COIMBATORE: There has been a huge spike in the number of scrub typhus or rickettsial fever cases, a very rare form of fever, in the last two months. Doctors say they have seen at least 28 cases in the last two months.

Scrub typhus fever began surfacing in the city around three to four years ago. Two major hospitals in the city, PSG Hospitals and Kovai Medical Centre and Hospitals (KMCH), say they have seen 100 and 60 patients respectively over the past two years. An observational study by PSG Hospitals recently found that of the 633 fever cases the hospital treated between 2012 and 2014, 95 tested positive for scrub typhus fever, said Dr Dinesh Narasimhan, general physician at the hospital.

Coimbatore has officially recorded only one scrub typhus case in the last one year, because unlike dengue, malaria or H1N1, it is not mandatory to officially report scrub typhus cases.

CHENNAI: City doctors are seeing an increasing number of patients with scrub typhus, a fever usually seen in rural areas. From a diseases caused by mites, prevalent during winter, it is now seen all through the year, say doctors.

Infectious diseases specialist Dr V Ramasubramanian says one in every ten fever patients is found to have scrub typhus. "It is a very treatable condition, provided the patient comes in early. We are now seeing people even from posh areas within the city," he says. The lack of affordable testing facilities for the illness is a dampener for early diagnosis, he points out.

SHIMLA: Scrub typhus fever claimed life of a 22-year-old from Theog area of Shimla on Saturday. He was initially brought to Indira Gandhi Medical College and Hospital (IGMCH) for treatment, but later referred to the PGIMER, Chandigarh, in a very critical condition. This is the third death in the state in the last three months and second in a week due to the high-intensity fever.

On Wednesday, a 40-year-old woman from Chotta Shimla had died of scrub typhus. She had been admitted to IGMCH on July 19. ?For the past three weeks I have been getting numerous patients with symptoms including fever which then leads to jaundice, kidney problems, breathing problems, low blood platelet count and then coma. Most of the tests including typhoid, hepatitis tests show negative. The patients are usually from the rural belt,? said Dr Plaban Das, Director, Darjeeling Dooars Medical Association.
A 60 year male was brought dead to a nursing home from Bijanbari.
His relatives complained of a 3-day fever accompanied by the above symptoms. A 40- year-old with similar symptoms in a critical state was referred to Siliguri (as Darjeeling does not have any ICU facility) where he died.
?A 72 year old woman was brought in from 10th Mile, Lopchu with such symptoms. While treating her I noticed a black eschar (a bite mark of a mite.) I immediately suspected Scrub typhus. A virus [probably not a virus. - Mod.TY] is spreading through small cattle to people in Rajasthan and other parts of the country that looks like bed bug [?] but is more dangerous than dengue. The disease, called scrub typhus, with DAH (diffuse alveolar hemorrhage) is spreading among people through cattle in which platelets in blood declines rapidly and later blood starts oozing out from body parts and the victim dies. A person of Jaislamer district, suffering from scrub typhus and undergoing treatment at a private hospital in Jodhpur, died on Tuesday evening [1 Sep 2015]. The body was brought to Jaisalmer on Wednesday [2 Sep 2015].

Informing about this case, CMHO Dr NR Naik said that the man, a resident of Jhabra village of Jaisalmer district, who was staying at Mohangarh, consulted a private doctor. But with fever not subsiding, the family members brought him to Jaisalmer and during the investigation at a private hospital, the doctors suspected dengue and referred him to Jodhpur, where he was admitted at a private hospital. The platelet counts kept falling and he died on Tuesday. Dr Naik said, the dengue report for the patient came out negative but was suffering from scrub typhus, which is spread through mites in small cattle.

The patient was administered many units of blood, but there was no improvement in his health and later blood started flowing out in lungs and he died.

Naik said that last year [2014] too, many dengue patients were found in Jaisalmer and the health dept as preventive measure has started putting tamiflos [sic. temephos] in water ponds and tanks and started spraying parathrium [sic. permethrin]. Wherever rainwater is accumulated in pits, burnt oil has been put and the health department is totally active in preventing dengue. On the other hand, an expert said that scrub typhus is a serious fever which is spread by mites. This is spreading in Jaipur and nearby areas. This is probably the 1st case in Jodhpur division in which the victim died. [During 2014] 200 cases of scrub typhus came to light in Rajasthan; generally it spreads among people working in fields.

[This sounds like hemorrhagic fever, although scrub typhus can cause disseminated intravascular coagulation and bleeding. They likely used the Weil-Felix test, which was probably positive for them to make the diagnosis of scrub typhus. No mention is made of whether tests were done to rule out Crimean Congo henorrhagic fever virus infection in this case. Although the illness described in the news report above is compatible with a diagnosis of scrub typhus, the news report above fails to mention in the description of the clinical picture the presence of an eschar, a black lesion at the site of the chigger bite, that is often a useful diagnostic clue.

There was a significant outbreak of scrub typhus in Rajasthan in 2014. Because scrub typhus can present in a non-specific manner, its diagnosis can be difficult and sensitive, and specific diagnostic tests may not be widely available.

"Scrub typhus is caused by the rickettsia-like bacterium _Orientia tsutsugamushi_ that is transmitted by the bite of a chigger mite present in shrubberies. Symptoms develop 6 to 21 days after the chigger bite. Fever, headache, lymphadenopathy, and myalgias are common. A characteristic black eschar that is useful for making the diagnosis may be seen at the site of a chigger bite and a maculopapular rash may also be present. Shimla, Sept. 15 -- The spreading fatal scrub typhus, also known as World War-2 fever, has seemingly set the alarm bells ringing for the health department.
