      Deaths at Aoujeft following a "mystery disease"
      15/11/2010

      Nine people died in different localities Aoujeft (moughataa the wilaya of Adrar) as a result of an "unknown illness" revealed Mohamed Ould Elmoctar Hmein Amar Mayor Aoujeft. He said one death was recorded Monday, November 15 in the locality of "Tennoumert" ... Where two people died yesterday.
      . Other deaths from this "unknown disease" had been reported several days ago by the Mauritanian media.

                NOUAKCHOTT — An outbreak of Rift Valley fever has struck the north Mauritanian town of Aoujeft, causing "loss of human life" and killing cattle, official sources said, while the press put the death toll at 17.

                "Cases of Rift Valley fever have caused loss of human life and at the level of cattle," the official Mauritanian news agency AMI reported, without giving casualty figures.

                According to the independent local press, the "epidemic" has claimed 17 lives and sick people have been sent to hospitals in Nouakchott.

                "The ministers of health and the interior, visiting the region, have given strict instructions to the people of Adrar (where Aoujeft lies) to avoid consuming locally produced meat and milk," AMI added.

                These bans on foodstuffs essential to the local population are expected to last until laboratory analyses and tests that have been under way since last Thursday on contaminated animals are complete, the agency said.

                Rift Valley fever is a disease transmitted by infected mosquitos that mainly affects livestock but can also contaminate human beings, particularly young men who are looking after diseased cattle.

                The death rate is usually below two per cent of infected people, since Rift Valley fever is generally a mild disease among people, causing headaches and muscle pain and flu-like fever, according to the World Health Organisation.

                No specific treatment is available and there is no human vaccine for Rift Valley fever, which gets its name from the part of Kenya where it was first identified in 1931.

                        Amazing. Please rename and move this thread.

                        In post #3, RVF was one of the leading suspects, and the ProMED post from November 4th also mentioned RVF as a possibility.

                        Thanks to all our newshounds, we had word on this outbreak a MONTH before the international media. Somewhere between 12-15 of the deaths are explicitly described in this thread (depending on your interpretation of the translations).

                          Rift Valley fever is a disease that primarily affects animals but can

                          against the disease, which is transmitted by mosquitoes and affects

                          Rift Valley fever bears the name of the region of Kenya where it was
                          identified in 1931. The disease has hit Kenya, Somalia and Tanzania
                          in the years 1967/68, and the last major outbreak was reported in

                          Rift Valley fever (RVF) is a viral zoonosis that primarily affects
                          animals but also has the capacity to infect humans
                          . Infection can
                          cause severe disease in both animals and humans. The disease also
                          results in significant economic losses due to death and abortion
                          among RVF-infected livestock.

                          RVF virus is a member of the _Phlebovirus_ genus, one of the 5 genera
                          in the family _Bunyaviridae_. The virus was 1st identified in 1931
                          during an investigation into an epidemic among sheep on a farm in the
                          Rift Valley of Kenya. Since then, outbreaks have been reported in
                          sub-Saharan and North Africa [Mauritania]. In 1997-98, a major
                          outbreak occurred in Kenya, Somalia and Tanzania, and in September
                          2000, RVF cases were confirmed in Saudi Arabia and Yemen, marking the
                          1st reported occurrence of the disease outside the African continent.

                          The vast majority of human infections result from direct or indirect
                          contact with the blood or organs of infected animals. The virus can
                          be transmitted to humans through the handling of animal tissue during
                          slaughtering or butchering, assisting with animal births, conducting
                          veterinary procedures, or from the disposal of carcasses or fetuses.
                          Certain occupational groups such as herders, farmers, slaughterhouse
                          workers and veterinarians are, therefore, at higher risk of
                          infection. The virus infects humans through inoculation, for example
                          via a wound from an infected knife or through contact with broken
                          skin, or through inhalation of aerosols produced during the slaughter
                          of infected animals.

                          There is some evidence that humans may also become infected with RVF
                          by ingesting the unpasteurized or uncooked milk of infected animals.
                          Human infections have also resulted from the bites of infected
                          mosquitoes, most commonly the _Aedes_ mosquito. Transmission of RVF
                          virus by hematophagous (blood-feeding) flies is also possible. To
                          date, no human-to-human transmission of RVF has been documented, and
                          no transmission of RVF to health care workers has been reported when
                          standard infection control precautions have been put in place
                          . There
                          has been no evidence of outbreaks of RVF in urban areas.

                          While most human cases are relatively mild, a small percentage of
                          patients develop a much more severe form of the disease. This usually
                          appears as one or more of 3 distinct syndromes: ocular (eye) disease
                          (0.5-2 percent of patients), meningoencephalitis (less than one
                          percent) or haemorrhagic fever (less than one percent).

                          The total case fatality rate has varied widely between different
                          epidemics but, overall, has been less than one percent in those
                          documented. Most fatalities occur in patients who develop the
                          haemorrhagic icterus form.

                            Amogjar Fever: The Wali on the scene

                            The sudden death this morning [5 Nov 2010] of a camel herder and that
                            yesterday's guide-camel, whose body was burnt publicly by the
                            veterinarian, has relaunched with a vengeance the serial "weird fever
                            of Amogjar."

                            To appreciate the situation himself, to inquire for news of affected
                            families following the death of some of their members and to calm
                            somewhat heated minds, the Adrar Wali visited the disputed area this
                            morning ([Thu 4 Nov 2010]).

                            The visit [of the Wali] comes after the [visit of the Chief Medical
                            Officer of PMI from Atar the day before yesterday [3 Nov 2010] and
                            the visit of the Regional Health Director (DRASS) yesterday [4 Nov
                            2010]. The Wali was accompanied by the Hakem d'Atar, the regional
                            army leaders, the gendarmerie, the police, the representative of CSA
                            and those from the [Ministry of the] Environment. We recall that upon
                            his return last night [3 Nov 2010], the DRASS [Regional Health Team]
                            brought with them 2 young children that were immediately admitted to
                            the hospital in Atar.

                            In the beginning of the winter season when there is usually a
                            resurgence of diseases which affect both humans and animals, some
                            people are concerned about the scourge that killed several animals in
                            Inchiri, Tagant Dhar Adrar (Ain Savr) and now Baten (Atar-Amogjar),
                            that it might be more widespread.

                            Many families[in fact have claimed] some losses due to sudden deaths
                            in their livestock.

                            [In the 1st post on this outbreak, there was mention of 3 deaths and
                            2 additional cases in this outbreak, with presenting symptoms of
                            fever, arthralgias and vomiting. There were also conflicting reports
                            on the involvement of camels in the affected area.

                            In this follow-up newswire there is mention of one additional human
                            death, and it appears as though there have been livestock deaths
                            occurring in recent times in the affected area and at least one camel
                            death (that was cremated/burned). Reviewing prior reports from
                            Mauritania, ProMED has posted on outbreaks of Rift Valley fever (RVF)
                            and Crimean-Congo hemorrhagic fever (CCHF) in Mauritania. The scant
                            information that has been available on this current outbreak does not
                            mention hemorrhagic phenomena associated with the fatalities -- while
                            not ruling out CCHF, the added fact that clinical disease has not
                            been report in CCHF infections of camels, it makes this diagnosis less likely.

                            If in fact there has been an increase in livestock (caprines,
                            camelidae and possibly bovines if there are the herders in the
                            general area) mortality in the affected areas, it is possible that
                            there may be ongoing RVF in the affected zones.

                            Once again, there are many other etiologies for a clinical
                            presentation of fever, arthralgias and vomiting that should still be
                            considered in the differential diagnosis. We await results of further
                            studies, as well as more information on the possibility of a
                            concurrent increase in livestock mortality.
