Pekanbaru Teen in Critical Condition, Coma After Contracting Bird Flu
A17-year-old boy from Pekanbaru, Riau, is now in a critical condition after being infected with the deadly bird flu virus.
“The patient is in a critical condition and in a coma,” said Azisman Saad, head of the bird flu team at Arifin Achmad General Hospital in Pekanbaru.
Azisman said the boy, identified only as JF, was isolated in intensive care after suffering high fever and trouble breathing.
“He needs a breathing machine to survive,” Azisman said, adding that the provincial health agency had sent the boy’s blood sample to the Ministry of Health and the test result came back positive for H5N1 avian influenza.
Evy, one of the boy’s relatives, said the family did not know where JF could have contracted the virus because he had no history of direct contact with sick poultry.
Evy said JF fell into coma on Friday after experiencing some bird flu symptoms, including trouble breathing, fever and diarrhea.
Azisman said the hospital had handled two other positive cases of bird flu since December, both occurring in children below the age of 5. One of the patients died on Dec. 31, while the other was recovering and had been released from isolation.
Azisman said a 5-year-old girl from Indragiri Hulu died because she received treatment too late and was brought to the hospital in a very bad condition.
Tjandra Yoga Aditama, director general of communicable diseases at the Health Ministry, said 20 people had been confirmed as having contracted bird flu in 2009, of which 19 had died.
The findings meant Indonesia remained the country with the highest number of bird flu cases and fatalities. Since the outbreak first emerged in 2003, 137 people have died in Indonesia, while the virus has killed less than 300 people worldwide. http://thejakartaglobe.com/news/peka...ird-flu/351922

"Previously these patients was treated for two days in the hospital of Santa Maria. Lalau when his condition was critical, just referred to Arifin Achmad Hospital," said Dr. Azisman Sa'ad, Chairman Tim Bird flu Arifin Ahmad Hospital, as confirmed Tribune, Sunday (10 / 1).
About YF conditions, Azisman explained, the patient was in critical condition. Can be said in a coma.
"It's very hard situation. We have carried out according to the procedure implementation stylist still MOH full potential," said Azisman.
Explained, 150/80 patients tekananan blood, 38.5-degree body temperature, respiration 60 times per minute and the pulse 146. Pulmonary condition was whitened all. Handling is being done by Azis is the installation of a ventilator to assist breathing.
"So the status of the patient when we received was ARDS (Acute Respiratory Distress Syndrome, ed). But we still try to the maximum," said Azisman.
Azisman said, diperolahnya of information, the patient had contact with dead birds while still in Painan. But this is denied sister YF, initials K.
"He was never in contact with dead chickens. Also there are no chickens died suddenly at around the house," said K.
Suspect case of bird flu were experienced by the second YF at the beginning of this year 2010. Previous Arifin Ahmad Hospital also treat patients suspect bird flu called Anisa (5) from the village of Bangkal Malang, Kelayang district, Indragiri Hulu. Anisa finally declared healthy despite bird flu postif stated.
Watchful
The last two cases is certainly worth considering aware some time ago in Pekanbaru reported last about ten days in chickens died suddenly in RW 6 RW 9 Sub-District and Kulim. This information is presented head of the Office of Agricultural Pekanbaru, Sentot Djoko Prayitno the Tribune last week.
Began to find again the case of birds died suddenly, and there are victims who became suspect bird flu, according to Dr. Azisman make a sign from the outbreak of bird flu virus spreading. This is one possible cause is the beginning of the rainy season.
"All you need do is, if people find any please drop dead birds be reported to the authorities. Especially when accompanied by the citizens of sudden high fever. Please hurry reported, either to the RT, RW, health center, or to the hospital and the government the other, "continued Azisman.
According to Djoko Prayitno Sentot, on rainy weather, the virus can easily develop. Avian influenza (AI) is well developed at a temperature below 22 degrees. At the temperature 25 degrees he was already disabled. Whereas at temperatures above 30 degrees he was dead.
Cases of bird flu in poultry is commonly found in backyard poultry in the 500's tail. As for the maintenance of large scale it found no cases. However, a clear poultry around the discovery of chickens died suddenly had to be destroyed.
Bird flu is an infection by the virus H5N1 subtypes of influenza A (H = hemagglutinin; N = Neuraminidase), so far found no scientific evidence of human transmission. But the current state of the bird flu virus had a mutation in the human experience that can lead to spread from human to human. But certainly this disease can infect humans, through the polluted air of the virus.
People who show bird flu symptoms as ordinary flu, including fever, cough, sore throat, shortness of breath and sometimes accompanied by diarrhea, but her condition rapidly declined drastically. If not immediately help, the patient could die.
Family Convinced Kesurupan
The family suspect bird flu patients YF, YF trusted if possessed by spirits. Not suspected bird flu diagnosed sperti Arin Ahmad Hospital doctors. This was disclosed when found pasein family in a side alley Tribune isolation room Arifin Ahmad Pekanbaru hospitals.
Told the patient's family, one day last week, YF go swimming at a public pool in the area of Jalan Hang Tuah. YF go with some other family members. According to family members, after swimming YF fever. YF body temperature higher. YF could even say the words that can not be understood by many people. And then he almost fainted.
"We believe he was kemasukkan (keserupan, red), while swimming yesterday afternoon. Look at his body's not that hot. We believe the negative (-red bird flu)," said the patient's initials biological brother K.
Told the family, they've asked YF disease to 'smart people'. So, smart people say the language was issued YF is the language of Satan. Not another human language.
"We wonder what he was saying, when he should speak Minang," said one family YF.
The family was then still pursue the path of alternative medicine, through the smart people. However, efforts are also still running medical. "What must we worry. But we certainly hope he can return semubuh. Because he should have had to go to school," said K.
When Tribune visited, some family members seem worried YF waiting in hospital corridors. Among those who sat there on the mat in the hall dibentang hospital, about 50 meters from the isolation room. (HNK)

The patient suspect YF bird flu (17), from Painan, Pesisir Selatan, West Sumatra, in the critical condition through to this news was written, on Sunday (10/1) struck 20,00 of WIB. Pasien was treated in space of the bird flu isolation of RSUD Arifin Ahmad Pekanbaru, since Saturday (9/1). Observation of the Stand, from outside isolation space, YF terbaring weak and was installed the implement ventilatar to help his breathing. YF was in Pekanbaru since two weeks ago to go on holiday in his older brother's house in Pekanbaru. However, two days ago, he was attacked by the hot fever. His family in Pekanbaru carried him to RS Saint Maria, on Friday (8/1). The family's information, the YF condition increasingly worsened in the hospital. YF until unconscious. Finally YF by the side of RS Saint Maria was reconciled to RSUD Arifin Ahhmad, on Saturday (9/1).
"Beforehand this patient has been treated two days in RS Saint Maria." Lalau has during his condition been critical, just was reconciled to RSUD Arifin Ahmad, said Dr Azisman Sa'ad, Chairman Tim of the Handling of Bird Flu of RSUD Arifin Ahmad, during it was confirmed the Stand, on Sunday (10/1). Concerning the YF condition, Azisman explained, the patient was in the critical condition. Could be said in the comma situation. It was "quite very heavy that his situation." We carry out penata laksanaan in accordance with the procedure continue to the Department of Health as optimally as possible, said Azisman. Explained, tekananan the patient's blood 150/80, the temperature of the body 38.5 levels, breathing 60 times per the minute and the pulse 146. The condition for the lungs has faded all. The handling that was carried out according to Azis was the ventilator fitting to help breathing.
"So the status" of the "patient during us received already ARDS (Acute Respiratory Distress Syndrome, red)." However we continued to try maximal, said Azisman. Azisman said, from information that diperolah him, the patient could contact with the poultry died during still was in Painan. However this was denied by the older brother YF, had the initials K. "He had not been touching with the chicken died." Also did not have the chicken died suddenly around the house, said K. The case suspect bird flu that was experienced by YF was what was second in the beginning of this 2010. Beforehand RSUD Arifin Ahmad also treated the patient suspect bird flu was named Anisa (5) from the Village of Bangkal Malang, Kecamatan Kelayang, Kabupaten Indragiri Hulu. Anisa had finally been stated healthy although being stated postif bird flu.
Be on the alert Dua this last case ought to be definitely guarded against considering some time before in Pekanbaru it was reported around the last ten days was found by the chicken died suddenly in RW 6 and RW 9 Lim Ku- districts. This information was sent by Pertanian Pekanbaru Section Head, Sentot Djoko Prayitno to the Stand, last week. From the re-discovery of the case of the poultry died suddenly and had casualties that to suspect bird flu, according to Dr Azisman made the sign begin to spread him the spreading of the bird flu virus. This of one of his causes was to begin him the rainy season. "That must be carried out was, if the resident found the existence" of the "poultry died suddenly helped immediately it was reported to the authorised side." Moreover if being accompanied with the existence of the feverish resident high suddenly. Please fast was reported, whether to RT, RW, Puskesmas, or to the hospital and the other government side, continued Azisman.
According to Sentot Djoko Prayitno, in the weather of the rain season, the virus could easily developing. The virus avian influenza (AI) this developed good in the temperature was supervised by 22 levels. In the temperature 25 levels he has begun to be non active. Whereas in the temperature on 30 levels he died. The case of bird flu to the poultry generally is found in the maintenance of the poultry under 500 tails. Whereas for the large scale maintenance precisely was not was found by the case. However, it was clear the available poultry in and around the discovery of the chicken died suddenly that must be destroyed. Bird flu was the infection by the influenza virus A the Sub- Type H5N1 (H = hemagglutinin; N = Neuraminidase), until this was not found by scientific proof the existence of the spread between humankind. But in this current situation the bird flu virus did not yet experience the mutation to humankind that could result in the spreading from humankind to humankind. However that was certain this Illness could attack humankind, through dirtiest air the virus.
The person who was attacked by bird flu showed the sign as being affected by common flu, including the fever, the cough, was sick the throat, breathless and sometimes was accompanied by diarrhoea, but his condition was very fast descended drastic. When immediately was not helped, the sufferer could die. The Yakin Kesurupan family of the patient's Family suspect YF bird flu, more trusted if YF kerasukan the supernatural being. Not the assumption of bird flu sperti that was diagnosed by the doctor RSUD Arin Ahmad. That was revealed by the family pasein when being met the Stand in the path samping space of the bird flu isolation of RSUD Arifin Ahmad Pekanbaru. Told by the patient's family, one day last week, YF went to swim in the public's swimming pool in the region of Street Hang Tuah. YF went with several of the family's other members. According to the family's member, sepulang swam YF the fever. The temperature of the YF body increasingly high. YF in fact could say words that could not be understood by many people. In fact at that time he was almost unconscious.
"We were convinced he this kemasukkan (keserupan, red), during yesterday swam at noon." Saw his body would not hot that. We were convinced the negative (flu birds-red), said the brother of the patient's uterus had the initials K. Told by the family, they have asked about the YF illness to the 'clever person'. Then, the clever person said the language that was dismissed by YF was the devil's language. Not the natural language again. "We were surprised also what he said, in fact necessarily he spoke Minang," said one of the YF families. The previous then family continued to try to get the route of alternative medical treatment, through the clever person. However medical efforts also continued to be undertaken. "That was certain we worried." But we definitely hoped he could semubuh came back. His matter necessarily he must have gone to school, said K. When the Stand visited, several people of the family's YF member appeared worried was waiting in the hospital path. Among them there are those that sat termenung on the mat that dibentang in the hospital path, around 50 metre from the room of the isolation. (hnk)

We will know eventually if WHO is being notified because WHO tabulations are always based on the onset date. As note by Commonground in a post above at least two of these individuals had symptom onset in December of 2009. If WHO updates the cumulative table to include these two cases in 2009, then Indonesia is submitting the data to WHO. If the WHO updates don't include these two cases in 2009 or they are included in a future cumulative count by the Indonesia MOH (Depkes) for 2010, it will be obvious that WHO is not receiving official data from Indonesia on the number of human infections of H5N1 nor the details of those infections.

Re: Riau, Sumatra: 1 confirmed H5N1 bird flu death; total of 3 confirmed cases
My current analysis on this 17 yo old.
Post #2 is from a site called Jakartaglobe. This is a new site for me, but I haven't been tracking Indonesia for about a year now....so it may not be new at all.
At the bottom of the article, I believe it states two references, or initials of the poster, and one reference:
Quote:
JG, Antara
We have the Antara article posted here, dated 1/10 (which is also the date of the Jakartaglobe article). The Antara article is Post #13. No where in the Antara article does it state the patient was positive at any given time. The reason we thought they said he was "positive" was because of the paragraph underneath a title within the body of the article labeled "Positive"
Quote:
Which is translated as:
Quote:
“Until now we don’t know how he could be infected because there was no bird death around his house. So far JF is studying in West Sumatera,” said Evy. Positive.
Azizman stated since December 2009, Arifin Achmad hospital had treated three bird flu cases. Two patients were toddlers from Kabupaten Indragiri Hulu (Inhu) and the other was JF from Pekanbaru.
So, in my notes, I will record this information. He is currently negative.
We have two definite confirmed in Riau, the two toddlers from Indragiri Hulu (Inhu).
