A two-year-old girl from East Jakarta died on Nov. 29 after developing symptoms on Nov. 18, it said. "Investigations into the source of her infection suggest exposure at a live bird market."
The two latest cases took Indonesia's known number of bird flu infections to 139, including 113 deaths, since 2003, according to the United Nations health agency.
It was not immediately clear why the WHO toll does not include a 15-year-old Indonesian girl in central Java whose doctor said she had died in early November. Presence of the virus had been confirmed in a health ministry lab, according to Agus Suryanto, head of the medical team treating the girl.
Worldwide, there have been 389 cases including 246 fatalities since 2003, WHO says. Vietnam has the second highest number of cases (106) among 15 countries with known human cases.

Positive testresult from a regional health ministry laboratory, negative testresults from the two national - WHO-acknowledged - labs.

The national labs are the only ones who can officially say a patient is positive or negative for avian flu.

Testresults from regional labs are used for guidelines in treatment of the patient and local containment procedures.

The primary focus of this week's WHO gathering is to discuss, and hopefully resolve, issues pertaining to the sharing of virus samples and vaccines.

Indonesia is claiming IP (Intellectual Property) rights on viruses that emerge from within their borders, and believe that they should control their distribution and use.

Indonesia has refused to `share' virus samples until guarantees are made that they will be provided with vaccines, made from these samples, at an affordable price.

Additionally, Indonesia has been slow to report human cases of H5N1 infection - preferring to provide aggregate totals every few months instead of real-time reporting.

This dispute has been ongoing for two years, and during that time the world has had very little access to virus samples from Indonesia, creating a dangerous blind spot for scientists hoping to detect changes in the virus.

While the official death toll stands at 113 in Indonesia, due to inconsistent and oft times inaccurate testing, few observers believe that number fully reflects the situation.

The attending physician said the MOH labs confirmed the case (which was subsequently denied by MOH).

Local health agency chief Tatik Suryanti said test results from two laboratories in the capital came back positive Wednesday.

The above comments confirm an H5N1 case in Riau Province and question why a fatal infection of a 15F in central Java was not reported. The WHO situation update also described confirmation of a fatally infected 2F from East Jakarta.

The 9F had been described by the local media, but the 2F was not. However, there have been a large number of recent suspect cases and clusters in the local media, raising concerns over undercounts, as indicated in Reuter’s comment on the lab confirmed 15F.

The reporting of the two recent cases is a marked improvement over reports of earlier cases this year, which were delayed by weeks or months. However, the steady stream of suspect cases or clusters in Indonesia and reports that 50% of poultry sold in Java was H5N1 positive create causes for concern.

However, recent reports have indicated 50% of the poultry sold in markets in Jakarta, Banten, and West Java is H5N1 positive, raising concerns of widespread human cases in the area. The two most recent cases were reported relatively soon after confirmation. However, earlier cases were reported weeks or months after confirmation, raising serious concerns.

In addition to the news blackout on confirmed cases, other lab confirmed cases and clusters have been denied and in other cases, index cases of H5N1 confirmed clusters have been said to be due to lung inflammation, dengue fever, and typhus.

The concerns surrounding these cases has been compounding by a withholding of samples from third party analysis, and a lack of sequences data. The last human H5N1 sequences are from patients who died almost two years ago (January 2007).

Recently a large PCR confirmed cluster was reported in Makassar, and was reported to be lab confirmed was denied by the Ministry of Health. The denial was followed by the placement of a WHO team into the region to monitor H5N1 in poultry. The H5N1 situation in poultry has also been unclear since Indonesia has not filed an OIE report since 2006 and has halted shipment of poultry samples to the WHO regional lab in Australia.

These denials of lab confirmed cases and the withholding of samples and sequences have raised serious pandemic concerns.

Timely reports of confirmed cases and more interactions with third parties, including release of samples and sequences, would be useful.

On the 21st of November extensive chicken culling was conducted around the area in the village (Desa Sekijang), although the few chicken that were tested were negative, as noted in the toggletext translation below.

Khawatir Flu Burung


WARGA RT03 RW02 Desa Sekijang Kecamatan Bandar Seikijang Kabupaten Pelalawan memusnahkan ratusan ekor ayam kampung. Pemusnahan dilakukan warga dan tim dari Dinas Peternakan Kabupaten Pelalawan, Jumat (21/11), di kawasan perkebunan PT Guna Dodos karena dikhawatirkan unggas yang dimusnahkan tersebut mengidap flu burung.
Pemusnahan ini menyusul penemuan suspect flu burung atau virus Avian Influenza (H5Nl) dalam tubuh Ririn (9) siswi kelas III SD. Acara pemusnahan dipimpin Kadisnak Drs HM Zubir Umar, disaksikan Manajer PT Guna Dodos, Frans, para asisten kebun, kepala desa setempat dan pejabat kecamatan.
Kadisnak Drs HM Zubir Umar mengatakan, ayam yang dimusnahkan ditangkap dari areal dalam radius 100 meter dari rumah Suptianto, orang tua Nining yang kini dirawat di RSUD
Arifin Ahmad Pekanbaru. Jumlah yang dimusnahkan kaJi ini sebanyak 105 ekor ayam, namlU1 besar kemungkinan akan ada pemusnahan susulan jika masyarakat masih khawatir teltular.
Pemusnahan di.lakukan atas permintaan warga karena khawatir tertular. Dari hasi I rapid test kami, sembilan ekor ayam yang diperiksa, semuanya negatif. Artinya tidak ada penularan H5Nl. "Kebetulan hasil rapid test, sembilan sampel ayam negatif flu burung. Tapi warga tetap khawatir dan minta ayam-ayam milik mereka dimusnahkan saja, ya kita lakukan," terangnya.
Meski negatif flu burung, sampel hasil tes dikirimkan ke Balai Pengkajian veterineer Bukit Tinggi Sumatera Barat, untuk pemerik saan lanjutan.
Kepada warga, Zubir Umar meminta, jika menemukan gejalagejala awal flu burung, segem melaporkan kepada Dinas Petemakan atau Dinas Kesehatan di Pangkalan Kerinci. Hal ini lmtuk menjaga jangan sampai penyebaran virus H5Nl menghinggapi manusia. "K ita segera koordinasi dengan Dinas Kesehatan dalam rangka mencegah penyebaran. Mungkin kandang-kandang ayam perlu penyemprotan desinfektan lagi," terangnya. (hud)

Avian influenza situation in Indonesia -- WHO update 45

The Ministry of Health of Indonesia has announced 2 new confirmed cases of
human infection with the H5N1 avian influenza virus. A 9 year old girl from
Riau Province developed symptoms on 7 Nov 2008 and was hospitalized on 12
Nov 2008. She recovered and was discharged from hospital on 27 Nov 2008.
Laboratory tests confirmed the presence of the H5N1 avian influenza virus.
Investigations into the source of her infection indicate poultry deaths at
her home on 2 Nov 2008.

The 2nd case, a 2 year old girl from East Jakarta, developed symptoms on 18
Nov 2008, was hospitalized on 26 Nov 2008, and died on 29 Nov 2008.
Laboratory tests have confirmed infection with the H5N1 avian influenza
virus. Initial investigations into the source of her infection suggest
exposure at a live bird market.

Of the 139 cases confirmed to date in Indonesia, 113 have been fatal.

Apparently the latest suspected bird flu patient died in Sari Asih Hospital. Later she was brought to Tangerang Hospital. Sarih Asih Hospital is not a dedicated bird flu hospital, may be a private hospital. Did not check it yet.
Tangerang hospital is an 'avian influenza hospital".

Metrotvnews.com, Tangerang: A servant in Tangerang south, Banten, died. He it was suspected was affected by bird flu. Before dying, he experienced hot high. Casualties were named Sutinah.

The woman from Pungelan, Banjarnegara, Central Java, this lived in his employer's house in Street Kutilang Blok M 3, Bintaro Jaya, Tangerang.

The owner of the house claimed to be Sutinah experienced the high fever and immediately was brought to the hospital.

Sutinah could be treated for three days in the Sari Asih Hospital, Tangerang. After dying, he was carried to the Public Hospital of Tangerang City.

The team of the doctor will examine the Sutina body to ascertain the illness that was suffered by him.

Suspected birdflu patient dies


Tangerang (ANTARA News) - A suspected birdflu patient of Bintaro, Tangerang, Banten, known by her initials as Su (24) died on Tuesday.

Su's aunt, Sumiati, said before she died her niece was intensively treated at Sari Asih hospital in Ciledug, Tangerang.

She said Su initially had breathing problems, cough and a high fever and as her condition worsened her family then took her to the nearest hospital.

"However she finally died after being treated in the hospital for three days," she said.

Su's body was taken to Punggelan in Banjarnegara, Central Java, for burial.

The head of the Tangerang health service, Hari Heryanto, said his office could not as yet confirm if the cause of Su's death was birdflu virus.

"Her blood sample is still being examined and the result of laboratory tests is needed to confirm if the cause is really the virus," he said.

The district of Tangerang which is located west of capital city Jakarta has been known as an endemic area and recorded the highest number of birdflu cases so far totalling 17 that led to 15 deaths. (*)

The official RSUD Tangerang has taken the sample of the saliva and the liquid to casualties's throat to confirm positive or not that was relevant was affected by the bird flu virus. However, to get results of the inspection was needed by time three days.

A had said since 2005 through to 2008 Tangerang regencies was the endemic area of bird flu with the number of most cases namely 18 cases, and 16 people including dying (not yet including casualties Sut).

As many as 10 from 36 subdistricts in the Tangerang Regency were the endemic area of bird flu. Heryanto said his side got the fund from APBD 2008 for the special program the socialisation of bird flu like the simulation, the training of the health power and the community.
