    The correspondent of the MAI in the wilaya of Adrar reported, quoting several sources, the deaths of two women, which occurred two days ago, in the resort of Agjett, under the common Tawaz (45 km East of the town of Atar), following an illness, which remains unknown.

    The caller also stated that two members of the family are still under medical supervision in hospital Atar, stating that their health has improved, according to the doctor.

    The deaths of two women coincided with the death of camels in the same area. The veterinary services of the Wilaya could not, so far, determine the cause of these strange deaths.

    Some citizens expressed concern that these deaths are due to consumption of contaminated meat.

    Such a hypothesis has however been rejected by local authorities who have stated that this may be a malaria affecting individuals physically weak, noting that neighbors of the victims who ate the same meat, are healthy.

    The authorities noted that women who have died, have passed away, a week after consuming the meat "suspicious".

    According to a report by a veterinary mission that visited in recent days, the site concerned "no general symptom has occurred in animals" in this region.

    The report concluded that it is impossible to confirm the existence of a disease, without the detection of specific symptoms and conducting tests on samples taken from animals exhibiting these signs.

    These same services have also mentioned cases of deaths in the ranks of camels in other mughatas the wilaya, which they believe requires intensified surveillance measures against the contagious animal diseases.

    The correspondent of the MAI in the Wilaya also reported that the Wali of Adrar, Saidou Sall, in a statement Monday in Atar, indicated "that the victims died after suffering from fever and muscle pain ".

      So we appear to have three fatalities, two sisters and a brother, and at least one (and possibly three) additional cases. Symptoms mentioned so far are fever, joint and muscle pain, and vomitting, which is really too non-specific to suggest a specific etiology. The incubation period seems to be very short as well, as all the illnesses and deaths have occurred in the past four days.

      CCHF is consistent with this description (and possible illness in camels), and has been reported from Mauritania before:



      Rift Valley fever is also consistent with both the illness in humans and aminals, and it too has been reported in Mauritania:



      There was also a report on an unidentified hemorrhagic illness in Nouakchott last year:



      Anthrax and plague would also be good symptomatic matches, and do exist all over the world, but I cannot find a report of either illness from Mauritania.

        The above thread is from two weeks ago and also is from Nouakchott, mentioning four undiagnosed fatalities in a prison within 48 hours, but with no symptomatic description provided.

          This source suggests six total cases, all with roughly simultaneous onset, raising the possiblity of a poisoning as well.



          Adrar: Fever unknown [kills] two girls

          The inhabitants of the town of Amogjar, located 35 km north of Atar, was surprised last Saturday (October 30) by a fever that has hit several people at once. Vomiting, joint pain and death of two young girls 15-16years. The Regional Directorate of Health in Adrar dispatched the Chief Medical Officer of PMI Atar on the scene. Having consulted all present (a dozen tents), the doctor brought a serious case is currently hospitalized in the hospital in Atar and Nouakchott during evacuation. During the mission, three other people came after him to the hospital in Atar. The case of the latter do not appear serious, as the hospital director.

          While parents speak ill of food poisoning due to consumption of camel meat for several days or insecticide sprayers left by the anti-locust, doctors can not seem to determine the disease. They did several blood tests and others, that they are being sent in specialized laboratories.

              [The 2 newswires above mention that the clinical presentation of
              these cases has been one of high fever, arthralgia (joint pains), and
              vomiting. Locally, there are numerous hypotheses being proposed for
              the possible etiology of these deaths and illnesses, including
              malaria, and an association with the consumption of camel meat, with
              rumors that there may have been a camel die-off in the area where the
              fatalities resided. As with many as yet undiagnosed outbreaks
              associated with high fatalities, newswire reports often present
              conflicting pieces of information reflecting a level of panic and
              concern among the population and a health sector that is trying to
              reassure the population. This appears to be the case where there are
              rumors of a camel die-off, consumption of camel meat that is
              suspected to be contaminated, and results of a field trip that
              theoretically identified only one camel death in the area.

              Just looking at the reported symptoms, high fever, arthralgia, and
              vomiting, with an observed high case fatality rate, the differential
              diagnosis would be a lengthy list of diseases including those of
              viral, bacterial, and parasitic etiologies, and including diseases
              that are known zoonotic as well as those not known to be zoonotic.


              The rumors surrounding a possible concurrent camel die off in the
              area combined with a history of consumption of camel meat increases
              the temptation to put known zoonotic diseases higher on the
              differential diagnosis (this presumes a probable contact with the raw
              meat and blood of animals who may have been infected with viral
              agents such as Crimean-Congo hemorrhagic fever and Rift Valley
              fever). But it is possible that these cases are not related to
              possible diseases in the camels in the area.

              It should be noted that the Adrar region (province) of Mauritania is
              in the Saharan desert (the majority of the country is located in
              desert and semiarid regions), with migratory populations that live in
              encampments near water sources. From the above given information, it
              sounds as though this outbreak is occurring in one such encampment.
              While rain is scarce in this region, sudden deluges do occur. Keeping
              that in mind, another disease that comes to mind with the
              presentation of arthralgias, vomiting, and fever is leptospirosis.


              One can keep discussing the factors in favor of one or more of the
              possible etiologic agents, but at this point it seems prudent to
              await further information from knowledgeable sources in the region on
              results of laboratory studies conducted on the hospitalized cases.

                Whatever this is, it is almost certainly spread by the camels themselves, as we have reports of a couple more deaths with contact with camels. CCHF is quite likely.

                    This post suggests that this may be a false alarm - the three (or four?) deaths are blamed on malnutrition, and those ill are stated to have recovered. No indication of which pathogen is suspected to be the cause of the fever, nor what the suspected cause of the camel deaths is.
