Novel Influenza A (H1N1): School closures in Germany

In Germany?s Federal State Hesse (Hessen) in the region of the university town Gie?en, today another school had been closed precautionary by local Authorities. Novel influenza A (H1N1) had been confirmed in the case of a male student, who had been in Munich nearly 2 weeks ago (There had been several confirmed cases in Munich at that time). And there is a suspected case of novel influenza in a girl from the same school, who had been in contact with the confirmed case. Health Authorities are waiting for further test results.

There already had been school closures in Germany for clusters of novel Influenza in a Japanese school in D?sseldorf and another school in Cologne (North Rhine-Westphalia)

Dillenburg/Wetzlar. Weil ein Sch?ler im Lahn-Dill-Kreis an Schweinegrippe erkrankt ist, bleibt seine Schule in Dillenburg bis einschlie?lich Freitag vorsorglich geschlossen. Das teilte der Lahn- Dill-Kreis am Mittwoch in Wetzlar mit. Bei dem Zehntkl?ssler der Goldbachschule im Ortsteil Frohnhausen, einer Haupt- und Realschule mit etwa 430 Sch?lern, war der Erreger nachgewiesen worden. Eine Mitsch?lerin gilt als dringender Verdachtsfall. Ihre Testergebnisse stehen noch aus. In Hessen sind bislang mehr als 20 Menschen an Schweinegrippe erkrankt. Der Junge war vor knapp zwei Wochen von einer Klassenfahrt aus M?nchen zur?ckgekehrt.

Die Testergebnisse der Sch?lerin seien negativ, sagte eine Sprecherin des Lahn-Dill-Kreises in Wetzlar. At least in the RKI-study, that was adopting the SEIR-model, the effect of e.g. school closures depends on the basic reproduction number and the time, you started the non pharmaceutical interventions (as early as possible).

In any case, the main effect in the delay of time is due to the early interventions of the public health service (type 1, KfM 1). The total count of cases will be changed only minimally. The shape of the epidemic wave will be shifted to the right and it will be less high but broader instead.

In other words: The Epidemic within Germany couldn?t be stopped, as long as the Pandemic is going on ?outside?. So, after containment efforts failed, you can only change the number of cases at a certain period of time, but you can?t significantly reduce the total number of cases.

Nevertheless that time delay may be helpful. The epidemic event will not be overwhelming the capacities of health care facilities, the results of therapeutic measures will become better with time and experience. The numbers of workers will not go below essential limits. In this model of the first epidemic wave in any population without pre-existing immunity, you will not have time enough to gain protection provided by vaccination.​

in the UK now has this meaning: Epidemic within UK has reached the advanced stage and the effects of school closures on the epidemic curve are low. In other words: It's no longer effective, it's too late !

But as for Germany, it might not be too late yet to close schools, in order to get some more time (with less damage) until the vaccination will be available.

Also, this virus mainly attacks school-children.

For me as a parent another thing is important: being on top of it.

In the UK the virus is spreading like wildfire. Not yet here in the Netherlands.

When the time comes - and it will come - I would like to hear it fast if / when there is an outbreak at the schools of my 2 children.

Probably we cannot escape the swinefu, probably we will be well after a "mild" or not so mild infection. But you cannot exclude a serious infection, a small percentage in children will end up in ICU and some of them will die.

It seems very important you stay on top of it. I am not sure if all the teachers will be alert and signal childeren with symptoms.

You don't know in advance if you or your child will be dangerously ill or not. No one can guarantee me that.

And indeed it seems ridiculous to go on with the swineflu party named school in case of an infection reported.

So I can't prevent my children to fall ill, but I want to create a situation in which they will be monitored .

Best way to do so is keeping them home I suppose. I am sure grandma will help. :)

gsgs, same to me. I really like these curves of St.Louis vs. Philadelphia, 1918. The main difference is, that we now have antivirals and that we probably will have vaccination, that might sharply cut the epidemic curve. As long as there is any correlation between cases and deaths (and of course it is !), every intervention (school closures e.g.) will contribute to a better outcome.

Dutchy, I totally agree with you. And i'd like to point out that there always is a gap between decisions for individuals (parents and children) and decisions for countries (politicians, State officials, Health Authorities).

No further cases of swine flu had been confirmed.

Ein Sch?ler der zehnten Klasse war positiv auf die Influenza A/H1N1 getestet worden. Die Testergebnisse weiterer Verdachtsf?lle waren negativ ausgefallen.
Nach Angaben des Robert-Koch-Instituts betr?gt die Gesamtzahl der seit dem 29. April best?tigten Schweinegrippe-F?lle bundesweit 505. Die mit gro?em Abstand meisten Erkrankungen seien mit 240 F?llen in Nordrhein-Westfalen aufgetreten. In Hessen seien bislang 30 F?lle best?tigt worden. Zehn Betroffene davon h?tten sich nicht im Ausland mit der Grippe angesteckt, sondern bei bereits Infizierten in Deutschland.

Now total of 12 cases confirmed (+4) in highschool Alexander von Humboldt-Gymnasium. Highschool remains closed for the whole week. Local Authorities warned public of hysteria. Obviously many Berlin residents fearfully went to their doctors, after they heard about school closure due to swine flu.

Bayreuth, Bavaria, July 6, 2009: Two female students of the Bayreuth highschool (Richard-Wagner-Gymnasium) have been newly confirmed cases of novel influenza A (H1N1). They have no idea, where the two girls had been infected. Local authorities decided to close the highschool as a precaution to stop the spread of the virus.

Berliner Schweinegrippevirus aus Texas?

Am Berliner Alexander-von-Humboldt-Gymnasium trotzt zumindest die Lehrerschaft der Schweinegrippe. An der Schule in Berlin-K?penick wurden acht F?lle von Schweinegrippe gez?hlt, worauf der Unterricht in der laufenden Woche gestrichen wurde. Die Lehrer erscheinen trotzdem t?glich, denn nach Aussagen des Schulleiters st?nde auch ohne Sch?ler viel Arbeit an. Das Gymnasium will den Unterricht am Montag, den 13. Juli, wieder aufnehmen ? gerade rechtzeitig: Am 14. Juli gibt es in Berlin Zeugnisse und dann beginnen die Sommerferien.​

Than could we finaly register for the posters, that there were/are an worldwide intentional concertated strategy of NON real state of the matter awareness, joint with inadequate measures, as the NON early total closures from the very begining of this pandemic, to the actual partial closures when it is already exponentialy spreading.

Why such everywhere copy and paste of bad measures, when it is obvious to everybody they did not any barriering?
A strategy of doing nothing special, only to leave it roaming until everybody came in contact.

A global handwashing (some countries which did more than the above are excluded from the rant).

The fact is that the famous Internet, even if tried hardly, did not make enaugh diference to make any substantial changes to such wrong guideliness, at the same time when it induced screamings for other things.

So how did they named them, catastrophists?, casandras?, conspiracy theorists?, freaks?

NY was lucky.
It all started there at one school St.Francis)
They got the Cancun-virus and it was particularly mild in these
children. Only 50 died in a town of 8M.
They are immune now (until...) and NY is almost back
to zero after this wave.

Some cities or countries might consider to do the same
deliberately. So, what virus-strain do they have
in Berlin,Bayreuth ?

Germany: National Pandemic Plan: School closures are useful to reduce peak burden of Pandemic. Quoted scientific literature (Ferguson et al. Nature) says school prophylaxis would require antiviral stockpiles of 72 or 102% of population size. Ministry of Health says: Germany's stockpile of antivirals (doses of treatment courses for Germany's population of 84 millions) currently is: 30 %.

Das IfSG sieht ggf. seuchenrechtliche Ma?nahmen (? 28 ff IfSG), wie das Schlie?en von Kinderg?rten und Schulen sowie anderen Gemeinschafts-einrichtungen, das Verbot von Veranstaltungen oder gr??eren Menschenansammlungen und die Quarant?ne/Ab-sonderung von Kranken, Krankheits- und Ansteckungsverd?chtigen, vor (siehe hierzu auch Ziffer 8.2). Da Kinder zu den wichtigsten Multiplikatoren von Influenza-Infektionen geh?ren, kann eine Schlie?ung von Kinderg?rten und Schulen je nach epidemiologischer Situation sinnvoll sein. Schulschlie?ungen auf dem H?hepunkt der Pandemie wirken sich mathematischen Modellierungsstudien zufolge g?nstig auf die Spitzen-Erkrankungsraten, aber offenbar kaum auf die Erkrankungsraten insgesamt aus [2]. Auch Besuchsverbote in medizinischen Einrichtungen und Pflegeeinrichtungen k?nnen - der Situation angemessen - im Pandemiefall erwogen werden. Die angeordneten Ma?nahmen sollen im Rahmen eines bundesweit einheitlichen Vorgehens erfolgen, ihre Umsetzung muss vor Ort durch die zust?ndige Beh?rde sichergestellt werden.

School closure (Fig. 3d?f) causes a small reduction in cumulative attack rates, but a more substantial reduction in peak attack rates (of up to 40%). Such a reduction in peak incidence could mitigate stresses on healthcare systems and absenteeism in the critical workforce. Closure of 50% of workplaces can enhance the impact of school closure, but at higher economic cost (Fig. 3d?f).

For a socially targeted prophylaxis policy, best use is made of drugs by targeting classmates or close work colleagues rather than the entire population of a school or workplace (see Supplementary Information). School/workplace prophylaxis could have a dramatic impact on attack rates (Fig. 3d?f), but this requires antiviral stockpiles of 72% or 102% of population size for the moderate and high transmissibility scenarios, respectively. The Robert Koch Institute has published a new report on how to change Germany?s strategic plan according to the evolving epidemiological situation.

Preventive measures still could include closure of schools and kindergartens, even for some weeks. But the possible benefit of school closures and similar decisions has to be in balance with the undesired effects of such incisive measures.
