Patient admitted with novel coronavirus

Sunnybrook Health Sciences Centre is caring for a patient who has a confirmed case of the novel coronavirus that originated in Wuhan, China.

On Thursday evening (January 23) an adult male presented to the Sunnybrook Emergency Department with fever and respiratory symptoms. The patient was screened and in addition to his symptoms, had confirmed recent travel history to Wuhan, China.

Sunnybrook’s Emergency Department team, isolated the patient immediately and took the necessary precautions to ensure the safety of staff and other patients. Diagnostic testing was conducted and specimens were sent to the Public Health Lab in Ontario to confirm the diagnosis. Today, the diagnosis of novel coronavirus was confirmed.

“I want to thank our front-line teams and the great efforts of those in Public Health, for acting so quickly and efficiently to ensure the patient received an accurate diagnosis and is being cared for safely and effectively,” said Dr. Andy Smith, President and CEO of Sunnybrook Health Sciences Centre. “We are fortunate our teams have been preparing for this type of situation and they performed perfectly.”

Right now, the patient is in stable condition and has been admitted to an isolation room. He will undergo treatment to manage his symptoms and a number of teams in the hospital will be caring for him over the next few days.

“There has been a tremendous amount of learning since the 2003 SARS crisis, which has helped prepare us for a situation like this,” said Dr. Jerome Leis, Sunnybrook’s Medical Director of Infection Prevention and Control. “We followed our usual practice of systematic screening which allowed is to rapidly identify this patient, send appropriate testing, and institute the necessary precautions. I feel confident in our ability to respond appropriately and limit the spread of infection.”

Sunnybrook remains open and clinics and procedures will continue to operate as per normal. The hospital is safe for patients and visitors.

Current situation


On December 31, 2019, cases of undiagnosed viral pneumonia were first reported by health authorities in Wuhan, China. The cause has now been confirmed as a new coronavirus, known as 2019-nCoV, which has not previously been identified in humans.

Coronaviruses are a large family of viruses that circulate in animals including humans and may cause illness in them. Human coronaviruses are common and can range from mild illnesses such as the common cold, to severe illnesses such as Severe Acute Respiratory Syndrome Coronavirus (SARS CoV) and Middle East Respiratory Syndrome Coronavirus (MERS CoV).

Risk to Toronto residents


As of January 25, 2020, Toronto Public Health (TPH) has received notification of Toronto’s first presumptive confirmed case of the novel coronavirus (2019-nCoV) in a resident who recently returned from Wuhan/China. The individual is stable and is hospitalized. The risk to Toronto residents remains low. Toronto Public Health continues to actively monitor the situation in collaboration with provincial and national health agencies, and stakeholders that include local hospitals, airports and community agencies.

Diagnosis and testing


Coronavirus infections are diagnosed by a healthcare provider based on symptoms and laboratory tests. Travel history is also important. There is a specific test for 2019-nCoV to confirm the infection if it is suspected.

Recommendations to Toronto residents


There is currently no vaccine to prevent 2019-nCoV. The best way to prevent infection is to avoid being exposed to this virus. Prevention measures include:
  • Wash your hands often with soap and water or use an alcohol-based hand sanitizer.
  • Avoid touching your eyes, nose, and mouth with unwashed hands.
  • Avoid close contact with people who are ill.
  • Stay home when you are ill.
  • Cover your cough or sneeze with a tissue, then immediately throw the tissue in the garbage and wash your hands.
  • If you don’t have a tissue, sneeze or cough into your sleeve or arm.
  • Clean and disinfect frequently touched objects and surfaces.
Symptoms of the 2019 Novel Coronavirus infection


A novel coronavirus causing pneumonia has been detected in Wuhan, China. Symptoms have included:
  • fever
  • cough
  • difficulty breathing
If you have traveled to Wuhan, China and develop symptoms of 2019 Novel Coronavirus infection, avoid contact with others and follow-up with your health care professional.

Tell your health care professional:
  • your symptoms
  • where you have been travelling or living
  • if you have had direct contact with animals (for example: visited a live animal market)
  • if you have had close contact with a sick person, especially if they have had fever, cough or difficulty breathing.

[h=3]What does a presumptive confirmed or presumptive positive case mean?[/h]
The presumptive positive result comes from a local “rapid” test that gives health authorities a head-start on their work, according to Dr. Peter Donnelly from Public Health Ontario.

Public Health Ontario developed a “specific accurate and rapid test” for the novel coronavirus, he said. The National Microbiology Laboratory in Winnipeg also has a test, albeit slightly different.

The laboratories collaborate with each other, he explained.

“What we’ve agreed is that because we know our test is rapid and reliable, we can actually make a positive result from one of our tests actionable,” he said.

“So in other words, we don’t wait.”

The presumptive positive result becomes a confirmed case once the Winnipeg laboratory provides its result.

A man in his 50s travelled from Wuhan to Guangzhou, China before arriving in Toronto on Jan. 22. Public health officials say he became ill within a day and self-reported by having a family member call 911.

“The emergency service was aware of his travel history. They used full precautions,” said Dr. Barbare Yaffe of Toronto Public Health at a Saturday press conference.

The man was taken to Sunnybrook Hospital with fever and respiratory symptoms, and is currently in stable condition in a negative-pressure room, with staff taking precautions.

A test from the Public Health Ontario Laboratory confirmed the diagnosis. Toronto Public Health has been speaking with the man to determine exactly who he’s had contact with.

“The last word I heard was there was very little by way of contact,” says Dr. Eileen de Villa, the city’s medical officer of health.

She added that their understanding is that the man took “private transportation” home, not mass transportation.

“But we are absolutely making sure that we get the history of all the places the individual has been. That’s part of regular public health practice, in order that we can then determine who needs to be informed and what sorts of measures they need to take,” she said.

OTTAWA -- The first Canadian patient diagnosed with a novel form of coronavirus began showing signs of illness on the plane that brought him back from the disease's epicentre., Canada's Chief Public Health officer said Sunday as she stressed the risk of future infection is low.

Dr. Theresa Tam said the man in his 50's, currently in stable condition in a Toronto hospital, showed mild symptoms on the flight that brought him back to the country from China earlier this week.

Tam said authorities are now working to help track some of his fellow passengers, but said the case demonstrates that the country's public health protocols are working...

...The province of Ontario health officials on Saturday said a man in his 50s developed a respiratory illness after arriving in Toronto on Jan 22 from Wuhan, the epicentre of the outbreak of the viral disease, and was being treated in a public hospital and in isolation.

Theresa Tam, Canada's chief health officer, told reporters in Ottawa that the patient had mild symptoms while on the flight, though he did not report his condition after landing at the Toronto airport.

The individual arrived via China Southern Airlines flight CZ311 from Guangzhou to Toronto. Tam said health officials are in the early process of contacting other travellers on that flight.

But Tam said other people on the flight or airport that were not close to the patient should not be overly concerned...

Canadian medical officials have confirmed a second case of coronavirus in Toronto and are closely watching an additional 19 people who have respiratory symptoms and travelled to the Hubei region of China, while laboratories scrutinize their test results.

Ontario Health Minister Christine Elliott said the second patient is the wife of the first Canadian patient, who federal officials said departed Wuhan, China and boarded China Southern Airlines Flight CZ311, which landed at Pearson from Guangzhou, China on Jan. 22...

Results from the federal government’s microbiology lab in Winnipeg have confirmed the earlier finding of a second case of the 2019 novel coronavirus in Toronto, Ontario Health Minister Christine Elliott said Wednesday.

The woman in her mid-50s was aboard China Southern Airlines Flight CZ311 on Jan. 22, along with her husband, which landed at Toronto Pearson from Guangzhou.

Both she and her husband had spent time in Wuhan, Hubei Province, the epicentre of the coronavirus outbreak...

The man was being treated at Sunnybrook Health Sciences Centre after arriving on a flight from China last week.

"Over the course of the week, his status continued to improve to the point where he no longer required in-patient care," the hospital said in a statement.

The man was being held in a negative pressure room after being admitted to the hospital on Jan. 23, a day after returning from Guangzhou, China.

The patient's wife also has a confirmed case of the virus, although she has been at their home in self-isolation. Public health officials said Thursday that she is "doing well" and hasn't shown symptoms of infection.

Both patients are in their 50s, health officials said.

Fourth case of novel coronavirus confirmed in Canada


A fourth case of the novel coronavirus that has killed more than 200 people in China and sickened thousands of others has been confirmed in Canada.

The new case is in a patient in London, Ont., according to an Ontario government source.

The case has been confirmed by Canada’s National Microbiology Lab in Winnipeg, the source added.

The province’s chief medical officer of health is expected to share more details about the case at a news conference at 2:30 p.m. EST Friday.

Three other cases of the new virus, known as 2019-nCoV, have been confirmed in Canada, one in British Columbia and two in Ontario.

TORONTO - Today, Dr. David Williams, Chief Medical Officer of Health, confirmed Ontario's third case of 2019 novel coronavirus. The case is located in London, Ontario.

Given the newness of the 2019 novel coronavirus and as part of the continuing cooperative efforts between Public Health Ontario (PHO) and the Public Health Agency of Canada (PHAC), Ontario has been sharing all test samples with the National Microbiology Lab in Winnipeg. As part of the National Microbiology Lab's efforts to ensure quality control, additional testing was performed on the samples provided. As a result of this additional testing, a case that was previously deemed negative by PHO's lab has now been found to be positive by the National Microbiology Lab, which uses a more sensitive test that can identify the smallest trace presence of the virus. The majority of cases previously considered negative have been confirmed as negative by the National Microbiology Lab.

To provide the public with the greatest certainty, Ontario's reporting will now include a new category that more accurately reflects the province's status: presumptive negative. That said, for any person found presumptively negative by the PHO lab, there is an extremely low risk of transmitting the virus.

The newly confirmed case, a female in her 20s, arrived asymptomatic in Toronto on January 23, 2020 and was transported to London by way of a private vehicle. On January 24, 2020, the individual started to exhibit symptoms and that same day was assessed and tested at London Health Sciences Centre. She was released with follow up by the Middlesex-London Health Unit. This individual has had limited exposure to other individuals and has otherwise been in isolation since January 24, 2020, with regular contact by the Middlesex-London Health Unit to monitor her condition. She is now recovered and is well.

"I want to re-emphasize that the risk to Ontarians remains low," said Dr. Williams. "It is clear that we are learning more and more about the coronavirus each day, and our testing procedures are evolving and getting more and more precise, which is good news for everyone throughout Ontario and Canada. By working hand-in-hand with our partners across the health care system we are in a very good position to contain the virus and better protect the health and wellbeing of individuals and families across the province."

While requiring validation by the National Microbiology Lab, PHO's lab will continue to offer the province's public health system an early indication of a case's status, allowing the province and local public health officials to quickly and effectively implement all necessary protocols to contain and treat any individuals who have 2019 novel coronavirus.[h=3]Media Contacts[/h]
 
Summary

Asymptomatic patient female in 20's and thankfully went into self-isolation upon return to Canada from left Wuhan on Jan 22, arrive Canada Jan 23 midday. Went to hospital on 24th. Not admitted and sent home for monitoring. University hospital London. Western University student but was not on campus at all. (ironically University Hospital is on the Western Campus so I don't know how they can say that)


The initial test on the sample last week was negative but the sample was retested this week when the test was improved and came back with a positive test with very low viral load. They are still re-running tests on all the earlier cases that have been deemed negative to make sure there aren't any false negatives.

Bottom line is that the patient was asymptomatic on the flight and she was wearing a mask so there is no risk to anyone else on the flight because Public Health Ontario's view is that the evidence for asymptomatic transmission is weak. At least they don't have any evidence to that case but they are not coasting on that, they are double checking and checking with their partners.


Guidelines for releasing from self-isolation are 2 negative tests 24 hours apart but they are working on seeing if that is appropriate.

Question from a reporter regarding ensuring hospitals have enough stock of masks: We have regular lines with the hospitals about stock lines of masks and gowns and if there are any gaps in services we would like to know, the hospitals are going to have to work internally to maintain their existing stock on masks. The reporter stating that Ontario hospitals do not have enough masks but the Chief Medical Officer is not aware of the report the reporter was asking about.

TORONTO — Today, Dr. David Williams, Chief Medical Officer of Health, confirmed a new presumptive positive case of the 2019 novel coronavirus (COVID-19) in Toronto. Ontario's first three cases are all resolved, with each of those patients having two consecutive negative tests at least 24 hours apart.

On February 21, an adult woman arrived to Canada from China and presented at North York General Hospital's emergency department at the advice of Telehealth Ontario with an intermittent cough that was improving.



As per established infection, prevention and control protocols, the patient was cared for at North York General Hospital using all appropriate precautions including being isolated and was tested for COVID-19. Being mildly ill, the woman was discharged home and, per protocols, went into self-isolation.

Toronto Public Health was notified and has been following up with the patient since. The Public Health Ontario Laboratory confirmed today (February 23) that the laboratory testing for COVID-19 is presumptive positive, with the sample now having been sent to the National Microbiology Lab in Winnipeg for confirmation.



Given the individual's clinical assessment and history, there is a low risk that she was infectious. The individual followed all protocols and wore a mask throughout her travels back to Toronto and, since landing, the woman has had very limited exposure to other individuals. The province is coordinating with local public health units to ensure that passengers in close proximity to the woman on the plane are contacted and monitored.

"Because of all the proper protocols and procedures that are in place to contain this virus and exposure to others was limited, I want to assure the public that the risk to Ontarians remains low," said Dr. Williams. "Protecting the health and well-being of individuals and families across the province remains our top priority and we continue to vigilantly monitor for and contain any and all new cases."



Dr. David Williams and Dr. Barbara Yaffe, Associate Chief Medical Officer of Health, will be joined by Dr. Eileen de Villa, Medical Officer of Health for Toronto, to provide an update on the situation at a 9:00 a.m. briefing tomorrow (February 24) at the Queen's Park media studio.
