The last WHO update for Indonesia was January 22, which described cases from 2008. UN members strengthened reporting requirements for diseases that had the potential for spread beyond national borders and H5N1 was added as a notifiable disease that required reporting within 24-48 hours of confirmation.
The failure of WHO to issue updates on any 2009 cases, even after acknowledgement of lab confirmation by the Indonesian MOH, highlights extreme oversight failures, continues to be hazardous to the world’s health.

Blood samples taken from two-and-a-half-year-old Wahyu Ibnu Saputra, who died last week in Pekanbaru, Riau, have tested positive for the bird flu virus, an official said on Wednesday.
Azizman Saad, coordinator of the bird flu prevention team at the Arifin Ahmad hospital in Pekanbaru, said confirmation that Wahyu died from bird flu had been received by the Riau Health Office several days ago.
"The victim is believed to have contracted the virus from dead chickens in his surrounding areas," Azizman told The Jakarta Post.
Indonesia's bird flu death reached 119 as of early this month, according to data from the national bird flu commission. Blood samples of patients believed to have been infected with the virus are usually sent to the Health Research Agency of the Ministry of Health in Jakarta.
Following the confirmation, Azizman urged Wahyu's family members and neighbors in Tanjung Rhu village, Pekanbaru to go to the nearest community health center to have a check-up.
"There has not been any evidence that the bird flu infection can be transmitted from human to human, but the case shows the bird flu outbreak took place in the victim's area. It's better for those who had made contact with the victim to go to the health center even though they do not have any health problems," he said.
Azizman explained that Wahyu was intensively treated for high fever and coughing and breathing difficulties in an isolated room at Arifin Achmad hospital.

Meanwhile, Susi Herlinda, a member of the Pekanbaru Legislative Council, criticized the administration for its failure to anticipate the bird flu infection. The death of the victim showed the administration was not aggressive enough in campaign about the danger of bird flu, she said.

In other cases, relatives developed symptoms but recovered after treatment with Tamiflu, and were said to be H5N1 negative. In other instances, the earlier contacts were said to have died of other diseases that are associated with bird flu symptoms, such as lung inflammation, thyphus, or dengue fever.
These clusters in Indonesia continue to cause concern. Reporting of confirmed cases by WHO has ceased, as has the flow of sequence data from poultry or patients, even though Indonesia has reported the highest number of fatalities and the case fatality rate remains at 80%.
The inability of WHO to exercise oversight remains a hazard to the world's health.

In the past, hasn't there been a question of family transmission? Usually, it seemed to be mother-child or child-mother, with the occasional sibling. (Am I remembering incorrectly?)
This possible child and now father & grandparents is falling out of that pattern, right? Do we know if the grandparents are maternal or paternal? That's something else to know, in considering family transmission, correct?
Sorry, if this is too basic and I'm showing my ignorance in the science part of this ... but it came to mind, this afternoon.

The family genetics stories are fairy tales. There are several examples of transmissions that don't follow blood lines.

Blood samples taken from the 2-and-a-half-year-old chld who died last
week [23-27 Mar 2009] in Pekanbaru, Riau, have tested positive for
the bird flu virus, an official said on Wednesday [1 Apr 2009].
Azizman Saad, coordinator of the bird flu prevention team at the
Arifin Ahmad hospital in Pekanbaru, that confirmation that the child
died from bird flu had been received by the Riau Health Office
several days ago. "The victim is believed to have contracted the
virus from dead chickens in his surrounding areas," Azizman told The
Jakarta Post. Indonesia's bird flu death [toll] reached 119 as of
early this month, according to data from the national bird flu
commission. Blood samples of patients believed to have been infected
with the virus are usually sent to the Health Research Agency of the
Ministry of Health in Jakarta.
Following the confirmation, Azizman urged the child's family members
and neighbors in Tanjung Rhu village, Pekanbaru to go to the nearest
community health center to have a check-up. "There has not been any
evidence that the bird flu infection can be transmitted from human to
human, but the case shows the bird flu outbreak took place in the
victim's area. It's better for those who had made contact with the
victim to go to the health center even though they do not have any
health problems," he said.
Azizman explained that the child was intensively treated for high
fever and coughing and breathing difficulties in an isolated room at
Arifin Achmad hospital. "One day before he died, his condition was
improving but, finally, his condition [deteriorated] and he fell
unconscious. Six doctors tried to save his life, but to no avail," he
said. Azizman explained that before the disclosure of the laboratory
tests the child was believed to have died from multi-organ failure.
Rini Hermiyati of the Pekanbaru Health Office said she had assigned a

The Pekanbaru Livestock Husbandry Office made the decision to cull

Meanwhile, Susi Herlinda, a member of the Pekanbaru Legislative
Council, criticized the administration for its failure to anticipate
the bird flu infection. The death of the victim showed the
administration was not aggressive enough in campaign about the danger
of bird flu, she said. "It has become a habit of our people,
everybody acts only after somebody falls victim to a certain
diseases. The death of this child should have been used as momentum
to improve," she said. "Related institutions have to create awareness
among the public about the danger of the bird flu," Susi added.

Pekanbaru, Riau – Health service of Pekanbaru announced exceptional bird flu status (KLB) on neighborhood of bird flu victim, Wahyu Ibnu Saputra (2.9), Jalan Kuantan Jalan Kuantan VII, RT 7, RW 4, Kelurahan Tanjung Rhu, Kecamatan Limapuluh. Health service conducts three standard programs for prevention of bird flu virus spreading.
Actions taken are: Area monitoring to Jalan Kuantan VII, RT 7, RW 4, Kelurahan Tanjung Rhu, Kecamatan Limapuluh; Proclaiming bird flu alert status to people who had contact history with victim; and Giving tamiflu and hospital recommendation to people who develop high fever.
This is mentioned by Pekanbaru health service officer, dr Rini Hermiyati Thursday (2/4). For this prevention purpose, Health service is working together with Agriculture service Pekanbaru.
“People with high fever will receive Tamiflu administration and recommendation for regional hospital. This is done for bird flu exceptional circumstances status areas”, said Rini. Investigation and preventive measures will be done for 30 days started from Monday (30/3) to neighborhood 500 meter around victim’s house.
Bird flu H5N1 virus has killed 3 people in Riau since 2007. Two fatalities reported in 2007 at Kecamatan Rumbai and Marpoyan Damai; and one fatality in 2009 at Kecamatan Lima Puluh.

In this news report two additional individuals in Pekanbaru, Riau, are showing signs of possible influenza. They lived about 500 m (about 1/3 mile) from Ibn Wahyu Saputra (the young child who was confirmed with H5N1 and died on March 26). It is not clear if these two people are the grandparents of Ibn Wahyu Saputra who have been reported as showing symptoms as noted in a post up above.
