There was never any formal media reports on the results of H5N1 testing of Yenti (Yanti) from Padang who died in July 2010. Yenti is not included in any Indonesia Ministry of Health reports on H5N1 patients (Link)

Hoepfully the test results from this young child will be made public when they are known. A quick check of the Indonesia news articles indicates that test results are usually only reported with the patients test negative for H5N1.

A quick check of the Indonesia news articles indicates that test results are usually only reported with the patients test negative for H5N1.

The Ministry of Health is the only one to announce H5N1 positives.
Hospitals are not allowed to do so.

Pasaman Barat – A 5-year-old girl, Indri Rahmi Wati, resident of Kampung Sungai Janiah, Jorong Sungai Janiah Nagari Talu, Kecamatan Talamu Kabupaten Pasaman Barat (Pasbar) had died in M Djamil hospital, West Sumatera, on Monday (25/10).

She was suspected of having bird flu H5N1 infection.

Patient had received medical help for four hours at internal medicine isolation unit of M Djamil hospital before the death.

Head of Health Service in Pasbar, Yandra Fery confirmed the existence of bird flu suspect patient in Pasbar.

Indri, had been admitted to Talu public health center and then transferred to Jambak Pasbar regional hospital. Since her condition was deteriorating, the hospital referred her to M Djamil hospital, where she was immediately treated at internal medicine unit.

Meanwhile, M Djamil hospital had collected patient’s blood and throat swab samples for further examination in Indonesia Ministry of Health Research and Development laboratory (Balitbangkes).

Lung examination or chest x-ray done by M Djamil hospital showed pneumonia which indicates bird flu infection.

Field investigation showed evidence of died chickens around patient’s house. However, those chickens showed symptoms of convulsion, and apparently did not suddenly die as commonly observed in bird flu infected birds.

Yes, and how often does that happen?

Every couple of months the MOH retrospectively reports one or two cases, usually those that resulted in death.

We know that the majority of infected individuals infected with H5N1 (not just in Indonesia) had close association with sick or dying poultry infected with H5N1. Almost every news report about suspected cases in Indonesia are in areas with reported H5N1 poultry infections. A year or two ago the H5N1 circulating in Egypt infected numerous children, most of whom survived. Why is it that there there are almost no survivors of H5N1 in Indonesia?

We don?t know the number of H5N1 patients in Indonesia.

Some serosurveillance studies were done, the results were not published.

Dr Nidom - an authority - once said the percentage of inhabitants of Jakarta with H5N1 antibodies could be as high as 70%. Having antibodies is not the same thing as having fallen ill of H5N1, however such a high percentage is meaning something I suppose.

If someone falls ill, develops high fever, some may stay at home, if it gets serious the patient may die. This happens all the time, every day: people "die of a fever" , could be tuberculosis related pneumonia, malaria, or dengue, or one of many other diseases, or may be even H5N1. Nobody will ever know.

Others may go to a nurse at a Community Health Center, if available. If the nurse suspects possible H5N1, you may get a course of Tamiflu. If you were in time you have a good chance to be cured. Nobody will ever know if it was H5N1.

If you have H5N1 and you are a little late to get medical help, the Tamiflu won't help. Or you won't receive Tamiflu at all. Wrong diagnose or, may be you went to a private hospital: they don't have Tamiflu.

These are the majority of cases we see reported: the patients fall ill, seek help a little late (not suspecting bird flu), are not diagnosed initially as a H5N1 case, their health is deteriorating, in the end they are referred to a dedicated bird flu hospital. The survival rate is rather low: less than 25% or so.

The relation between high death rate in Indonesia and late treatment (and/or after wrong initial diagnosis) is also reflected in several scientific studies.

Things can go wrong in the testing procedure, the timing etc, we had a lot of discussion on this here at FT. Survivors tend to test negative .

It may be most survivors are not tested at all, they stay home and get better or get a course of Tamiflu at the health center and are cured.

If you look at it from the point of view of the health authorities the current bird flu policy is rather succesfull :

Many villages have volunteers who help to spot bird flu.
A huge network of community health centers with Tamiflu at hand.
100 or more hospitals with trained staff and equipment to treat bird flu patients.
Money from abroad pouring in to fund all kinds of bird flu projects.
Number of confirmed cases going down.
A homemade H5N1 vaccine is being developed.

So compared to health problems like tuberculosis (some 200 deaths every day) bird flu seems to be 'under control' in Indonesia. May be including "information control"?

I have felt for a long time that we may be missing many bird flu cases that may be survivable with little treatment.

The endemic H5N1 situation with the animals/environment in Indonesia presents a real risk to humans - and has for many years.

Absolutely, DEPKES can claim it has successfully controlled and contained H5N1 human infections in Indonesia. They will continue to claim success up until the day the H5N1 virus reassorts or recombines and becomes as easily transmissible as the novel H1N1 pandemic strain in 2009. We can only hope that if that event occurs that the new strain will be less virulent.

The fact the H5N1 has not yet become a pandemic influenza strain is NOT because of Public Health control measures in Indonesia or any other country with human H5N1 cases. The ONLY reason H5N1 is not a pandemic influenza strain is because it can not yet be easily transmitted between humans.
