What's causing cholera to spread?

The tragic story of 22-year-old Saint Helene and her 15-month old daughter Cherie began like many here in Haiti. About two weeks ago, Saint Helene bought a bus ticket to Artibonite, a city about an hour north of Port Au Prince. Visiting with friends there last week, they had likely heard about the cholera outbreak that was unfolding in front of them. The good news: When Saint Helene and Cherie headed back to Port-au-Prince a few days later, they felt perfectly fine. Asymptomatic, as we say in the medical world. What Saint Helene or Cherie could not have known at that time is they were likely carrying the cholera bacteria back to the nation?s capital...

...the vast majority of patients with the cholera bacteria actually don?t have any illness at all. Also, it was only several days after an exposure before Saint Helene began to feel ill.

PORT-AU-PRINCE, Haiti, Oct. 29 (UPI) -- The death toll from the cholera epidemic in Haiti hit 330 Friday, health officials said.

Gabriel Thimote, the minister of health, said Friday afternoon 25 of the deaths were reported in the previous 24 hours, Le Nouvelliste reported. He said 4,714 people had been hospitalized with the disease.

PORT-AU-PRINCE, Oct 30 AFP - Haiti has reported seven new deaths from the cholera outbreak, bringing to 337 the number killed by the diarrhoeal disease since it first appeared earlier this month.

With 4764 people infected and under medical treatment, the death rate has nevertheless slowed from earlier this week, officials said on Saturday.

Laboratory Test Results of Cholera Outbreak Strain in Haiti Announced

PORT-AU-PRINCE, HAITI ? The Haitian Ministry of Public Health and Population has received the results of laboratory testing showing that the cholera strain linked to the current outbreak in Haiti is most similar to cholera strains found in South Asia. More information about this strain, including the possibility that it might be found in other regions of the world, is anticipated from additional studies. The findings were reported as part of laboratory collaboration between the National Public Health Laboratory (NPHL) in Haiti and the U.S. Centers for Disease Control and Prevention (CDC) in Atlanta.

The rapid identification of the outbreak strain as Vibrio cholerae serogroup O1, serotype Ogawa and antimicrobial susceptibility profiles were reported last week by the NPHL. The new findings from CDC's laboratory are based on a method of "DNA fingerprinting" called pulsed field gel electrophoresis (PFGE), which analyzes DNA patterns that can then be compared with PFGE patterns of cholera strains from other regions of the world. The PFGE testing was performed on 13 bacterial isolates recovered from patients with cholera in Haiti. The PFGE analysis shows that these isolates are identical, indicating that they are the same strain and similar to a cholera strain found in South Asia.

Haiti cholera deaths rise sharply
The number of people known to have died from a cholera epidemic in Haiti has increased markedly.

Health officials say 105 more people have died since Saturday, bringing the total to 442.

They said there had been a 40% jump in the number of new cases.

As of 3 November, the Haitian Ministry of Public Health and Population (MSPP) reported 6 742 cholera cases including 442 deaths.

The departments where confirmed cases have been reported include Artibonite, Centre, Nord, Nord-Ouest, and Ouest.

Laboratory test on the cholera strain linked to the current outbreak in Haiti conducted by the US Centers for Disease Control and Prevention (CDC) in Atlanta showed that it is most similar to cholera strains found in South Asia.


PAHO/WHO Response

PAHO/WHO and health partners continue to support the Haitian Ministry of Public Health and Population in their response to the cholera outbreak. Priority is given to measures for protection of families at the community level, strengthening primary health care centers already operating across the nation, and establishing a network of special cholera treatment centers and designated hospitals for treatment of severe cases.

Current preventive measures being used to control the outbreak include treating ill people with oral rehydration solution, providing access to safe water, and encouraging good hygiene and sanitation practices.

ARTIBONITE, 4 November 2010 (IRIN) - A colourful cholera prevention poster in Haiti has images of smiling people getting water from a tap, a man using a sturdy latrine and people walking a sick boy to a health centre.

But these vital tools for fighting the infectious bacterial disease - which by 3 November had killed at least 442 people in Haiti and infected more than 6,742 - are absent in most communities, such as the village of Deslandes in the Petite Rivi?re d?Artibonite District.

Here, there is one well for 600 people that rarely functions, the village has no toilets and reaching the nearest health centre requires crossing a river. Most people use the River Artibonite - thought to be the source of the epidemic - as their primary source of drinking, bathing and laundry water.

Rural areas like Deslandes are badly off as well. Open defecation is the rule - a practice that is all the more dangerous when cholera is present. Less than 40 percent of Haitians have access to toilets, according to UNICEF-Haiti.

?When people get sick they relieve themselves in the woods like all of us,? Deslandes resident Ovid Floville, 50, told IRIN. ?[Once they are too weak] and cannot stand any more, they stay at home and their whole body gets covered in diarrhoea.?

He said people scrub their homes with river water. NGOs have brought bleach and other supplies to nearby villages but Deslandes is isolated and access difficult, noted local pastor Solomon Tomas.

PORT-AU-PRINCE (Reuters) - Earthquake-hit Haiti escaped a fresh disaster threatened by Hurricane Tomas, but the storm caused flooding that killed eight people and increased the contagion threat from a deadly cholera epidemic, the government and aid workers said on Saturday.

Amid widespread relief that the hurricane largely spared crowded camps in the Haitian capital housing 1.3 million quake survivors, the international humanitarian operation was turning its attention back to the two-week-old epidemic, which has killed just over 500 people and sickened more than 7,000.

"We do expect the infection rate to jump up due to the flooding and to the bad sanitation conditions in many areas," Christian Lindmeier, spokesman in Haiti for the Pan American Health Organisation/World Health Organisation, told Reuters...

United Nations representative said that the engineers were forced to open the valves of a dam on the river, whose level of the reservoir was too high, to allow water to escape and reduce the pressure on the structure.

Officials suspect at least 120 cases of cholera in Haiti capital

PORT-AU-PRINCE, HAITI - Health officials said Monday that they are examining at least 120 suspected cases of cholera in Haiti's capital, the most significant indication yet that the epidemic might have spread from outlying areas to threaten as many as 3 million people.

Timothee said many of the hospitalized patients are believed to have recently arrived from parts of Haiti such as the Artibonite Valley, where the epidemic was first registered and has done its most ferocious damage.

More than 6,400 of the known 8,138 cases to date have been in the agricultural region, clustered around the Artibonite River.

At least 114 of the people suspected of having the disease in the capital are in the Cite Soleil slum, the expansive oceanside shantytown at the capital's far northeastern edge and its closest point to the valley.

In the Old Military Airport camp in the capital, a Red Cross worker with a megaphone marched through the shacks recently, announcing that cholera had arrived and warning people to wash their hands, sterilize vegetables and clear garbage from around their shacks.

The camp houses some 50,000 people packed onto an abandoned airfield. Aid groups including Doctors Without Borders, the British Red Cross and Partners in Health are building a special cholera treatment unit at the camp. Despite the fact that it is not yet fully operational, patients are arriving with severe diarrhea...

"As you can see, we make the decontaminations with chlorinated water," says Dr. Anany Prospero with Partners in Health. He says this unit will be used to assess possible cholera cases, provide initial treatment and refer the more serious cases to a local hospital. It will operate 24 hours a day.

Prospero says camp residents are being urged to come to the facility immediately if they have severe diarrhea or vomiting.

Those who don't receive appropriate health care "can die in four to six hours," he says.

They've already had a half-dozen suspected cholera cases here; it could take a few days for lab tests to confirm them.

Outbreak 'likely'

Health minister Ariel Henry said that a sizeable outbreak in Port-au-Prince now appeared likely.

"It's coming," Mr Henry told the AFP news agency.

He also said that at least one person in Port-au-Prince had died of the disease.

On Tuesday the ministry said across Haiti the death toll from the epidemic had risen to 583, from 544 on Monday.

The number of sick people receiving treatment had risen from 8000 to more than 9000 within 24 hours, the ministry said.
