No -- I don't think these 111 from Nakhon Phanom are included in the 144 total from around the country. I think these are additional folks just being watched in case they develop any symptoms -- they don't have any at this point -- just have had contact with sick/dead poultry somehow.

That's my impression -- the 144 total from around the country are people with flu-like symptoms who have had contact with poultry ... the additional 111 from Nakhon Phanom are folks who have had contact with sick/dead poultry, but have no symptoms -- villagers with sick/dead chickens, soldiers who have been (presumably) culling, people collecting samples, etc.

The patients' names were not released by officials but they were referred to as a 17-year-old youth and a 42-year-old woman. Dr. Thawat said the young man had touched ducks as both he and the woman worked in a duck slaughterhouse in the eastern province of Chachoengsao, not far from Bangkok.

According to Dr. Kasiwat Sripradit, the hospital director, the two patients were admitted with high fever and bad cough.

Avian Influenza surveillance in human
As at August 2, 2006.
Bureau of Epidemiology, Department of Diseases Control, Ministry of Public Health
Since January 1, 2006 to August 2, 2006, the Bureau of Epidemiology has received reports of influenza or pneumonia cases in Avian Influenza Surveillance Network from the Provincial Health Offices and Disease Prevention and Control Regional Offices. The investigation and analysis were summarized as follows:
1. Cumulative number of patients under surveillance are 2,318 cases; 67 provinces. Today reports are 164 cases; 107 cases from Phichit, 14 from Sukhothai, 4 each from Nan, Phitsanulok, Phetchabun and Kampaengphet, 3 each from Kanchanaburi, Chiengmai, Uttradit and Nakhonphanom, 2 each from Nakhonpathom, Prachinburi, Lampang and Nakhonsawan, and 1 each from Nonthaburi, Lopburi, Singburi, Suphanburi, Prachubkirikhun, Nakhonnayok and Maehongson.
2. Confirmed human case of avian influenza 2006 = 1 death cases; from Phichit province.
3. There are 279 suspect case reported, of which waiting for laboratory result.
Conclusion:
· In 2005, there are 5 confirmed human cases of avian influenza, with 2 death cases.
· In 2006, there is 1 confirmed human dead case of avian influenza.

Thai definitions
This is useful. The definitions of "suspected case", etc., that the Thai authorities us:
*Case definitions for Avian Influenza in Humans in Thailand (1 June 2006)
1. Suspect: An individual
- whose body temperature is more than 38°C and who has at least one of the following symptoms: muscle pain, cough, breathing difficulty, or shortness of breath, or physician suspects pneumonia or influenza, and - who has a history of direct contact with sick or dead poultry in the last 7 days or there were reports of unusual poultry deaths in the village in the last 14 days or had been looking after another pneumonia patient in the last 10 days before illness onset, but
- who does not have a specimen which complies with the recommendation for laboratory testing for Influenza A/H5.
2. Probable case: a suspect
- who developed signs and symptoms of, or died from, acute respiratory failure.
3. Confirmed case: a suspect
- who has a final standard laboratory confirmation of Influenza A/H5 through at least one of the following methods:
A. Single RT-PCR method using two primer/probe sets, or using specimens collected from at least two different locations (such as throat swab and nasopharyngeal aspirate, etc.), or using at least two specimens collected from a patient at different period of illness),
B. Viral culture
C. Neutralization test (four-fold antibody increase between acute and convalescent serum)
4. Case under investigation: a patient
- whose additional clinical information, and/or history of exposure to risk factors, and/or laboratory results are needed before any conclusion can be made.
5. Excluded case: a patient
- who does not fall under any of the above definitions and is therefore excluded.

Interesting post #18 from FrenchieGirl- "we have only 70 tablets of Tamiflu in our drug stockpile. A suspected bird flu patient needs two tablets a day for 5 consecutive days. If we run out we still could get more supply within an hour from nearby hospitals." I wonder: 1) are all suspected cases put on the Tamiflu immediately? 2) 70 tablets would care for 7 patients for 5 days of treatment- what preps are in place for those who require more care? 3) would HCW's be receiving Tamiflu at the same time? or would they need to register with symptoms? 4) Thailand is presenting with many suspected cases- and the list seems to increase- is this 70 indicative of a limited supply? or simply limited information?

Two patients hospitalized for human flu have been put in an isolation ward on suspicion of having bird flu in Thailand's eastern province of Chachoengsao, as doctors conducted laboratory tests to confirm the diagnosis, media here reported Thursday.
Thawat Suntrajarn, director of the Department of Disease Control, said that the two patients, who had been earlier admitted to Ban Pho Hospital in Chachoengsao with high fever and bad cough and confirmed an infection of human influenza, were isolated in a closed-circuit television equipped camera to monitor for bird-flu related symptoms around the clock.
"Because they are having a human influenza, further tests are needed to confirm what type of the human flu virus they have and whether they also have bird flu," Thawat was quoted by Thai News Agency as saying.
Authorities didn't release the patients' names but referred them as a 17-year-old youth and a 42-year-old woman. Thawat said both of them worked in a duck slaughterhouse in Chachoengsao, not far from Bangkok.
A local livestock official said the province has raised some 5 million chickens in farms but no bird flu outbreak has been reported in local poultry.

Public health official reports no new bird flu cases
BANGKOK, Aug 4 (TNA) - Public Health Ministry lab tests confirmed that three suspected bird flu cases in Lop Buri and Chachoengsao tested negative to the H5N1 bird flu virus, a senior ministry official said
Friday.
Department of Disease Control director-general Dr. Thawat Suntrajarn said the nine-year-old girl who died in hospital in the central province of Lop Buri Thursday, as well as two patients with flu symptoms in hospital in Chachoengsao, were not infected with the H5N1 virus but with Type A flu virus.
Dr. Thawat said the child did not have direct contact with chicken or other birds and at the first stage, the cause of death was believed to be severe pneumonia.
The other two suspected cases were workers who slaughtered ducks in a processing factory but they
were raised under sanitary conditions and were free of disease.
Moreover, there have been no suspicious deaths of poultry in the province.
Dr. Paichit Warachit, director-general of the Department of Medical Sciences said that provincial health offices have implemented preventive measures against bird flu.
As of August 4, there were 39 outstanding suspected cases awaiting lab test results, according to Dr. Paichit. (TNA)-E004

Yes it is. What they meant, though, is a normal human influenza A virus--H1N1 or H3N2. Those two strains can and do cause severe pneumonia, killing patients in the process. It is relatively unusual for them to kill a 9 year old girl however.
