Father and son in Solo Hospital (Java) tested negative for bird flu:


Two patients suspect the resident's bird flu Kedungampel Kulon RT 1/8, Cawas, of Klaten had finally been permitted to come home from RSUD Dr Moewardi, on Friday (7/3/2008).

The two patients were stated by the bird flu negative.

The two patients were their respective Sudarmaji (37), and his child of Ibnu Fajar Hidayat (9).

Results of the laboratory of Litbangkes Department of Health said, the two residents who beforehand were sentenced suspect bird flu evidently the bird flu negative.

I would like to advance an alternative explanation

I think we have a new, virulent strain of New Castle disease emerging in particular areas of Indonesia. NDV is known to cause disease in humans, usually from direct aerosol contact (in lab, by veterinarians or handling of infected poultry).

In the cases where families clusters (parent, offspring) were treated with tamiflu, and where disease rapidly progressed to a point requiring ventilllators, survival is usually dismal. Yet here, we have within the last few weeks, several clusters where these patients survived and were discharged. In some cases, bird deaths were reported, but H5N1 in home or nearby poultry was not confirmed.

Perhaps it's time for WHO and the CDC to take a close look at the NDV strains in these Indonesian human H5N1- cases, where bird deaths were investigated, and reported to be infected with NDV, rather than HPAI.

It would be a rather interesting development.

"..Clinical Disease in Humans

Humans are among the many species that can be infected with NDV in addition to at least 241 avian species.1,50 The most common sign of infection of humans is conjunctivitis that develops within 24 hours of NDV exposure to the eye. In the first reported case, it was also noted that the preauricular lymph node on the involved side was swollen and tender. General symptoms of headache, discomfort, and slight chills occurred within 48 hours of the initial infection, but lasted for only 24 hours. Local treatment was used to relieve symptoms of the conjunctivitis, which cleared completely within a week. Virus was reisolated from the lacrimal fluids of the infected individual, who became seropositive within 2 weeks after the exposure. 47 Subsequently, reported infections were similar and usually resulted in self-limiting eye infections characterized by unilateral to occasionally bilateral conjunctivitis, edema of the eyelids, congestion of the conjunctiva, tearing, and pain without involvement of the cornea. Rarely, NDV infection has resulted in symptoms of fever, chills, headache, pharyngitis, depressed appetite, photophobia, and general apathy.50,51 A history of aerosol exposure was associated with most of the individuals who developed a generalized infection.52
Many of the documented cases involved infections in laboratory workers who accidentally splashed hightiter NDV-infected egg fluids into their eyes, veterinary laboratory diagnosticians who performed postmortem examinations on infected birds or handled infectious tissues, workers in poultry processing plants, and poultry vaccination crews.1,52 The absence of many recent reports suggests that such infections may be commonplace and that when they occur, they are mild and self-limiting.1 The use of personal protective equipment and biological safety cabinets in laboratories has undoubtedly reduced the exposure of current laboratory workers.
The virulence of NDV isolates does not appear to differ for humans in contrast to the wide difference in virulence for chickens. Experimentation with a virulent field isolate was the cause of the initial human infection, but similar signs can result from infections with low virulence NDV strains, such as B1, which are commonly used as ND vaccines.53 No evidence exists to support human-to-human transmission,1,51 but the potential for human-to-bird transmission exists. Because of the mild, self-limited infection of humans by NDV, the risk of substantial illness and pandemics is unlikely. Multiple treatments of humans with NDV administered by inhalation54 or IV55 without substantial adverse effects are further evidence that the risk of severe illness is low, although low-grade fevers that subsided in 24 hours did occur after inhalation therapy in 8 of 23 vaccine-treated patients. In the inhalation studies, it was also noted that mild conjunctivitis was a consequence of accidental eye inoculation..."

They would be glad to tell you "it was only a Newcastle disease variant.Have to ask Dr niman what the recombination chances are and what the results might be.Then add some dengue/chik to the mix...

Abstract
An unknown virus was isolated from a lung biopsy sample and multiple other samples from a patient who developed a lethal case of pneumonia following a peripheral blood stem cell transplant.
A random PCR-based molecular screening method was used to identify the infectious agent as avian paramyxovirus 1 (APMV-1; a group encompassing Newcastle disease virus), which is a highly contagious poultry pathogen that has only rarely been found in human infections.
Immunohistochemical analysis confirmed the presence of APMV-1 antigen in sloughed alveolar cells in lung tissue from autopsy.
Sequence from the human isolate showed that it was most closely related to virulent pigeon strains of APMV-1.
This is the most completely documented case of a systemic human infection caused by APMV-1 and is the first report of an association between this virus and a fatal disease in a human.

Mayor orders chicken cull in Makassar following bird flu outbreak



Makassar, South Sulawesi (ANTARA News) - Makassar Mayor Ilham Arief Sirajuddin has ordered the culling of poultry living within a radius of one kilometer from a bird flu outbreak at the cisity`s IDI complex in Antang.

"This is to follow up on the discovery of bird flu infection in the area last week," Mayor Sirajuddin said here on Wednesday.

The culling was needed to curb the bird flu outbreak following the sudden deaths of tens of chickens in the Antang area, Makassar, last week. A Makassar resident was also believed to have been infected with the H5N1 avian influenza (AI) virus.

The patient, identified as Ririn (12 years old), is currently being treated intensively at Wahidin Sudirohusodo Regional Public Hospital, Makassar.

"We have received a patient who has symptoms of bird flu. However, we cannot yet confirm it yet because we need to examine samples of her blood first," Dr Kadir, medical director of Wahidin Hospital, said.

Two to three days were needed to get the results of laboratory tests on the patient`s blood sample, he said.

According to data of the Indonesian Health Ministry recently, AI has so far infected a total of 129 people in Indonesia of whom 105 have died. Indonesia`s AI mortality rate is the highest in the world. (*)

Upon re-reading the story, there was only one person who had "high fever, breathless, coughs, and outside blood from the nose since the last three days.", although one additional family member was initially suspected.

The other 7 famly members may have been examined after only minimal symptoms - out of an abundance of caution.

Bird flu epidemic in East Lampung continues to spreading



Bandarlampung (ANTARA News) - A pandemic (???) bird flu is further spreading in East Lampung district, the local animal husbandry agency`s animal health division chief Dewanto said here on Wednesday.

Dewanto said the epidemic caused by the humid weather as well as public ignorance of matters relating to bird flu, has affected 21 of 346 villages in East Lampung.

However, villagers have not completely stopped selling chickens, or eradicating sick fowls by burning them and burying their carcasses, he said.

Dewanto admitted the government had difficulties in putting sick chicken in quarantine because their chickens are freeling roaming in their neighborhood.

The Animal Husbandry Agency in association with village administrators enlightened the villagers on how to deal with bird flu, Dewanto said.

As the result, few villagers stopped eating chicken for fear of being infected with bird flu.

In the meantime, the bird flu epidemic has also spread to other districts in Lampung province.

JAKARTA, March 13 (Xinhua) -- Eight people from the same village in Indonesia's Lampung province were admitted to hospital allegedly for developing bird flu symptoms, local press said Thursday.
The patients, including two babies and two teenagers, all come from Way Laga village where dozens of chickens have died of the avian influenza.
They were sent to the state-run Abdul Moeloek Hospital in provincial capital Bandarlampung with high fever and respiratory problems after eating the infected chickens, reported the national Antara news agency.
Lampung is located in the southern tip of Sumatra island.
"The patients are placed in the isolated rooms and have undergone laboratory tests. But it takes several days before we get the results," the hospital's director Dr Wirman was quoted as saying.
At least 102 people have died from bird flu out of some 125 reported cases in Indonesia since the outbreak was confirmed in 2003.
Jakarta has been the hardest hit city in the country, with 25 deaths resulting from the 29 cases reported.

6 Residents Long Evacuated Necessity

Eight residents of the Batusuluh Atas Village, Waylaga, of Panjang, yesterday were treated in RSUD Dr. Hi. Abdul Moeloek (RSUDAM).

Namun, dari sepuluh pasien suspek flu burung, dua di antaranya Aniti dan Lala menolak dirawat di rumah sakit. Sementara dua lainnya, Sanpidi dan Imas pada Rabu malam kabur dari rumah sakit, sekitar pukul 21.30 WIB.

Menurut petugas RSU Abdul Muluk, Kamis (13/2/2008), semalam sudah tidak menemukan bapak dan anak ini di ruang isolasi bagi para penderita atau suspek flu burung.

Two from 10 patients with the assumption were attacked by the bird flu virus (AI) that was treated in RSUD Abdul Moeloek could escape from the Gawat Darurat Installation on Wednesday afternoon (12/3).
The two patients who could escape this was named Imas Supriyana (10) and Santibi (52).

Officially Peternakan Banda Lampung and the official of the Way Laga Community Health Centre picked up the two patients with the assumption was attacked by bird flu on Thursday morning (13/3).
Both of them were prepared to return to the hospital after receiving the explanation, all the patient with the assumption or positive was attacked by bird flu was borne by the regional government.

The Aning condition (32), villagers Cimarias, Kecamatan Pamulihan, of Kabupaten Sumedang, West Java, that was treated in Cempaka RSUD Sumedang Space since Monday (10/3) because of being expected infected the bird flu virus, began to improve.
Expected, the hot sign high and breathless that was suffered before being treated was the sign from Tuberkolosis, not bird flu.
Nevertheless, explained public relations of RSUD Sumedang Agus Suyaman, the side he continued to be on the alert and continue to send the sample of Aning blood.
