Chicken Cull in Bali After Avian Influenza Outbreak

On the heels of a rabies outbreak in Bali, officials on the tourist island are now wrestling with the shadow of avian influenza.

Made Badra, the head of Badung district?s animal husbandry, marine and fisheries agency, said on Tuesday that one person was being treated for suspected bird flu infection, while 133 chickens from the Jagapati Abiansemal village had been culled after one out of five of the sick birds, selected randomly from the group, tested positive for the H5N1 virus last week.

Indonesia remains the world?s most-affected country from the virus, with 113 human deaths from 139 cases as of Jan. 19, according to the World Health Organization. Indonesia?s failure to reign in the disease ? which is endemic among domestic birds in the vast majority of Indonesia?s 33 provinces ? has prompted critics to warn a global pandemic could originate from Indonesia.

?We conducted the culling [last] Thursday, Friday and [Tuesday] morning and eliminated 133 chickens,? he said, adding that local residents had voluntarily given up their birds to be destroyed.

Badra also said that a local resident, Putu Pramayoga, was experiencing bird-flu like symptoms after coming in contact with sick poultry. He was given antibiotics and antiviral drug Tamiflu and his condition was said to be improving.

?This morning I have checked. The fever and the cough have gone,? he said, adding that Putu?s tissue samples had been sent to Sanglah hospital in Denpasar to find out whether or not he was infected with bird flu.

?There is no rapid test for humans. Laboratory tests take time, so we cannot confirm whether or not Putu was infected by the H5N1 virus.?

Badra said that in an effort to curb the spread of the virus, the agency would conduct a drive to disinfect chicken farms and slaughter houses.

?We will also reactivate surveillance in nearby villages to monitor the transportation of chickens,? he said.

Badra said awareness of bird flu in Badung had improved after a small outbreak of the virus struck the district in 2007, killing two people.

The state-run Antara news agency reported on Tuesday that 11.5 percent of Lampung Province?s 2,330 villages, a total of 268 villages, had tested positive for bird flu among birds in January.

Sunaryo Kasman, section head of animal and veterinary health for Lampung?s husbandry and animal health agency was quoted as saying that the survey conducted by the agency in January 2009 indicated that the virus had spread to eleven subdistricts.

Sunaryo also said that in 2004, the H5N1 virus killed more than 1.8 million birds in Lampung, the highest rate in the province, while in 2008 the number of dead birds fell to 25,282.

The decreasing figure was the result of increasing awareness in the community and faster response times when bird flu cases were detected, he said.

A local man suspected of having bird flu rested in his home in Jagapati village, Badung, a place well-known for cockfighting, while officials continued to cull fowls in the area.

Putu Pranayoga, 20, was resting in his bedroom when The Jakarta Post visited. Dwi Tresna Ningsih, a field doctor who has been treating and observing Putu, said he began showing symptoms of influenza Monday, just a few days after the report of a possible bird flu outbreak in the area.

"We've given him Tamiflu *the medicine required to treat bird flu suspects* and we'll continue to observe him every five hours," Tresna said Tuesday.

She said Putu seemed to be getting better but declined to rule out the possibility of bird flu, saying that she was still waiting for results of Putu's blood test, which was sent to Jakarta on Monday.

"We can't know for sure until we've received the results of his blood tests, which we will get in two weeks," she said.

It is the latest resurgence of the much-dreaded avian flu in Bali since the death of a 29-year-old woman, who allegedly died after being infected by the H5N1 virus on August 2007.

The virus was first detected on Wednesday last week, when a villager reported the sudden and nearly simultaneous death of 15 fowls in the area to the Badung Animal Husbandry and Fisheries Agency.

The dead fowl tested positive of bird flu.

The agency has since culled as many as 133 fowls in the district, with further culling to continue in the days to come until all the villages' estimated 180-fowl population has been eliminated.

The announcement above indicates that at least four other people in Indonesia have been locally confirmed with bird flu. All four of these "locally confirmed cases" have died. These deaths would have occurred after Jan 2, the last reported death in a WHO update.

There have been a number suspected cases since Jan 2, but the three listed below died and are likely to be officially confirmed in the future. Perhaps someone else has other suspected individuals that may have died from bird flu.

TAK hanya unggas di Kelurahan Cilendek Timur Bogor Barat yang positif flu burung. Sumber air di kolam pembuangan RT 02/03 juga positif mengandung virus H5NI. Hal tersebut berdasarkan hasil tes laboratorium Badan Penelitian Veteriner Bogor. Warga sempat panik ketika mengetahui kondisi tersebut. Apalagi sebelumnya jenazah Ruslan, penderita suspect flu burung 29 Desember 2008, dimakamkan di lokasi itu. Jadi, beberapa warga meminta penjelasan Dinas Kesehatan dan Dinas Pertanian Kota Bogor untuk memberikan jaminan keamanan dan kesehatan.
Kemarin, Dinkes dan Distan mengadakan penyuluhan di Majelis Taklim Al Istiqomah RW 04 Kelurahan Cilendek Timur. Hadir dalam kesempatan itu DSO Flu Burung dari Dinas Kesehatan Kota Bogor Dwi Susanto. Dia menjelaskan, pencemaran sumber air itu dipastikan bukan dari makam Ruslan. Sebab, prosedur penanganan jenazah suspect flu burung mengikuti standar WHO. Pertama dibungkus menggunakan plastik khusus antikuman lalu dilapisi aluminium foil. Ditambah pelapis khusus yang terbuat dari seng dan terakhir dimasukkan dalam peti kayu. Empat lapisan tersebut dijamin tidak akan menularkan virus keluar.
Sementara Kasi Kesehatan Hewan dan Produksi Dinas Pertanian Kota Bogor drh A Maolana menuturkan, pencemaran terjadi bukan di kolam penampungan yang biasa digunakan untuk keperluan warga. "Sampel air yang positif diambil dari kolam pembuangan warga. Diduga karena banyak unggas air yang berkeliaran di lokasi tersebut. Sedangkan kolam penampungan air milik warga masih negatif dan relatif aman digunakan sehari - hari," terangnya. Karena itu, Dinas Pertanian mengimbau kepada warga mengkandangkan unggasnya. Terutama unggas air yang diketahui lebih kuat menahan virus dalam tubuh, sehingga tidak mati. Meski terjangkit, itu bisa menjadi pengantar penularan virus.
Dari hasil pemeriksaan Dinas Pertanian, di sekitar lokasi tersebut belum ditemukan unggas mati akibat virus flu burung. Sementara Kepala Dinas Kesehatan Kota Bogor dr Triwanda Elan mengungkapkan, meski telah ada korban suspect flu burung hingga dua orang, pihaknya belum berani mengeluarkan status siaga flu Burung untuk Kota Bogor. "Sekarang kita terus waspada dan masih memonitor semua orang yang sakit dan ada kontak dengan unggas mati," ucapnya saat dihubungi Radar Bogor, tadi malam. "Penentuan status siaga dilakukan terhadap korban manusia, sementara unggas kita baru melakukan dengan penanggulangan, salah satunya sosialisasi kepada warga," ujarnya. Sosialisasi ini kian intensif dilakukan di daerah rawan. INDONESIA: Steps taken to confront bird flu but danger remains

[This is the first reference I have seen that the Bali patient has been hospitalized. It could be an error]

JAKARTA, 12 February 2009 (IRIN) - Indonesia is making progress in its fight against avian influenza but the danger posed by the H5N1 virus to humans remains high, specialists said.

Last month, the Indonesian Health Ministry announced that two more people had died of the disease, bringing the death toll to 115, the highest in the world.

Investigations indicated that both victims visited wet markets before contracting the disease.

On 8 February, a 20-year-old man on the resort island of Bali was hospitalised for suspected bird flu after dozens of chickens in his village tested positive for the virus. Tests were being carried out to determine whether he had contracted bird flu.

However, Bayu Krisnamurthi, executive director of Indonesia's National Committee for Bird Flu Control and Pandemic Preparedness, said there was no evidence the virus had mutated into a form that could make it easier for humans to be infected.

"Bird flu has now become like other infectious diseases spread by animals. The virus is around us, but there's no clinical evidence that it is any more dangerous than before," Krisnamurthi told IRIN.

Progress

James McGrane, team leader of the UN Food and Agriculture Organization's Influenza Control Programme in Indonesia, said the government had made substantial progress in bringing avian influenza under control, but more needed to be done.

"The disease appears to have been contained, with data appearing to indicate that both overall incidence and geographic spread are decreasing, although the disease remains entrenched in some areas," McGrane told IRIN.

"These advances have been made despite the unprecedented challenges presented to avian influenza control given Indonesia's massive and diverse poultry population," he said.

But McGrane said Indonesia needed to work harder to tackle the disease, particularly on Java, where most of the deaths have occurred.

Twelve of Indonesia's 33 provinces have recorded human cases of bird flu while only two provinces have had no avian flu in poultry, the national bird flu committee said.

Chairul Anwar Nidom, a virologist with the Tropical Disease Centre at Airlangga University in Surabaya, said a common policy on bird flu was lacking among government agencies, making controlling the disease more difficult.

"By and large the government has done a good job and people are aware of the danger of bird flu," Nidom said.

But Nidom criticised the government's policy of vaccinating poultry rather than culling, believing that it masks the virus, and ultimately contributes to its mutation.

"There's still no common agreement on this issue among government agencies. The agriculture and farming sector sees the need for vaccinations to save the economy. We are at a crossroads, having to choose between saving poultry and protecting human beings," said Nidom.

Nidom, who first revealed the outbreak of bird flu in Indonesia, said the threat of a pandemic remained high given the unpredictable nature of the H5N1 virus.

"Frankly, we still don't know exactly the virus's pattern of infection and its predispositions. As long as it remains so, it will remain a threat to humans," he said.

Krisnamurthi said despite the progress, there was no room for complacency and the government continued its campaign to warn the public about the danger of bird flu.

Last month, the Indonesian National Committee for Bird Flu Control and Pandemic Preparedness, supported by the UN Children's Fund (UNICEF), launched workshops on pandemic preparedness in 10 cities across the country, he said.

Since last year Indonesia has adopted a policy of only publicly reporting human infections from time to time rather than as they occur.

In addition, the country has been locked in a dispute with the World Health Organization over sharing virus samples for the past two years. Health Minister Siti Fadilah Supari has insisted Indonesia will not share virus samples unless it receives assurances it will be given any vaccine made from the viruses.

The Bird Flu Information Corner, a joint project between Kobe University, Japan and and the Institute of Tropical Disease, Airlangga University, Indonesia has two updates on their site today.



The first appears to clear the Bali suspect of having the H5N1 virus, although additional blood tests will be conducted.


The second comes from Metro News TV, and it relays information about 4 people who have come down with fevers after consuming possibly infected poultry in Bekasi, which is about 15 miles east of Jakarta.









The four are being treated at home with Tamiflu, pending medical tests. Apparently, their conditions are not serious at this time.



These are, of course, only suspect cases.

Nyoman Sutedja also announced that I Putu Prana Yoga (19), bird flu suspected patient resident of Dusun Sibang was negative bird flu infection after treated at home for several days. However, Bali health service will still send the blood sample for reconfirmation examination.

Metrotvnews.com, Bekasi – Four people residents of Desa Sukamulya, Kecamatan Sukatani, Kabupaten Bekasi, West Java developed high fever after consuming poultries which possibly infected by bird flu virus. Three days ago about 42 chickens belong the people around the neighborhood were dead.



Dead chicken samples have been confirmed positive bird flu virus. The four victims have been treated by Tamiflu by medical team of local public health center. Up to now, victims haven’t been admitted to the hospital until blood tests result issued.
