WASHINGTON (AP) ? A new study finds that the way swine flu multiplies in the respiratory system is more severe than ordinary winter flu.

Tests in monkeys, mice and ferrets show that the swine flu thrives in greater numbers all over the respiratory system, including the lungs, and causes lesions, instead of staying in the head like seasonal flu.

In addition, blood tests show that many survivors of the 1918 flu pandemic seem to have immunity to the current swine flu, but not to the seasonal flu that hits every year.


Those were findings from a study by a top University of Wisconsin flu researcher that was released Monday and will be published in the journal Nature.

The researcher, Yoshishiro (yosh-ee-hero) Kawaoka (cow-a-woka), said he is more concerned about swine flu because of these results.

MADISON - A new, highly detailed study of the H1N1 flu virus shows that the pathogen is more virulent than previously thought.

Writing in a fast-tracked report published today (July 13, 2009) in the journal Nature, an international team of researchers led by University of Wisconsin-Madison virologist Yoshihiro Kawaoka provides a detailed portrait of the pandemic virus and its pathogenic qualities.

In contrast with run-of-the-mill seasonal flu viruses, the H1N1virus exhibits an ability to infect cells deep in the lungs, where it can cause pneumonia and, in severe cases, death. Seasonal viruses typically infect only cells in the upper respiratory system.

"There is a misunderstanding about this virus," says Kawaoka, a professor of pathobiological sciences at the UW-Madison School of Veterinary Medicine and a leading authority on influenza. "People think this pathogen may be similar to seasonal influenza. This study shows that is not the case. There is clear evidence the virus is different than seasonal influenza."

The ability to infect the lungs, notes Kawaoka, is a quality frighteningly similar to those of other pandemic viruses, notably the 1918 virus, which killed tens of millions of people at the tail end of World War I. There are likely other similarities to the 1918 virus, says Kawaoka, as the study also showed that people born before 1918 harbor antibodies that protect against the new H1N1 virus.

And it is possible, he adds, that the virus could become even more pathogenic as the current pandemic runs its course and the virus evolves to acquire new features. It is now flu season in the world's southern hemisphere, and the virus is expected to return in force to the northern hemisphere during the fall and winter flu season.

To assess the pathogenic nature of the H1N1 virus, Kawaoka and his colleagues infected different groups of mice, ferrets and non-human primates - all widely accepted models for studies of influenza - with the pandemic virus and a seasonal flu virus. They found that the H1N1 virus replicates much more efficiently in the respiratory system than seasonal flu and causes severe lesions in the lungs similar to those caused by other more virulent types of pandemic flu.

"When we conducted the experiments in ferrets and monkeys, the seasonal virus did not replicate in the lungs," Kawaoka explains. "The H1N1 virus replicates significantly better in the lungs."

The new study was conducted with samples of the virus obtained from patients in California, Wisconsin, the Netherlands and Japan.

The new Nature report also assessed the immune response of different groups to the new virus. The most intriguing finding, according to Kawaoka, is that those people exposed to the 1918 virus, all of whom are now in advanced old age, have antibodies that neutralize the H1N1 virus. "The people who have high antibody titers are the people born before 1918," he notes.

Kawaoka says that while finding the H1N1 virus to be a more serious pathogen than previously reported is worrisome, the new study also indicates that existing and experimental antiviral drugs can form an effective first line of defense against the virus and slow its spread.

There are currently three approved antiviral compounds, according to Kawaoka, whose team tested the efficacy of two of those compounds and the two experimental antiviral drugs in mice. "The existing and experimental drugs work well in animal models, suggesting they will work in humans," Kawaoka says.

Antiviral drugs are viewed as a first line of defense, as the development and production of mass quantities of vaccines take months at best.

In addition to his appointment at UW-Madison, Kawaoka also is a professor at the University of Tokyo. The new study was funded by grants from the U.S. National Institutes of Health, and the Japanese Ministry of Education, Culture, Sports, Science and Technology.

infected Americans ?

If 1918 gave antibodies, then maybe 1976 too ?
although that was only vaccine, not the real thing and only one vaccination,

Wow, people born before 1918. Not a lot of those people still around. More fascinating yet is, the 1918 Spanish flu remnants were still circulating until 1957. Why then are people only showing high antibody titres if they born then before 1918? That tells me this is closer to a disease circulating before 1918 not afterwards.

Nothing new. Both Kawahoka and Garcia-Sastre provided earlier results with animals studies where antibodies against H1N1 (1933/Wsn) and A/Tx/91 - A/New Caledonia reduces mortality in mice inoculated with lethal dose of reconstructed 1918 virus and reassortants variants. Here at FT Sci Library are available some abstracts about their work.

The 2009 pandmeic is a re-run of 1918. The paths are coincident.

1918 was a H1N1 swine/human recombinant. The survivors were infected with the 1918 strain and have HIGH titers (over 1000) still (in samples collected a few years ago).

The sera were collected from REAL 1918 survivors (human) a few years ago. The HIGH titers against 1918 were published several years ago by Kawkaoka in Nature.

Many of us have, for a variety of reasons, suspected this for some time.

WASHINGTON, July 13 (Reuters) - The new H1N1 influenza virus bears a disturbing resemblance to the virus strain that caused the 1918 flu pandemic, with a greater ability to infect the lungs than common seasonal flu viruses, researchers reported on Monday.

Tests in several animals confirmed other studies that have shown the new swine flu strain can spread beyond the upper respiratory tract to go deep into the lungs -- making it more likely to cause pneumonia, the international team said.

In addition, they found that people who survived the 1918 pandemic seem to have extra immune protection against the virus, again confirming the work of other researchers.

"When we conducted the experiments in ferrets and monkeys, the seasonal virus did not replicate in the lungs," said Yoshihiro Kawaoka of the University of Wisconsin, who led the study.

The H1N1 virus replicates significantly better in the lungs."

The new swine flu virus has caused the first pandemic of the 21st century, infecting more than a million people, according to estimates, and killing at least 500. The World Health Organization says it is causing mostly moderate disease but Kawaoka said that does not mean it is like seasonal flu.

"There is a misunderstanding about this virus," he said in a statement. "There is clear evidence the virus is different than seasonal influenza."

Writing in the journal Nature, Kawaoka and colleagues noted that the ability to infect the lungs is a characteristic of other pandemic viruses, especially the 1918 virus, which is estimated to have killed between 40 million and 100 million people.

OLD PROTECTION

They tested the virus in blood samples taken from nursing home residents and workers in 1999 in California, Wisconsin, the Netherlands and Japan.

People born before 1920 had a strong antibody response to the new H1N1 virus, meaning their body "remembered" it from infection early in life. This finding supports a study published in Nature in August that also found people who survived the 1918 pandemic still had immune protection against that virus.

Flu viruses change constantly, which is why people can be re-infected and why the vaccine must be changed regularly. Current seasonal strains of H1N1 are distant cousins of both the 1918 pandemic strain and the new H1N1 strain.

"Our findings are a reminder that swine-origin influenza viruses have not yet garnered a place in history, but may still do so, as the pandemic caused by these viruses has the potential to produce a significant impact on human health and the global economy," the researchers wrote.

Other tests showed the virus could be controlled by the antiviral drugs Relenza, made by GlaxoSmithKline <GSK.N>, and Tamiflu, made by Roche AG <ROG.VX>, the researchers said.

The World Health Organization said on Monday that vaccine makers should start making immunizations against H1N1 and that healthcare workers should be first in line to get them.

Companies working on an H1N1 vaccine include Sanofi-Aventis <SASY.PA>, Novartis AG <NOVN.VX>, Baxter International Inc <BAX.N>, GlaxoSmithKline <GSK.L>, Solvay <SOLB.BR> and nasal spray maker MedImmune, now part of AstraZeneca <AZN.L>. (Editing by Doina Chiacu)

so 1918 is only 91 years away genetically, while 1957 is 130 years away



can't they find the exact epitopes which provide protection ?


what is it, that protects after 90 years but often
doesn't protect in vax after just ~2 years ?

1918 was an H1N1 human / swine recombinant. 2009 is heading in the same direction (swine picking up early H1N1 polymorphisms). The 2009 pandemic virus knows EXACTLY what it is doing, and it has been obvious for some time now (as soon as the pandemic strain was identified as swine H1N1).

The pandemic virus doesn't "know" anything. It is not a conscious entity. It may be predisposed to follow a certain course of development, but it's not thinking.

1957 was H2N2 which replaced H1N1. H2N2 is a DIFFERENT serotype, which has NOTHING to do with the numbers above.

You would be quite surprised. It has been evolving for eons and KNOWS exactly what it is doing (this isn't Kansas).

Aha! Thanks GSGS. That makes perfect sense.
It makes NO sense. 1957 was H2N2 and a different serotype. 1918 was half swine H1N1 and half human H1N1. Pandemic 2009 influenza is swine H1N1.
