KUALA LUMPUR: A 36-week-old foetus was saved but the mother, aged 29, died of Influenza A(H1N1) complications.

The drama first unfolded at the Malacca hospital on Tuesday when the pregnant woman, who came in with Influenza-like illness, was admitted when doctors found the foetus in distress inside the mother's womb.

"Doctors who realised that the foetus was undergoing a foetal distress syndrome had to make a quick decision to save its life, especially after laboratory results confirmed that the woman was positive for Influenza A (H1N1)," said Health Minister Datuk Seri Liow Tong Lai.

He said the woman, who was treated with Tamilfu, was wheeled into the operating theatre where the surgeon managed to successfully remove the foetus from the mother's womb.

The woman, he added, subsequently suffered complications and was in the intensive care unit where she died on Friday due to 'adult respiratory distress syndrome'.

"The newborn is negative for H1N1. Since it was born without reaching the full term of pregnancy, doctors are closely monitoring the baby which is in stable condition," said Liow.

Meanwhile, a 54-year-old diabetic, also from the same family and confirmed positive for A(H1N1), died due to complications at the same hospital.

Liow said the deceased was brought to the hospital with ILI on Tuesday and was immediately warded and treated with the anti-viral drug, Tamiflu.

"Throat swabs taken from her showed positive for the disease," he said, adding that due to her uncontrolled diabetic condition, she suffered 'severe metabolic acidosis' which led to kidney failure and sepsis.

He said the deceased lungs also revealed that she had pneumonia related to A(H1N1).

The death toll from Influenza A(H1N1) either directly or due to complications now stands at 80 cases.

"The two deaths clearly show how important it is for people in the high-risk category to seek treatment the moment they have an onset symptoms of the disease. It is not necessary to have fever but if they have all other symptoms related to A(H1N1), they must go the nearest government hospital," said Liow.

He said people with chronic diseases, asthma, diabetes, respiratory problems, kidney failure, pregnant women and children must see a doctor the moment they have fever (not necessarily), cough, cold, headache and body pain.

"Those in high risk groups are immediately treated with Tamiflu to avoid complications due to their co-morbid health conditions such as chronic diseases and obesity," he added.

Citing the two deaths, Liow warned that delay in seeking treatment could lead to complications and death.

Although no new cluster cases were reported within the last 24-hours, Liow said, the ministry was not taking the situation lightly as there was at least some 50 confirmed H1N1 cases every week since last month.

Currently he said, there were 351 people being treated in hospitals nationwide with ILI and of the number 28 were confirmed positive for H1N1.

"We have five of the confirmed cases being treated in ICUs due to them suffering complications due to their co-morbid health conditions," he added.

UALA KUBU BARU: A 54-year-old woman and her 29-year-old daughter died of Influenza A (H1N1) related illnesses at the Malacca Hospital on March 31 and Friday respectively.

Health Minister Datuk Seri Liow Tiong Lai who confirmed the cases here Saturday, said both deaths were due to complications and were not direct H1N1 deaths.

Currently, the total H1N1 death remained at 77 cases, excluding the latest two cases, he told reporters after being briefed by the Kuala Kubu Baru Hospital staff.

Liow explained that the woman who had diabetes tested positive with the H1N1 virus when she was admitted at the hospital on March 31.

It was her diabetes, he said that had led to the complication of severe metabolicacidosis, causing an infection to the kidney and its eventual failure.

?Therefore, her death was due to diabetes complication,? he added.

The victim?s 29-year-old daughter was 36 weeks pregnant and was admitted on the same day at the hospital where she was diagnosed with Foetal Distress Syndrome. Later she showed symptoms of influenza-like illness (ILI) and tested positive with H1N1.

Urging the public not to panic, Liow said it was not a second outbreak of the virus.

He, however, said the ministry would issue a media statement every Wednesday or update on the situation to the media regularly.

?I hope everyone would take care of themselves and not to be in a crowd if they have fever and cough,? said Liow, reminding hospital management and medical officers to be on alert for suspected A(H1N1) cases.

Meanwhile, BERNAMA reported Education director-general Tan Sri Alimuddin Mohd Dom urging schools to take preventive measures to check the spread of A(H1N1).

?I urge schools to refer to the Education Ministry circular as the directive remained the same as that issued last year,? he told reporters after the opening of the National Co-curriculum Gala 2010 by Deputy Prime Minister Tan Sri Muhyiddin Yassin at Kem Tangok in Bachok, Kelantan here Saturday.

Alimuddin said the Health Ministry reported a rising trend for H1N1 since three weeks ago with 50 positive cases weekly.

Liow had said that there were 10 influenza like illness (ILI) cluster cases reported in Kelantan, Perak, Kedah, Selangor and Malacca last week.

They were at SK Selat Bagan Nyior, Pulau Dayang Bunting, Langkawi (48), SM Teknik Langkawi (43), Purnama Beach Resort, Pulau Pangkor (31) and SMK Langkawi Pulau Tuba, Langkawi (30 cases).

I do not understand how someone can say that these two deaths are not related to influenza virus.
The mother had a metabolic acidosis and subsequent kidney problem, which can be triggered by many diseases, including the 'flu
The daughter is said to enter with fetal distress, but usually fetal distress doesn' t cause maternal death. It 's more likely that the mother's infection has caused fetal distress.

I agree tetano. I'm still wondering who the 80th person is. Could just be a mix up.

No Indication Of H1N1 Second Wave Despite Two Deaths, Says Liow

KUALA KUBU BAHARU, April 3 (Bernama) -- Health Minister Datuk Seri Liow Tiong Lai Sunday said that there was no indication that the second wave of Influenza A (H1N1) despite two H1N1 deaths reported at the Melaka Hospital over the past four days.

He said the first case involved a 54-year-old woman who died of diabetic complications on Wednesday although she had tested positive for H1N1.

The woman was admitted to Melaka Hospital on Tuesday after showing symptoms of influenza-like-illness (ILI) and was given Tamiflu treatment, but she died the following day of complication called Severe Metabolic Acidosis, he told reporters after visiting the Kuala Kubu Baharu Hospital here.

Liow said the second case involved the woman's daughter who died of Adult Respiratory Distress Syndrome.

He said the 29-year-old was 36 weeks pregnant and admitted to the hospital due to pregnancy complications.

"At the same time, she was also showing symptoms of ILI and was tested positive for H1N1. Tamiflu treatment was given almost immediately to the woman, the same day a caesarean section was conducted to save her baby.

"After the surgery, the woman was admitted to the intensive care unit (ICU) but she suffered a complication known as Adult Respiratory Distress Syndrome and died yesterday," he said.

Liow said the premature baby, however, was tested negative for H1N1 and was currently in stable condition at the ICU.

The minister said the two deaths proved that those with chronic diseases like diabetes, asthma, kidney problems as well as children and pregnant women were in the high-risk group of H1N1 infection.

Meanwhile, he also called on the people here to continue taking precautionary measures following the increase of the ILI cluster cases, especially with the coming Hulu Selangor by-election scheduled on April 25.

Liow said the ministry would work closely with the Election Commission to prevent the spread of H1N1 and advise those who were unwell not to get involved in the by-election campaign.

As at 6pm yesterday, 28 people were tested positive for H1N1, while 375 ILI cases were being treated at hospitals nationwide.

The H1N1 vaccine is now available for the high-risk group at 104 government health facilities nationwide.

Wife?s last wish to hubby ? look after family and mum

MALACCA: Be strong and don?t worry, these were the last words Mustaza Sarmat, 29, said to his wife Surmani Mah Nor as he held her hands before she underwent a Caesarean operation to save their child at the Malacca Hospital last Wednesday.

?I was by her side. I told her to be strong. I even promised her that both of us would raise our family together,? a tearful Mustaza said as he related his last moments with his wife who slipped into a coma during the operation and died two days later on April 2.

?She told me to look after our family and her mother,? he added. Surmani, 29, and her mother, Maimunah Dimah, 54, who died on March 31, had both contracted the A(HINI) virus.

He said that Surmani, who had a mild fever and slight cough, had sought treatment at a private clinic near their home in Jalan Jasin, Merlimau.

He added a routine pre-natal check up at the Malacca Hospital on March 29 revealed the baby?s heart was beating rapidly, prompting doctors to perform a Caesarean to save the girl.
The 2.95kg baby is free from the A(HINI) virus and is in an incubator at the hospital.

?We have yet to name her,? said Mustaza, a contract worker with Indah Water Consortium in Muar.

Meanwhile, Maimunah?s husband and Surmani?s father, Mah Nor Mah Nasir, 65, was still in shock over the family?s double loss but accepted it as fate.

He said the family had not told Surmani?s three-year-old daughter Nuraira Batrisyia of her mother?s death.

He added that Mustaza?s family and his youngest daughter Fatimah, 17, were put under house quarantine for a week since Friday and that the authorities were monitoring their condition daily.

[She passed away on March 31st.]

Excerpt:

Referring to the two deaths associated with H1N1 infection in Melaka Hospital reported on 31 March and 2 April, is involved in a two-child family. The first case was a 54-year-old woman who is a diabetic (diabetes mellitus). He was admitted on March 30 with symptoms of Inclusion and further treated with Tamiflu. Laboratory tests confirmed he was positive for H1N1. However, he also experienced problems, uncontrolled diabetes, the complications caused "severe metabolic acidosis" due to total kidney failure, other bacterial infections (sepsis) and diabetes was not controlled. X-ray of the lungs did not show any signs of pneumonia to be associated with H1N1. So his death was directly caused by complications of diabetes.

His son aged 29 years 36 weeks pregnant was admitted to hospital on the same day with signs of asphyxiation baby in the womb (Foetal Distress Syndrome). In the same she showed symptoms of Inclusion and confirmed H1N1 positive. Tamiflu treatment began, and to save the babies, performed surgery on the same day. After surgery he continued to put into the intensive care unit but had suffered complications and died yesterday (2 April) due to "Adult Respiratory Distress Syndrome." Tests on the baby she was negative for H1N1 but the baby is being treated at the Intensive Care Unit for close monitoring since he was premature. The baby's condition is stable so far.
