Hong Kong: Human Swine Influenza Outbreak at Male Ward of Siu Lam Hospital Update (10/29/09)

The following is issued on behalf of the Hospital Authority:

Regarding an earlier announcement on 13 patients of a male ward in Siu Lam Hospital (SLH) confirmed with Human Swine Influenza, SLH announced the following updates today (October 29):

There are 13 more patients (aged 27 to 52) in the concerned ward and two other male wards presenting with fever and flu symptoms.

Viral tests were arranged for the patients and they were confirmed with Human Swine Influenza.

One patient (aged 47) with epilepsy, Down's syndrome and severe mental retardation was transferred to Intensive Care Unit of Tuen Mun Hospital (TMH) today. He is in critical condition and requires ventilation support. He is prescribed with Tamiflu and antibiotics.

The remaining 12 patients are also being treated under isolation in TMH and in stable condition.

We are closely monitoring the condition of all patients.

Infection control measures of the hospital have been stepped up.

Admission to and discharge from the hospital are suspended, while restricted visiting policy is implemented.

Patients' relatives have been informed about the situation. All other patients and the staff in the hospital are under close surveillance.

The cases have been reported to the Hospital Authority Head Office and the Centre for Health Protection for follow up.

His condition continued to deteriorate and he succumbed yesterday (October 31) at 10.50pm.

Hong Kong: Fatal case of human swine influenza at Queen Elizabeth Hospital (11/2/09)

The following is issued on behalf of the Hospital Authority:

A spokesperson of Queen Elizabeth Hospital (QEH) made the following announcement regarding the death of a human swine influenza (HSI) patient today (November 2):

A 55-year-old male patient with high risk factors in health, developed flu symptoms and attended the Accident and Emergency Department of QEH on October 7.

He was admitted for clinical treatment and was prescribed with Tamiflu and antibiotics.

Positive result of HSI was confirmed on the same day.

His condition became critical on October 8 and he was transferred to the Intensive Care Unit and required ventilation support.

The patient passed away today (November 2) at 1.56pm.

The hospital would like to express condolences to the patient's family.

The case has been reported to Hospital Authority Head Office and Centre for Health Protection.

A spokesperson of Prince of Wales Hospital (PWH) made the following announcement today (November 4) regarding a new critical case of human swine influenza (HSI):

A 51-year-old male patient with hypertension and obesity was admitted to a private hospital on October 29 due to fever, cough and shortness of breath.

Positive result of HSI was confirmed on the same day and he was prescribed with Tamiflu and antibiotics.

He was transferred to the medical ward of PWH on October 31 and later admitted to the Intensive Care Unit.

He is now in critical condition and requiring ventilation support.

The hospital will continue to closely monitor his condition.

Hong Kong: Opening remarks by SFH on human swine flu (11/05/09)

Following is a transcript of opening remarks made by the Secretary for Food and Health, Dr York Chow, on human swine flu vaccine at a stand-up media session at the West Wing lobby of Central Government Offices today (November 5):

I announce that today the Government has secured a supply of human swine flu H1N1 vaccine for Hong Kong, following completion of procurement procedures by tender today.

Sanofi Pasteur S.A. succeeded in the tendering exercise.

The Government has placed an order for three million doses of vaccine at a price of HK$237 million.

This company is selected because their proposal meets our stringent requirements.

We attach greatest importance to efficacy and safety.

The unit price is HK$79, which is considered reasonable.

The first batch of vaccine totalling 500,000 doses will reach Hong Kong in mid-December.

The remaining 2.5 million doses will arrive by batches in January.

By the end of December, we will provide free vaccination to five target groups of a combined total of about two million people.

These groups are:
(a) healthcare workers;
(b) chronic patients and pregnant women;
(c) children aged from six months to under six years;
(d) Elderly aged 65 and above; and
(e) pig farmers and slaughterhouse workers.


Scientists worldwide also consider them high-risk groups who should be vaccinated.

We will also provide the vaccine to private doctors at cost including administrative cost.

Those who do not belong to the target groups may seek vaccination from them at their own cost.

For dosage, those who are aged 10 or above require one dose; others aged below 10 require 2 doses.

Babies less than six months old do not need the vaccination.

Two doses should be administered one month apart.

The vaccination programme is a voluntary scheme.

Details will be announced later this month.

Over the past few weeks, many countries have started their own human swine flu vaccination programme.

According to the World Health Organization, there is no unusual adverse effect observed and human swine flu vaccine has the same risk comparable to that of seasonal flu vaccine.

As such, I would like to appeal to the target groups to take the vaccination to protect them from human swine flu.

Hong Kong: Tamiflu-resistant human swine influenza virus detected (11/5/09)

The Department of Health's Public Health Laboratory Services Branch (PHLSB) today (November 5) detected a strain of human swine influenza (HSI) virus which was resistant to oseltamivir (Tamiflu).

A department spokesman said the virus was identified during PHLSB's routine sensitivity tests of the HSI virus to oseltamivir and zanamivir.

"This is the fifth time Tamiflu resistance in HSI virus has been found in Hong Kong and is the fourth local case.

"Tests showed that this strain is sensitive to zanamivir (Relenza)," he said.

The virus was isolated from the specimen taken from a 74-year-old man who developed flu-like symptoms on October 4.

He was admitted to United Christian Hospital on October 7 for acute exacerbation of his chronic lung condition.

He has already recovered and was discharged from hospital on October 13.

His nasopharyngeal aspirate specimen tested positive for HSI.

Further test results today showed that the virus was resistant to Tamiflu.

The man had a previous history of taking a full course of Tamiflu in late September during another admission for acute exacerbation of his chronic lung condition.

His family member was asymptomatic.

The spokesman said there was no evidence of further transmission of Tamiflu-resistant HSI from the patient.

The PHLSB conducts routine sensitivity tests on specimens taken from confirmed HSI patients.

So far more than 5,500 HSI samples have been tested for antiviral sensitivity in Hong Kong.

The case will be reported to the World Health Organisation (WHO), the spokesman said.

Hong Kong has an intensive surveillance system for antiviral resistant influenza viruses.

"We will closely liaise with the WHO and overseas health authorities to monitor the global development of antiviral-resistant HSI virus," he said.

Hong Kong: Supply of swine flu vaccine secured (11/5/09)

Supply of human swine influenza (HSI) vaccine to Hong Kong has been secured with the award of the relevant contract today (November 5).

Following a tender process, the decision was taken to acquire three million doses of HSI vaccine from French manufacturer Sanofi Pasteur S.A., which met a host of strict Government tender requirements, at HK$79 per dose.

The Government's emphasis in the tender was on effectiveness and safety.

The first batch of 500,000 doses will be delivered by mid-December 2009 and the remaining 2.5 million doses are expected by January 2010.

Hong Kong: Designated Flu Clinics revise opening hours (11/5/09)

The following is issued on behalf of the Hospital Authority:

The Hospital Authority (HA) today (November 5) reminded the public of the new opening hours of the eight Designated Flu Clinics (DFCs) on Saturdays, Sundays and public holidays from this Saturday (November 7) in view of alleviation in service demand.

The eight DFCs will run a half-day service (9am to 1pm) during the weekends and holidays, while full-day service (9am to 5pm) will be maintained from Mondays to Fridays.

The HA spokesman said the HA would continue to closely monitor the service demand at DFCs and adjust the service hours to meet the needs as and when necessary.

To wrap up the situation in the past week (October 29 - November 4), the HA spokesman said a total of 1,292 patients with flu-like symptoms attended the eight DFCs.

Among all confirmed human swine influenza patients, 69 are now staying in public hospitals for treatment, with 54 in stable condition, six in serious and nine in critical condition.

Furthermore, the newly confirmed cases in the past week included 30 staff members of public hospitals (eight male and 22 female, comprising two doctors, seven nurses, four allied health professionals, one administrative/clerical staff and 16 supporting staff).

So far 1,068 HA staff members have been confirmed with human swine influenza.

Among them, 1,044 have already recovered and returned to work.

The spokesman reminded the public that the test for human swine influenza is not routine now because the treatment of patients would not depend on the outcome of the test.

Under normal circumstances, both DFCs and Accident & Emergency Departments will only conduct laboratory tests for human swine influenza on the following specific groups of patients:

  • pregnant women;
  • children at or under 12 months old;
  • healthcare workers including staff from residential homes;
  • patients living in institutions without outbreak; and
  • influenza patients with persistent fever (over 38C) or whose condition deteriorates 48 hours after treatment.
Public hospitals and clinics will only prescribe symptom control medication to influenza patients with mild conditions.

Patients can consider seeking consultation first with private doctors and clinics.

Who will perform the inoculation of human swine flu vaccine?
Regarding the discovery of human swine flu virus in pigs, is pork still safe for consumption?

Secretary for Food and Health:
First of all, the details of the vaccination programme will be announced later this month. We probably will be using mostly the public resources, particularly our clinics and services of the Hospital Authority and the Department of Health right now. We should be also approaching the private sector to see in what capacity they can help us.
Regarding the infection of pigs recently, first of all, it has been proven that this virus can be transmitted from humans to pigs in different countries, including Canada and United States as well as now in Hong Kong, and also reported on the same day is a case in Taiwan as well. We feel that this is actually not a new discovery, it is something that is already well established. It is not unusual. There is no additional risk to our population because I think human infection is already spreading in the community. As far as the eating of pork is concerned, I can reassure that pork is very safe especially when you eat pork, you usually cook them and the virus will be killed under a temperature of 70 degree Celsius or above. There is no worry of eating pork at all.

Reporter:
Is it still safe to use Auxiliary Medical Service (AMS) staff for inoculation?

Secretary for Food and Health:
We have not decided exactly the capacity of AMS in the whole programme. Of course, we will be using mostly our public resources, the existing resources in our public clinics.

Reporter:
(about human swine flu vaccine)

Secretary for Food and Health:
Our contract with Sanofi Pasteur S.A. will be based on what they have bid so far. We can be pretty assured that the vaccine will be arriving at the time that is being promised.

Reporter:
Why not including young people in the vaccination programme?

Secretary for Food and Health:
Regarding the group of young people, this is also the group that we think even though they may have susceptibility for infection, the chance of them developing into serious infection and also complication including mortality is much lower than the other groups. This is balancing the benefits of vaccination. We believe the five groups that I have mentioned should be vaccinated first.

Reporter:
Is December too late for Hong Kong?

Secretary for Food and Health:
Our flu season is usually about two months later than the other countries that are giving vaccination right now. We are roughly following the same type of pattern. We are also observing other countries as well. As I have mentioned earlier, normally our two peaks are separated apart for quite a long period. Summer peak is usually in July and August, but this year it extends to almost the end of September and early October. If our second peak starts at the same time, it will be around about February and March next year. But we are also preparing just in case it comes in January, we are still vaccinating the groups at risk much earlier in December instead.

Are you under pressure to include school children into the target group (for human swine flu vaccination)?
Are you going back to ask the expert group to look at this or are you going to wait until you get enough swine flu vaccines to include them ?

Secretary for Food and Health:
As I have told the legislators earlier, we are considering any type of groups that may be at risk apart from the existing five groups that have already been identified.
It is important to note that we are not just providing vaccines so that people just get it but rather, I think we have to balance the risk of having the vaccine and also the risk of not having the vaccine.
The scientific experts have identified the five groups at the moment and they would be the priorities for the coming vaccination programme.
We will be following up on the situation in case during the winter peak, there are actually other groups, including students, that may be affected.
We will be monitoring the situation and then decide.

Reporter:
But if you include the school children, would you have to buy more vaccines?

Secretary for Food and Health:
We will see what the consumption of the vaccines is like. I think just like the experience in other places, if the epidemic is going to be severe, then more people will rush to get the vaccine.But if the peak is not going to be that severe, I can foresee that there will be some people choosing not to be vaccinated.
So this is a very difficult situation to fully predict.
But we have to make preparation for all scenarios and in case there is a need to vaccinate students as well, we will be also getting ready for it.

Reporter:
Are you concerned that the take up rate of the swine flu vaccination is not as you expected among the target groups. What are you going to do?

Secretary for Food and Health:
I think in view of the relatively mild situation of the human swine flu in Hong Kong for the last few months, some of the Hong Kong people may be a bit complacent about what will happen in winter.
But I have to stress that as we cannot predict fully what the situation will be like, I would advise at least the at-risk groups in the community should receive vaccination because judging from the possibility and also the risk, the chance of them getting a more serious illness or maybe even facing the risk of death is there.
So, it is important to appeal to them that they should have themselves vaccinated.

Guangdong, Hong Kong and Macao tripartite meeting on infectious diseases

Key medical and public health officials and professionals from Guangdong, Macao and Hong Kong will attend the Ninth Tripartite Meeting on the Prevention and Control of Infectious Diseases in Zhuhai on Thursday and Friday (November 12 and 13).

During the two-day meeting, experts from the three places will discuss public health issues of mutual concern including the overall situation and control and preventive measures on infectious diseases, monitoring of human swine influenza and anti-microbial resistance, and collaboration of medical and public health experts in the three places.

Led by the Director of Health, Dr PY Lam, the Hong Kong delegation will comprise representatives from the Department of Health and the Hospital Authority.

Hong Kong: Siu Lam Hospital update on human swine influenza outbreak in male ward (11/10/09)

The following is issued on behalf of the Hospital Authority:

Regarding an earlier announcement on 42 patients of three male wards in Siu Lam Hospital (SLH) confirmed with human swine influenza, the hospital announced the following update today (November 10):

Two more patients (aged 38 and 47) in one of the concerned wards have presented with fever and flu symptoms.

Viral tests were arranged for them and they were confirmed to have human swine influenza.

The patients are being treated under isolation in Tuen Mun Hospital and their condition is stable.

Infection control measures at the hospital have been stepped up.

Admission to and discharge from the hospital have been suspended, while a restricted visiting policy has been implemented.

Patients' relatives have been informed about the situation.

All other patients and the staff in the hospital are under close surveillance.

The cases have been reported to the Hospital Authority Head Office and the Centre for Health Protection for follow up.

Hong Kong: CHP investigating a Legionnaires' Disease case (11/11/09)

The Centre for Health Protection (CHP) of the Department of Health is investigating a confirmed case of Legionnaires' Disease.

The case involves a 59-year-old man living in Cheung Chau.

He developed fever, cough and myalgia on October 23 and consulted doctors.

He was admitted to Queen Mary Hospital on October 30, and was transferred to Grantham Hospital on November 3 for further management.

He is now in stable condition.

Blood tests revealed that he had been infected with Legionella bacteria.

The CHP's investigation showed that the patient had no recent travel history.

His home contact was asymptomatic.

This is the 37th case of Legionnaires' Disease reported this year.

Thirteen cases were reported in 2008, 11 in 2007 and 16 in 2006.

Hong Kong: Critical case of human swine influenza at Tuen Mun Hospital (11/11/09)

The following is issued on behalf of the Hospital Authority:

The spokesperson of Tuen Mun Hospital (TMH) made the following announcement regarding a newly confirmed critical human swine influenza (HSI) case today (November 11):

A five-year-old girl with global developmental delay, epilepsy and asthma developed flu symptoms on November 6.

She attended the Accident and Emergency Department of Tuen Mun Hospital on November 9 and was admitted to the intensive care unit for observation and treatment.

Positive result for human swine influenza was confirmed on November 10 and she was prescribed with Tamiflu and antibiotics.

Her condition deteriorated to critical today (November 11) and she requires ventilation support.

The hospital is closely monitoring her condition.

Hong Kong: HA weekly statistics of Designated Flu Clinics and admitted human swine influenza patients (11/12/09)

The following is issued on behalf of the Hospital Authority:

Regarding the services of the eight Designated Flu Clinics (DFCs) and the confirmed human swine influenza patients admitted to public hospitals, the Hospital Authority (HA) today (November 12) provided the following wrap up of the past week:

In the past week (November 5-11), a total of 1,175 patients with flu-like symptoms attended the DFCs.

Among all confirmed human swine influenza patients, 72 are now staying in public hospitals for treatment, with 59 in stable condition, seven in serious and six in critical condition.

Furthermore, the newly confirmed cases in the past week included five staff members of public hospitals (three male and two female, comprising one nurse and four supporting staff).

So far 1,073 HA staff members have been confirmed with human swine influenza.

Among them, 1,069 have already recovered and returned to work.

The HA spokesman reminded members of the public that the new opening hours for the eight Designated Flu Clinics on Saturdays, Sundays and public holidays have been effective since November 7 in view of alleviation in service demand.

The eight DFCs will run a half-day service from 9am to 1pm during weekends and public holidays, while full-day service (9am to 5pm) will be maintained from Mondays to Fridays.
