Chinese woman dies of H5N1 strain of bird flu

Last updated: Sunday 18 January 2009 16:00 UTC

A woman in the Chinese province of Shandong has died of the H5N1 variant of the bird flu virus. This is the second death as a result of bird flu in a month. The 27-year-old woman fell ill two weeks ago. The health authorities in the eastern province have confirmed that she was infected with the potentially fatal variant of the virus.

Nearly 250 people have died as a result of the H5N1 strain of bird flu since 2003. The disease is transmitted by direct contact with infected birds or their excrement.

China reports second bird flu death this month

Agence France-Presse | 01/19/2009 12:07 AM
Printer-friendly version | Send to friend
BEIJING - A woman has died of bird flu in east China's Shandong province, state media said Sunday, reporting the nation's second death from the disease this month.
The 27-year-old, surnamed Zhang, fell ill on January 5 and died Saturday evening, the Xinhua news agency said, citing the Shandong provincial health department.
The national disease prevention and control centre on Sunday confirmed that she was infected with the H5N1 strain of avian influenza, according to Xinhua.
It brings to 22 the number of people in China who have died from the virus since 2003.
Earlier this month, a 19-year-old woman died of bird flu in Beijing -- the country's first fatal case in nearly a year -- after handling ducks she had bought in a market.
The report of Zhang's death came a day after Chinese state media said that a two-year-old girl had fallen ill from bird flu. The toddler was reported to be in a critical condition in Sunday evening primetime TV news.
People who were in close contact with Zhang are now under close observation, but so far no one has fallen ill, the China News Service said.
China has reported the case to the World Health Organization (WHO), as well as health authorities in Hong Kong, Macau and Taiwan, according to the China News Service.
Bird flu outbreaks tend to happen in the winter months, when low temperatures create more favorable conditions for the virus to spread.
Prior to this month's cases, and in addition to the fatalities, 10 Chinese were known to have contracted bird flu but survived.
According to the WHO, about 250 people have died from bird flu worldwide since 2003.
Scientists fear the virus could mutate to jump easily from human to human, potentially sparking a global pandemic.

Fatal H5N1 Case in Shandong China Increases Concerns
Recombinomics Commentary 15:46
January 18, 2009

The 27-year-old woman surnamed Zhang, from Jinan, capital of China's eastern Shandong province, died on Saturday after falling ill on Jan. 5

The above comments describe the third confirmed H5N1 case in China this month (see updated map). This case is in addition to the fatality in Beijing (19F) and the critical case originating in Hunan (2F) and all three follow confirmation of two bird outbreaks in Jiangsu last month. The outbreaks in Jiangsu involved asymptomatic poultry infected with clade 7, a clade that is significantly different than the clade 1 and clade 2 targets of avian and human vaccines, raising concerns that clade 7 is silently spreading in China and cause fatal infections this month.

China reports two new cases of bird flu, one dead
18 Jan 2009 16:30:34 GMT

(Recasts with state media saying a woman had died)
By Ian Ransom
BEIJING, Jan 18 (Reuters) - A woman in eastern China has died and a two year-old girl is critically ill in northern China after becoming infected with bird flu, state media said on Sunday.
The 27-year-old woman from Jinan, capital of China's Shandong province, died on Saturday after falling ill on Jan. 5, Xinhua said, citing an unnamed official with the provincial health department. It gave her surname as Zhang.
The two-year-old girl, surnamed Peng, was found ill on Jan. 7 in central Hunan Province and taken to a hospital in her home province of Shanxi on Jan. 11, Xinhua news agency said, citing an unnamed official with the provincial health department.
After not reporting a single human infection in almost a year, China has now confirmed three cases in two weeks.
Health authorities said earlier this month a woman infected with bird flu had died in Beijing after buying ducks at a market in Hebei province, which surrounds the Chinese capital, sparking emergency checks of local poultry markets.
Experts said the case was not unexpected as the virus is more active during the cooler months between October and March, but pointed to holes in surveillance of the virus in poultry.
China's Agriculture Ministry said last week it had found no bird flu cases among poultry in Beijing or other areas surrounding the city during checks after the woman's death.
The H5N1 virus remains largely a disease among birds but experts fear it could change into a form that is easily transmitted among humans, and spark a pandemic that could kill millions of people worldwide.
With the world's biggest poultry population and hundreds of millions of backyard birds, China is seen as critical in the fight to contain bird flu.
GIRL CRITICAL
The national disease prevention and control center on Sunday confirmed Zhang, the 27-year-old woman, had been infected with bird flu, Xinhua said, but provided no other details.
China's Health Ministry said in a statement on its website (www.moh.gov.cn) that authorities had confirmed on Saturday that the two-year-old girl had been infected with the virus.
The statement did not say how the girl had become infected. There have not been any reports of outbreaks of the virus among birds in Hunan since May 2007.
"Currently, the girl's condition is critical. Shanxi health departments are currently fighting to save her with the guidance of a team of health experts," the Health Ministry said.
"All people who have had close contact with her are under strict medical observation," it said. It added that the World Health Organisation (WHO), and health authorities in Hong Kong, Macau and some other countries had been notified.
Calls placed to health departments in Shanxi province went unanswered.
WHO said China's Health Ministry had notified them of the toddler's infection, but could not provide more details. "We are staying in close contact with the Health Ministry," a spokeswoman from WHO's China office told Reuters.
Since the H5N1 virus resurfaced in Asia in 2003, it has infected 391 people, killing 247 of them, according to WHO figures released in mid-December.
The toddler's infection brings China's total to 33 human bird flu cases, of which at least 22 people have died. (Additional reporting by Lu Jiansheng; editing by Myra MacDonald)

Clade 7 Silent Spread Causing Fatal H5N1 Cases in China?
Recombinomics Commentary 17:24
January 18, 2009

Gene sequencing by the National Avian Influenza Reference Laboratory indicated that this virus is very similar to the avian influenza virus variant isolated in Shanxi in 2006.

The above comments are from the OIE report submitted by China for the two H5N1 outbreaks in Jiangsu, China (see updated map). Although 376,998 birds were culled, there were no fatalities listed in the December 26 report because the birds were asymptomatically infected. This raised concerns that similar infections at other farms led to the silent spread of clade 7, which was the clade in Jiangsu as well as Shanxi in 2006 and other poultry outbreaks in areas surrounding Beijing, as well as fecal samples from Hunan in 2006. There were also reports of clade 7 in Vietnam in 2008 prior to the outbreaks in Jiangsu.

The Jiangsu outbreak was followed by the confirmed case (19F) in Beijing, who had purchased and gutted ducks in Hebei and developed symptoms on Dec 24. It is unlikely that these birds had symptoms, and a survey of birds near Beijing failed to identify dead or dying poultry. However, a health care worker in Beijing did develop symptoms, raising concerns of increased efficiency of transmission to humans.

The Beijing case(s) were followed by the fatality in Shandong (27F), who developed symptoms on January 5. The possible exposure has not been disclosed.

These two cases were followed by the case (2F) in Hunan, who is in critical condition in Shanxi. Surveys of markets in Hunan and Shanxi failed to identify dead or dying poultry.

Thus, there has been no linkage of any of the three confirmed H5N1 to dead or dying poultry. However, vaccination of poultry with a poorly matched vaccine would yield asymptomatic birds shedding H5N1 virus, which would facilitate spread in immunized birds that could lead to fatal human infections.

More information on the H5N1 isolated from the three cases this month would be useful.

Clade 7 is readily distinguished from clade 1 or clade 2.1, 2.2, or 2.3, and the distinction can be seen in the HA cleavage site (clade 7 has a G at the third position instead of an R or S).

Shandong deceased source of infection of avian flu is still under investigation
01 years http://www.sina.com.cn 2009 on 22 day 00:07 Morning News

Experts said: At present, bird flu cases increased and the spring has nothing to do

Morning correspondent reported Jinan Yucai Yang

On the 20th evening around 6:00, Zhu Fang (a pseudonym) TV sitting next to the right and left arms around more than 1-year-old child, the right hand to the bottle to the child's mouth. TV News, the now playing Huaihua City, Hunan Province infected with highly pathogenic bird flu patients died on the morning news. Zhu Fang head, silently watching the children.

At 18:40 on January 17, Zhu Fang's wife, Zhang Xia (pseudonym) had been infected with highly pathogenic avian influenza died. January 5 coupled with the death of patients in Beijing, Huang Yan-qing, and 20 in Hunan Province on the morning of the death of patients with Wu, just 20 days, China has three causes of infection of highly pathogenic avian influenza deaths. In addition, Shanxi Province and one of the children infected with avian flu are being treated.

At a time when the peak period of Spring Festival travel season, the rapid increase in traffic would increase the risk of the spread of avian influenza? Yesterday afternoon, epidemiological experts Morning News reporters Cai Fang and Shi Biaoshi currently three cases of death are highly sporadic cases of avian influenza cases increase and there is no relationship between the spring, will not be large-scale outbreak, but did not rule out also there is the possibility of dissemination.

From the onset to death less than two weeks

Zhu Fang and his wife's home district in Jinan槐荫maestra Rong-xiang Garden North Road. Before the death of his wife, husband and wife in the district next to the roadside to sell "麻辣烫" fairly decent day.

Yesterday afternoon, Zhu Fang in an interview with reporters, recalled his wife, Zhang Xia from the onset to death of the whole process. "I still do not know how she is dead in the end?" Zhu side as if to say to himself, and now so well-developed medical, why come to the rescue, however?

According to Zhu Fang memories, Zhang Xia early on January 5 with fever, feeling unwell, his own point of eating cold medicine. January 10, his condition has worsened to a nearby therefore the Fifth People's Hospital of Jinan City doctor, the doctor said to be upper respiratory tract infection, but did not improve after吃了药. January 11, Zhu side with his wife to the Shandong Provincial Hospital emergency department, observation and treatment for two days, his condition worsened on the 13th the beginning of hospitalization.

"This has been the middle did not improve, it has been confirmed not only said that the 'unexplained pneumonia'." Chu told reporters that January 15, the wife in a serious condition, hospital, she was transferred to an isolation ward, but the disease remains not eased. At 2:00 on January 17, because not diagnosed anxious wife Zhu Fang chartered to take the hospitals, intend to the Beijing Union Hospital for treatment. "That just arrived in Langfang, Hebei, Beijing is drawing near, we were back to the Jinan, and was admitted to the infectious disease hospital, but did not rescue them."

January 19, Jinan City Hospital for Infectious Diseases covered Zhongtao business president in an interview with the media, said Zhang Xia from the onset to the illness came to a standstill, advanced at a very fast pace, especially lung infection at a very fast pace, experts doubt that she because of pneumonia of unknown causes. January 17, in the normative samples of patients with laboratory analysis, confirmed the infection of highly pathogenic avian influenza. "She later admitted to hospital, including the provincial expert group, including all the experts do, stationed in the treatment of infectious disease hospital for emergency treatment, this process is still very timely."

Source of infection is still under investigation

According to Mr. Zhu told the reporter that his wife helped in the streets to sell their own "麻辣烫" would inevitably be reflected in regular contact duck blood, chicken wings, chicken and other livestock products. Despite the current health sector has not yet been determined Zhang Xia real source of infection, but Mr Chu believes that his wife contracted bird flu, surely and contacts related to the livestock meat.

His wife passed away, because it is in close contact with Mr. Zhu De, is still in the medical observation period, the daily need for regular measurement of body temperature, but the doctor did not restrict him out. "The doctor says that at least seven days of observation time."

Zhang Xia after the death of Jinan City health and disease control departments to respond quickly, will work with Zhang Xia had had contact with 147 people have been isolated inspection, has not yet been found to have symptoms of infection.

Is it because Zhang Xia blood contact with dead chickens and ducks infected with bird flu virus? Yesterday afternoon, reporters call the Health Department of Shandong Province, the emergency office, answer the phone staff, told reporters words carefully, in the authoritative department before a final conclusion, they speculate Zhang inconvenience to the source of infection.

If Zhang is really the source of infection of chickens and ducks died of blood, then eat "麻辣烫" whether customers will be infected? Jinan City CDC staff, told reporters yesterday afternoon, "麻辣烫" itself strong enough to kill the high temperature of avian influenza virus, in the absence of symptoms such as fever, even in her booth eaten "麻辣烫" do not have to panic.

The comments are made when the report was posted yesterday. The above is the most recent from ProMED and the CP comments have been similar since 2004. He doesn't think H5N1 is much of a problem for the general population. He maintains that only a small number of genetically susceptable individuals get infected by H5N1 positive poultry.

Last season the H2H case in Jiangsu (that was written up in NEJM), was said to be due to poultry consumed at a restaurant, and the son who died ate dark meat, while his father had white meat, which accounted for the gap in disease onset dates! (and the son also had a diminished immune response becase he was receiving rabies shots for a dog bite). I really am not making this up. CP also invented a sibling in a cluster in central Vietnam in 2005 to prove that the H2H cluster wasn't H2H because the imaginary sibling wasn't infected.

Although entertaining, the comments are not reality based, regardless of posting date.
