Kotamobagu ? A family of 10, consists of 4 females and 6 males, rushed to Kandou hospital in Manado, Sunday (4/9) because of H5N1 suspicion. The family resides in Kelurahan Gogagoman.

The family developed high fever and then were brought to Datoe Binangkang hospital and later transferred to Kandou hospital.

The patients are now recovering, but so far there is no examination result of the cause of the disease.

In the meantime, about 25 chickens in Gogagoman died and tested positive H5N1 infection. The chickens died-off without showing common bird flu signs such as bluish comb/wattle and subcutaneous bleeding of fetlock, which is raising a suspicion of possible virus mutations, told the authority of Livestock Service in Kotamobagu.

The reports (here and elsewhere) seem to indicate that all individuals were from the same family (apart from one HCW? contact), and that all were symptomatic initially at the time of being placed in isolation.

What is not indicated is how many family members did not develop symptoms from this initial group; if every member of the family became ill at the same time, it is concerning as this could indicate a very high transmission rate - regardless of the type of flu involved. One would presume that rapid tests were positive to have prompted this action by the authorities?

Any further information on this family group, and especially the numbers of those who did not become symptomatic following exposure would be welcomed.

Bird flu virus (H5N1) has hit back in Kotamobagu. This time the attacks are far more sinister than the first event in late April. The first attack only resulted in dozens of poultry that had to be destroyed, this time 11 people in Kotamobagu had to be sent and treated in the department of special (General Hospital), Prof. dr Kandou, Malalayang Manado. After chickens were tested positive for H5N1 in Kotamobagu , apparently people who could have been in contact with the chickens were checked for flu-like symptoms.

10 people were feverish, they were sent to hospital in Manado. 1 person is escorting the 10. Their condition seems to improve, the fever seems to go down.

At first glance it looks like precautionary measures were taken, no serious health problems are reported, test results are pending.

This now seems inaccurate from the later reports and it would seem that instead chickens died and were tested, and family members became ill with high fevers and sought medical assistance independently - it also sounds as if necessary isolation procedures could not be put into place in the early stages. Although the media reports do imply that since that time proactive monitoring of locals has been put into place in the area where the chickens died; however, it isn't clear from these reports when the chickens died and how many days passed before these individuals became ill.

The reports about the suspect cases are also a little confusing; Up to 10 patients were reported as being from Gogagoman, but later reports imply at least two locations and several family groups. Additionally, one contact of one of the patients is reported as being in isolation and it is not clear who this might be, so I thought I would try to break this down so we can add to lists later if necessary.

Some further information on the evolution of the outbreak in local poultry and leading to the isolation of the family group.

It seems that the first chicken died on the Monday 29th August, and that Family 2 were close neighbours of family 1, and that all of them would have had the same direct exposure to infected chickens at a meal, so with the exception of the single person who one of the reports has indicated was isolated because of their contact with one of the suspects, any h2h transmission would seem unlikely. The date of common exposure would seem to be the 1st September, so if this is indeed human H5N1 infections, it would indicate an incubation period of around 48 - 72 hours. Addendum - A second set of samples have been sent to jakarta so if 'usual' practice is followed, the patients will be released and declared negative as soon as negative tests are achieved post tamiflu treatment for all in the group.
