Very bad news from China, as reported by Xinhua and confirmed by the World Health Organization: For the first time in 12 years, polio paralysis has surfaced in China. Four children, the oldest 2 years old, were diagnosed with polio in mid-July. They all live in Hotan prefecture in Xinjiang province (by weird coincidence, also the site of the latest Chinese food-safety scandal).

The generally accepted math, in polio detection, is that one verified case of polio paralysis represents up to 200 cases of silent infection. Those 200 undetected cases may not experience symptoms, but they can pass on the disease to others. As a result, one case of polio in an area that has been considered polio-free is an emergency. Four cases, as you can imagine, is much worse...

Wild poliovirus confirmed in China



1 September 2011 -The Ministry of Health, China, has informed WHO that wild poliovirus type 1 (WPV1) has been isolated from four young children, aged between four months and two years, with onset of paralysis between 3 and 27 July 2011.

All four cases are from Hetian prefecture, Xinjiang Uygur autonomous region, China.

Genetic sequencing of the isolated viruses indicates they are genetically-related to viruses currently circulating in Pakistan.
The last WPV case in China was reported in 1999, due to an importation from India. The last indigenous polio case occurred in China in 1994.

A national team of clinicians, laboratory experts, epidemiologists and public health experts has been dispatched to the affected region, to assist in the investigation and planning of response activities, and this team will be joined by international support as required.

National, Xinjiang autonomous region and local public health authorities are currently conducting an epidemiological investigation, including collection of stool specimens from contacts and evaluation of vaccine coverage.

The Ministry of Health plans to conduct an initial response vaccination campaign in early September, targeting 3.8 million children aged under 15 years in the key affected outbreak area, and children aged under 5 years in other areas of Xinjiang.

of 10 cases reported by the country this year [2011]. All cases were
reported in Xinjiang Uygur autonomous region, however this latest case
is the 1st outside the originally-infected Hotan prefecture (in Bazhou
prefecture). The most recent case had onset of paralysis on [5 Sep
2011].

Very interesting. China has reported only 10 paralytic infections to the WHO (http://polioeradication.org/Dataandmonitoring/Poliothisweek.aspx), so if this report of 17 deaths is accurate, as polio has less than a 10% fatality rate even in paralytic cases, they may be substantially underreporting.

Given that they reported even 3 non-paralytic cases in Beijing, I have my doubts about this report. And judging by the report below, it may contain an error.

URUMQI, Oct. 21 (Xinhua) -- China vaccinated about 3.89 million children against polio in one of its far western regions this month, bringing the region's vaccinated population to 60 percent after an August outbreak left one person dead and paralyzed 17 others, local health officials said Friday.

The latest round of vaccinations included children under 15 years old in six high-risk areas and children under five years old in eight low-risk areas in Xinjiang Uygur Autonomous Region, said Yin Yulin, head of the regional government's health bureau.

More than 9.3 million children and adults in Xinjiang, a vast region that is home to 21 million people, were vaccinated in September, the official said. The next round of vaccinations will start in November.

The large-scale vaccinations came in response to a polio outbreak in the region that started in August, the first cases of the disease in the country since 2000. One person died and seventeen people, including eight children, were left paralyzed...
