The West Jakarta Public Health Sub-agency, in cooperation with the Health Ministry, also sent a team to investigate the source of the disease and to monitor any signs of infection in neighboring residents.

"We have tested 50 people who had been near the victim and thankfully they have all tested negative," sub-agency head Dr. Ariani Murti said.

"We have not seen any signs or symptoms from the people in her immediate area."

Ariani said Kalideres is prone to bird flu because it was "very dirty" and some residents still kept chickens around their houses despite the prohibition by the city administration.

Earlier this year, a boy from the same area also died after becoming infected with bird flu.

He was one of six people living in the neighborhood who contracted bird flu.

Ariani said health services would continue to monitor Kalideres for the rest of the week.

She said her office needed much support from the public and residents to help prevent further infections.

"The people of Kalideres need to, please, stop keeping poultry in their homes, otherwise these cases will keep on occurring," she said.

The second most recent death from bird flu was a businessman identified by initials MS.

On Dec. 2, MS had a fever, acute coughing and respiratory problems. Two weeks later he died in RS Persahabatan Jakarta, East Jakarta. (anw)

W. Jakarta authorities search for bird flu

The West Jakarta municipality has been inspecting poultry in Kalideres subdistrict since Thursday, following the death of a 24-year-old woman due to bird flu in the area a day earlier.

A representative from the West Jakarta Animal Husbandry and Fisheries Agency said the entire subdistrict would be scanned and warnings would be given to unlawful bird owners.

"We found more than 40 birds, chickens and ducks owned by 11 residents in the community unit where the bird flu victim lived. None of the animals were certified," said Moris Sihombing, the head of the agency's animal supervision division.

The H5N1 virus -- the deadliest bird flu strain for humans -- has so far hospitalized 115 people nationwide and killed 94, including 24 deaths in Jakarta alone.

Indonesia tops the list for the highest number of bird flu cases globally.

The deceased, identified only as DF, died in Cengkareng Hospital in West Jakarta two weeks after doctors positively diagnosed her with bird flu.

Samples taken from 55 people who had been in close contact with the victim are being examined at the Health Ministry's lab, with the results to be released within a week, city health agency spokeswoman Tini Suryati said Thursday.

However, on Wednesday the head of the West Jakarta health subagency, Dr. Ariani Murti, told reporters 50 people had been tested for bird flu, with the results all negative.

Tini said her agency was unaware of any friends or relatives of the victim who had displayed typical symptoms of bird flu, including a high fever or respiratory problems.

Both agencies are yet to find a dead or ill bird in the neighborhood of the victim.

In an effort to control the spread of avian influenza in Jakarta, the City Council passed an ordinance in April controlling the farming and distribution of poultry.

According to the ordinance, poultry farmers are required to have a Jakarta identity card, permission from neighborhood authorities, documents detailing the number of birds kept and the size and shape of their enclosure and a license from the City Animal Husbandry and Fisheries Agency. Poultry farms must also be located at least 25 meters from any residential area.

Poultry farmers and slaughterhouse owners with preexisting businesses have six months to fulfill the requirements or their businesses will be shut down and their birds seized.

The Death Resulting From Bird Flu In Indonesia 81 Percent

Jakarta - The case of bird flu in Indonesia cumulatively reached 116 people, 94 people including dying.

Currently, their condition was still stable because his leucocytes were still normal.

Results of the sample inspection of blood, said Tini, could be known around one week.

As for results of investigation that was carried out by the Kesehatan Service around casualties's house was not found by the case of the fever.

Kepala Suku Dinas Peternakan dan Perikanan Jakarta Barat Chaidir Taufik mengatakan, instansinya telah menyisir dan memusnahkan unggas di Rt 01 dan 02, dua hari lalu. Sehari kemudian, kegiatan ini dilanjutan dengan menyisir RT 03, 04 dan 05. "Hari ini akan dilakukan penyisiran unggas terakhir di RT 06 dan 07," ujarnya pada Tempo, Jumat (28/12).

Hal tersebut dilakukan karena perempuan berinisial DF (24 tahun) warga Jalan Sumur Bor RT 02 RW 12 Kelurahan Kalideres diduga meninggal karena terkena virus Flu Burung pada Selasa lalu (25/12).

Kemarin, kata dia, pihaknya menemukan 66 unggas pangan dan hias milik 20 warga yang tinggal di RT 03, 04 dan 05. "Sesuai dengan adanya Peraturan Gubernur, kami telah memberikan peringatan kepada 20 orang tersebut untuk meniadakan unggasnya dengan dikonsumsi, dijual atau dimusnahkan dalam waktu seminggu," ujarnya.

I understand the report like this:

according to regulations it is forbidden to keep poultry in this region. After the latest death, the livestock service "combed" the area and found "illegal" poultry. You can get a permit after testing your poultry by a vet.

In a week time they want the area to be clean of "illegal" poultry.


Note: other reports mentioned poultry in this area tested negative for bird flu, no single positive case found in poultry. No mentioning of testing other species, like cats, dogs, etc.

The above translation suggest that the recent cluster in Serang, Indonesia tested negative because testing was primarily on samples collected after the start of Tamiflu treatment. Tamiflu inhibits the release of influenza from cells, so viral RNA levels in circulation decline. This decline has the potential of producing false negatives, which may account for the high case fatality rate in Indonesia. Patients treated early recover and test negative, even though they were H5N1 infected. Those who start treatment late, or have a high viral load, test positive but subsequently die.

The effect is most easily seen in clusters. One of the best examples was the Garut cluster. Although there were only three confirmed cases, the confirmed cases were in significant clusters. They were linked to earlier cases who died with bird flu symptoms, but samples were not collected for testing, so the index cases for the multiple clusters were not confirmed.

The positive cases led to the implementation of a Tamiflu blanket over the entire village. Many contacts developed symptoms and were hospitalized. Several were into the ICU, but none tested positive. A health care worker noted that many times initial tests are negative. However, the Tamiflu makes the subsequent test negative also, so the patient recovers but the recovery doesn’t lower the case fatality rate because H5N1 is never confirmed. Confirmation could be done with follow-up test on convalescent sera collected 3-4 weeks after symptoms, but such follow-up is rare.

This type of follow-up was planned for the outbreak in Turkey. Those cases also had problems with false negatives. The four siblings which included the index case all tested negative initially. Three of the four died and tested positive, but were only positive with lung samples collected at the time of death. A Tamiflu blanket was aggressively applied, but results from follow-up testing were never reported and it is not clear that samples were ever collected.

Comments from the latest cluster in Pakistan indicate testing there is also on samples collected after the start of Tamiflu treatment. Antibody testing of the negatives has been promised, but at this time only one of the nine patients were positive in local test have been confirmed by WHO reference labs.

The false negatives can lead to a lack of follow-up on contacts and a false termination of a transmission chain, which can lead to assurances that are not supported by the true infections or spread.

Two patients were suspected of by bird flu being treated in Flamboyan Rumah Sakit Hasan Sadikin Space since, on Monday (31/12) at dawn.

The Hasan Sadikit hospital (RSHS) Bandung last night again received Arlan (33), a patient was expected by bird flu.
