OF FAMILY OF DHEMAGARH NO-2 UNDER DHALPUR BPHC AND BAHBARI UNDER BIHPURIA BPHC CONSUMED WILD MUSHROOM. 5(FIVE) NOS. OF MEMBERS OF THE FAMILIES STARTED VOMITING,LOOSE MOTION AFTER TAKING WILD MUSHROOM. THEN THEY WERE ADMITTED IN NORTH LAKHIMPUR CIVIL HOSPITAL ON 1462012. OUT OF 5(FIVE) PATIENTS, 4(FOUR) NOS. OF PATIENTS ARE ADMITTED FROM DHALPUR BPHC AND 1)ONE) NO. OF PATIENT ADMITTED FROM BIHPURIA BPHC. THE AGE GROUP OF THE PATIENTS ARE......... 1. 11-20 YRS: 1 NO. 2. 21-30 YRS: 1 NO. 3. 31 -40 YRS: 3 NOS. 1. 5(FIVE) NOS. OF BLOOD SAMPLES WERE COLLECTED FOR LIVER AND KIDNEY FUNCTIONING RESR. 2. 4(FOUR) NOS. OF RECTAL SWAB SAMPLES WERE COLLECTED AND SENT TO ASSAM MEDICAL COLLEGE AND HOSPITAL FOR CULTURE AND SENSITIVITY TEST. RESULTS AWAITED. 1. MICROBIOLOGIST,IDSP(L) VISITED THE NORTH LAKHIMPUR CIVIL HOSPITAL AND MET THE PATIENTS AND COLLECTED RECTAL SWAB SAMPLES. 2. DSO INFORMED THE IC DHALPUR BPHC AND IC BIHPURIA BPHC AND REQUESTED HIM TO SEND THEIR HEALTH WORKERS TO THE AFFECTED AREAS FOR AWARENESS OF NOT TAKING ANY TYPE OF MUSHROOMS DURING THIS SEASON. 3. DSO(L) INFORMED THE IC SIMOLUGURI SHC AND REQUESTED TO TAKE PREVENTIVE MEASURES IN THE AFFECTED AREAS. ALSO HE IS REQUESTED TO KEEP CONSTANT TOUCH WITH DSO(L). 4. DR. S.SARMA, IC SIMOLUGURI SHC ALONG WITH A MEDICAL TEAM VISITED THE AFFECTED AREA AND REPORTED NO FURTHER CASES WERE FOUND IN THAT AREAS. Investigation Open PATIENTS ARE IMPROVING.

Acute Diarhheal Disease CHC- Maharajganj, Vill.- Janahi, Block- Maharajganj Raebareli 4 0 15/06/2012 2012-06-19 15:27:02 1045 All cases presented with symptoms of diarrhea and vomiting. The affected people are small children. O.T. Test Positive CHC level RRT team investigated the cases. Active house to house search was done. Cases of diarrhea with vomiting reported from one household of the village. The symptoms started due to consumption of stale food and contaminated drinking water. Health education given regarding consumption of freshly cooked food and clean drinking water. Adequate amounts of antidiarrheals, ORS packets, and chlorine tablets were given to the affected persons. Sample- Water No. of sample collected for H2s strip test: 4 Result of H2s strip test: Contaminated -4 1.Active search for any other cases in the area was conducted by the RRT. 2.The RRT members alon with SDM&HO, Nakachari BPHC aware the inhabitants on Health & Hygyene, Safe drinking water, Hand washing etc. 3. Health workers and other Paramedical Staff of Nakachari BPHC & Selenghat MPHC are alerted and the area is under constant surveillance for any other fresh cases. 4. Medicine distributed Norflox TZ,Norflox, Norflox TZ oral Susp.,ORS, Halazone Tab. etc. Investigation Closed 1.Present situtation is under control. 2.
