Roughly 48 hours ago (Oct 8th) Gert van der Hoek (aka Dutchy) on FluTrackers posted an Indonesian MetroTV report on two children hospitalized on Friday (Oct 7th) with severe respiratory symptoms on the resort island of Bali.

Given their reported contact with chickens, and the endemic nature of the H5N1 virus in Indonesia, bird flu was immediately suspected.

Over the next two days the newshounds on FluTrackers and the Flu Wiki posted more than 2 dozen translated reports on these two children, and yesterday we learned that preliminary testing indicated bird flu, and that both children were on respirators.

According to other reports (here & here) prophylactic Tamiflu is being provided to family members and close contacts to these boys, and the community has been told that Tamiflu would be provided to anyone who had contact with sick or dead chickens.

All of this is highly reminiscent of the events of August 2007, when an outbreak of H5N1 killed two women (and was suspected in the death of one’s 5 year-old daughter) on the island of Bali (see WHO reports here and here) (my bolding - S.)

Bali, with a population of just under 4 million, is a popular international tourist destination, although its reputation has suffered in recent years due to outbreaks of bird flu, dengue, and rabies.

A number of countries, including the US and Australia, have issued travel health warnings for Bali, and for the rest of Indonesia (see Here). (my bolding - S. )

Although news reports of bird flu outbreaks, and human cases, in Indonesia have declined over the past couple of years the virus remains endemic in that country, and continues to mutate and spread.

Reporting and surveillance in Indonesia – as it is in much of the world – leaves much to be desired. So while Indonesia reports 146 total deaths from the H5N1 virus, the actual number is unknown.


With these two new deaths, a good deal of attention will be focused on Bali over the next couple of days to see if additional cases show up

To summarise some key points that we need to remember here:-

a) as far as I can gather, the chickens tested negative for H5N1 but there were (as far as I can interpret these reports) chickens that died. This suggests that either the virus in these birds has mutated away from the testing reagents producing a false negative, or that they were not the transmission vector. This needs to be determined.

b) the children were not brought for treatment in a timely fashion, and sadly, this means that Tamiflu was not administered early, which will likely have played a factor in these children's deaths. My heart goes out to their poor parents

c) I think that only local tests have currently confirmed H5N1 and that we do not have confirmatory tests as yet from the central laboratory

Denpasar. Bali officials are on alert again after two children at Denpasar?s Sanglah General Hospital were diagnosed with the H5N1 strain of avian influenza.

The siblings, 5-year-old Nengah Rika and 10-year-old Wayan Aldiawan of Bangli district, were admitted to the hospital on Friday night with symptoms of avian influenza and placed in isolation.

The hospital?s medical and nursing director, Anak Agung Ngurah Jaya Kusuma, said on Sunday that the children had been in direct contact with dead poultry around their house and were treated at a local health clinic before being brought to Sanglah.

Officials are still waiting for results of further tests from Udayana University and the Health Ministry.

Bali Animal Husbandry Agency head Putu Sumantra said a team had been deployed to Bangli, but preliminary results showed no trace of H5N1. He said that given the preliminary test, he was not ready to conclude the birds died of avian influenza.

Three new Indonesian H5N1 deaths include two siblings

Oct 10, 2011 (CIDRAP News) ? Three Indonesian children have recently died from H5N1 avian influenza infections, including two siblings from the island of Bali, according to reports today from the media and the World Health Organization (WHO).

The WHO confirmed one of the deaths today, and its verification of the two from Bali would push Indonesia's H5N1 total to 181 cases, including 149 fatalities. The country has the most WHO-confirmed H5N1 cases and deaths in the world.

The first case involves a 1-year-old girl from the West Jakarta district of DKI Jakarta province, who died on Aug 25, the WHO said in a statement today. She got sick and was treated at a healthcare facility on Aug 8 then admitted to a private hospital on Aug 15.

An investigation into the source of her infection found that one of the girl's family members is a caterer who routinely processes poultry, including slaughtering, according to the WHO statement. Poultry was kept in the girl's neighborhood, but no chicken deaths had been reported within the 14 days before her illness.

Meanwhile, a hospital in Bali said yesterday that two children, a 10-year-old boy and his 5-year-old sister, died yesterday after a 2-day hospitalization, Deutsche Presse-Agentur (DPA) reported today.

The two children were admitted to Sanglah General Hospital in Denpasar on Oct 7 with H5N1 symptoms and placed in isolation, according to a Jakarta Globe story today. Though tests at the hospital were positive for the H5N1 virus, further tests are being conducted at Udayana University and at Indonesia's health ministry.

An official from the hospital said the children had direct contact with dead poultry in their home and had been treated at a local clinic before they were hospitalized, according to the Globe report.

Puto Sumantra, who heads Bali's animal health agency, told the paper that a team dispatched to investigate the poultry deaths found no sign of the virus on preliminary tests, though some of the carcasses have been sent for further lab tests. He added that the area has been disinfected and that samples have been taken from the childrens' family members.

The infections and deaths are Indonesia's first since early June and the first to strike the resort island of Bali since 2007. In August 2007, the WHO reported that the virus killed two women, ages 28 and 29. The younger woman was a poultry trader.

The virus was suspected in the death of the other woman's 5-year-old daughter, who had died about 2 weeks earlier.

WHO confirmation of the two infections and deaths of the Bali children would raise the global H5N1 total to 568 cases, including 334 deaths.

Avian flu claimed its first human victims this year on Sunday night after two children from Jehem village, Bangli, passed away in Sanglah Central Hospital in Denpasar.

Five-year-old N.R. died at 8:20 p.m., while her 10-year-old brother, W.A., died at around midnight. The medical team has confirmed that the two children were infected with the avian flu virus based on the results of three tests at different labs.

The last recorded fatalities due to avian flu in Bali were in 2007, claiming the lives of two patients from Tabanan and Jembrana, data from the hospital showed.

From 2007 up to this month, the hospital has treated 58 patients suffering from avian flu.

IGB Ken Wirasandhi from the Sanglah Hospital?s avian flu treatment team said that the two children were referred from Bangli regional hospital with breathing difficulties.

?They were already in a severe condition and losing consciousness, and they had had fever for 12 days. We tried our best by implementing the protocol for handling avian flu patients, but they didn?t survive,? he said.

The victims? bodies would be kept in the hospital?s morgue for a week because their family refused to cremate them right away as suggested by the medical team.

The team stated that bodies cremated immediately could prevent the possible further spread of the disease.

The family argued that it would not be possible for them to organize a cremation since their village was currently holding a religious temple festival.

Balinese Hinduism prohibits its adherents from organizing death-related rituals during major temple festivals.

Ken advised people to be alert to the spread of avian flu, especially in handling and cooking chicken. ?It?s been four years since the last death from avian flu, but now it has emerged again,? he added.

The Husbandry Agency has conducted rapid tests of samples taken from dead poultry in the victims? neighborhood, all of which showed negative results. It is now waiting for the results of further laboratory tests.

IGN Mahardika, a researcher of the avian flu virus, said Bali was still vulnerable to avian flu transmission due to ecological factors. He urged the Husbandry Agency to immediately send the samples taken during the rapid tests to a laboratory for more extensive tests.

?Laboratory research has shown that the virus remains H5N1, but it is necessary to keep studying the possibility of mutation,? he said.

Previous research by veterinarian IGN Badiwangsa in Klungkung has concluded that the avian flu virus emerges at times when local people hold numerous religious celebrations.
