Soldier may have first human case of bird flu here
April 22, 2008
A soldier who helped slaughter poultry infected with avian influenza might have the first human case of the disease in Korea, the Health Ministry said yesterday.
¡°The soldier slaughtered poultry on Friday and Saturday, and his symptoms meet the criteria of a suspected avian influenza patient under the World Health Organization manual,¡± the ministry said in a release. ¡°His condition, however, is close to that of a bacterial pneumonia patient, so we are closely monitoring him.¡±
The ministry quoted the Center for Disease Control and Prevention as saying a soldier had respiratory complications after killing infected chickens and ducks last week.
The soldier is being treated and monitored in a quarantine ward. The ministry said tests are being conducted and it will take one to three weeks for a final confirmation.
Since April 1, the Ministry of Food, Agriculture and Forestry has confirmed dozens of outbreaks of the highly contagious strain of the virus called H5N1, which is capable of infecting humans. Most outbreaks have been in South and North Jeolla.
The first H5N1 outbreak occurred in Korea in 2003, but there has never been a confirmed human case here. The virus has killed 207 people in 12 other countries since November 2007, according to World Health Organization data.
The release said the soldier¡¯s condition improved after he received antibiotics and that everyone who had contact with him were also given medicine and will be monitored for 10 days. The ministry issued the statement following a story in yesterday¡¯s editions of the Seoul Shinmun, a daily newspaper, which reported that a 22-year-old corporal in Sunchang, North Jeolla, may have gotten avian influenza.
Quoting a government report, the newspaper identified the corporal only by his family name, Cho.
After returning to his base after the culling, Cho developed a high fever starting on Sunday and was transported to the Army Hospital in Seongnam, the report said.
¡°Cho¡¯s temperature went up to 39.8 degrees Celsius [103.6 F] and he fell unconscious, so we suspected it was an avian influenza infection,¡± Capt. Wi Seong-hyeon of Cho¡¯s unit told the newspaper. ¡°We transported him to the army hospital and reported the case to the Sunchang health authorities.¡±
The ministry later denied that the soldier in question became unconscious.

I thought the stories indicate one has been tested, but the high number of others with symptoms have led to the suspicion that bacterial pneumonia may be the diagnosis (although its not clear that there are any test results to distiguish the possibilities). South Korea checks soldier for bird flu infection

SEOUL, April 22 (Reuters) - South Korea is testing to see if one of the workers taking part in a mass cull of poultry infected with bird flu may have become infected with the deadly virus, the health ministry said on Tuesday.
The ministry said it would take about one to three weeks to confirm if the soldier, said to be 22-years old by local media, had been infected with the H5N1 strain of bird flu.
"The soldier's symptoms are consistent with that of suspected AI (avian influenza) patients, according to World Health Organisation standards, but the clinical symptoms also resemble bacterial pneumonia so we are on a close watch," the ministry said in a statement.
The soldier, who is being treated in a quarantine ward, had a high fever and respiratory problems. He has shown signs of improvement after being treated with antibiotics, it said.
On Monday, South Korea's farm ministry said it planned to cull a record 5.3 million birds as it announced its 17th case of bird flu in three weeks, in what has become the country's fastest and biggest outbreak of avian influenza.
No human deaths from the disease have been reported so far from the country.
Some 240 human deaths have been reported globally from the H5N1 strain and 381 confirmed cases of infection since 2003, according to World Health Organisation data. (Reporting by Jon Herskovitz and Lee Jiyeon; Editing by Keiron Henderson) http://www.alertnet.org/thenews/newsdesk/SEO333941.htm

Health officials in South Korea are carrying out medical tests to determine whether a soldier taking part in a mass poultry cull aimed at curbing the spread of birdflu may have become infected.
The Health Ministry says it will take up to three weeks to find out whether the 22-year-old soldier has contracted the H5N1 virus, and says it is possible he may have bacterial pneumonia.
The soldier was one of several hundred soldiers who've been assisting the government carry out a planned cull of five million ducks and poultry, as the government seeks to stop its worst outbreak of birdflu since the virus emerged in 2003 from spreading.
In the past three weeks, 17 human cases of the disease have been reported, but there have been no fatalities.

The above article seems to be saying that the soldier(s) are infected with H5N1 (and lab confirmed), but the symptoms (pneumonia) are due to bacteria. This is a VERY dangerous position.
The military decided to cut the number of troops deployed to help slaughter avian-influenza infected poultry after news broke that a soldier may have contracted the disease.
On Tuesday, according to the Defense Ministry, it had already reduced the number of troops from 200 to 100 in Gimje, North Jeolla Province.
Military authorities said the reduction reflects that the destroying work has almost finished, but observers speculate that it was the news about the soldier's suspected contraction of the influenza that moved them to back out.
On Monday, a day before the reduction plan, 22-year-old Cho, who had been working in Sunchang, North Jeolla Province, displayed high fever and other symptoms similar to the disease after having helped bury infected chickens.
The Ministry for Health, Welfare and Family Affairs swiftly explained that it could be simple pneumonia and that it would take up to three weeks to identify the disease, but there was still concern the disease could quickly become human-infectious.
``We believe the corporal is unlikely to have contracted avian influenza, but we are taking every precaution with other troops,'' a military official told Yonhap.
It was earlier reported that the Ministry for Food, Agriculture, Forestry and Fisheries requested the military authorities deploy troops to help destroy the birds, adding up to a million chickens and thousands of ducks. However, the defense ministry was reluctant to do so, fearing families of the dispatched would claim the possibility of human infection of the disease.
Earlier, renowned doctor Kim Woo-ju of Korea University Medical Center had warned that the avian influenza was spreading in an unusual pattern and added his concern the disease could turn into a human -infectious one.
On Tuesday, two more suspected cases of bird flu were reported in Iksan, North Jeolla Province, with more than 2,500 and 450 chickens dead, respectively. As of Tuesday, of 49 reported cases of suspected avian influenza outbreak, 26 have been confirmed to be highly pathogenic.

Military Withdrawal from H5N1 Culling in South Korea
Recombinomics Commentary 15:08
April 22, 2008
The military decided to cut the number of troops deployed to help slaughter avian-influenza infected poultry after news broke that a soldier may have contracted the disease.
``We believe the corporal is unlikely to have contracted avian influenza, but we are taking every precaution with other troops,'' a military official told Yonhap.
The above comments describe the withdrawal of the military from culling operations in South Korea. The withdrawal follows the news on the hospitalized soldier who appears to have initially tested positive for H5N1. Media reports described additional hospitalized patients, but their link to culling operations or the military is unclear.
The poultry outbreak in South Korea is at record levels, and the military was deployed for the first time. There are almost 50 suspect or confirmed locations, and recently plans were announced to increase testing on duck farms. Although there have been reports of H5N1 in dead waterfowl, H5N1 can also cause asymptomatic infections in waterfowl, even though the same virus is lethal in chickens and humans.
However, there have been prior reports of mild H5N1 in Egypt and the H5N1 status of symptomatic patients in India and Bangladesh remains unclear, because most are not tested. The outbreak in South Korea is unusual because the temperature in Korea is milder, but the H5N1 is spreading at record levels and rates in South Korea, raising concerns of genetic change and altered transmissibility.
Milder human cases raise concerns about human to human transmission, because infected hosts are more mobile, leading to more contacts and opportunities for transmission.
Sequence data from the earlier poultry outbreaks would be useful. Last season the H5N1 in South Korea was the Uvs Lake strain, but since that outbreak there has been human to human transmission in Pakistan as well as record levels of H5N1 in India and Bangladesh.

[Eleven cases... It seems to refer to epizootics... - Nine people were serologically confirmed H5N1 positive in ealry epizootics events in Rep. of Korea during past years. None was added to WHO list of human confirmed cases because their illness didn't match clinical definitions for H5N1 influenza (ie: mild, asymptomatic cases). IOH
I don't understand the statement about 'two o three weeks to confirm tests' when it could be enough two o three days or less. Clearly, samples are going to be sent to a foreign laboratory. Rep. of Korea surely has technology and skill workers to get results in a couple of days. It's really disappointing delay.
