OF PATIENTS ARE ADMITTED FROM DHALPUR BPHC AND 1)ONE) NO. OF PATIENT ADMITTED FROM BIHPURIA BPHC. THE AGE GROUP OF THE PATIENTS ARE......... 1. 11-20 YRS: 1 NO. 2. 21-30 YRS: 1 NO. 3. 31 -40 YRS: 3 NOS. 1. 5(FIVE) NOS. OF BLOOD SAMPLES WERE COLLECTED FOR LIVER AND KIDNEY FUNCTIONING RESR. 2. 4(FOUR) NOS. OF RECTAL SWAB SAMPLES WERE COLLECTED AND SENT TO ASSAM MEDICAL COLLEGE AND HOSPITAL FOR CULTURE AND SENSITIVITY TEST. RESULTS AWAITED. 1. MICROBIOLOGIST,IDSP(L) VISITED THE NORTH LAKHIMPUR CIVIL HOSPITAL AND MET THE PATIENTS AND COLLECTED RECTAL SWAB SAMPLES. 2. DSO INFORMED THE IC DHALPUR BPHC AND IC BIHPURIA BPHC AND REQUESTED HIM TO SEND THEIR HEALTH WORKERS TO THE AFFECTED AREAS FOR AWARENESS OF NOT TAKING ANY TYPE OF MUSHROOMS DURING THIS SEASON. 3. DSO(L) INFORMED THE IC SIMOLUGURI SHC AND REQUESTED TO TAKE PREVENTIVE MEASURES IN THE AFFECTED AREAS. ALSO HE IS REQUESTED TO KEEP CONSTANT TOUCH WITH DSO(L). 4. DR. S.SARMA, IC SIMOLUGURI SHC ALONG WITH A MEDICAL TEAM VISITED THE AFFECTED AREA AND REPORTED NO FURTHER CASES WERE FOUND IN THAT AREAS. Investigation Open PATIENTS ARE IMPROVING.

The affected people are small children. O.T. Test Positive CHC level RRT team investigated the cases. Active house to house search was done. Cases of diarrhea with vomiting reported from one household of the village. The symptoms started due to consumption of stale food and contaminated drinking water. Health education given regarding consumption of freshly cooked food and clean drinking water. Adequate amounts of antidiarrheals, ORS packets, and chlorine tablets were given to the affected persons. Females of age group 15-30 highly affected. Since 3 days Chlorination plant was out of order and pipeline leakage is suspected. 2 Water Samples sent for Bacteriological Testing at WASMO, Navsari, found ?ve for drinking. 2 Stool Samples sent at Microbiology Dept, GMC, Surat for testing of Cholera found ?ve for Cholera. Coli diarrhoea 2)Two Drinking water samples collected from different sources. One from Tap Water. one from Tubewel on 040612 Reasult: Not contaminated after 48 hours. 1) DSO and Epidemiologist visited to the Jail on 040612 and inspected the situation 2)Food materials examin and advised to discard flour(Atta), which is probable unfit for human consumption 3) Jail authority is requested to provide boil waterto the prisoners. 4)To mantain the hygine in the jail custody advised to the prisoners. Medical College , Barpeta not recieved till now. 2. ANM of Major char SC issked for testing H2S strip test in an around areas and the house of the deceased. DSO asks Jt. Director of Health Services, Barpeta on 21612 for a medical team to Major Char , the village of the deceased for field verification , water testing , health education etc. Investigation Open The patient was treated at F.A.A. Medical College Hospital since 20 days before , died at F.A.A. Medical college Hospital on 10612 as informed by the respective SC, report recieved in S form for the week no 24.

Affected Village : Rupahi MPHC: Chouldhowa MPHC. 5 0 19/06/2012 2012-06-25 12:53:15 1109 Three families of Rupahi village under Chouldhowa MPHC of Boginodi BPHC , Lakhimpur District consumed Wild Mushroom on 19062012 . After consuming mushroom ,4 (Four) Nos. of persons started vomiting and loose motion. Then all the patients were admitted to North Lakhimpur Civil Hospital (NLCH) on 1962012. The age group of the patients are ??? 1. 1-10 YRS: 1 NO. 2. 11-20 YRS: 1 NO. 3. 21-30 YRS: 1 NO. 4. 31-40 YRS: 1 NO. 5. 41-50 YRS: 0 NO. 6. 51- ABOVE: 1 NO. 5 (Five) nos of blood Sample were Collected for Liver and Kidney Functioning test. 1. Microbiologist, IDSP (L) visited the North Lakhimpur Civil Hospital and meet the patients. 2. The SDM&HO ic Boginodi BPHC and ic Chouldhowa MPHC are informed and requested to send the health workers to organize Awareness Camp, awareness meeting and house hold survey at the affected areas. There is water logging due to improper drainage system. Poor socio-economic background residing at T.E. line. 1) 4 nos of samples of drinking water collected in H2S kit from different sources (Tube-Well),3 nos samples shows no bacterial contamination. 2) 4 nos of sample of drinking water collected for MPN test from different sources (Tube-Well) and sent to AMCH Microbiology Deptt. for Bacteriological Examination on 190612, the result is awaited.3) The Stool Sample could not be collected due to lack of fresh affected case. 1.The Medical Team from Khowang PHC visited the T.E. on 120612 & 140612.2.The DIET of Dibrugarh(HQ) visited the site on 19062012 and collected water samples for MPN test (4 nos.),and H2S Strip-test (4 nos.) from the drinking water source. Also Health education was given to the community regarding water borne diseases and about precaution to be taken against the diseases. JNM Medical College,Raipur(C.G.) and according to the report Vibrio cholerae 01 "ogawa" grown was found. Stool sample of patient collected and Vibrio cholerae 01 "ogawa" grown found.Test was done by medical college raipur. Investigated by RRT team . Door to door survey conducted & requesting health worker to report similar cases in their area. Water purifications measures under taken through chlorination. Intensified house to house fever surveillance done by field staff. Anti Larval activity is being carried out by field staff and VHSC member. Health Education about Mosquito breeding and safe drinking water practice done. Residual Chlorine level checked found NIL. Chlorination of OHT done. Investigation Open Fever outbreak may be due to high vector Aedes mosquito density . Weekly Anti larvel work without omission of breeding places may be reduced vector density.Neiboring villages of 1KM radius may be conduted fever surveillance by PHC Staff. Health education may play effetive roll to change the people water storing behavior . Affected area is under vigil surveillance by PHC Team.

All the affected children were of 3 to 18 age group. Male and Female were equally affected. There was no casualty whatsoever. All the affected were discharged on the same day after being treated on 30062012 Nil Gentamycine, Metronindazole, ORS, RL & Multivitamin were given. Investigation Open No new cases of Food Poisoning from the affected area has been reported so far. Information was published on Dainik Hindustan on 01072012.The Information was scanned and verified and was found true. Everything is under control right now.
