The above translation describes a hospitalized child with bird flu symptoms, which developed after handling poultry that died. The poultry in the area was H5N1 confirmed. The area is between Demak and Semarang and close to another suspect case (50F) from the same sub-district (see updated map).
In addition to the two suspect cases in Karangtengah, there were two fatal cases on the east side of Sermarang. Thus, all four cases were within a few miles of each and have been reported over the past month.
The initial fatal case (15F) was lab confirmed, yet the Ministry of Health denied the positive lab results. Thus, although there have been four suspect cases, including two fatalities, in a relatively small area with H5N1 confirmed poultry, none of the patients in this geographical cluster are WHO confirmed cases.
Although there were two recently confirmed cases in Indonesia in Riau and East Jakarta, clusters have been under-reported, including familial clusters where the index case was mis-diganosed as having lung inflammation, dengue fever, and typhus. All index cases died, as did the H5N1 confirmed family member, yet none of the index cases are official cases and none of the confirmed cases are official clusters. After these three clusters were reported, Indonesia imposed a news blackout. Moreover, a large cluster in Makassar, which involved PCR confirmed cases, was also denied.
Thus, the continuing lack of confirmation of familial or geographical clusters remains a cause for concern.

Re: Indonesia: Human Cases 8 Oct 2008 +
H5N1 outbreak in poultry in Riau, screening of inhabitants of 3 villages. This seems to be a typical example of how rapid response teams do their job. These rapid reponse teams seem to be implemented in the `hot` provinces.
If someone has flu-like symptoms he/she will be put on Tamiflu and stay at home. If the health doesn't improve when using Tamiflu, the patient will be transfered to a hospital. There samples will be taken to be tested locally and in the national laboratories of Dep of Health.
Bird flu, residents of three of villages supervised by doctors
on Tuesday, December 23 2008
After results of rapid tests and results of the laboratory in the Penyelidikan Hall and the Veteriner Research (BPPV) the Tinggi Hill positive, the Health Service (Diskes) the Indragiri Hilir Regency (Inhil), immediately descended to the location of the occurrence of hundreds of tails of the poultry that died as a result of being attacked by the virus H5N1, that is in the Concong Subdistrict some time before.
More than 300 residents, that was in three villages that became the area tertular immediately was given by the health test by the Diskes Inhil team, from results of the test that was carried out by the team's doctor who came from this Diskes Inhil was not found by the patient who had the sign was infected by the bird flu virus, like the fever and the temperature of the high body.
We did not find the existence of the sign to the resident, said Kadiskes Inhil, H Irsyal Rusad, on Monday (22/12). Three villages that it was indicated became the infecated area , was the Se- Rumpun Jaya Village, Dusun Bersama Jaya, and the Pecan Jaya Village, Kecamatan Concong.
Because at this time already more than time that was given by the spread of the virus H5N1 this, that is for 14 days, then began to be carried out by mass medical treatment against 300 more the resident that [were] in the contaminated area, from at this time the area was stated sterile, because of the incubation period already through.
He said, against 300 residents that was in the location, at this time could have been grateful, because of being not was found by the significant sign, but like that, against the resident must continue to do what was given by Diskes and the Pertanian Service, namely counselling around the spread and his danger this virus against humankind, the resident must continue to carry out the step in anticipation, as cleaning the pen and bringing or putting the poultry from and to the area lain.

Re: Indonesia: Human Cases 8 Oct 2008 +
Approximate locations -
Two current (or three) suspected human bird flu cases in Desa Kedunguter: -6.9529 110.575
One previous suspected case from Desa Donorejo in late November: -6.946 110.611
Note: There are two locations identified as Donorejo on the map below. I believe that the one circled in green is Desa Donorejo. These two desa are only 2-3 miles apart.
