A tan s?lo cinco d?as de que el Ministerio de Salud bonaerense haya negado que la gripe A fuera la causa del brote de una aguda infecci?n que afect? a 11 personas en el hospital de Carmen de Areco, el Ministerio de Salud de la Naci?n inform? ayer que el genoma viral del virus de la gripe A fue hallado en las muestras de cuatro de los pacientes afectados, que fueron analizadas en el Instituto Malbr?n.

Aunque la mayor?a de los pacientes afectados evoluciona en forma favorable, dos enfermeras del hospital que contrajeron la infecci?n segu?an ayer internadas en estado cr?tico.

Los estudios epidemiol?gicos se orientar?n esta semana a confirmar que el virus provino de los Estados Unidos, inform? el citado ministerio.
Lo que a?n se desconoce es de qu? cepa de gripe A se trata, si la que circul? el a?o pasado en el ?ltimo invierno en la Argentina o la que circula actualmente en el hemisferio norte.

"Si se tratara de la cepa que circul? el a?o pasado en la Argentina, las personas que se vacunaron el a?o pasado tendr?an a?n cierta protecci?n, ya que ?sta dura aproximadamente unos 10 meses", dijo a LA NACION la doctora Carla Vizzoti, jefa del Programa Nacional de Control de Enfermedades Inmunoprevenibles del Ministerio de Salud de la Naci?n.
Vacuna en camino

La funcionaria se?al? que "a fines de febrero comenzar? la distribuci?n de la nueva vacuna antigripal [que se elabora a partir de los serotipos del virus que actualmente est?n circulando, y que difieren de los del a?o pasado]. El caso de Carmen de Areco es un ejemplo de c?mo el personal de salud est? expuesto al virus y que tiene que protegerse, para no contagiar a los pacientes".

EMERGENCIA EN EE. UU.

En este invierno del hemisferio norte, la gripe A mostr? un comportamiento m?s virulento que el a?o pasado. El gobernador de Nueva York declar? en emergencia a la salud p?blica ante los brotes de la enfermedad. S?lo en ese estado norteamericano se detectaron hasta mediados de enero pasado 20.000 casos de influenza con una cepa distinta que, seg?n los diagn?sticos, provoca s?ntomas m?s severos y graves que las temporadas anteriores..

Influenza A(H1N1)pdm09 was not cited in any of the above articles in Spanish (''Gripe A'').

Since in the US the overwhelming majority of influenza A viruses circulating are of subtype H3N2, it is likely that the cluster of Argentinian cases are also caused by the same subtype.

The hyperacute clinical presentation of these nosocomial cases, however, could signal the presence of other pathogens and until a more precise aethiological studies will be performed and results made public, I would remain cautious about the actual causative agent in this cluster.

Malbr?n?, inform? hoy que tras los estudios realizados en el Laboratorio de Virus Respiratorios dependiente del organismo, se detect? la presencia de genoma viral de Gripe A a trav?s de la t?cnica de PCR convencional en muestras de 4 de los pacientes afectados por el brote de neumon?a aguda en el hospital ?Nuestra Se?ora del Carmen?, de la ciudad bonaerense de Carmen de Areco.

Aunque ya se han descartado la mayor?a de las otras causas que se hab?an sospechado cuando se detect? el brote, los ex?menes de laboratorio continuar?n debido a que algunos de ellos requieren repetir muestras luego de unas semanas para detectar cambios en la respuesta inmunol?gica de anticuerpos en los pacientes.


Quienes deben vacunarse contra la gripe

En 2011, la Presidenta de la Naci?n Cristina Fern?ndez de Kirchner decidi? incorporar la vacuna antigripal al Calendario Nacional de Inmunizaciones. Se trata de una vacunaci?n anual que se brinda gratuitamente en hospitales y centros de salud p?blicos de todo el pa?s a:

> Todos los trabajadores de la salud
> Todas las mujeres embarazadas, en cualquier momento de la gestaci?n.
> Todas las mujeres que tienen beb?s menores de 6 meses.
> Todos los ni?os entre 6 meses y 24 meses inclusive.
> Ni?os y adultos que tienen entre 2 y 64 a?os con enfermedades cr?nicas
(respiratorias, card?acas, renales, diabetes, obesidad m?rbida), con receta m?dica.
> Mayores de 65 a?os.

Las personas integrantes de estos grupos deber?n vacunarse anualmente, independientemente de haber recibido tratamiento previo con antivirales.

Another question (if this is just H1N1) is why would pH1N1, after a very large circulation over the last four years and inevitable multiple exposures for healthcare workers (who very likely have had vaccines in the past) suddenly cause such a high level of severe disease and/or virulence?

"Since the beginning of the problem, and even when results were not available, the Ministry of Health took preventive measures and locking action for this type of pathology," said the provincial director of Primary Health Care, Luis Crovetto.

"The joint effort and work with the Ministry of Health of Nation, has allowed us to move forward day by day in the diagnostic investigation and continue therapy instituted from day one," he stressed Crovetto, who coordinated actions in blocking Carmen de Areco hospital.

Of the 11 hospitalized, two were discharged with personalized daily checks by phone. In the hospital of La Plata Rossi's two critically ill patients while other four are stable and progressing favorably.

Desde el Ministerio de Salud bonaerense recomendaron a la poblaci?n vacunarse antes de viajar a los Estados Unidos en base al brote epid?mico que registra en este momento dicho pa?s.

"Debe tenerse [en cuenta] como prevenci?n de que Estados Unidos est? pasando por un brote epid?mico. Ser?a conveniente que quien concurra tenga la protecci?n de la vacuna porque es una herramienta altamente efectiva para prevenir la salud", se?al? en declaraciones a TN el director de Atenci?n Primaria de dicha cartera, Luis Crovetto.

Sobre la situaci?n que atraviesan las nueve personas afectadas de gripe A, cuyo contagio se produjo en el hospital de Carmen de Areco, Crovetto detall?: "Dos se encuentran de alta, 7 est?n evolucionando satisfactoriamente y pr?ximos a finalizar su tratamiento, y otras dos siguen en terapia intensiva en estado cr?tico".

Por otro lado, agreg? que hace 12 d?as que no se registran nuevos casos de afectados, ni en el personal ni en la comunidad, aunque contin?an con las tareas de investigaci?n para conocer su procedencia.

"En principio, como antecedente de importancia, hay un trabajador que hab?a viajado a los Estados Unidos. El Instituto Malbr?n sigue trabajando para ver qu? variedad es y establecer si existe correspondencia con este dato", agreg?.

Crovetto aprovech? las circunstancias para desmentir algunas versiones que hablaban de la circulaci?n viral en la Argentina y, especialmente, en la provincia de Buenos Aires, donde "existe una alta cobertura de vacunaci?n, del 93%"..

Tras la confirmaci?n de cuatro casos de Gripe A en el hospital de Carmen de Areco, el gobernador bonaerense Daniel Scioli pidi? ?tranquilidad? a la poblaci?n y afirm? que no hay circulaci?n de la enfermedad, ya que se encuentra ?encapsulada? en ese centro de salud.

"Quiero transmitir absoluta tranquilidad, (la Gripe A) est? absolutamente bloqueada, limitada y encapsulada en esos casos" de Carmen de Areco, donde est?n internadas cuatro personas, asegur? Scioli en declaraciones a Radio 10.

Por su parte, el ministro de Salud de la Provincia, Alejandro Collia, confirm? hoy que una enfermera que contrajo el virus en Orlando, Estados Unidos, "ser?a el nexo epidemiol?gico" de los cuatro casos de Gripe A detectados en el hospital Nuestra Se?ora del Carmen.
Hasta el momento, asegur?, s?lo est? probado que "cuatro de los once pacientes que fueron hospitalizados con los mismos s?ntomas contrajeron el virus". Y destac? que desde que se cerr? el servicio de terapia intensiva del hospital, hace 12 d?as, no se detectaron nuevos casos.

Gov't denies Swine Flu outbreak, suggests flu shot before visiting US
After four people were diagnosed with Swine Flu in a Carmen de Areco hospital, the head of the BA province Health Ministry?s Primary Care office, Luis Crovetto, suggested that people planning on visiting the US should get a flu shot before traveling.

He also rejected rumours saying there was a Swine Flu outbreak in the country.

Regarding the patients currently being treated, he stated that ?one of them recently visited the US, and the Malbr?n Institute is treating him to determine which virus strain they are dealing with.?

The Government official clarified that ?neither the country nor the province are dealing with a Swine Flu outbreak? and said the population should not be worried.

He concluded by assuring that ?every infected patient was treated quickly? and that no other cases were registered.

This is apparently a quote from an official source. If the traveler is a nurse, it seems unlikely to be the index case. Might there be some co-infection going on that worsened the illness of the two nurses?

BUENOS AIRES --- Four H1N1 cases, two of them seriously, the first in 2013, have been detected in a hospital in the town of Carmen de Areco Argentina (150 km northwest of Buenos Aires), reported Monday a official source.

The advice comes after are detected cases of swine flu in hospital in Carmen de Areco.

The Buenos Aires Governor Daniel Scioli, today called "quiet" after eleven confirmed cases of influenza A in the provincial territory and assured that no viral circulation noting that the disease is "encapsulated" to hospital patients Carmen de Areco.
