The boy was admitted to a Hong Kong hospital on May 28 with convulsions after arriving from Guangdong's capital city of Guangzhou. He tested positive for the H5 strain.

The boy was confirmed to have been in contact with a live duck between May 17 and 19 at a farmer's market in Guangzhou. However, tests conducted at the market and surrounding markets, as well as a hospital in Zhuangzhou that had admitted the boy, found no signs of a bird flu outbreak, said He Jianfeng, director of the Epidemic Studies Institute of the Guangdong Disease Control Center.

He said the province has launched an avian influenza response plan and is keeping track of the whereabouts of those who were in close contact with the child, as well as monitoring their health.

Laboratory results have confirmed that a 2-year-old boy from Guangzhou does have H5N1 Influenza A, the Centre for Health Protection said today [2 Jun 2012], adding his condition has changed from stable to serious.

He is in intensive care at Princess Margaret Hospital and has developed obstructive hydrocephalus.

His parents are in quarantine at the hospital and lab tests for the influenza were negative.

In the private clinic the boy attended 2 healthcare workers and another patient who attended the same clinic also tested negative.

Updates on H5N1 case

The Centre for Health Protection (CHP) of the Department of Health today (June 3) provided further updates on the Influenza A (H5N1) case involving a two-year-old boy.

The CHP's epidemiological investigation found that the boy had been to Anhui, China with parents from May 20 to 22, during which he had no direct poultry contact. He had visited a wet market with live poultry near his residence in Guangzhou in mid-May.

The boy is currently receiving isolation treatment at Princess Margaret Hospital (PMH). His condition remained serious.

The CHP continued contact tracing work.

Apart from the boy's parents, the CHP had traced contacts with the patient in the private medical clinic in Mong Kok, ambulance and Caritas Medical Centre (CMC), totalling around 80 people. Among them, two healthcare workers from CMC developed some respiratory symptoms and nasopharyngeal aspirates taken from them were tested negative for Influenza A (H5).

Earlier, laboratory tests of the boy's parents, two health care workers of the private clinic and another patient who attended the same clinic were all tested negative for Influenza A (H5).

The CHP has issued letters to doctors yesterday, reminding them to pay special attention to patients who have visited wet markets or had poultry contact in Guangzhou. They are also reminded to report any suspected H5 cases to the CHP.
As at noon today, a total of 10 public enquiries have been received on the DH hotline (2125 1111).

Investigation continues.

A two-year-old boy who is seriously ill with bird flu at Princess Margaret Hospital missed out on treatment for the disease for several days after doctors at another hospital failed to diagnose H5N1, it emerged last night.
The toddler is in intensive care with obstructive hydrocephalus, or water on the brain, a spokesman for the Centre for Health Protection said.

His Hong Kong father and mainland mother do not have symptoms but have been quarantined at the hospital.

The boy was transferred to Princess Margaret on Friday from Caritas Medical Centre where he had been treated in a general pediatrics ward for five days.

A spokeswoman for the Hospital Authority told The Standard that Caritas doctors suspected upper respiratory tract infection.

"We can't put all patients with flu in isolation and it depends on the case," she said.

When asked when the antiviral drug Tamiflu was given to the boy, she said: "What I can tell you is the boy has been given Tamiflu ... at Princess Margaret."

The boy is believed to have contracted bird flu when he and his mother bought a live duck at a wet market in mid-May in Guangzhou and had it dressed on the spot. The Centre for Food Safety said there is no registered poultry farm within 30 kilometers of the patient's usual residence in Guangzhou.

On May 23, the boy developed fever and a runny nose and was taken to two doctors in Guangzhou within three days. He came to Hong Kong on May 26 and visited a private clinic in Mong Kok.

He started convulsing and was taken to the accident and emergency department of Caritas Medical Centre a week ago.

About 80 contacts of the boy at the private clinic, ambulance, and Caritas including children are being traced.

Two health-care workers from Caritas developed symptoms but were not diagnosed with H5N1.

University of Hong Kong virology professor Guan Yi said the virus will be tested to see whether it has mutated.

The Agriculture, Fisheries and Conservation Department inspected all 30 local chicken farms on Saturday and gave them a clean bill of health.

"The department has also tested 145 samples collected from local farms and all were found negative for H5 virus," a spokesman said.

Infectious disease specialist Lo Wing-lok said it is incredible that live ducks are still available in wet markets in Guangzhou and slaughtered in front of customers.

"Guangzhou is one world away from us," he said.

Further updates on H5N1 case

The Centre for Health Protection (CHP) of the Department of Health today (June 4) provided further updates on the Influenza A (H5N1) case involving a 2-year-old boy.

Further tests conducted by the Public Health Laboratory Services Branch of the CHP on the Influenza A virus isolate from the nasopharyngeal aspirate taken from the boy showed that the H5 gene of the isolate belonged to clade 2.3.2.1, which is the same clade as the isolates from wild birds detected in 2011 and 2012 and in the imported human infection case in late 2010.

So far all the genes characterised belong to avian origin and there is no evidence of resistance to the antiviral agent oseltamivir (Tamiflu).

The boy is currently receiving intensive care in the Paediatric Intensive Care Unit of Princess Margaret Hospital.

Investigation into the cause of his obstructive hydrocephalus is still under way. His current condition remains serious.

The CHP so far has kept some 80 contacts under medical surveillance.

Nasopharyngeal aspirates taken from the parents and the five contacts who reported respiratory symptoms tested negative for Influenza A (H5).

As at 5pm today, a total of 20 public enquiries have been received on the DH hotline (2125 1111).

The investigation is continuing.

Hong Kong authorities have confirmed that a two-year-old boy has been hospitalized with the bird flu. Although the bird flu alert level has been raised to ?serious?, health authorities are urging people not to panic.

The toddler is from Guangzhou in southern China and had been running a fever for weeks before his family took him to a private clinic in Hong Kong on May 26. After testing positive for the H5N1 Influenza A virus, he was placed in intensive care in the Princess Margaret Hospital. His parents have been quarantined there as well, although they tested negative for the virus.

Now the search is beginning for the source of the infection.

"A preliminary investigation showed that the boy went to a poultry market once with his mother. But within a radius of 13 kilometers from their home in Guangzhou, there is no such market. So we have to investigate further."

Protocol dictates that a 13-kilometer radius from the source of the infection will be declared an import control zone. Imports of poultry products will be suspended for 21 days.

Because of the high risk of infection, Hong Kong authorities are checking all those who may have had contact with the toddler. He spent five days at the Caritas Medical Centre before he was diagnosed with the bird flu and transferred to the Princess Margaret Hospital. Two healthcare workers there began to show symptoms, but later tested negative.

The bird flu virus that left a two-year- old boy fighting for life in hospital has not mutated but health experts warn Hongkongers not to let their guard down.

Experts said the quick detection of the case means Hong Kong has the right system to catch in midtrack the deadly H5N1 avian flu - seen as a potential pandemic strain even before H1N1 swine flu emerged in 2009.
The boy, who caught the virus in Guangdong, remains in serious condition in the pediatric intensive care unit at Princess Margaret Hospital.
Doctors gave him the antiviral drug Tamiflu in a bid to save his life.
He was admitted for five days to a general pediatric ward at Caritas Medical Centre and treated for obstructive hydrocephalus - or water in the brain - before the H5N1 diagnosis.

Hong Kong health officials have asked the Guangdong authorities to monitor the boy's grandmother who lives with the family.
Children in the Caritas ward were among 80 people put on medical surveillance for 10 to 14 days. Others include doctors and staff at a private clinic in Mong Kok the boy visited and the ambulance workers who transferred him.
The toddler caught bird flu when a duck that his mother bought in a Guangzhou wet market was slaughtered in front of them.
He also has an "atypical" presentation of hydrocephalus, which puzzled doctors, said Malik Peiris, professor of virology at the University of Hong Kong School of Public Health.
Gene sequencing analysis announced last night by the Centre for Health Protection found the H5N1 that infected the boy belongs to the dominant strain - clade 2.3.2.1, the same one as isolates from wild birds detected this year and last, and in an imported human infection case in late 2010.
University of Hong Kong professor of virology and gene sequencing expert Guan Yi said the strain has been detected in Hong Kong, the mainland, Mongolia and Eastern Europe.
The center added that the genes are of avian origin and there is no evidence that the strain is resistant to Tamiflu.
Peiris said: "I don't think we can remain relaxed, we should remain vigilant about H5N1.
"We know this virus remains endemic and established in poultry in many countries all the way to Bangladesh, India, Indonesia, Vietnam Cambodia, Guangdong and other provinces, infecting poultry and humans."
Although the virus can be transmitted to humans, this one case "does not dramatically change the situation one way or the other," Peiris said.
Bird flu measures in Hong Kong are still effective and people do not have to worry about eating poultry within the territory, he added.
"But people should be careful of going to live poultry markets when they travel overseas especially to endemic areas."

1.There is no bird flu in Anhui, Goungdong, Hong Kong, in ANY market, relative or suspect, out of 80 contacts.
2. The boy caught it at a market buying a duck.
3. The boy has the 2.3.2.1. clade.

On June 2, Hong Kong's Center for Health Protection raised the Pandemic Influenza Warning Status from "Alert" to "Serious" in light of the confirmed avian influenza (H5N1) infection of a 2 year old boy who had traveled to Hong Kong with his parents from Guangzhou. Avian influenza is a type of Influenza A virus that mainly affects birds and poultry. Occasionally it can be transmitted to humans if a person has close contact with infected birds/poultry.
The boy appears to have been infected by the virus when he was exposed to live poultry at a Guangzhou wet market in mid-May. Neither the boy's parents nor his treating physicians have contracted the virus indicating that the case appears to be isolated and that human-to-human transmission appears unlikely.
Nevertheless CHP advises increased vigilance to prevent avian influenza, including heightened attention to the following measures:

Avoid touching poultry, birds or their droppings, because they may carry the avian influenza virus; young children in particular should avoid exposure to live poultry.
Wash hands thoroughly with liquid soap and water immediately in case of contact with poultry, birds or their droppings;
Keep hands clean and wash hands properly;
Cover nose and mouth while sneezing or coughing with tissue; if no tissue is available, cough or sneeze into upper sleeve or elbow. Don't use your hands. Dispose sputum or secretions wrapped in tissue paper into rubbish bins with lids. Wash hands with liquid soap and water afterwards;
Build up good body resistance and have a healthy lifestyle. This can be achieved through a balanced diet, regular exercise, adequate rest, reducing stress and no smoking;
Poultry and eggs should be thoroughly cooked before eating;
Maintain good environmental hygiene;
Maintain good indoor ventilation;
Avoid crowded places with poor ventilation if feeling unwell;
Seek medical advice if fever or respiratory symptoms develop.
