Influenza Virus Antigen Detection by Immunofluorescence Assay (IFA)
Clinical specimens from 1107 patients with ILI and ARI were investigated by IFA in 36 RBLs for detection of antigens of influenza viruses, parainfluenza, RSV and adenoviruses. A total of 51 (4.7%) influenza cases were diagnosed including:

- 2 (0.2%) influenza (H1N1+H1N1v) cases in 1 city,

- 13 (1.2%) influenza A(H3N2) cases in 4 cities,

- 36 (3.3%) influenza B cases in 13 cities.

Mid-value for IFA detection of influenza and other non-influenza respiratory viruses in the country was estimated as 25.0%, including non-influenza respiratory viruses as 20.3% accounting as 12.4%, 6.1% and 1.8% for parainfluenza, adeno- and RS-viruses, correspondingly (table 2).

Conclusion
During the week ending January 02.2011 the overall proportion of respiratory samples tested by virus isolation, IFA and rRT-PCR positive for influenza was estimated as 9.9%. A total of 8 influenza viruses were isolated including 2 pandemic A(H1N1)v viruses, 1 influenza A(H3N2) viruses, and 5 influenza B viruses. Epidemic thresholds of morbidity for separate cities were exceeded in 17 RBLs of 49 collaborating with WHO NIC in St.-Petersburg. Morbidity level in these cities was ranged from 0.4% to 1.7% of overall population.This week nationwide ILI and ARI registered morbidity level (53.7 per 10 000) was lower than the national baseline by 13.8% that can be explained by New Year holidays in Russia.

During the reported week (January 17 - 23.2011) investigations on influenza virus isolation were conducted in 22 Regional Base Laboratories (RBLs) participating in influenza surveillance in collaboration with two WHO NICs (St.-Petersburg, Moscow). As result of 412 clinical samples testing 15 (3.6%) pandemic A(H1N1)v influenza viruses were isolated in Astrakhan, Moscow, St.-Petersburg and Khabarovsk, 5 (1.2%) influenza A(H3N2) viruses were isolated in Vladivostok and 7 (1.7%) influenza B viruses were isolated in Voronezh, Murmansk, Novosibirsk, St.-Petersburg and Vladivostok (table 1).

Influenza and ARI morbidity data. During the week ending January 23.2011 epidemic thresholds of morbidity for overall population were exceeded in 19 of 59 cities. Morbidity level in these cities ranged from 0.6% to 1.9%.The nationwide registered ILI & ARI morbidity level (82.6 per 10 000) was higher than the national baseline by 32.6%. Morbidity level varied in 59 cities widely (from 0.2% to 1.9% of overall population of cities).

Etiology of ILI & ARI morbidity. The overall proportion of respiratory samples tested by virus isolation, IFA and rRT-PCR positive for influenza was estimated as 24.5%. Influenza A(H1N1)v and B viruses dominated in etiology of morbidity in Russia. According to sentinel surveillance data investigation of clinical samples from 55 SARI patients in 8 cities showed 23 influenza pandemic A(H1N1)v, 6 influenza B and 1 influenza А(H3N2) case.

During the reported week (January 24 - 30.2011) investigations on influenza virus isolation were conducted in 22 Regional Base Laboratories (RBLs) participating in influenza surveillance in collaboration with two WHO NICs (St.-Petersburg, Moscow). As result of 505 clinical samples testing 18 (3.6%) pandemic A(H1N1)v influenza viruses were isolated in Kaliningrad, Moscow, St.-Petersburg and Khabarovsk, 4 (0.8%) influenza A(H3N2) viruses were isolated in Vladivostok and 32 (6.3%) influenza B viruses were isolated in Voronezh, Krasnodar, Moscow, Novosibirsk, St.-Petersburg and Vladivostok (table 1).

Epidemic thresholds of morbidity for overall population specific for each city were exceeded in 35 cities. Morbidity level in these cities ranged from 0.5% to 2.5%.The nationwide ILI & ARI morbidity level registered this week (118.0 per 10 000) was higher than the national baseline by 89.4%.

Etiology of ILI & ARI morbidity. The overall proportion of respiratory samples tested positive for influenza by virus isolation, IFA and rRT-PCR was estimated as 36.1%. Influenza A(H1N1)v and B viruses dominated in etiology of morbidity in Russia. According to sentinel surveillance data investigation of clinical samples from 36 SARI patients in 9 cities showed 14 influenza pandemic A(H1N1)v cases, 1 influenza А(H3N2) and 10 influenza B cases.

Epidemic thresholds of morbidity for overall population specific for each city were exceeded in 51 cities from 59 ones under surveillance. Morbidity level in four of these cities reached up 2.0 - 2.2%. The nationwide ILI & ARI morbidity level registered this week (142.7 per 10 000) was higher than the national baseline (62.3) by 129%.

As result of 742 clinical samples testing 15 (2.0%) pandemic A(H1N1)v influenza viruses were isolated in Astrakhan, Ekaterinburg, Kaliningrad, Moscow, St.-Petersburg and Vladivostok, 9 (1.2%) influenza A(H3N2) viruses were isolated in Vladivostok and Chelabinsk and 29 (3.9%) influenza B viruses were isolated in Astrakhan, Kaliningrad, Moscow,St.-Petersburg, Orenburg and Vladivostok (table 1).

During the reported week (February 07 - 13.2011) investigations on influenza virus isolation were conducted in 25 Regional Base Laboratories (RBLs) participating in influenza surveillance in collaboration with two WHO NICs (St.-Petersburg, Moscow). As result of 635 clinical samples testing 50 (7.9%) pandemic A(H1N1)v influenza viruses were isolated in Kaliningrad, Moscow, St.-Petersburg, Tula, Vladimir, Orenburg, Yaroslavl and Vladivostok, 4 (0.6%) influenza A(H3N2) viruses were isolated in Vladivostok and St.-Petersburg and 43 (6.8%) influenza B viruses were isolated in Astrakhan, Voronezh, Kaliningrad, Moscow, Novosibirsk, St.-Petersburg, Orenburg and Vladivostok (table 1).

Morbidity level in seven of these cities reached up 2.0% - 3.2%. The nationwide ILI & ARI morbidity level registered this week (129.2 per 10 000) was 2.1 times higher than the national baseline.

Etiology of ILI & ARI morbidity. The overall proportion of respiratory samples tested positive for influenza by virus isolation, IFA and rRT-PCR was estimated as 34.7%. Pandemic influenza A(H1N1)v and B viruses dominated in etiology of morbidity in Russia. According to sentinel surveillance data investigation of clinical samples from 49 SARI patients in 8 cities showed 17 influenza pandemic A(H1N1)v cases, 1 influenza А(H3N2) case and 6 influenza B cases.

Since week 40.2010, 77 influenza viruses were characterized antigenically: 56 were pandemic influenza A(H1N1)v virus A/California/07/09 like, 10 were influenza A(H3N2) virus A/Perth/16/09 like and 11 were influenza B/Brisban/60/08 virus like (B/Victoria/2/87 lineage). 3 influenza B viruses were characterized genetically at Research Institute of Influenza, all 3 belonged to clade B/Brisbane/60/2008 (B/Victoria/2/87 lineage).

Epidemic thresholds of morbidity for overall population (specific for each city) were exceeded in 30 cities from 59 ones under surveillance. Morbidity level in four of these cities reached up 2.0% - 3.2%. The nationwide ILI & ARI morbidity level registered this week (110.0 per 10 000) was 1.8 times higher than the national baseline. However it seams that peak of morbidity is passed.

As a result of 697 clinical samples testing 58 (8.3%) pandemic A(H1N1)v influenza viruses were isolated in Astrachan, Voronezh, Kaliningrad, Moscow, N. Novgorod, St.-Petersburg, Cheboksary and Vladivostok, 3 (0.4%) influenza A(H3N2) viruses were isolated in Vladivostok and 34 (4.9%) influenza B viruses were isolated in Astrakhan, Kaliningrad, Krasnodar, Moscow, Novosibirsk, Omsk, St.-Petersburg and Vladivostok (table 1).

The nationwide ILI & ARI morbidity level registered this week (85.4 per 10 000) was 1.4 times higher than the national baseline.

During the reported week (February 21 - 27.2011) investigations on influenza virus isolation were conducted in 26 Regional Base Laboratories (RBLs) participating in influenza surveillance in collaboration with two WHO NICs (St.-Petersburg, Moscow). As a result of 681 clinical samples testing 59 (8.7%) pandemic A(H1N1)v influenza viruses were isolated in 11 cities located in different regions of Russia, 3 (0.4%) influenza A(H3N2) viruses were isolated in one Far East city and 32 (4.7%) influenza B viruses were isolated in 8 cities of European part of the country and Far East region (table 1).

Since week 40.2010, 139 influenza viruses were characterized antigenically: 101 were pandemic influenza A(H1N1)v virus A/California/07/09 like, 11 were influenza A(H3N2) virus A/Perth/16/09 like and 27 were influenza B/Brisban/60/08 virus like (B/Victoria/2/87 lineage). 3 influenza B viruses were characterized genetically at Research Institute of Influenza, all 3 belonged to clade B/Brisbane/60/2008 (B/Victoria/2/87 lineage).

During the week 11 ending March 20.2011 epidemic thresholds of morbidity for overall population (specific for each city) were exceeded in 4 cities from 59 ones under surveillance. Morbidity level in these cities reached up 0.9% - 1.1% for overall population. The nationwide ILI & ARI morbidity level registered this week (69.9 per 10 000) was above the national baseline by 12.2%.

During the reported week (March 14 - 20.2011) investigations on influenza virus isolation were conducted in 22 Regional Base Laboratories (RBLs) participating in influenza surveillance in collaboration with two WHO NICs (St.-Petersburg, Moscow). As a result of 625 clinical samples testing 35 (5.6%) pandemic A(H1N1)v influenza viruses were isolated in 7 cities located in different regions of Russia and 17 (2.7%) influenza B viruses were isolated in 5 cities located in European part of the country and Siberia (table 1).

Influenza and ARI morbidity data
During the week 14 ending April 10.2011 registration of acute respiratory morbidity cases in Russia continued to decrease. Epidemic thresholds of morbidity for overall population (specific for each city) were exceeded more than 50% only in one city (Veliky Novgorod) from 59 ones under surveillance. In two sities epidemic thresholds were exceeded moderately. The nationwide ILI & ARI morbidity level registered this week (54.6 per 10 000) decreased below the national baseline by 12.4%.
