Bird flu
More

JAKARTA, Aug 22 (Reuters) - A 6-year-old Indonesian girl being treated in hospital tested positive for bird flu on Tuesday, a government official said, as the World Health Organisation ruled out human transmission in a village with a series of confirmed and suspected bird flu cases.
The girl from Bekasi, an eastern suburb of Jakarta, is the 60th case of avian influenza in Indonesia, 46 of which have been fatal.
"The girl is still alive and receiving treatment in hospital. She is getting better," I Nyoman Kandun, director general of communicable disease control, told reporters.
Indonesia has seen a steady rise in human bird flu deaths this year and the virus is endemic in poultry in nearly all of the provinces of the sprawling archipelago.
Fears of a possible new cluster arose after a 35-year-old woman from the West Javan village of Cikelet who died was confirmed to have been infected by bird flu.
Her 9-year-old daughter died a week before after showing signs of bird flu, although no samples were taken for testing.
They are among 18 people in the area being investigated for suspected bird flu.
One of them, a 9-year-old girl, died last week while a 17-year-old youth with is infected with the H5N1 virus, but has stayed at home after refusing to be hospitalised.
"Though some of these undiagnosed deaths occurred in family members of confirmed cases, the investigation has found no evidence of human-to-human transmission and no evidence that the virus is spreading more easily from birds to humans," the WHO said in a statement.
Bird flu remains essentially an animal disease, but experts fear it could mutate into a form that can be passes easily among humans.
Indonesia, which has been criticised for not doing enough to stamp out H5N1, has so far refused to do mass culling of poultry, citing the expense and the logistical difficulties because of millions of backyard fowl.

No Evidence Indonesia Bird Flu Spreading

By ZAKKI HAKIM
The Associated Press
Tuesday, August 22, 2006; 11:40 AM

JAKARTA, Indonesia -- Investigators found no evidence that bird flu was spreading among humans in a remote part of central Indonesia where two people died from the virus and a third was sickened, local and international health workers said Tuesday.

Four other people died in the Cikelet region of West Java province before swab samples were taken to determine the cause, officials said, and test results were pending for five others with symptoms of the disease, including a 61-year-old man hospitalized early Tuesday.

Health workers said poultry in the poor region, accessible by motorcycle or on horseback, began dying in large numbers in July after villagers purchased chickens from outside the area and incorporated them into local flocks.

"As the population had no experience with this disease, high-risk behaviors commonly occurred during the disposal of carcasses or the preparation of sick or dead birds for consumption," WHO said in a statement.

"Though some of the undiagnosed deaths occurred in family members of confirmed cases, the investigation has found no evidence of human-to-human transmission and no evidence that the virus is spreading more easily from birds to humans," the statement added.

The H5N1 virus has killed at least 141 people worldwide since it began ravaging Asian poultry stocks in late 2003, 46 of them in Indonesia, the worst-affected country, according to WHO.

Most people have been infected after coming into contact with sick birds, but experts fear the virus could mutate into a form that spreads easily among humans, possibly sparking a pandemic.

Nyoman Kandun, a senior Indonesian health official, said that while there was no evidence of mutation in Cikelet, authorities were carrying out tests on everybody who had been in contact with victims.

All had come back negative so far, he said.

Some 2,400 residents in the area also were being given free doses of the bird flu medication Tamiflu, Kandun said, and an agricultural official said at least 2,500 chickens had been killed in effort to stamp out the disease.

The H5N1 virus is considered endemic in poultry in 29 of 33 provinces across the archipelago, said national bird flu coordinator Bayu Krisnamurthi.
"We are racing against the virus, especially as we approach the rainy season where, just like a common flu, the risk of catching it increases."

Another Suspect H5N1 Fatality in Bekasi
Recombinomics Commentary

August 23, 2006

Suwardi, for example. He claimed to be his wife died with the sign similar to bird flu, every few the day before Nursifa fell ill.

Apart from Nursifa, RSPI Sulianti Saroso treated a bird flu patient in this month. However the patient that also from Bekasi finally had died. Mega Saputra blew out the last breath after could undergo the maintenance in ICU space for three days.

The above translation describes another suspect H5N1 bird flu fatality in Bekasi. The sequence of H5N1 from Bekasi does not match bird isolates, suggesting the patients are being infected by H5N1 in an alternate source. The recovering patient, Nursifa, has been H5N1 confirmed, but has no history of contact with poultry.

As indicated above, even fatal cases in the area are not tested for H5N1 even though there have been H5N1 deaths in the Bekasi area since last year.

Surveillance of patients with bird flu symptoms who do not have a contact with dead or dying birds remain poor, and the number of fatal cases in Bekasi and other regions in Indonesia, including Jakarta, is likely much higher than the reported cases.

Medan, Kompas - The Government of the North Sumatran Province asked for the handling of the case of bird flu better be decentralised or handed over to the area. This request was related the virus H5N1 that at this time it was confirmed spread to four regencies in North Sumatra. The government of the centre was asked to help the procurement of means and the infrastructure that were needed so that the area could carry out the maximal handling.

According to the Head of the Information Body and North Sumatran Province Communication, Eddy Syofian, at this time every time the government wanted to ascertain the sample of the waste of the poultry or positive human blood bird flu, must be sent to Jakarta. Moreover at this time the central Public Hospital (RSUP) Adam the Owner was treating a patient suspect bird flu from Simalungun.

“Sebenar him widely the spreading of the bird flu virus in North Sumatra, the handling of the government better should be not focussed whereas that centralistic. As long as could be done in the area, why wasn't the handling of the bird flu decentralised saja,” said Eddy in Medan, on Thursday (24/8).

Eddy gave an example, ought to North Sumatra had facilities of the health laboratory like the Department's laboratory of the Health in Jakarta, to confirm positive and not him the sample that it was suspected was infected by the bird flu virus. “Selain that, the available hospital in the area also must be equipped by facilities of isolation space for anticipation of the extraordinary incident like bird flu ini,” he said.

At this time RSUD Adam the Owner who was the hospital of bird flu of handling reconciliation in North Sumatra did not yet have special isolation space. The patient suspect and positive bird flu was only treated in one of the installation space treated inap the Rindu A2, that some rooms were also occupied by the non bird flu patient.

The North Sumatran Provincial Government according to Eddy during 2006 allocated the Rp budget 1 billion to build isolation space in RSUP Adam the Owner. This fund was taken from the APBD Change in 2006. “Khusus for the Kabanjahen Hospital, the Department of the Health also helped the development of isolation space and Rp equipment 1 miliar,” he said.

The North Sumatran Provincial Government further Eddy also asked that the government of the centre made areas of the spreading of bird flu the place of the supplies support (buffer stock) Tamiflu. “Saat this stock Tamiflu in North Sumatra 100 doses, the protective equipment 120 set,” he said.

Default My dumb question for the day

While it's hard to say what Tamiflu blanketeers are doing out in the villages, I am assuming that individuals with bird flu symptoms that are rushed to the hospital are swabbed for H5N1 before they are given Tamiflu. Hence it may not in many cases be the Tamiflu that is responsible for the initial negative tests. Is NAMRU2 doing any testing for other influenza strains? Or is additional testing not possible because the sample is used up in the initial H5N1 test procedure

I keep wondering, especially considering there is an epidemic of H5N1 among Indonesian poultry, whether tthe H5N1 test for humans no longer works properly because of mutations in H5N1 strain or whether this virus has combined with other influenza viruses and the tests don't pick it up for that reason.

Have any of the experts chimed in on this phenomenon?
