were not considered protected (immune) or they had received no dose of vaccine, or they had no proof of vaccination or that they were not eligible for vaccination, or that the information was not known to the subject. Among them, there are 29 children under one year who had not yet reached the age to receive their 1st dose of vaccine with three of them who received one dose of vaccine post-exposure.

In addition, 12 cases have been reported in persons deemed immune because of their age (born before 1970)...

                    Vaccination history of cases

It is estimated that in 605 cases (81%), the patient could not be considered protected (immune) for one of a variety of reasons:
                    • Had never received the vaccine
                    • Had no proof of vaccination
                    • Was not eligible to be vaccinated
                    • Did not know whether he or she had ever been vaccinated or contracted the disease
Of these, 29 were infants under age one, the minimum age to receive the first dose of the vaccine.

                    Measles vaccination in Qu?bec

The regular vaccination schedule recommends that the first dose of MMR (measles-mumps-rubella) vaccine be administered at 12 months and the second at 18 months. The effectiveness of the MMR vaccine is over 95% after two doses and protection against the disease is lifelong.
To protect the public, a school-based vaccination campaign is currently underway. To find out more, see the school-based measles vaccination campaign page.

Health officials are asking parents of school-aged children to fill out a pamphlet, noting whether their child has had both doses of the vaccine.

In the case of an outbreak in a school, all unvaccinated children will be asked to leave school for a couple of weeks, until the outbreak is over.

Health officials estimate 35,000 children in Montreal will need one or both shots.
